Severe adenovirus-associated pneumonia complicated by biopsy-evaluated acute kidney injury in an immunocompetent adult: a case report.

BACKGROUND: Adenovirus infection is typically self-limited in immunocompetent individuals. Severe adenovirus-associated pneumonia with renal involvement is uncommon and has been predominantly reported in immunocompromised hosts. Acute kidney injury (AKI) during viral infections may occur through inflammatory or tubular mechanisms even in the absence of immune-complex-mediated glomerular disease. CASE PRESENTATION: An 18-year-old previously healthy immunocompetent woman presented with severe pneumonia. Upper respiratory tract PCR was positive for adenovirus, while blood and urine cultures were

Epstein Barr virus antigen-induced autoantibodies against complement C1q exacerbate renal disease in lupus-prone mice.

BACKGROUND AND AIMS: Systemic Lupus Erythematosus (SLE) is a complex autoimmune disease characterized by the development of autoantibodies against multiple antigens, including complement C1q, starter molecule of the classical pathway. Anti-C1q autoantibodies (anti-C1q) are not only a biomarker of disease activity but believed to contribute to the pathogenesis of proliferative lupus nephritis. Previous studies demonstrated that a key immunogenic site of C1q (so-called 'A08') shares an identical sequence with Epstein-Barr-Virus (EBV) Nuclear antigen-1, and that anti-C1q can be induced by this EB

Predominant HLA class I associations with lupus nephritis susceptibility, histology class and reduced renal function in patients with systemic lupus erythematosus: a retrospective case-control study.

To investigate associations between (human leukocyte antigen) HLA alleles and overall lupus nephritis (LN) susceptibility, biopsy-defined histologic classes and reduced renal function in Taiwanese patients with systemic lupus erythematosus (SLE). We conducted a retrospective case-control study using data from the Taiwan Precision Medicine Initiative. The matched cohort included 388 SLE patients with biopsy-confirmed LN and 381 SLE patients without LN. HLA alleles were imputed from single nucleotide polymorphism (SNP) array data using HIBAG. Multivariable logistic regression was employed to eva

Serum sialic acid binding immunoglobulin-like lectin-1 (sSIGLEC-1) in Egyptian patients with lupus nephritis: correlation with renal activity in non-European ancestry.

Serum sialic acid binding immunoglobulin-like lectin-1 (sSIGLEC-1) is a type I interferon-associated biomarker previously linked to lupus nephritis (LN) in European ancestry populations, but its utility in non-European cohorts remains poorly defined. This study aimed to validate the association of sSIGLEC-1 with LN in Egyptian SLE patients (non-European ancestry) and to test its correlation with world health organization (WHO) pathological classes, chronic kidney disease (CKD) stages, systemic lupus international collaborating clinics-renal activity score (SLICC-RAS), systemic lupus erythemato

Completion Nephrectomy Outcomes Following Prior Partial Nephrectomy in Hereditary Kidney Cancer Syndromes.

Hereditary kidney cancer syndromes predispose patients to multifocal, bilateral and/or recurrent renal tumors, requiring repeated interventions over a lifetime. Despite nephron-sparing surgery being the preferred management strategy, some patients ultimately require completion nephrectomy-radical nephrectomy following prior ipsilateral partial nephrectomy-when tumor burden, declining renal function, surgical complications, or pre-transplant indications necessitate definitive kidney removal. Data on outcomes in this population remain limited. We evaluated a prospectively maintained cohort of 58

The effect and mechanism of TIPE in promoting M2 polarization through DcR3 in the microenvironment of colorectal cancer.

BACKGROUND: Elevated expression of tumor necrosis factor α-induced protein 8 (TIPE) is observed in colorectal cancer (CRC) and is known to regulate decoy receptor 3 (DcR3) expression while promoting M2 polarization of macrophages. However, the precise role of DcR3 in macrophage M2 polarization remains to be elucidated. METHODS: We employed bioinformatic analysis alongside in vitro and in vivo experimental approaches to investigate the regulatory mechanism by which TIPE influences macrophage M2 polarization in CRC. RESULTS: TIPE expression in CRC tissues positively correlates with M2 macropha

Reconsidering Race-Based Medicine in Pediatric Rheumatology: Challenges and Opportunities for Equitable Care.

Despite growing evidence of their limitations, race-based practices in pediatric rheumatology-those that rely on race or ethnicity to influence diagnosis and treatment-continue to shape care, often reinforcing health disparities. The assumption that biologic or genetic differences exist between racial groups oversimplifies complex health issues and perpetuates health inequities. This article examines persistent race-based practices in pediatric rheumatology, particularly in the interpretation of laboratory results and clinical decision-making, and highlights their clinical limitations. For exa

Immune Imbalance in Primary Membranous Nephropathy at Single-cell Resolution.

BACKGROUND: Primary membranous nephropathy (pMN) often progresses to end-stage renal disease (ESRD) in the absence of immunosuppressive therapy. The immunological mechanisms driving pMN progression remain insufficiently understood. METHODS: We developed a single-cell transcriptomic profile of peripheral blood mononuclear cells (PBMCs) from 11 newly-diagnosed pMN patients and 5 healthy donors. Through correlation analysis, we identified potential biomarkers for disease stratification and poor prognosis. RESULTS: Expression levels of several proinflammatory factors were significantly increased

Different profiles of fatty acids between leukocytes and whole blood in children with idiopathic nephrotic syndrome.

Idiopathic nephrotic syndrome (INS) is the most common pediatric glomerular disease, characterized by proteinuria, hypoalbuminemia and edema and caused by an immune dysregulation of T and B cells. Fatty acids (FA) are involved in immune response, with omega-6 prevailing in pro-inflammatory states and omega-3 promoting anti-inflammatory effects. While previous studies of INS assessed FA profile in blood or serum, which may be influenced by many systemic and dietary factors, the intracellular FA metabolism in white blood cells of children with INS, critical to immune cell activation, remains sti

CpG methylation changes associated with hyperglycemia in type 1 diabetes occur at angiogenic glomerular and retinal gene loci.

Chronic hyperglycemia is a major risk factor for glomerular or retinal microangiopathy and cardiovascular complications of type 1 diabetes (T1D). At the interface of genetics and environment, dynamic epigenetic changes associated with hyperglycemia may unravel some of the mechanisms contributing to these T1D complications. In this study, blood samples were collected from 112 young patients at T1D diagnosis and 3 years later in average. Whole genome-wide bisulfite sequencing was used to measure blood DNA methylation changes of about 28 million CpGs at single base resolution over this time. Chro

The intelligent podocyte: sensing and responding to a complex microenvironment.

Podocytes are key components of the glomerular filtration barrier - a specialized structure that is responsible for the filtration of blood by the kidneys. They therefore exist in a unique microenvironment exposed to mechanical force and the myriad molecules that cross the filtration barrier. To survive and thrive, podocytes must continually sense and respond to their ever-changing microenvironment. Sensing is achieved by interactions with the surrounding extracellular matrix and neighbouring cells, through a variety of pathways, to sense changes in environmental factors such as nutrient level

GALNT14 deficiency: connecting multiple links in the IgA nephropathy pathogenetic chain.

IgA nephropathy (IgAN) is a highly prevalent type of primary glomerulonephritis. IgAN involves mesangial deposition of immune complexes leading to complement activation, inflammation, and glomerular injury. A key hit for pathogenesis involves aberrant O-glycosylation in the hinge region of IgA. Despite its prevalence, however, the mechanisms underlying IgAN remain incompletely understood. In this issue of the JCI, Prakash and colleagues used whole-exome sequencing of two IgAN probands to identify loss-of-function variants in GALNT14 leading to loss of the enzyme GalNAc-T14, which is involved i

The Role of Glomerular and Serum Expression of Lymphocyte Activating Factors BAFF and APRIL in Patient with Membranous and IgA Nephropathies.

Increased activity of B lymphocytes underpins many autoimmune conditions. A key component of the humoral immune response involves the A PRoliferation-Inducing Ligand (APRIL) and B-cell-activating factor (BAFF) system. These proteins are responsible for the activation, maturation, and survival of B lymphocytes, playing a pivotal role in autoimmunity. Therefore, targeting the BAFF/APRIL system proves promising for the treatment of various autoimmune diseases. Meticulous research into pathomechanisms of lupus nephritis (LN) has enabled the introduction of biological treatments targeting the BAFF-

Identifying the potential therapeutic targets of tertiary lymphoid structure in IgA nephropathy based on bioinformatics.

Studies have found that there is tertiary lymphoid structure (TLS) in IgA nephropathy (IgAN), and the existence of TLS has an impact on renal function, creatinine, and proteinuria in patients. We aim to explore the potential molecular mechanisms and therapeutic targets of TLS in IgA nephropathy by bioinformatics methods, hoping to provide treatment methods. The datasets GSE226840, GSE237120, and GSE116626 from the Gene Expression Omnibus (GEO) database were employed to investigate the potential therapeutic targets of TLS in IgAN. The R was used to obtain the differentially expressed genes (DEG

CXCL12/CXCR4 modulates macrophage efferocytosis to induce glomerular crescent formation and fibrosis via ELMO1/DOCK180/RAC1 signaling in ANCA-associated glomerulonephritis.

BACKGROUND: ANCA-associated glomerulonephritis (AAGN) is a leading cause of uremia in children, driven by macrophages (Mφs) that mediate crescent formation and fibrosis. Despite their critical role, the signals governing monocyte recruitment and macrophage polarization in AAGN remain unclear. METHODS: We utilized single-cell sequencing to dissect the cellular dynamics of AAGN and conducted in vitro and in vivo experiments to explore the role of the CXCL12/CXCR4 signaling axis in monocyte recruitment and macrophage efferocytosis. Protein interaction analyses further delineated the downstream s

Dopamine and its effects on the respiratory system: A narrative literature review.

Dopamine is a neuromodulator molecule that is involved in several systems in the human body. As a neurotransmitter, it plays a role in regulating reward, pleasure, and motor control in the brain. Beyond its well-known central nervous system functions it significantly influences peripheral systems including kidneys, circulatory system, and notably, immune system. It increases glomerular filtration rate and renal blood flow in the kidneys, while it increases aortic pressure and cardiac output in the circulatory system. Crucially, dopamine and its receptors have been identified on various immune

A Case of Myeloproliferative Neoplasm-Related Glomerulopathy Secondary to Primary Myelofibrosis With Acute Leukaemic Transformation.

Glomerular diseases associated with myeloproliferative neoplasms (MPN) are rare, and most often present with proteinuria and kidney impairment. Its natural history is not well described, although it has been associated with poor prognosis in described cases. Here, we present a case of MPN-related focal segmental glomerulosclerosis (FSGS) secondary to primary myelofibrosis (PMF) and describe its progression with transformation of PMF to leukaemia. A 52-year-old gentleman was referred for lower limb swelling on a background of primary myelofibrosis requiring splenectomy 3 months prior. Kidney fu
