Autosomal dominant tubulointerstitial kidney disease in Chinese patients.

BACKGROUND: Autosomal dominant tubulointerstitial kidney disease (ADTKD) is recognized as a significant cause of genetic kidney disease. However, data from Asian populations remain limited. METHODS: We evaluated 70 patients meeting KDIGO clinical criteria for ADTKD at Peking University First Hospital between 2010 and 2024. Genetic testing was performed using whole-exome sequencing, with PacBio long-read sequencing employed for cases meeting clinical criteria but lacking identifiable causal variants. Plasma and urinary uromodulin levels were measured and compared between ADTKD, IgA nephropathy

Mechanisms and Targeted Therapeutic Strategies for Inflammation and Renal Fibrosis in Hyperuricemic Nephropathy.

Hyperuricemia (HUA) is a growing global health issue driven by economic development and lifestyle changes. Approximately 75% of uric acid in humans is excreted renally. Excess uric acid deposits in renal tissues, promoting tubulointerstitial fibrosis and leading to hyperuricemic nephropathy (HN), which is characterized by urate crystal deposition, chronic interstitial nephritis, and renal fibrosis. The pathogenesis and progression of HN involves dysregulated activation of multiple signaling pathways, including MAPK, Nrf2/HO-1/NQO1, PI3K/AKT, and ASK1/JNK/c-Jun pathways, which facilitate diseas

Beyond Ingestion: Nasal Inhalation-Induced Aristolochic Acid Nephropathy from a Traditional Herbal Remedy - A Case Report.

INTRODUCTION: Aristolochic acid nephropathy (AAN), a progressive form of interstitial nephritis, is predominantly associated with the oral ingestion of herbal remedies containing aristolochic acid (AA). CASE PRESENTATION: This report details a rare case of AAN linked to nasal inhalation of a traditional herbal preparation. An 80-year-old monk with chronic kidney disease and rapidly declining kidney function underwent kidney biopsy, revealing severe interstitial fibrosis, tubular atrophy, and the presence of aristolochic acid-DNA adducts, confirming the diagnosis. Further investigation uncover

Lipid droplets in felid kidneys: prevalence and composition by lipidomics.

INTRODUCTION: An accepted and common phenotypic curiosity of Felidae is the presence of intracytoplasmic lipid droplets in renal proximal tubule epithelial cells (RPTEC), also frequently observed in urine (lipuria). Both outcomes are currently considered, and taught, as incidental - without obvious pathophysiological consequence. This contrasts markedly with clinical (human) medicine, where lipid vacuoles in RPTEC are usually associated with metabolic or chronic disease, such as chronic kidney disease (CKD). Despite domestic felids having a high incidence of CKD with advancing age, no study ha

Tirzepatide-associated interstitial kidney injury.

We report a case of interstitial nephritis, likely secondary to tirzepatide. A 66-year-old man with type 2 diabetes, stage 3b chronic kidney disease, and other metabolic comorbidities experienced a progressive decline in renal function. Serum creatinine rose from 1.1 mg/dL (SI: 97.2 µmol/L) to 2.11 mg/dL (SI: 186.5 µmol/L) (reference range, 0.6-1.2 mg/dL [SI: 53-106 µmol/L]); estimated glomerular filtration rate (eGFR) fell from 68 to 34 mL/min/1.73 m2 over 8 months while receiving escalating doses of tirzepatide. Despite cessation of other medications and supportive care, renal dysfunction pe

Chronic Granulomatous Interstitial Nephritis Presenting Five Years After Intravesical Bacillus Calmette-Guérin Therapy: A Case Report and Literature Review.

Intravesical administration of Bacillus Calmette-Guérin (BCG) has been the standard treatment for non-muscle invasive bladder cancer (NMIBC).  Its efficacy in preventing recurrence and tumor progression has been well established; however, rare immune-mediated complications, particularly parenchymal renal involvement, pose significant diagnostic challenges. This report describes an exceptional case: that of a 73-year-old man who developed renal failure five years after his last BCG instillation. Histopathological examination revealed granulomatous interstitial nephritis. This case is remarkable

Recurrent IgG4 Disease-Related Tubulointerstitial Nephritis Treated With Rituximab As Maintenance Therapy.

Immunoglobulin G4-related disease (IgG4-RD) is a rare chronic immune-mediated multisystem disease with renal manifestations of tubulointerstitial nephritis (IgG4-TIN) and membranous nephropathy (IgG4-MN). IgG4-RD and its sequelae are treated with immunosuppressive therapy, including steroids and rituximab (RTX), which have demonstrated high efficacy in achieving disease remission. We describe a patient who was diagnosed with pancreatitis secondary to IgG4-RD through laboratory and imaging studies. Despite being on RTX, he developed acute kidney injury (AKI) and was diagnosed with IgG4-TIN via

Acute tubular injury versus acute interstitial nephritis in the kidney precision medicine project.

BACKGROUNDAcute interstitial nephritis (AIN) is a common cause of acute kidney injury (AKI), but the diagnosis may be missed as kidney biopsies are rarely obtained when acute tubular injury (ATI) is suspected.METHODSThe Kidney Precision Medicine Project is a cohort study that obtains kidney biopsies from individuals with AKI, which undergo pathologic and molecular interrogation. We compared ATI and AIN cases among the first 60 AKI participants.RESULTSOn clinicopathologic adjudication, 30 patients (50%) had a primary adjudicated diagnosis of ATI, 13 (22%) patients had AIN, 9 (15%) had diabetic

Spatial analysis reveals cellular microenvironments and mechanisms of inflammation and injury in acute interstitial nephritis.

Acute interstitial nephritis (AIN) is present in 15-20% of all acute kidney injury cases but lacks proven effective therapies beyond corticosteroids. Using high-resolution imaging mass cytometry and single-cell spatial transcriptomics to analyze human kidney biopsies with AIN, acute tubular injury (ATI), and reference tissue, we show that interferon-γ-induced CXCL9-CXCR3 signaling is markedly increased in AIN, with significantly more predicted interactions relative to ATI, dominated by homotypic T cell-T cell amplification networks concentrated in lymphoid aggregates. Tubular cells, predominan

Soluble urokinase plasminogen activator receptor (suPAR) is associated with intrarenal chronicity features on diagnostic and repeat kidney biopsies in lupus nephritis.

BACKGROUND: Soluble urokinase plasminogen activator receptor (suPAR) is linked to organ damage in systemic lupus erythematosus (SLE). We investigated whether serum suPAR associates with histological activity and chronicity in diagnostic and 12-month repeat kidney biopsies in lupus nephritis (LN). METHODS: suPAR levels were determined using ELISA in serum samples from 23 LN patients (19 incident) before and 12 months after initiation of immunosuppressive therapy, and from 15 age- and sex-matched healthy controls. Correlations with NIH activity index (AI) and chronicity index (CI) scores were a

An "All-in-One" Kidney Biopsy in AIDS: Crescentic Immune Complex-Mediated Glomerulonephritis, CD8-Driven Interstitial Nephritis, and Superimposed Thrombotic Microangiopathy.

INTRODUCTION: Human immunodeficiency virus (HIV)-associated kidney disease encompasses a diverse spectrum of glomerular, tubulointerstitial, and vascular pathologies. While HIV-associated nephropathy and immune complex-mediated glomerular disease in the setting of HIV are well-recognized entities, their simultaneous presentation with thrombotic microangiopathy (TMA) and diffuse infiltrative lymphocytosis syndrome (DILS) has not been previously documented. We report a novel case of advanced HIV infection manifesting as crescentic lupus-like glomerulonephritis (GN), CD8-predominant tubulointerst

Significant kidney impairment in pediatric inflammatory bowel disease and practice points for pediatric gastroenterologists.

OBJECTIVES: Children with inflammatory bowel disease (IBD) have an increased risk of developing kidney disorders, which may cause significant kidney function impairment (SKI) or lead to chronic kidney disease (CKD). In this study we aimed to provide insights in causes and diagnoses of SKI cases and to provide recommendations for pediatric gastroenterologists for children with IBD and SKI. METHODS: Cases of SKI in children with IBD (<19 years) were collected from the international, prospective PIBD-SETQuality Safety Registry. A monthly survey was sent to participating pediatric gastroenterolog

Renal limited sarcoidosis presenting with non-caseating granulomatous interstitial nephritis in the absence of extrarenal manifestations: a case report and literature review.

Renal limited sarcoidosis is an exceptionally rare condition characterized by non-caseating granulomas confined solely to the kidneys, without involvement of other organs. Its diagnosis is challenging because of the absence of systemic manifestations. We present a case of a 74-year-old woman with a history of type 2 diabetes mellitus, papillary thyroid carcinoma, and osteoporosis, who showed progressive renal dysfunction. Laboratory findings revealed elevated serum creatinine and urinary β₂-microglobulin, indicating tubular injury. Kidney biopsy demonstrated granulomatous interstitial nephriti

Chronic Cough With Reduced Diffusing Capacity and Preserved-Ratio Impaired Spirometry in a Young Never-Smoker With Systemic Lupus Erythematosus: A Diagnostic Pitfall.

Chronic cough in patients with systemic lupus erythematosus can be difficult to evaluate, especially when initial testing is unrevealing. We report the case of a 33-year-old woman with long-standing systemic lupus erythematosus, biopsy-proven lupus nephritis, prior extrapulmonary pericardial tuberculosis, and no active or passive smoking exposure who developed a persistent dry cough for six months. She was initially treated for gastroesophageal reflux disease with pantoprazole, without meaningful relief. Chest radiography and swallowing assessment were normal. Initial pulmonary function testin

Mikulicz Disease Revealing IgG4-Related Tubulointerstitial Nephritis: A Case Report and Literature Review.

Background and Clinical Significance: IgG4-related disease (IgG4-RD) is a chronic fibroinflammatory, immune-mediated multisystem disorder that can mimic neoplastic, infectious, or autoimmune conditions. Among its head-and-neck manifestations, IgG4-related dacryoadenitis and sialadenitis, historically referred to as Mikulicz disease, should be distinguished from the classical Mikulicz syndrome, which describes secondary lacrimal and salivary gland enlargement due to other systemic disorders. Renal involvement, most commonly in the form of IgG4-related tubulointerstitial nephritis (IgG4-TIN), is

Distal Renal Tubular Acidosis as a Renal Manifestation of Sjögren's Syndrome: A Case Report.

Distal renal tubular acidosis (dRTA) is a rare but recognized renal complication of Sjögren's syndrome (SS), often resulting from autoimmune-mediated damage to acid-base transporters in the distal nephron. It commonly presents with electrolyte abnormalities including hypokalemia and metabolic acidosis. A 47-year-old woman with chronic kidney disease presented with recurrent hypokalemia, mild proteinuria, and sicca symptoms. Workup revealed non-anion gap metabolic acidosis, a positive urine anion gap, and renal potassium wasting consistent with dRTA. Autoimmune testing showed a high-titer ANA (
