Mikulicz Disease Revealing IgG4-Related Tubulointerstitial Nephritis: A Case Report and Literature Review.

Background and Clinical Significance: IgG4-related disease (IgG4-RD) is a chronic fibroinflammatory, immune-mediated multisystem disorder that can mimic neoplastic, infectious, or autoimmune conditions. Among its head-and-neck manifestations, IgG4-related dacryoadenitis and sialadenitis, historically referred to as Mikulicz disease, should be distinguished from the classical Mikulicz syndrome, which describes secondary lacrimal and salivary gland enlargement due to other systemic disorders. Renal involvement, most commonly in the form of IgG4-related tubulointerstitial nephritis (IgG4-TIN), is

Distal Renal Tubular Acidosis as a Renal Manifestation of Sjögren's Syndrome: A Case Report.

Distal renal tubular acidosis (dRTA) is a rare but recognized renal complication of Sjögren's syndrome (SS), often resulting from autoimmune-mediated damage to acid-base transporters in the distal nephron. It commonly presents with electrolyte abnormalities including hypokalemia and metabolic acidosis. A 47-year-old woman with chronic kidney disease presented with recurrent hypokalemia, mild proteinuria, and sicca symptoms. Workup revealed non-anion gap metabolic acidosis, a positive urine anion gap, and renal potassium wasting consistent with dRTA. Autoimmune testing showed a high-titer ANA (

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Renal disorders in Autoimmune Polyendocrinopathy Candidiasis Ectodermal dystrophy (APECED): a systematic review.

BACKGROUND: Autoimmune polyendocrinopathy candidiasis ectodermal dystrophy (APECED), also known as autoimmune polyendocrine syndrome type I (APS-I) is an inborn error of immunity (IEI) with an immune dysregulation phenotype, mainly characterized by endocrine and non-endocrine manifestations including adrenal insufficiency, chronic mucocutaneous candidiasis, and ectodermal dystrophy. Renal disorders seem to be a significant morbidity in APECED patients, requiring further investigations. METHODS: The literature search was conducted in PubMed, Web of Science, and Scopus databases using relevant

Case report: Renal malformations in wild roe deer (Capreolus capreolus) in Central Poland.

Kidney diseases are observed in various wild animal species, more often noted in captive than in free-ranging animals. There are few reports in roe deer (Capreolus capreolus). Two kidney malformations were encountered while reviewing and collecting the roe deer samples. Kidney malformations were found in samples from two roe deer in Central Poland, one in 5-year-old female near Węgrów (50 km northeast of Warsaw) and one in 7-year-old male near Rawa Mazowiecka (75 km southwest of Warsaw). The female had a smaller (5 cm long and weighed 20 g) shrunken kidney (no prominent histopathological chang

A Case of Sarcoidosis Diagnosed Based on Progressive Renal Dysfunction and Lymphadenopathy.

Several series have suggested that kidney involvement (as defined by either histological changes in the kidney or a decline in kidney function in the absence of a biopsy) occurs in approximately 10 to 50 percent of patients with sarcoidosis. A large study examining more than 1200 hospitalized patients with sarcoidosis reported that kidney manifestations were present in approximately 6% of cases. The case is that of an 89-year-old woman. She experienced paroxysmal nocturnal dyspnea starting four days before her visit. Three days before coming to our hospital, she consulted a nearby clinic, lead

A Rare Coexistence of Antineutrophil Cytoplasmic Antibody (ANCA)-Associated Vasculitis and IgG4-Related Disease: A Case Report and Literature Review.

Autoimmune kidney diseases can present with overlapping clinical and pathological features, posing diagnostic and therapeutic challenges. Elderly patients with comorbidities are particularly vulnerable to atypical or coexisting disease presentations. We present a rare case of an 83-year-old male patient with acute kidney injury (AKI) and multiple comorbidities, diagnosed with both antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) and immunoglobulin G4-related disease (IgG4-RD). The patient exhibited characteristic features of AAV, including positive P-ANCA and elevated mye

The Tubulointerstitium in Health and Disease: More Than What Meets the Eye.

The tubulointerstitium is a critical component of the nephron. It is a highly vascularized compartment that plays an important part in kidney health and function. The tubulointerstitium orchestrates the clinical effects of volume control, electrolyte and acid-base balance, medication and toxin secretion, hormone production, immunological and inflammatory processes, and tubular repair processes. These important interconnected processes within the tubulointerstitial compartment that are independent of glomerular filtration require an intact tubulointerstitium to achieve optimal kidney function.

Clinical Tools for the Diagnosis of Acute Tubulointerstitial Disease.

Acute tubulointerstitial nephritis (ATIN) is a common cause of acute kidney injury (AKI), occurring in up to a quarter of kidney biopsies performed for AKI. ATIN can lead to chronic kidney disease if not promptly diagnosed and managed. The most common cause is drugs, with antibiotics being the most commonly reported cause. Widespread use of proton pump inhibitors and increasing use of immuno-oncotherapy, such as immune checkpoint inhibitors, are contributing to increased incidence of ATIN. However, there are diagnostic challenges. The time of onset of AKI in ATIN is variable, and diagnosis is

Rationale and Design of the International Prospective Study of CKD of Uncertain Etiology in Agricultural Communities.

INTRODUCTION: There has been an alarming increase in the incidence of a chronic kidney disease (CKD) of unknown etiology primarily affecting young individuals engaged in agricultural activities in Mesoamerica and South Asia. Despite extensive research over the past 2 decades, causes remain unclear. The disease is characterized by progressive loss of kidney function with the absence of heavy proteinuria and hematuria. The International Prospective Study of CKD of Unknown Etiology in Agricultural Communities (CURE study) aims to do the following: (i) identify factors associated with kidney funct

Emphysematous pyelonephritis in a diabetic child: a case report and review of the literature.

INTRODUCTION: Emphysematous pyelonephritis is a life-threatening condition characterized by gas accumulation within the kidney and the surrounding tissues, often resulting in tissue necrosis. Although frequently reported in adults, emphysematous pyelonephritis is an extremely rare condition in children, with only a few documented cases in the literature. We present the case of a child with insulin-dependent diabetes mellitus who had initially been admitted with a diagnosis of diabetic ketoacidosis that was later revealed to be emphysematous pyelonephritis. Effects of vaping on kidney function: a systematic review on acute kidney injury and chronic kidney disease.

The increase use of vaping, especially in the younger population, has led to increased scrutiny of its health effects, particularly on renal function. This article reviews the current literature on the association between vaping, nicotine exposure, and renal impairment, focusing on the development of acute kidney injury (AKI) and chronic kidney disease (CKD). Nicotine and other chemicals in e-liquids may induce microangiopathy, leading to vasoconstriction and reduced renal perfusion, thus contributing to AKI. Chronic exposure to nicotine also promotes an inflammatory response, increasing the r

Genetic Factors Related to the Development or Progression of Mesoamerican Endemic Nephropathy.

Over the past two decades, Mesoamerican endemic nephropathy (MeN) has become a major public health problem in certain regions of Mexico and Central American countries. The etiology of this disease is multifactorial, and important environmental factors have been described, such as chronic heat stress, recurrent episodes of dehydration, infections, and exposure to toxins of chemical and biological origin. Genetic and epigenetic factors have been proposed to play significant roles in MeN. Recent studies have analyzed the role of these factors in MeN. In some cases, these factors appear to be asso

Xanthogranulomatous Pyelonephritis in a Socioeconomically Disadvantaged Immigrant Child: A Case Report.

Xanthogranulomatous pyelonephritis (XGP) is a rare, chronic inflammatory kidney disease characterized by progressive destruction of the renal parenchyma, typically associated with obstructive uropathy and nephrolithiasis. It is especially uncommon in pediatric patients and may be mistaken for malignancy or other renal infections, leading to delays in diagnosis. We present the case of a 13-year-old socioeconomically disadvantaged and immigrant (SDI) girl who developed advanced XGP due to unmonitored renal stones and limited access to healthcare. She presented with prolonged fever, flank pain, w

Tubulointerstitial Nephritis - A Histopathological Study of Renal Biopsies.

Tubulointerstitial nephritis (TIN) is a frequent cause of acute kidney injury. TIN has a reported incidence ranging from 1% to 10% of all renal biopsies. Among patients with chronic renal failure, the incidence ranges from about 22% to 33.5%. We studied the histological features of renal biopsies with TIN, evaluated its etiology and correlated TIN with clinical features and investigations. Our study extended over a period of seven years, with 50 cases. The slides were stained with hematoxylin and eosin and special stains like periodic acid-Schiff, Jones methenamine silver and Masson's trichrom

Targeting ferroptosis: novel therapeutic approaches and intervention strategies for kidney diseases.

Chronic kidney disease (CKD), characterized by structural, functional, and metabolic derangements, remains a leading cause of end-stage renal disease (ESRD) with profound global health burdens. The kidney's high oxygen demand for blood filtration renders it exquisitely sensitive to redox imbalance-an aberration common to both CKD and acute kidney injury (AKI) that, when coupled with iron dysregulation, unleashes ferroptosis: a non-apoptotic, iron-dependent form of regulated cell death driven by iron accumulation, lipid peroxidation, and antioxidant defense impairment (e.g., GPX4/SLC7A11 dysfun
