BACKGROUND: Acute oxalate nephropathy is a progressive kidney condition characterized by the accumulation of oxalate crystals, causing interstitial nephritis and tubular damage. Excessive intake of vitamin C, obesity, chronic inflammatory bowel disease, and gastric bypass surgery are risk factors that can cause oxalate nephropathy. However, the risks associated with this condition and its management deserve further investigation.
CASE PRESENTATION: A 76-year-old woman had undergone Roux-en-Y gastric bypass surgery seven years prior following a diagnosis of gastric cancer. Four years previousl

Granulomatous interstitial nephritis (GIN) is rare in childhood and is associated with poor outcomes. We report a 6-year-old girl who developed nephrotic-range proteinuria following chronic non-steroidal anti-inflammatory drug (NSAID) use for juvenile idiopathic arthritis (JIA). Other possible etiologies were ruled out, and the diagnosis of GIN was confirmed by renal biopsy. NSAIDs were henceforth withheld and prednisolone was started, which resulted in amelioration of proteinuria. Strong suspicion, quick diagnosis and early initiation of therapy improve the outcome in GIN and prevents or dela

BACKGROUND: In 1987, a report from this unit described the changing indications for open adrenalectomy over a 15-year period. The indications for adrenalectomy had switched from it being the principal therapeutic procedure used in advanced breast cancer in the early 1970s, to being predominately performed for Cushing's disease or incidental, asymptomatic, adrenal masses by the early 1980s. The aim of the present study was to evaluate the changes in the presentation and management of adrenal disease in the last 15 years and to compare these findings with our previously published results.

BACKGROUND: Autosomal dominant tubulointerstitial kidney disease caused by uromodulin mutation (ADTKD-UMOD) is a hereditary condition characterized by progressive renal dysfunction, often requiring renal replacement therapy by middle age. A notable feature is a strong family history of chronic kidney disease (CKD) and hyperuricemia; however, the absence of urinary abnormalities often delays diagnosis. To facilitate early CKD management, a diagnostic method that is simpler than genetic analysis yet strongly indicative of ADTKD-UMOD is needed.
METHODS: Serum and urinary UMOD levels were measure

BACKGROUND Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have become important in the treatment of diabetic kidney disease (DKD). Despite their reno-protective effect, kidney injury from GLP-1 RAs has been rarely reported. The reported kidney injuries varied from mild symptoms to dialysis at presentation and occurred 2 days to 2 years after drug initiation. We report a case of a patient with DKD who developed an episode of interstitial nephritis after dulaglutide administration. CASE REPORT A 63-year-old woman with stage 3b diabetic kidney disease and depressive disorder was prescribed

A 10-year-old boy with a history of febrile urinary tract infections presented with a hypertensive crisis and thrombotic microangiopathy (TMA). Functional and genetic complement testing was normal, and TMA resolved with blood pressure control, suggesting a primary hypertensive aetiology. Renal biopsy confirmed chronic tubulointerstitial nephritis (CTIN), likely secondary to recurrent pyelonephritis and renal scarring after ruling out other potential causes. High-grade vesicoureteral reflux was confirmed by voiding cystourethrography. After 4 years, he has progressed to stage III chronic kidney

ANCA-associated vasculitis (AAV) is a group of rare small vessels vasculitis that preferentially affect the kidneys, lungs and upper airways. Although the detailed pathophysiology remains unclear, genetic background has been shown to play a role in sporadic forms of AAV. The discovery of these susceptibility genes (and associated biological pathways) involved in AAV have shaped the current understanding of AAV pathophysiology. In addition to common genetic polymorphisms, specific rare inborn errors of immunity (IEI) have been described with a high frequency of ANCA (antineutrophil cytoplasmic

Radiolabeled fibroblast activation protein inhibitors (FAPIs) have emerged as promising tracers for molecular imaging, particularly in the field of oncology. However, their potential extends beyond cancer applications to a wide range of non-oncological conditions characterized by fibrosis, inflammation, and tissue remodeling. This review addresses the expanding role of FAPI-based imaging in non-malignant diseases, related to its ability to visualize and quantify fibroblast activation, a key process in various pathological conditions. We discuss the mechanism of action of FAPIs, their radiolabe

INTRODUCTION: Infectious and inflammatory diseases of the genitourinary tract account for approximately 42% of all outpatient urology cases, and disorders such as cystitis, pyelonephritis, and prostatitis are often characterized by a chronic, recurrent course. One of the key contributors to recurrent urinary tract infections is inappropriate therapy that does not take into account the local microbial spectrum and pathogen resistance profiles.
AIM: To determine the spectrum of uropathogens and their antimicrobial resistance patterns in patients with cystitis, pyelonephritis, and prostatitis.

BACKGROUND: Feline morbillivirus (FeMV) has been associated with renal pathology in cats; however, the specific pathological alterations caused by FeMV infection remain controversial. This study aimed to investigate histopathological changes, viral localization, and apoptotic activity in the kidneys of FeMV-infected cats.
METHODS: Kidney tissues from 150 deceased cats with suspected or confirmed chronic kidney disease (CKD) were screened for FeMV using conventional reverse-transcription PCR (cRT-PCR). Positive cases were genotyped and quantified for viral load using reverse-transcription digi

Two cases of spontaneous cholesterol embolism, which followed different clinical courses, acute and chronic renal failure, are presented and histopathological lesions are compared. Both cases were diagnosed as cholesterol embolism post-mortem. Case 1 (a 66-year-old man) had acute onset of illness with fever, leucocytosis and renal failure, diagnosed as vasculitis, and died of rupture of an abdominal aortic aneurysm. Case 2 (an 84-year-old man) had eosinophilia of unknown aetiology for 7 years with intermittent worsening of renal function and died of sepsis. Case 1 had diffuse cholesterol cryst

Central noradrenergic (NA) neurons are key constituents of the respiratory homeostatic network. NA dysfunction is implicated in several developmental respiratory disorders including Congenital Central Hyperventilation Syndrome (CCHS), Sudden Infant Death Syndrome (SIDS), and Rett Syndrome. The current unchallenged paradigm in the field, supported by multiple studies, is that glutamate co-transmission in subsets of central NA neurons plays a role in breathing control. If true, NA-glutamate co-transmission may also be mechanistically important in respiratory disorders. However, the requirement o

Drug-induced hypomagnesemia is an adverse effect with the potential for serious and fatal outcomes. Although rare, chronic use of proton pump inhibitors (PPIs) can cause hypomagnesemia due to impaired intestinal absorption, mainly attributed to reduced transcellular transport of magnesium via transient receptor potential melastatin 6 (TRPM6) and 7 (TRPM7) channels. However, a reduction of magnesium paracellular absorption due to the downregulation of intestinal claudins has also been reported. PPI-induced hypomagnesemia can trigger other concomitant electrolyte derangements, including hypokale

Little is known about the effect tubulointerstitial nephropathies have in modulating maternal-fetal outcomes in pregnancy. Therefore, we analyzed the main outcomes of pregnancy in these women to gain a better understanding of the role of a reduction in maternal kidney mass. From the Torino Cagliari Observational Study (TOCOS) cohort, we selected 529 patients with a diagnosis of tubulointerstitial disease and focused on 421 patients with chronic kidney disease (CKD) stage 1, without hypertension but with proteinuria less than 0.5 g/day at referral. From a cohort of 2969 singleton deliveries fro

INTRODUCTION: The use of proton-pump inhibitors (PPI) is linked with infrequent but serious adverse events, including acute kidney injury, chronic kidney disease (CKD) and progression of CKD. Data on renal safety in routine use of PPI are more relevant to clinical practice. We studied whether such use of PPI is associated with renal dysfunction.
METHODS: Patients taking PPI for at least six weeks had serum creatinine tested pre (n = 200) and post (n = 180) recruitment. These patients were then advised to follow-up: those taking PPI for at least 90 days in the next six months (n = 77) and at l

RATIONALE & OBJECTIVE: Glomerular disorders have a highly variable clinical course, and biomarkers that reflect the molecular mechanisms underlying their progression are needed. Based on our previous work identifying plasminogen as a direct cause of podocyte injury, we designed this study to test the association between urine plasmin(ogen) (ie, plasmin and its precursor plasminogen) and end-stage kidney disease (ESKD).
STUDY DESIGN: Multicenter cohort study.
SETTING & PARTICIPANTS: 1,010 patients enrolled in the CureGN Cohort with biopsy-proven glomerular disease (focal segmental glomerulosc
