Case Report: staged electron beam radiotherapy for inoperable primary ectopic abdominal extramammary Paget disease.

Extramammary Paget disease (EMPD) is a rare cutaneous adenocarcinoma that usually arises in apocrine gland-bearing areas, whereas primary ectopic EMPD of the abdominal skin is exceptionally uncommon. We report a 79-year-old woman with a five-year history of progressively enlarging eczema-like lower abdominal-pelvic skin lesions, including erosion, exudation, scaling, crusting, and pruritus, initially treated as chronic dermatitis without improvement. Histopathology showed large atypical Paget cells with focal dermal microinvasion; immunohistochemistry was positive for cytokeratin 7 (CK7) and G

Serum fibroblast growth factor-23 and magnesium concentrations in cats with acute kidney injury: association with disease pathophysiology.

Cats with acute kidney injury (AKI) have a high mortality rate and are at risk of progression to chronic kidney disease (CKD). Fibroblast growth factor-23 (FGF-23) and magnesium, which are involved in mineral homeostasis and AKI outcomes, may contribute to this progression. This study aimed to evaluate serum FGF-23 and magnesium concentrations at diagnosis in cats with AKI and to investigate their associations with clinical outcomes. This retrospective study included client-owned cats with AKI. Serum FGF-23 and magnesium concentrations were measured at diagnosis, and their associations with AK

Fetal growth restriction leads to prolonged nephrogenesis and gene dysregulation in the adult kidney.

Low birthweight increases chronic kidney disease (CKD) risk. Using a gestational low protein (LP) mouse model, we hypothesized that protein restriction would alter nephrogenesis and adult gene expression. Pregnant CD-1 dams received normal protein (NP) or LP diets. Pups were studied on postnatal days 2-7 to assess the nephron progenitor cell population. Adult mice underwent transdermal GFR measurement and histologic evaluation, bulk RNA sequencing, and cationic-ferritin-enhanced MRI. Nephrogenesis was protracted in LP mice compared with NP mice. Adult LP mice had fewer glomeruli. LP males had

Gut-Intervertebral Disc Axis: Gut Microbiome-Driven Immune-Metabolic Imbalance and Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IDD) serves as a critical structural basis for chronic low back pain, severely impairing patients' quality of life and imposing a significant socioeconomic burden. Current interventions remain largely limited to symptomatic treatments including pharmacological analgesia and surgical intervention. The recently proposed "gut-disc axis" suggests that the gut microbiome can reshape the endplate-disc microenvironment by influencing intestinal barrier integrity, microbial metabolite profiles, and host immune-metabolic status. Existing evidence indicates that dysbios

High-Fat Diet Remodels Plasma Membrane Rigidity to Potentiate Colonocyte Stemness in Mice.

BACKGROUND & AIMS: Obesity and consumption of a high-fat diet are established risk factors for colorectal cancer. However, the cellular mechanisms linking dietary lipid exposure to intestinal stem cell dysregulation remain incompletely defined. We therefore investigated whether a high-fat diet perturbs plasma membrane homeostasis to potentiate colonocyte stemness in mice. METHODS: Mice were placed on a chronic 12-week high-fat diet or control (low-fat diet) feeding regimen, or a short-term 1-week high-fat diet exposure. In addition, genetically obese leptin receptor-deficient (db/db) vs (db/+

Granzyme A participates in intestinal inflammation induced by PFOA through affecting PAR1/β-catenin/TBX3/IL24 pathway.

Perfluorooctanoic acid (PFOA) has been associated with damage to multiple organs, and its effects on the gastrointestinal system have attracted increasing attention. However, the mechanism underlying PFOA-induced intestinal toxicity remains unclear. In this study, we found that PFOA disrupted the intestinal barrier and induced intestinal inflammation in mice, accompanied by increased GzmA expression. Mechanistically, GzmA upregulation promoted intestinal inflammation through the GzmA-PAR1 signaling axis, which enhanced the interaction between caveolin-1 and β-catenin and facilitated the nuclea

From ultra-processed to biointelligent foods: The role of food biotechnology in the transition toward functional and health-oriented nutrition.

BACKGROUND: The global rise in the consumption of ultra-processed foods (UPFs) has been consistently linked to gut dysbiosis and low-grade systemic inflammation, and has been associated with the increasing prevalence of chronic non-communicable diseases and global mortality. These findings highlight critical limitations of current food systems and underscore the need for innovative nutritional paradigms aimed at supporting metabolic and intestinal homeostasis. SCOPE AND APPROACH: This review critically examines advances in food biotechnology as a promising framework to counteract UPF-induced

Rethinking Percutaneous Coronary Intervention: The Era of Ultra-Low-Contrast Intervention.

Iodinated contrast remains a key component of percutaneous coronary intervention, yet it poses a significant risk for contrast-associated acute kidney injury (CA-AKI), particularly in patients with chronic kidney disease (CKD). This review summarizes the evidence, procedural workflow with stepwise approach including clinical implications of ultra-low-contrast PCI (ULC-PCI). We reviewed recent literature describing strategies for contrast reduction, imaging and physiology-guided PCI, and subsequent clinical outcomes. ULC-PCI incorporates techniques such as intravascular imaging (IVUS/OCT), frac

The kidney and IGF-1 axis as a central regulator of linear growth and adult height in pediatric health and disease.

The kidney plays an important role in normal linear growth throughout childhood by regulating systemic insulin-like growth factor-1 (IGF-1) bioavailability through the filtration and processing of IGF-binding proteins and by serving as a target organ for growth hormone (GH). In pediatric chronic kidney disease (CKD), impaired renal function is associated with reduced availability of bioactive IGF-1 despite normal or modestly elevated total IGF-1 concentrations, reflecting altered binding protein dynamics and functional GH-IGF resistance. These changes are frequently accompanied by impaired gro

Repetitive circadian disruption contributes to pain hypersensitivity and alters the transcriptional rhythms in spinal cord in male mice with neuropathic pain.

Circadian disruption increases the risk of neurological disorders, yet its impact on neuropathic pain (NP), a type of pain caused by nerve lesion, remains poorly understood. This study examined how three weeks of repetitive circadian disruption affects mechanical sensory thresholds and the spinal transcriptome in male C57BL/6JRj mice subjected to spared nerve injury (SNI) by using the chronic jet lag (JL) model. We also compared behavioural and transcriptomic outcomes between SNI and sham-operated control mice under either JL or a regular light/dark (LD) schedule. Mechanical hypersensitivity w

The algorithmic epistemic inversion in gastroenterology: how social media and psychological vulnerability fuel secondary risks in IBD and IBS management.

The unchecked proliferation of medical misinformation on short-video platforms has crystallized into a critical digital public health crisis. For patients managing chronic gastrointestinal diseases such as IBD and IBS, recommendation algorithms routinely override evidence-based medicine (EBM), engendering a phenomenon we term "algorithmic epistemic inversion." This inversion amplifies unproven "miracle cures" and fear-based narratives, driving population-level secondary risks, ranging from indiscriminate probiotic abuse and extreme elimination diets, malnutrition, and orthorexia nervosa. Altho

Uncommon indications in chronic pain for intrathecal drug delivery systems: a narrative review of the literature.

Intrathecal drug delivery systems (IDDS) have been used successfully for a number of refractory chronic pain conditions. The most common pain indications for the use of IDDS include cancer-related pain and postlaminectomy syndrome. IDDS can also be used to treat a variety of other less common refractory pain conditions, including central neuropathic pain, peripheral neuropathic pain and craniofacial pain. The aim of this article was to review the existing literature on the efficacy and clinical utility of IDDS for the management of uncommon pain indications, including spinal cord injury-relate

Complications leading to device explant in patients with chronic non-cancer pain receiving intrathecal drug delivery.

INTRODUCTION: Intrathecal drug delivery (IDD) is often used in the management of refractory chronic non-cancer pain, particularly persistent chronic low back pain in the setting of previous lumbar spine surgery (persistent spinal pain syndrome type 2 (PSPS2)). Complications of IDD leading to device explant in patients with chronic non-cancer pain have not been well characterized. This study evaluated complications leading to device explants in a large cohort of patients with chronic non-cancer pain treated with IDD. METHODS: A retrospective study was undertaken on patients with chronic non-ca

Abdominal aortic calcification index and prognosis in patients with chronic kidney disease: a meta-analysis.

BACKGROUND: Abdominal aortic calcification index (ACI), a quantitative computed tomography-based measure of abdominal aortic calcification burden, has been increasingly investigated as a prognostic marker in patients with chronic kidney disease (CKD). However, the association between ACI and clinical outcomes in CKD remains uncertain. This meta-analysis evaluated the association of baseline ACI with cardiovascular event-free survival (CVEFS) and overall survival (OS) in patients with CKD. METHODS: PubMed, Embase, Web of Science, Wanfang Data, and CNKI were searched for cohort studies evaluati

Natural Polysaccharide-Based Biomaterials for Skin Wound Healing: Immunomodulatory Mechanisms and Macrophage M2 Polarization.

Efficient cutaneous wound healing relies on the phenotypic transition of macrophages toward an anti-inflammatory, pro-reparative M2-like state. Non-healing chronic wounds are pathologically characterized by the breakdown of this polarization balance. In this review, we synthesize recent research on biomaterials fabricated from naturally occurring polysaccharides with intrinsic immunomodulatory activity, mainly represented by hydrogels that modulate macrophage phenotypic transitions. We first dissect the immune microenvironment of wound healing and elaborate on the core regulatory networks gove

Chronic sleep deprivation induces ovarian ferroptosis and premature ovarian insufficiency linked to IL-17 signaling.

OBJECTIVE: Sleep deprivation (SD) disrupts female reproductive homeostasis, yet the mechanisms linking chronic sleep loss to premature ovarian insufficiency (POI) remain poorly defined. METHODS: We analyzed sleep patterns and menopause timing in 147,814 UK Biobank women using multivariable logistic regression. We established a chronic SD mouse model and performed integrated multi-omics profiling, including serum proteomics and ovarian RNA sequencing, with pharmacological rescue using the ferroptosis inducer Erastin and inhibitor Ferrostatin-1. RESULTS: Healthier sleep patterns were associate

Systematic Review of Exercise Interventions and HRQOL in Predialysis CKD.

INTRODUCTION: Health-related quality of life (HRQOL) is frequently impaired among individuals with chronic kidney disease (CKD), even before dialysis initiation. Exercise has been proposed as a strategy to improve HRQOL, but the extent of this association remains unclear in patients with predialysis CKD. This systematic review aimed to characterize the literature examining the relationship between exercise and HRQOL in patients with predialysis CKD. METHODS: A systematic review was conducted, guided by Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines and its exten
