Robot-Assisted Partial Nephrectomy in a High-Complexity Renal Tumor in a Horseshoe Kidney.

INTRODUCTION: Horseshoe kidney is an uncommon congenital fusion anomaly that can make renal tumor surgery especially challenging because of altered rotation, limited mobility, variable vascular supply, and an unpredictable collecting system (1-7). This video presents a robot-assisted partial nephrectomy for a high-complexity renal tumor in this setting. CASE PRESENTATION: A 33-year-old man, with ECOG 0 and no relevant comorbidities, was diagnosed with a 7.5-cm solid renal mass in the central posterior portion of the left moiety of a horseshoe kidney. The lesion had a RENAL score of 10p. Contr

Robot-assisted buccal graft uretero-pyeloplasty for pediatric recurrent UPJO: a multi-center case-series.

Redo pyeloplasty is technically more complex due to more significant scar tissue and risk for poor ureteral blood supply affecting outcomes. Use of buccal graft uretero-pyeloplasty was originally reported in the pediatric population back in 2017 as an alternative for complex reoperations. We hypothesize that in highly selected group of pediatric patients undergoing redo pyeloplasty, a buccal graft uretero-pyeloplasty would demonstrate efficacy. A total of 6 institutions identified patients undergoing a robot-assisted buccal uretero-pyeloplasty from 2014 to 2025. Demographic, pre-operative, int

Development of an Improved Cadaveric Training Model for Endoscopic Combined Intrarenal Surgery Using Fresh-Frozen Cadavers.

OBJECTIVES: Endoscopic combined intrarenal surgery (ECIRS) is technically demanding and has a prolonged learning curve. Reports describing ECIRS training using fresh-frozen cadavers are limited, partly because intrarenal and pyelovenous backflow may prevent adequate hydronephrosis. This study aimed to establish an optimized ECIRS training model using fresh-frozen cadavers. METHODS: ECIRS cadaver surgical training (ECIRS-CST) was conducted between April 2021 and October 2022 and evaluated across two periods. In the first period, standard ECIRS-CST was performed. In the second period, laparosco

Renal Candidal Bezoar in an SGLT2 Inhibitor User with an Indwelling Ureteral Stent: A Case Report and Review of Literatures.

Renal candidal bezoar is a rare cause of obstructive pyonephrosis and usually occurs in patients with diabetes mellitus, immunosuppression, urinary obstruction, or indwelling urinary devices. We report an 88-year-old man with type 2 diabetes treated with dapagliflozin, stage 3 chronic kidney disease, and a history of ureteral stricture managed with ureteral stenting who developed recurrent candidal pyonephrosis and extensive fungal bezoar involving the ureter and renal pelvis. Initial treatment at a referring hospital included percutaneous nephrostomy drainage, fluconazole therapy, ureteroscop

The Hidden Hitchhiker: Retrocaval Ureter Masquerading as Isolated Pelviureteric Junction Obstruction.

The coexistence of true pelviureteric junction obstruction (PUJO) and a retrocaval ureter in a single, non-duplicated collecting system is exceptionally rare, and this case represents a "hidden retrocaval ureter," in which an upstream PUJO masked the classical radiological features, transforming the diagnosis into an unexpected intraoperative discovery. Our case highlights such a peculiar case, where a 70-year-old man with longstanding intermittent right loin pain was found to have right hydronephrosis and a serum creatinine of 1.7 mg/dL. Computed tomography showed a markedly dilated extra-ren

Primary Diffuse Large B-cell Lymphoma of the Urinary Bladder: A Case Report and Narrative Review of Treatment Modalities and Clinical Outcomes.

Primary diffuse large B-cell lymphoma (DLBCL) of the urinary bladder is a rare extranodal lymphoma, representing a very small proportion of both bladder malignancies and extranodal lymphomas. We report a 73-year-old, chronically institutionalized woman with Eastern Cooperative Oncology Group (ECOG) 3-4 who presented with abdominal/flank pain, fever, hydronephrosis, and polymicrobial urosepsis. A bladder mass was detected incidentally on CT and subsequently confirmed by cystoscopy and transurethral resection of bladder tumor (TURBT). Histopathology showed a CD20-positive, CD5/CD10-negative larg

Biopsy-Proven Prostatic Metastasis From Diffuse-Type Gastric Adenocarcinoma Presenting With Bilateral Malignant Ureteric Obstruction: A Case Report.

Metastatic involvement of the prostate and bladder trigone from gastric adenocarcinoma is exceedingly rare. We report the case of a 38-year-old man with previously treated diffuse-type gastric adenocarcinoma who developed bilateral hydroureteronephrosis during surveillance. Cystoscopy demonstrated an Inflamed prostatic urethra with early papillary changes and abnormal trigonal mucosa with bilaterally narrowed ureteric orifices. Retrograde studies revealed bilateral mid-ureteric strictures without intraluminal lesions. Biopsies from the prostatic urethra and trigone confirmed metastatic gastric

Robot-assisted advancing spiral flap pyeloplasty for complex ureteropelvic junction obstruction and ureteral atresia: a preliminary experience in 27 patients.

BACKGROUND: Long-segment ureteropelvic junction obstruction (UPJO) (typically 12≥3 cm) and ureteral atresia present major reconstructive challenges due to fixed-length limitations of conventional flap techniques. This study evaluates the feasibility and safety of a robot-assisted advancing spiral flap pyeloplasty (RASFP) with dynamically adjustable flap design as a proof-of-concept for complex cases. METHODS: A single-center retrospective study (August 2022-July 2025) included 27 patients with long-segment UPJO (median stricture length 3 cm) or ureteral atresia. All patients underwent RASFP u

Performance of the Quadruple-D Score Versus Other Nephrolithometric Tools in Obese Patients Undergoing Percutaneous Nephrolithotomy.

OBJECTIVE: To evaluate whether the CT-based Quadruple-D score predicts stone-free status after percutaneous nephrolithotomy (PCNL) in obese patients and to compare it with established nephrolithometric scores, including SCR, CROES, Guy's Stone Score, and S.T.O.N.E. METHODS: We retrospectively analyzed obese adults (BMI > 30 kg/m²) who underwent PCNL at a tertiary center between January 2013 and June 2025 and had preoperative non-contrast CT and 1-month postoperative imaging. The Quadruple-D score (0-4) assigned one point each for stone volume <500 mm³, attenuation <1000 HU, skin-to-stone dist

Divergent outcomes of delayed ureteropelvic junction stenosis after renal cyst sclerotherapy: two case reports.

Rare long-term complications, such as ureteropelvic junction (UPJ) stenosis, can occur after renal cyst puncture and sclerotherapy and lead to severe hydronephrosis with renal impairment. We illustrate these risks with two examples: in one case, a delayed diagnosis led to severe perirenal fibrosis and renal failure at operation; in the other case, early laparoscopic pyeloplasty preserved renal function. The marked differences illustrate that this complication often has a mysterious onset and long latency, highlighting the importance of early detection and treatment. Clinicians should maintain

Diffusion models vs. DCGANs for class-imbalanced lung cancer CT classification: A comparative study.

Effective lung cancer detection from CT scans remains critically challenged by class imbalance where benign and normal cases are underrepresented, leading to biased machine learning models with reduced sensitivity for minority classes and potentially missed diagnoses in cancer screening applications. We present a comprehensive comparative analysis of Diffusion Models and Deep Convolutional Generative Adversarial Networks (DCGANs), both incorporating modern architectural enhancements including spectral normalization, self-attention mechanisms, and conditional generation, for addressing class im

Rapid and accurate preoperative diagnosis of urinary tract infections in urolithiasis patients: clinical validation of the HB&L system.

BACKGROUND: Preoperative urinary tract infections in urolithiasis patients represent a significant clinical challenge, with infection rates of 7–28% increasing hospitalization duration and complicating surgical management. Rapid diagnostic platforms are needed to replace traditional culture-based methods that delay targeted therapy. METHODS: This study evaluated 158 urolithiasis patients’ urine specimens using both the HB&L rapid detection system and traditional culture methods. The HB&L system utilized 500 µL of urine incubated for 4.5 h, while conventional culture used 10 µL inoculated onto

A Highly Uncommon Presentation: A Consecutive Three-Case Series of Malignant Ureteral Obstruction in Long-Term Survivors of Non-small Cell Lung Cancer.

Malignant ureteral obstruction (MUO) is an exceptionally rare complication in non-small cell lung cancer (NSCLC), particularly among long-term survivors. We report three cases of MUO arising in patients with stage IV or recurrent NSCLC, all of whom had survived 3.6-4.0 years after their initial diagnosis. Two of the three cases harbored epidermal growth factor receptor (EGFR) mutations, and MUO in EGFR-mutated NSCLC has not been previously described, suggesting a potential association with prolonged survival under targeted therapy. All patients developed hydronephrosis and renal dysfunction re
