Robotic appendiceal onlay flap ureteroplasty combined with Boari flap-psoas hitch ureteroneocystostomy for repair of the 20 cm ureteral avulsion: initial experience and case report.

BACKGROUND: Ureteral avulsion is a rare but serious complication of ureteroscopic lithotomy. As a common treatment for long segment ureteral avulsion, ileal ureteral replacement has many complications, such as anastomotic site stricture and retrograde infection, and prolonged exposure of the intestinal mucosa to urine may result in postoperative metabolic acidosis. Autologous tissue is becoming increasingly popular in ureteroplasty, including oral mucosa, appendix and bladder. Recent advancements in ureteral reconstruction techniques have highlighted the utility of the appendix and bladder, bu

Safety and Efficacy of Renal Sympathetic Denervation Using Percutaneous Radiofrequency Ablation, a New Hypertension Treatment.

RATIONALE AND OBJECTIVES: Conventional catheter-based renal sympathetic denervation (RDN) as a treatment for patients with treatment-resistant hypertension cannot ablate renal sympathetic nerves ≥4 mm from the renal artery. We developed a percutaneous radiofrequency ablation (pRFA) method to ablate remote renal sympathetic nerves. This study aimed to assess the feasibility, efficacy, and safety of RDN with pRFA. MATERIALS AND METHODS: Fourteen Beagle dogs were divided into RDN and control groups. The RDN group underwent computed tomography (CT)-guided pRFA to ablate the sympathetic nerves aro

[Pyonephrosis Caused by Ureteral Stricture Due to Syphilitic Granuloma: A Case Report].

An 87-year-old woman was admitted to our hospital with anorexia, lightheadedness, and fever. She was referred to our department with a diagnosis of right pyonephrosis due to stenosis of the right ureter. The severe stenosis made it impossible to place a ureteral stent. Nephrostomy was performed and the infection improved. Antegrade pyelography and contrast CT did not reveal the cause of the stenosis. Considering the reduced quality of life due to the nephrostomy and the possibility of ureteral stricture due to malignancy, the patient underwent right nephrectomy at a later date. Pathological ex

HMGB1-mediated macrophage polarization to M2 phenotype promotes ureteral stricture: therapeutic potential of HMGB1 inhibitors.

BACKGROUND: Ureteral stricture is a common condition characterized by fibrotic narrowing of the ureter, often resulting in obstruction and impaired renal function. The condition is typically associated with chronic inflammation, epithelial injury, and abnormal wound healing responses, leading to excessive deposition of extracellular matrix proteins. High Mobility Group Box 1 (HMGB1), a damage-associated molecular pattern (DAMP) protein, plays a key role in promoting inflammation and fibrosis in various tissues. However, its specific role in the pathogenesis of ureteral stricture remains unclea

Laparoscopic onlay-flap ureteroplasty using cecal appendix.

INTRODUCTION: The management of ureteral strictures longer than 1-2 cm must be treated by major surgery (1, 2). The strictures located at the distal part of the ureter can be managed by a ureteral reimplantation using a psoas hitch or a Boari flap depending on its proximity to the bladder (3). Those located at the proximal ureter can be treated by a pyeloplasty (4). The ureteric strictures in the mid-ureter are the ones that pose a greater challenge for the urologist because a ureteral substitution is needed, either using a segment of the intestine or a buccal mucosa graft (5, 6). Our main obj

Ureteral stricture formation after endoscopic removal of obstructing stones: could it be predicted with well-assessed radiological parameters?

The possible role of well-assessed radiological parameters in the prediction of ureteral stricture formation in cases with impacted obstructive ureteral calculi has been evaluated. 46 adult patients with or without ureteral stricture formation after ureteroscopic stone management were included. In addition to stone size and some certain radiological parameters including ureteral wall thickness (UWT) of the involved ureter at the impacted stone site was also measured and noted on computed tomography (CT) images. Parameters were evaluated in two subgroups of cases, namely: Group 1: patients in w

Newly modified 'pseudo flap' without compromising vascularity to enhance repair of long distal ureteral loss: A retrospective analysis of a prospective database.

OBJECTIVE: To present an alternative technique called pseudo-flap for reconstructing long ureteral defects as an alternative to Boari flap. Despite being used for more than 70 years by urologists for tension-free reconstruction of distal and mid-ureteral defects, the Boari flap exhibits high complication rates, with an average of 27% (range 5.5%-30.4%). These complications arise from compromised blood supply, attributed to incisions made on all three sides of the flap and dependence on the flap base as the sole source of blood supply. METHODS: We retrospectively reviewed patients who underwen

Radiotherapy induced ureteric stenosis in locally advanced cervical cancer: A review of current evidence.

BACKGROUND: Concurrent chemo-radiation followed by high dose rate brachytherapy is the standard of care for locally advanced cervical cancer. The proximity of the ureters to the tumor volume risks ureteric stenosis. Here we outline the current understanding of radiotherapy induced ureteric stenosis in patients treated for cervical cancer, focusing on the incidence, risk factors, clinical consequences, and management. METHODS: Searches on EMBASE, PubMed, Science Direct, and Google Scholar were performed for publications reporting on radiotherapy, cervix cancer and ureteric stenosis. Multi and
