A study was made of 50 fused kidneys; 41 postmortem specimens and 9 living patients' records were reviewed. Three patterns of renal fusion were identified--midline fused horseshoe kidney [15], laterally fused horseshoe kidney [21], and crossed renal ectopia [14]. These morphologic renal patterns were correlated with coexisting abnormalities of the urinary tract and other organ systems. The case of a patient is reported in whom pre-operational infusion urography indicated severe hydronephrotic lesions of both kidneys. It was attempted to preserve the kidneys by performing pelviplasty on the one side and ureteral implantation on the other side. Four patients with idiopathic retroperitoneal fibrosis were found to have characteristic obstruction and anterior displacement of the lumbar vena cava. Varying degrees of venous collateral circulation were present, depending on the degree of vena caval compression. All 4 patients underwent ureterolysis to relieve the hydronephrosis and to confirm diagnosis by biopsy. Two patients were treated with corticosteroids postoperatively. A safe and effective method for long-term relief of renal obstruction is presented. This method has been successful in buying time for patients in stiuations in which other approaches are hazardous or more complex. Its use can be modified for short-term therapeutic drainage of the upper urinary tract.

These "greyscale" images were obtained using a standard Nuclear Enterprises Diasonograph 4102B and a Varian 620/L digital computer interfaced to this Diasonograph. The authors present a differential diagnosis of echo-free renal masses and emphasize that such lesions are not always simple cysts. Of 312 renal sonograms, 15 showed sonolucent masses which proved to be something other than a simple cyst. The necropsy reports of 294 cases of anencephaly and 50 cases of iniencephaly have been examined, and a tubulated list of associated malformations produced. Cases were divided by sex and the presence or absence of spina bifida. Forty-one per cent of the series had other malformations, and other malformations were more common in those cases with spina bifida than in those without. This paper describes five patients, including four siblings in one family, who have diabetes insipidus, diabetes mellitus, optic atrophy and deafness (the DIDMOAD syndrome). Amoxicillin (AMPC), an oral preparation of broad spectrum synthetic penicillin, was used for treatment of 10 cases with urological infection, and the following results were obtained. 1. AMPC at a daily dose of 750 approximately 1,500 mg was administered for 7 approximately 49 days to 5 cases of acute cystitis, 2 cases of acute epididymitis and one case each of calculous hydronephrosis, chronic prostatitis and postoperative cystitis. Considerable functional disturbances of the urodynamics, consisting of hypotonia, hypokinesia, and dyskinesia of the pelvis and upper third of the ureters were detected by television excretory pyeloscopy and motion-picture urography in female rabbits after prolonged administration of progesterone and estradiol dipropionate. These findings suggest a role of hormonal imbalance in the pathogenesis of disorders of the urodynamics that are the possible causes of development of pyelonephritis, nephrolithiasis, hydronephrosis, and other diseases of the kidneys in women.

A study of the literature has revealed 11 cases of calcium "milk" in hydronephroses. We have been able to observe five further cases in a period of 14 months. The criteria annunciated by Murray are stressed, since they will raise a suspicion of the presence of calcium "milk" on views taken with a vertical x-ray beam. Two more indications are described for temporary ex vivo perfusion of kidneys with revascularization of these organs as autografts to orthotopic or heterotopic locations.
