Psoas Abscess: A Possible Missed Diagnosis in the Emergency Department.

A woman in her 70s was admitted to the Emergency Department with non-specific symptoms, such as fever, cough, dysuria, and hip pain with limited mobility, along with elevated inflammatory markers. She was initially diagnosed with a lower respiratory tract infection. However, she was readmitted to the hospital three days later due to worsening symptoms. The patient was eventually diagnosed with a secondary psoas abscess caused by underlying appendicitis and was successfully treated. This case highlights the diagnostic challenge of psoas abscess and underscores the importance of considering this

A Case-Based Review of Advanced Surgical Techniques in the Treatment of Incidentally Diagnosed Fibrolamellar Hepatocellular Carcinoma.

Fibrolamellar hepatocellular carcinoma (FL-HCC) is a rare liver tumor that typically affects young adults without underlying liver disease. This case describes an incidental diagnosis of FL-HCC in a healthy 25-year-old male who presented to the Emergency Department with left flank pain. Imaging revealed a ureteropelvic junction stone with moderate hydronephrosis, as well as an unexpectedly large right hepatic mass. Further evaluation confirmed the diagnosis of FL-HCC. The patient underwent a successful right hepatectomy utilizing the anterior approach with hanging maneuver and extra-Glissonian

Comparison of nuclear renographic renal cortical transit time with diuretic drainage time parameters in congenital hydronephrosis.

INTRODUCTION: Ureteropelvic junction obstruction (UPJO) is a frequent cause of congenital hydronephrosis (CHN). There is no gold standard diagnostic test for UPJO. Serial diuretic renogram is often used as a surrogate for diagnosing UPJO. Cortical transit time (CTT) is a renogram parameter reported helpful for management. Our primary objective was to study the association of CTT with other diuretic renogram parameters, and secondarily with clinical management. METHODS: We retrospectively reviewed 295 charts for CHN and pyeloplasty. We included 64 consecutive pyeloplasties (treatment group), a

Obstructive Pyelonephritis With Enterococcal Bacteremia Despite Unremarkable Urinalysis.

Obstructive pyelonephritis can be diagnostically challenging when urinary findings are unremarkable. Enterococcus faecalis is a well-known cause of complicated urinary tract infections and bacteremia; however, its clinical presentation may be atypical in patients with obstruction. A 73-year-old man presented with fever and left flank pain. Urinalysis and urinary sediment examination on admission were normal, and a urine culture was not obtained during the initial evaluation. Two sets of blood cultures yielded E. faecalis. Non-contrast computed tomography revealed a 12-mm left ureteral stone wi

A Rare but Critical Presentation in Primary Care: A Case Report on Unilateral Ureterolithiasis Leading to Postrenal Acute Kidney Injury.

Distal ureteral calculi measuring ≤5 mm are generally expected to pass spontaneously and are commonly managed conservatively. However, persistent obstruction may occur and can result in renal impairment. A 31-year-old previously healthy male presented with lower abdominal discomfort, dysuria, and penile pain. Initial noncontrast computed tomography (CT) revealed a 4 mm calculus at the left ureterovesical junction without hydronephrosis. He was discharged with conservative management, including nonsteroidal anti-inflammatory drugs (NSAIDs), oral cefixime 400 mg daily, hydration advice, and foll

Renal Ultrasound in Patients With Preauricular Skin Tags: Is It Necessary and Who Needs It?

Introduction: Congenital preauricular lesions, including skin tags, accessory tragi, and pits, have long been associated with renal anomalies due to their shared embryologic origin. Despite the common practice of recommending routine renal ultrasound screening for these patients, the necessity of this protocol has not been conclusively validated. Methods: A retrospective review was conducted at a single institution from 1996 to 2025. A total of 109 patients presenting with congenital preauricular lesions were identified. Data extracted included demographics, comorbidities, syndromic status, he

Feasibility and efficiency of the use of pulsed thulium: YAG (Thulio) laser for ECIRS: a multicenter evaluation.

INTRODUCTION: The pulsed Thulium: YAG (p-Tm: YAG) laser represents a recent technological advancement in endourology, offering high pulse frequencies and energy levels for efficient lithotripsy. While its efficacy has been demonstrated in retrograde intrarenal surgery (RIRS) and percutaneous nephrolithotomy (PCNL), data on its application during endoscopic combined intrarenal surgery (ECIRS) remain limited. This multicenter retrospective study evaluated the safety, efficiency, and clinical performance of the p-Tm: YAG laser during ECIRS. METHODS: Data were collected from 179 consecutive patien

Urothelial Bladder Cancer With Cardiac Metastasis: A Case Report.

We report a case with a large muscle-invasive bladder TCC and cardiac and disseminated metastases. A 74-year-old man was referred to our department due to a decline in general health, associated with macroscopic hematuria for the past two months, in the context of urinary infection. CT imaging revealed a large bladder tumor with bilateral hydronephrosis, suspicion of peritoneal carcinomatosis, and locoregional lymphadenopathies suggesting secondary involvement. Histology of the bladder tumor confirmed the diagnosis of high-grade infiltrating urothelial carcinoma. The 18-FDG PET scan detected d

The role of voiding cystourethrogram in the assessment of bilateral antenatally-detected isolated hydronephrosis.

INTRODUCTION: Isolated hydronephrosis (IH) is considered the most common cause of congenital hydronephrosis. Although voiding cystourethrogram (VCUG) is one of the main diagnostic tools in the workup of IH, there is debate about performing it routinely for all cases. In this study, we evaluated the role of VCUG in assessing bilateral IH in terms of its utility and potential complications. Moreover, we aimed to evaluate the outcome of the management of bilateral HN. PATIENTS AND METHODS: A retrospective review of patients' charts who presented with IH with non-visualized retrovesical ureters f

Incarcerated Ventral Hernia Requiring Emergent Repair in an Adult With Down Syndrome: Diagnostic and Postoperative Challenges in an Underrepresented Population.

The manifestation of ventral hernias is quite common, with an increased risk of progressing to incarceration/strangulation, which imposes the need for emergent surgery. Patients with Down syndrome (DS) present unique anatomical, physiological, and neurodevelopmental characteristics that may complicate diagnosis and postoperative management. In general, DS has been well studied for perioperative risk primarily with pediatric and cardiac populations, yet emergency general surgical outcomes in adults remain poorly studied. In our case report, a 59-year-old woman with DS and Alzheimer's disease un

Beyond Hydronephrosis: Perirenal Fluid on Screening Ultrasound in Patients With Renal Colic at a Basic Emergency Service.

Screening ultrasound has been progressively integrated into the evaluation of renal colic in emergency settings, enabling rapid identification of indirect signs of urinary obstruction, such as hydronephrosis, and supporting clinical triage when laboratory testing is limited. This article reports a case of recurrent severe renal colic refractory to analgesia, in which screening ultrasound demonstrated moderate unilateral hydronephrosis associated with perinephric/perirenal fluid. We discuss the clinical significance of this combined finding in light of the available evidence, acknowledging that

Antimicrobial strategies for ureteral stent removal after radical cystectomy: a comparative cohort study.

INTRODUCTION: Patients undergoing radical cystectomy (RC) face a heightened risk of infectious complications around the time of ureteral stent removal. This study aimed to determine whether a culture-guided antimicrobial strategy reduces infectious complications after stent removal compared with an empirical nitrofurantoin regimen. METHODS: We analyzed 200 patients who underwent RC with urinary diversion and ureteral stenting. Patients received either empirical nitrofurantoin or a culture-guided antimicrobial strategy based on pre-removal urine cultures for stent removal. The primary endpoint

An audit of delivery plans for babies with antenatal urinary tract abnormalities: the mothers' perspective.

OBJECTIVES: There is little evidence guiding the delivery of babies who are antenatally detected to have antenatal urinary tract abnormalities (AN-UTA), and some clinicians would deliver early purely because of AN-UTA. With growing evidence of adverse events resulting from early-term delivery this study aimed to assess delivery patterns for babies with AN-UTA. METHODS: Parents of children referred to our service with AN-UTA from May 2022- Sept 2023 were asked about their recollection of delivery discussions and methods. Data were collected on antenatal scans, delivery timing and method and th

Acute Obstructive Uropathy Complicated by Spontaneous Fornix Rupture and Hemorrhagic Urinoma.

Acute ureteric obstruction is a common urological presentation, but complications such as spontaneous calyceal rupture leading to a urinoma and hemorrhage are rare. This report details the case of a 43-year-old male who presented with acute right flank pain and chills due to severe right hydroureteronephrosis caused by two large obstructive ureteric stones. Initial computed tomography (CT) of the kidneys, ureters, and bladder (KUB) revealed a large peri-renal fluid collection (approximately 6.9 cm) with heterogeneous hyperdense components (measuring approximately 47 Hounsfield units), consiste
