Vesicoureteral reflux and anorectal malformations.

BACKGROUND: Renal and urinary tract anomalies are frequently associated with anorectal malformations (ARMs) and may adversely affect long-term renal outcomes, if not detected early. However, reliable clinical predictors for significant urologic abnormalities across different ARM phenotypes remain poorly defined. OBJECTIVE: To determine the prevalence and grade distribution of vesicoureteric reflux (VUR) in neonates with ARMs, and to explore its association with renal and urinary tract anomalies, the complexity of the ARM phenotype, and other factors are associated with high-grade VUR. METHOD

Outcomes of antenatal hydronephrosis and other renal abnormalities in a population-based cohort.

INTRODUCTION: Hydronephrosis, commonly detected during second trimester ultrasound screenings, can signal underlying renal abnormalities. In 2018, Sweden introduced a national guideline recommending follow-up for fetal renal pelves with an anteroposterior diameter (APD) ≥6 mm at the second trimester ultrasound. This study evaluates the impact of the guideline by assessing the incidence and resolution of hydronephrosis and other renal abnormalities. Secondary outcomes included postnatal renal function, frequency of febrile urinary tract infections (UTI), and the need for surgical intervention b

A deep learning-based study on automated CT diagnosis of kidney stones, hydronephrosis and pyonephrosis.

Automated identification of urinary system diseases on non-contrast computed tomography (NCCT) can facilitate early diagnosis and inform treatment decisions. However, kidney stones, hydronephrosis, and pyonephrosis share overlapping NCCT appearances and require diagnostic cues at different spatial scales, making accurate identification within a unified framework challenging. To address this, we constructed an integrated CT-based framework for automated identification of urinary system diseases. First, the collected CT data were split into training, validation, and test sets and processed using

A Silent Obstruction: Ureteropelvic Junction Syndrome Presenting As Resistant Hypertension in an Adult.

Resistant arterial hypertension warrants systematic evaluation for secondary causes, including rare structural anomalies of the urinary tract. Ureteropelvic junction obstruction can lead to chronic hydronephrosis, progressive renal dysfunction, and sustained activation of the renin-angiotensin-aldosterone system. We present the case of a 50-year-old woman with resistant hypertension referred for secondary cause evaluation. The patient reported paroxysmal facial flushing, headaches, palpitations, and abdominal distension. Physical examination revealed a large, non-tender mass in the right upper

End-Stage Kidney Disease Following Reflux Nephropathy Since the 2011 AAP Guidelines.

Age-associated sarcopenia decreases mobility and is promoted by cell senescence, inflammation, and fibrosis. The mitochondrial enzyme arginase-II (Arg-II) plays a causal role in aging and age-associated diseases. Therefore, we aim to explore the role of Arg-II in age-associated decline of physical activity and skeletal muscle aging in a mouse model. Young (4-6 months) and old (20-24 months) wild-type (wt) mice and mice deficient in arg-ii (arg-ii-/-) of both sexes are investigated. We demonstrate a decreased physical performance of old wt mice, which is partially prevented in arg-ii-/- animals

Association between job insecurity and cardiovascular diseases in workers with type 2 diabetes mellitus.

In organs, the correct architecture of vascular and ductal structures is indispensable for proper physiological function, and the formation and maintenance of these structures is a highly regulated process. The analysis of these complex, 3-dimensional structures has greatly depended on either 2-dimensional examination in section or on dye injection studies. These techniques, however, are not able to provide a complete and quantifiable representation of the ductal or vascular structures they are intended to elucidate. Alternatively, the nature of 3-dimensional plastic resin casts generates a pe

Trifid epiglottis in a neonate with choanal atresia and hydrometrocolpos: the third reported case and the first symptomatic neonatal presentation.

BACKGROUND: Trifid epiglottis (TE) is an extremely rare congenital laryngeal anomaly. To the best of our knowledge, only two cases have previously been reported in the literature. Both of them have been incidentally detected and associated with polydactyly. CASE PRESENTATION: We report the third known case of TE in a two-day-old female neonate who was admitted to the emergency department due to abdominal distension, oliguria, and poor feeding within the first 24 h of life. Examination revealed stridor during crying, left postaxial polydactyly, right syndactyly, and an anteriorly displaced anu

Severe Inflammatory Cystitis and Ureteritis Following Neoadjuvant TCHP (Docetaxel, Carboplatin, Trastuzumab, and Pertuzumab) Therapy in HER2-Positive Breast Cancer: A Case Report and Literature Review.

Haematuria is a rare complication in patients receiving docetaxel, carboplatin, trastuzumab, and pertuzumab (TCHP) chemotherapy for breast cancer, with only a few cases reported. Here, we describe a 65-year-old woman with HER2-positive T2N1 breast cancer who developed visible haematuria and acute kidney injury (AKI) after her first cycle of neoadjuvant TCHP therapy. Ultrasound and computed tomography (CT) imaging of the renal tract demonstrated mild bilateral hydronephrosis and moderate bilateral hydroureter associated with inflammatory fat stranding, with findings most consistent with chemoth

Long-Term Outcomes of Neoadjuvant Chemotherapy (NAC) Prior to Bladder-Sparing Trimodality Therapy (TMT) for Patients With Muscle-Invasive Bladder Cancer (MIBC).

BACKGROUND: Neoadjuvant chemotherapy (NAC) followed by bladder-sparing trimodality therapy (TMT) is an emerging treatment option for selected patients with muscle-invasive bladder cancer (MIBC), but long-term outcome data remain limited. PATIENTS AND METHODS: MIBC patients treated with 3-4 cycles of cisplatin-based NAC followed by TMT (maximal TURBT and concurrent radiation with weekly cisplatin) at 2 Canadian cancer centers from 2008 to 2017 were retrospectively reviewed. This updated analysis assessed median disease-free survival (DFS), overall survival (OS), bladder-intact DFS (BI-DFS), an

Laparoscopic Management of a Symptomatic Paraduodenal Hernia Complicated by Ureteric Compression: A Case Report.

Paraduodenal hernias (PDHs) are a rare variant of congenital internal hernias that result from abnormal midgut rotation and represent the commonest type of internal hernia. They often result in intermittent or acute small bowel obstruction (SBO), but can present with rare complications. A 41-year-old male patient presented with episodic abdominal pain for eight months. There were no other associated symptoms. Imaging for suspected ureteric colic was done, and it was revealed that there were encapsulated small bowel loops in the left upper quadrant (LUQ). CT abdomen was consistent with a left P

A Leak Beneath the Graft: Description of a Rare Ureteral Injury Following Aortobifemoral Bypass.

Ureteral injury is an uncommon but clinically significant complication of major vascular surgery, including aortobifemoral bypass, due to the close anatomic relationship between the ureters and the aortoiliac vessels. Delayed diagnosis may lead to urinoma formation, urinary obstruction, infection, and contamination of prosthetic vascular grafts; however, clinical presentation is often nonspecific and may resemble more common postoperative vascular complications. We report a 59-year-old man who underwent elective aortobifemoral bypass with Dacron graft placement and presented two weeks later wi

A clinical entity of multiple ureteral fibroepithelial polyps with annular stenosis and tortuosity: clinical features and management considerations.

BACKGROUND: Ureteral fibroepithelial polyps (FEPs) are rare. The management of multiple polyps presents unique challenges, particularly when combined with other variants. These complex cases warrant individualized surgical planning. OBJECTIVE: To characterize a distinct subset of complex ureteral FEPs defined by the triad of multiple annular stenosis, tortuous ureter, and multiple polyps-collectively termed "MASTUMP," and to investigate its clinical, pathological, genetic features, and management considerations. DESIGN: Single-center retrospective study. METHODS: This study included nine pa

Ovarian Vein Syndrome Presenting as Recurrent Right-Sided Flank Pain: A Case Report.

Ovarian vein syndrome (OVS) is a rare cause of ureteral obstruction resulting from extrinsic compression by a dilated ovarian vein, most commonly affecting women of reproductive age. Its nonspecific presentation, including intermittent flank or abdominal pain, urinary symptoms, and recurrent UTIs, often mimics more common conditions such as ureterolithiasis, appendicitis, or gynecologic pathology, leading to delayed diagnosis. Imaging modalities, particularly contrast-enhanced CT, are essential for identifying dilated ovarian veins compressing the ureter and associated hydronephrosis. We repor

Extrapelvic Urinary Extravasation Caused by Retroperitoneal Metastasis of Pulmonary Squamous Cell Carcinoma: A Case Report.

A 52-year-old man with pulmonary squamous cell carcinoma, clinically staged as cTxN3M1c (stage IVB), received four cycles of first-line chemotherapy with cisplatin plus TS-1 followed by seven cycles of second-line nivolumab. Subsequent imaging revealed progressive enlargement of cervical lymph nodes, left adrenal metastasis, and retroperitoneal metastases. Over several days, he developed left flank and back pain and presented to the emergency department. An emergent contrast-enhanced abdominal computed tomography (CT) demonstrated contrast extravasation from the upper left ureter, left uretera

A Customized Reinforced 8-French Ureteral Stent with End-Piece to Decrease Ureteral Obstruction and Alleviate Stent-Related Symptoms: Evaluation of 92 Patients with 319 Ureteral Stent Procedures.

INTRODUCTION: The most common method for releasing renal obstruction consists of the placement of an indwelling double-pigtail stent. However, adequate insertion of the stent through an obstructed ureter does not necessarily guarantee renal decompression. Most studies have concluded that the stiffness and lumen diameter of stents are major factors for maintaining their patency. Nonetheless, such stents are also poorly tolerated, severely impairing the patients' quality of life. We aimed to evaluate the efficiency and tolerance of a customized reinforced 8-French stent in a consecutive series o

Hidden in Hydronephrosis: Mixed Epithelial and Stromal Tumor of the Kidney (MESTK) Mimicking Hydronephrosis.

Mixed epithelial and stromal tumor of the kidney is an uncommon renal lesion characterized by cystic architecture and hormonally responsive stromal components, which explains its predominance in women, particularly those with prolonged or fluctuating estrogen exposure. Its occurrence in men is unusual and can make diagnosis challenging. We present a case of a 43-year-old male who presented with vague abdominal discomfort and was found to have a markedly enlarged, non-functioning right kidney on imaging. Initial investigations suggested obstruction of the distal ureter and a non-functioning rig

Hepatocyte growth factor blocks epithelial-mesenchymal transition and fibrosis of ureteral epithelial cells in UPJO.

BACKGROUND: Ureteropelvic junction obstruction (UPJO) is a leading cause of congenital hydronephrosis. Its etiology is associated with dysregulated extracellular matrix (ECM) remodeling and epithelial-mesenchymal transition (EMT). While Hepatocyte Growth Factor (HGF) is a known antagonist of fibrosis in other organs, its role in human UPJO pathogenesis remains unexplored. METHODS: Stenotic ureteropelvic junction (UPJ) tissues from pediatric UPJO patients (n = 40) and normal ureteral tissues from duplex kidney controls (Ctrl, n = 10) were obtained in our hospital. Analyses included demographic

Delay of Secondary Diversion After Failed Ablation May Commit Patients With Posterior Urethral Valves to Worse Renal Short-term Outcomes.

OBJECTIVE: To compare the effects of secondary versus primary diversion on kidney and bladder function in infants with posterior urethral valves (PUV). Although primary valve ablation is considered the gold standard, it is controversial and typically reserved for severe cases with valve ablation being the gold standard. In cases that fail to improve, secondary diversion may be offered-after a delay that likely had a detrimental impact on renal reserve. METHODS: A retrospective review of our institutional database between 2000 and 2024 identified 81 patients with diversion. After exclusion of
