Iatrogenic ureteric injury is an uncommon but serious complication of gynaecological surgery, with significant potential for renal impairment, fistula formation, prolonged hospitalisation, and medico-legal consequences. The ureter is at risk not only in complex pelvic procedures but also during apparently straightforward operations, and injuries may go unrecognised at the time of surgery, contributing to delayed diagnosis and worse clinical outcomes. We present a case series of five female patients aged 33, 59, 45, 46, and 47 years, respectively, who underwent complex gynaecological surgery fo

Urinary tract injury during cesarean section is uncommon, but the risk is much higher in the case of multiple sections due to dense adhesions. Bladder injury is the most frequent. Accidental ureteric injury is also rare; however, the combination of bladder rupture, ureteric ligation, and massive uterine artery hemorrhage in a single operation is what happened in our case. Current guidelines recommend the placement of a ureteric stent after de-ligation, but this is a situation in which stenting was not technically achievable. We report a case of a 36-year-old woman, gravida 4 para 2, with two p

Uremia has been associated with slowed intestinal motility, resulting in loss of peristalsis and the development of paralytic ileus. We present a case of uremia-induced paralytic ileus in a 70-year-old woman. The patient was admitted with a 7-day history of abdominal pain, vomiting, and constipation. Initial evaluation revealed a soft non-tender abdomen with decreased bowel sounds. Urgent ultrasonography showed dilated gut loops and sluggish gut motility, while atrophic kidneys with hydronephrosis were also appreciated. Routine biochemistry labs revealed elevated creatinine and potassium level

BACKGROUND: We present a case of two major complications following insertion of a nephrostomy managed in a minimally invasive way. Our case is the first in the literature to describe this minimally invasive treatment technique for colon perforation in a completely asymptomatic patient.
CASE DESCRIPTION: A 75-year-old female patient with a history of bilateral obstructive uropathy secondary to bilateral radiation-induced ureteric strictures attended for bilateral nephrostomy placement. The patient had a history of endometrial cancer, treated previously by total hysterectomy and bilateral salpi

Parastomal hernia is a recognized long-term complication following ileal conduit urinary diversion, though associated urinary obstruction is not very common. We report the case of a 63-year-old woman with an extensive history of abdominal and urological surgery who presented with bowel obstruction symptoms, reduced conduit output, and acute kidney injury. Cross-sectional imaging demonstrated bilateral hydronephrosis and hydroureter along with bilateral pyelonephritis secondary to extrinsic compression of the ileal conduit by a parastomal hernia containing the cecum. Intraconduit catheterizatio

Hereditary hemorrhagic telangiectasia (HHT) is a multiple-organ disease. Herein, we report a case of rupture of an undiagnosed pulmonary arteriovenous malformation (PAVM) during pregnancy in which good outcomes for both mother and children were achieved with transcatheter embolization. The patient was a pregnant 32-year-old Pashtun woman, G4P3, with a history of cesarean section. She had undergone pregnancy checkups at our hospital for a dichorionic diamniotic twin pregnancy. In gestational week 20, the patient experienced loss of consciousness at home and was transported by ambulance to our h

Medullary sponge kidney (MSK) is a congenital collecting duct abnormality associated with nephrocalcinosis, recurrent nephrolithiasis, hematuria, urinary tract infections, and obstructive urological complications. Immune thrombocytopenia (ITP) is an acquired autoimmune disorder characterized by isolated thrombocytopenia and bleeding risk. We report a case illustrating how these two chronic conditions can interact indirectly to create a recurrent management dilemma. A 45-year-old woman with bilateral MSK and chronic ITP controlled with weekly romiplostim had a baseline platelet count of approxi

Stercoral colitis is an inflammatory complication of prolonged fecal impaction, with presentations ranging from mild colonic inflammation to life-threatening perforation. Although gastrointestinal manifestations are well recognized, secondary involvement of adjacent pelvic structures due to mass effect remains underreported and may contribute to atypical clinical presentations. An elderly woman with paranoid schizophrenia receiving long-term antipsychotic therapy presented with a three-day history of lower abdominal pain, distension, and urinary retention. BACKGROUND: Congenital anomalies of kidney and urinary tract (CAKUT) are the leading causes of chronic kidney disease (CKD) in childhood. Determining the clinical course, outcome, and prognostic factors of this heterogeneous disease group is important to provide appropriate management and follow-up. Therefore, we aimed to identify the risk factors of CKD in CAKUT and the differences in clinical courses between disease subgroups.
METHODS: Three hundred patients (M/F: 203/97) divided into 16 CAKUT categories were enrolled in the study. Logistic regression and survival analyses were performed to

BACKGROUND: Necrotizing fasciitis (NF) of the head and neck is a rare but rapidly progressive and life-threatening soft tissue infection that constitutes a true surgical emergency. Due to the complex anatomy of the cervicofacial region and the proximity to the upper airway, early diagnosis and management are particularly challenging, and delayed recognition is associated with high morbidity and mortality. This article aims to provide a concise and clinically oriented overview of the presentation, diagnostic pitfalls, and current management strategies for cervicofacial necrotizing fasciitis.

Urinary tract stones complicated by infection are relatively common, whereas renal abscess rupture complicated by paralytic ileus is extremely rare. On March 1, 2025, a 22-month-old boy was admitted to the Department of Pediatrics, Lhasa People's Hospital. He presented with secondary paralytic ileus resulting from renal abscess rupture secondary to nephrolithiasis. The patient was hospitalized with a 10-day history of cough, expectoration, abdominal pain and distension. Given severe abdominal distension and intestinal obstruction, emergency small bowel decompression and abdominal abscess drain

BACKGROUND: Renal tuberculosis is a chronic granulomatous, infectious kidney disease caused by tuberculosis infection, often leading to ureteral tuberculosis, bladder tuberculosis, prostate tuberculosis and genital tract tuberculosis. Its typical pathologic changes are tuberculous nodule formation accompanied by different degrees of caseous necrosis. In recent years, the incidence of atypical renal tuberculosis has increased significantly, with extremely high rates of misdiagnosis and missed diagnosis. Prolonged delays in diagnosis and treatment have led to severe consequences such as renal fa

Plasmablastic lymphoma (PBL) is an aggressive and uncommon variant of diffuse large B-cell lymphoma, most frequently associated with immunocompromised states. Although extranodal disease is characteristic, involvement of the urinary bladder is exceptionally rare and may mimic primary urothelial malignancy both clinically and histologically. We report the case of a 62-year-old man with a previous history of high-grade diffuse large B-cell lymphoma in remission who presented with acute gross haematuria. Imaging demonstrated bladder wall thickening with associated hydronephrosis. Transurethral re

Cystitis cystica et glandularis (CCG) is a benign bladder condition driven by chronic mucosal irritation. Although often incidental, extensive disease can cause distal ureteric obstruction and produce radiological and cystoscopic appearances that closely mimic bladder malignancy, creating significant diagnostic and management challenges. We report two men with histologically confirmed CCG whose clinical courses differed markedly. The first presented with bilateral hydronephrosis and bladder distortion, initially suspicious for urothelial carcinoma. Despite repeated resections, stenting and ne

Methicillin-resistant Staphylococcus aureus (MRSA) is an uncommon cause of urinary tract infection but can result in severe systemic complications, particularly in patients with indwelling urinary devices. We report a case of MRSA bacteremia associated with a ureteral stent in an 83-year-old woman who presented with a high-grade fever. Computed tomography demonstrated left hydronephrosis despite stent placement. Emergency stent exchange revealed whitish purulent debris in the bladder and draining from the stent side holes. Blood, bladder urine, and renal pelvic urine cultures all yielded MRSA.

Many varied presentations of genitourinary tuberculosis (GUTB) such as recurrent urinary tract infection, sterile pyuria (pus cells in urine despite a negative routine bacterial culture) with/without haematuria, irritating voiding symptoms, infertility, hydronephrosis and renal failure had been published in the literature. Tuberculous epididymo-orchitis (inflammation of the epididymis and testis) is an underreported and rare form of GUTB. Non-specific symptoms of epididymal tuberculosis (TB) are difficult to differentiate from cysts, tumours and other infections. Bilateral epididymal involveme

BACKGROUND: Sequential treatment of ankylosing spondylitis (AS) that includes tumour necrosis factor-α antagonists (anti-TNF agents) has been applied in most of the Western countries. Existing cost-effectiveness (CE) models almost exclusively presented the incremental CE of anti-TNF agents using a closed cohort while budget impact studies are mainly lacking. Notwithstanding, information on impact on total population health and societal budget as well as on actual incremental CE for a given decision time span are important for decision makers. This study aimed at quantifying, for different deci
