Spontaneous renal pelvis rupture with extrusion of calculus is an extremely rare event. This article reports a case of right pelvis rupture with perinephric abscess due to renal calculus, with discussion on the diagnostic approach used, and its management principle. The diagnosis was confirmed with intravenous urography using computed tomography. She underwent right ureteral splinting using double-J stent, ultrasound guided pigtail drain insertion initially followed by stone extraction later.

Genitourinary Tuberculosis (GUTB) primarily occurs due to haematogenous infection and represents the most prevalent form of extra-pulmonary tuberculosis. It typically presents with non-specific symptoms and is rare in children, making diagnosis challenging and, potentially, leading to complications such as renal failure, pyonephrosis, nephrocutaneous fistulae, and sepsis. We describe a unique case involving a four-year-old male child who developed renal tuberculosis secondary to a renal stone, resulting in pyonephrosis and colorenal fistulae.

A 80-year-old woman presented with a swelling and pain in the left thigh visited our hospital. CT showed an impacted stone in the left ureter, left hydronephrosis and abscess formation extending from the perirenal area to the left thigh. Immediately, the patient was admitted and left nephrostomy was placed. On day 3 of admission, drainage of abscess in the left thigh was performed with the cooperation of an orthopedic surgeon. On day 17, a contrast-enhanced CT showed shrinkage of the thigh abscess, however, showed the left non-functioning kidney and poor drainage of the left iliopsoas abscess.

Extravasation of urine due to a forniceal rupture at the pelviureteric junction is a seldom-seen complication. While the presence of pyonephrosis can lead to a retroperitoneal abscess, its fistulization into the peritoneal cavity is exceptionally rare. Here, we present the case of a 50-year-old man exhibiting clinical signs of peritonitis. An abdominal CT scan indicated retroperitoneal peritonitis due to the rupture of the fornix. This case underscores the uncommon manifestation of pyonephrosis with peritonitis and pyoperitoneum, attributed to bladder cancer.
