Trends in End-Stage Kidney Disease Due to Reflux Nephropathy in Children and Young Adults.

BACKGROUND AND OBJECTIVES: Up to 50% of children with urinary tract infections (UTIs) have vesicoureteral reflux (VUR). Severe VUR can lead to reflux nephropathy (RN) and end-stage kidney disease (ESKD). In 2011, the American Academy of Pediatrics UTI guidelines suggested selective voiding cystourethrogram (VCUG), departing from previous recommendations to obtain VCUG after all febrile UTIs in children aged less than 2 years. It is unknown whether the subsequent decrease in VCUG has led to increased preventable kidney damage. This study's objective is to evaluate recent trends in the US incide

Outcomes of antenatal hydronephrosis and other renal abnormalities in a population-based cohort.

INTRODUCTION: Hydronephrosis, commonly detected during second trimester ultrasound screenings, can signal underlying renal abnormalities. In 2018, Sweden introduced a national guideline recommending follow-up for fetal renal pelves with an anteroposterior diameter (APD) ≥6 mm at the second trimester ultrasound. This study evaluates the impact of the guideline by assessing the incidence and resolution of hydronephrosis and other renal abnormalities. Secondary outcomes included postnatal renal function, frequency of febrile urinary tract infections (UTI), and the need for surgical intervention b

The epidemiology of kidney and urinary tract infections in pediatric chronic kidney disease: prevalence, risk factors and association with GFR decline in the Chronic Kidney Disease in Children Study.

BACKGROUND: Urinary tract and kidney infections are potential complications in pediatric chronic kidney disease (CKD), especially among patients with congenital anomalies of the kidney and urinary tract (CAKUT). We characterized the prevalence of self-reported infections from childhood through young adulthood; evaluated risk factors; and quantified the association with CKD progression. METHODS: Infections in the past year were self-reported at annual Chronic Kidney Disease in Children (CKiD) study visits. Age-specific infection prevalences were stratified by sex and CKD diagnoses (glomerular,

End-Stage Kidney Disease Following Reflux Nephropathy Since the 2011 AAP Guidelines.

Age-associated sarcopenia decreases mobility and is promoted by cell senescence, inflammation, and fibrosis. The mitochondrial enzyme arginase-II (Arg-II) plays a causal role in aging and age-associated diseases. Therefore, we aim to explore the role of Arg-II in age-associated decline of physical activity and skeletal muscle aging in a mouse model. Young (4-6 months) and old (20-24 months) wild-type (wt) mice and mice deficient in arg-ii (arg-ii-/-) of both sexes are investigated. We demonstrate a decreased physical performance of old wt mice, which is partially prevented in arg-ii-/- animals

Association between job insecurity and cardiovascular diseases in workers with type 2 diabetes mellitus.

In organs, the correct architecture of vascular and ductal structures is indispensable for proper physiological function, and the formation and maintenance of these structures is a highly regulated process. The analysis of these complex, 3-dimensional structures has greatly depended on either 2-dimensional examination in section or on dye injection studies. These techniques, however, are not able to provide a complete and quantifiable representation of the ductal or vascular structures they are intended to elucidate. Alternatively, the nature of 3-dimensional plastic resin casts generates a pe

Genetics of CAKUT.

Congenital anomalies of the kidney and urinary tract (CAKUT) represent a heterogeneous group of developmental disorders and are the leading cause of pediatric chronic kidney disease worldwide. The phenotypic spectrum is broad, encompassing kidney agenesis, hypodysplasia, multicystic dysplastic kidneys, vesicoureteral reflux, obstructive uropathies, and other malformations affecting the kidneys, ureters, and urethra. Advances in genetics have begun to unravel the molecular pathways underlying these diverse phenotypes, yet the complexity of CAKUT reflects contributions from both monogenic varian

Low Rate of Urological Complications in Pediatric Kidney Transplantation Using a Stentless Ureteral Anastomosis.

BACKGROUND: Pediatric kidney transplant recipients are susceptible to a range of complications that may compromise allograft function and are associated with significant morbidity. Urologic complications represent an important subset that may necessitate multiple radiological and/or surgical interventions. METHODS: Our previously reported modified extravesical ureteroneocystostomy technique was designed to avoid routine stent placement at the time of transplant as well as to reduce postoperative urologic complications. Here, we reviewed 107 consecutive pediatric kidney transplant recipients t

A Neonatal Salt-Wasting Crisis Mimicking Congenital Adrenal Hyperplasia: A Case of Transient Pseudohypoaldosteronism.

Salt-losing crisis in the neonate is a life-threatening event manifested by hyponatremia, hyperkalemia, and metabolic acidosis. Whilst this biochemical finding is classically seen in congenital adrenal hyperplasia (CAH), conditions of aldosterone action, such as pseudohypoaldosteronism (PHA), can also present with these findings but require different care. A four-week-old term male infant is described who was brought in with vomiting, lethargy, and anuria. At the time of admission, he was in shock with marked hyponatremia (119 mmol/L), life-threatening hyperkalemia (9.8 mmol/L), metabolic acid

Maternal Risk Factors for Infants Born with Congenital Renal and Urinary Tract (CAKUT) Anomalies: A Cross-Sectional Assessment.

Congenital Anomalies of the Kidney and Urinary Tract (CAKUT) often results in renal failure. Alhough the pathophysiology of CAKUT is still not completely understood, several maternal risk factors have been identified associated with children with CAKUT so far. Therefore, the aim of this study was to assess the proportion of risk factors of mothers gave birth to the infants with CAKUT. This cross-sectional study was conducted over a six-month period in the Departments of Pediatrics, Pediatric Nephrology, Neonatology and Pediatric Urology in Dhaka Shishu Hospital, Bangladesh from May 2018 to Nov

Multicenter evaluation of ambulatory blood pressure in children with CAKUT: distinctive profiles in the cystic dysplasia subgroup.

BACKGROUND: Hypertension (HT) is an important risk factor in patients with congenital anomalies of the kidney and urinary tract (CAKUT), independent of the existing anomaly in childhood. This study aimed to investigate masked hypertension and/or blood pressure profiles in different subgroups with a normal glomerular filtration rate (GFR). METHODS: The study included participants from seven different pediatric nephrology centers in Türkiye. Ambulatory blood pressure monitoring (ABPM) was performed on patients aged 5-18 years, diagnosed with CAKUT, with a GFR above 100 ml/min/m2, and normal off

Congenital Anomalies of the Kidney and Urinary Tract in Down Syndrome: Prevalence, Phenotypes, Genetics and Clinical Management.

Down syndrome (DS), the most common survivable autosomal aneuploidy, is associated with a high prevalence of congenital anomalies of the kidney and urinary tract (CAKUT), significantly increasing the risk of chronic kidney disease (CKD). This review examines the diversity of CAKUT phenotypes reported in individuals with DS, focusing on anomalies affecting the kidney, ureter, bladder, and urethra. According to available literature, hydronephrosis is the most common renal anomaly, often secondary to other CAKUT phenotypes, followed by renal hypoplasia and glomerulocystic disease. Furthermore, ob

Radiological Discovery of Asymptomatic Megaureter: A Rare Urological Anomaly and Literature Review.

Megaureter, characterised by a dilated ureter without evidence of obstruction at the vesicoureteral junction, is a condition primarily associated with paediatric populations. It is often diagnosed early in life due to urinary tract infections, hydronephrosis, or other urinary symptoms. In contrast, megaureters in adults are rarely encountered, particularly in asymptomatic individuals. This discrepancy in age-related presentation raises important questions regarding the natural history, detection, and clinical relevance of the condition in older patients. This paper presents a rare case of an i

INTRODUCTION: Although rare, incontinent diversions still has a place in the management of challenging conditions. Ureterocutaneostomy (in refluxing or non-refluxing moieties) is a viable option for relief of obstruction. It can be performed open or laaproscopically. Here we report our laparoscopic technique of extraperitoneal cutaneous ureterostomy with the help of transperitoneal laparoscopy. MATERIAL AND METHODS: The patients who underwent laparoscopic ureterocutaneostomy between 2020 and 2024 are enrolled in the study and our hybrid technique is described. RESULTS: Seven patients are enr

Coiling in Renovascular Hypertension: A Case Report of Fibromuscular Dysplasia and Complex Urinary Tract Abnormalities.

BACKGROUND Arterial hypertension (AH) affects 4% of children, with the most common etiologies including renal parenchymal disease, renovascular abnormalities, and primary hypertension. Severe hypertension in a patient with a urinary tract defect is a rare conjunction, requiring investigation for other coexisting pathologies. CASE REPORT We present the case of a 4-year-old boy with bedwetting, daytime urinary incontinence, and an episode of urinary tract infection, which led to an extensive diagnostic evaluation. The workup revealed bilateral vesicoureteral reflux, posterior urethral valves (PU

Children with recurrent urinary tract infections: who are they and why do we need better prevention options?

BACKGROUND: Recurrent UTIs (rUTIs) in children can lead to renal scarring and chronic renal failure, if not managed early. The current standard of care involves antibiotic prophylaxis, which benefits remain controversial. There is a need for new interventions, other than antimicrobial use, to reduce the occurrence of rUTIs as well as the identification of groups to target. Our objective was to describe the pediatric population most susceptible to rUTIs, including renal scarring and adverse events associated with prolonged antibiotic use. METHOD: We conducted a single-centre retrospective char

Neonatal jaundice is associated with increased risks of congenital anomalies of the kidney and urinary tract and concomitant urinary tract infection.

The link between neonatal jaundice and urinary tract infection (UTI) remains debated, with congenital kidney and urinary tract anomalies (CAKUT) potentially playing a role. This population-based study aimed to analyze the correlations between neonatal jaundice, CAKUT, and concomitant UTI. The study cohort consisted of 2,078,122 live births from 2004 to 2014. We linked several population-based datasets in Taiwan to identify infants with unexplained neonatal jaundice and their mothers. The primary outcome was the rate of CAKUT occurring within 3 years after delivery, and the presence of concomit
