Analgesic abuse and kidney disease.

The analgesic syndrome, comprising renal disease, hypertension, peptic ulcer, anaemia and recurrent headache, accounts for wide-spread morbidity and mortality especially in Queensland and New South Wales. Epidemiological and clinical evidence gathered from many Western societies implicates unsupervised consumption of compound analgesic preparations, particularly those containing phenacetin, in the causation of the majority of cases. Laboratory experiments so far have failed to produce an entirely satisfactory model of clinical analgesic nephropathy. In small animals, papillary necrosis results

Minor analgesic use among high-school students: indications for renal morbidity and mortality.

Reported patterns of minor analgesic use by adolescents were taken from a baseline 1973 survey, and a follow-up 1974 survey. The differences between the sexes within each year were shown to be highly significant. Trend analysis in minor analgesic use included the year-to-year comparisons, the degree of minor analgesic use, and the rate of change in this use. The trends show the long-term process of minor analgesic use, and support the cause for concern about analgesic nephropathy.

Analgesic nephropathy: a major or minor problem?

Habitual analgesic takers run approximately three times the risk of developing chronic renal disease as nontakers or occasional takers. Over 200 deaths may occur each year in Australia from analgesic-induced renal disease, but this must be balanced against the benefit to society of simple, effective pain relief. This can be placed in perspective by contrast with the cost to society of cigarette smoking.

Bilirubin-associated renal papillary necrosis in the homozygous Gunn rat: light-and electron-microscopic observations.

Homozygous members of the mutant Gunn strain of Wistar rats suffer an inherited autosomal recessive deficiency of glucuronyl transferase resulting in unconjugated hyperbilirubinaemia and jaundice. Unconjugated bilirubin visibly accumulated in the interstitium of the renal papillary tip. The deposit is followed by necrosis, first of intersitital cells, loops of Henle and vasa recta and later of collecting ducts. The necrotic, acellular papillary tip eventually separates. Cortical scars correlate with established papillary necrosis. The bilirubin occurs in two ultrastructural forms, crystalline

Analgesic nephropathy--a continuing problem.

Analgesic-induced renal disease occurs more commonly than is recognized; unless specific inquiry is made, it is seldom diagnosed. Patients with chronic pain, especially those with hypertension, urinary tract infection, or renal insufficiency, should be suspect. The condition is preventable and even in the late stages is manageable if recognized. Analgesic abuse may result in end-stage renal disease; it accounts for a significant percentage of patients entering chronic hemodialysis or renal transplant programs.

The induction of renal papillary necrosis in Gunn rats by analgesics and analgesic mixtures.

Homozygous members of the mutant Gunn strain of Wistar rats genetically lack the enzyme uridine diphosphate glucuronyl transferase. "High" and "low" dose gavage feeding for 18-34 days of an analgesic mixture containing aspirin, phenacetin and caffeine (APC) confirmed the previously reported susceptibility of these animals to analgesic induced renal papillary necrosis. Heterozygotes do not share the gross enzyme deficiency of homozygotes and, when treated with APC under identical conditions, failed to develop renal papillary necrosis. Groups of homozygotes were dosed by gavage with aspirin, phe

Diseases causing end-stage renal failure in New South Wales.

The nature of the original renal disease was determined in 403 consecutive cases of end-stage renal failure, in 317 of which the clinical diagnosis was corroborated by histological examination of the kidney. Five diseases accounted for 20 or more cases--glomerulonephritis (31% of the total), analgesic nephropathy (29%), primary vesicoureteral reflux (8%), essential hypertension (6%), and polycystic kidneys (5%). In only four cases did renal failure result from chronic pyelonephritis without a demonstrable primary cause. Greater use of micturating cystography and cystoscopy and routine urine te

[Characteristics of the energy metabolism in the kidneys in analgesic nephropathy].

Effect of prolonged treatment with acetyl salicylic acid (100 mg/kg) on energy metabolism was studied in dog kidney. Functional conditions of kidney were altered, which was manifested by an increase in excretion of sodium and by impairement is osmotic concentrating of urine. Studies of energy metabolism revealed uncoupling of oxidation and phosphorylation and also some decrease in the activity of glycolytic processes. The observed alterations in bioenergetics of kidney tissue are considered as important factors in development of analgetic nephropathy.

Analgesic abuse in urological practice.

Thirty-three patients seen in a urological department had taken large doses of analgesic for prolonged periods of time. In 9 cases a diagnosis of pyramidal necrosis was established, while 4 other had pyelographic evidence of pyelonephritis, and analgesic abuse was probably an important aetiological factor in their renal condition. Analgesic nephropathy is easily overlooked unless patients are questioned routinely regarding their intake of analgesic. It is important to identify the group of patients without evidence of serious renal disease who are taking excessive quantities of analgesic and t

[Analgesic nephropathy].

Analgesic nephropathy is characterized by poor clinical symptoms. Abnormal urinary findings are rare. The disease is usually discovered if advanced renal damage has occurred with elevated serum creatinine, papillary necrosis, microhematuria and renal colics. There is abundant evidence, that abuse of phenacetin leads to analgesic nephropathy. Aspirin may have only an additive effect with phenacetin in causing renal damage. The primary medullary changes caused by phenacetin or one o f its metabolites are: Interstitial fibrosis, thickening of tubular basement membrane, loss of tubular epithelium

Analgesic abuse syndrome: a frequently overlooked cause of reversible renal failure.

The incidence of analgesic nephropathy in the United States is greater than previously reported. Because of the characteristic radiographic features of papillary necrosis, this diagnosis may be made while it is still clinically unsuspected. Early diagnosis is extremely important because cessation of analgesic abuse may avert progressive renal damage. Uncovering the diagnosis calls for special care in obtaining the telltale history. This must be sought in patients with radiographic evidence of papillary necrosis when a history of diabetes mellitus, obstructive uropathy, or sicle cell anemia is

The radiological diagnosis of analgesic nephropathy.

A study was made of the radiological findings in 57 patients with a clinical diagnosis of analgesic nephropathy. The ages of the patients ranged from 27 to 72 years, and females predominated over males in the ratio 4-7 to 1. Renal failure varied from mild to very severe and the patients showed the usual high incidence of analgesic-induced gastro-intestinal, haematological and neurological disorders. In 82% of cases, kidneys were of normal size, but there was radiological evidence of reduced renal function in 85%. Thus, a small kidney with good renal function was unlikely to be due to analgesic

Letter: The clinical course of patients with analgesic nephropathy.

Thirty-four patients with analgesic nephropathy were followed at intervals of 1 to 3 months with measurements of creatinine clearance and serum concentration of acetylsalicylic acid (ASA) for a total of 105 patient-years. Diagnostic criteria included total consumption of at least 2 kg of phenacetin and 3 kg of ASA, compatible tissue abnormality on biopsy and evidence of papillary necrosis on an intravenous pyelogram. Nephropathy was induced by the same compound analgesic containing ASA, pehnacetin, caffeine and codeine (APC&C) in 30. Phenacetin was removed from this preparation in 1970 and rep

Anti-inflammatory drugs and renal papillary necrosis.

The literature on renal papillary necrosis (RPN) associated with the administration of non-steroidal anti-inflammatory drugs (NSAID) to rats and man, is reviewed. RPN is almost universally reported after long term administration of NSAID to rats, reports being cited for an indomethacin analog, phenylbutazone, fenamic acids, fenoprofen and sudoxicam. Aspirin will also induce RPN in rats, and is probably the cause of the analgesic nephropathy' linked to abuse of aspirin/phenacetin combinations in man. RPN is reported at autopsy in human arthritics, but whether this is a facet of the disease, or

Analgesic renal papillary necrosis.

Analgesic nephropathy is well recognised. This is a retrospective review of 19 patients with the disease, who presented at Groote Schuur Hospital over a 4-year period. The diagnosis was made on historical and clinical grounds and on the radiological manifestations of papillary necrosis. The mean age was 49 years (26 - 74) with a sex ratio of 3,8 female : 1 male. The period of abuse varied between 3 and 20 years. Aspirin, phenacetin, codeine, and antipyrine were the compounds mainly ingested. If the abuse is stopped, renal deterioration can be halted. However, factors which lead to dehydration
