[A CASE OF DELAYED GRAFT FUNCTION FOLLOWING LIVING-DONOR KIDNEY TRANSPLANTATION WITH RECOVERY AFTER 28 DAYS OF HEMODIALYSIS].

Delayed graft function (DGF) is a rare complication following living-donor kidney transplantation, most commonly caused by ischemic acute tubular necrosis (ATN). We report a case in a 51-year-old woman with diabetic nephropathy who underwent an ABO-compatible living-donor kidney transplant from her younger brother. Although the procedure was completed without major complications, the patient developed persistent oliguria postoperatively. Serial allograft biopsies on postoperative days 1, 3, and 13 consistently showed findings of ATN without evidence of acute rejection or calcineurin inhibitor

Phytochemical Analysis of Moroccan Mercurialis annua L. and Its Nephroprotective Activity: Evidence Supporting Traditional Renal Uses.

Mercurialis annua L. (Euphorbiaceae) is traditionally used to treat renal disorders, although its nephroprotective activity remains poorly documented. This study characterized the phytochemical profile of the Moroccan hydroethanolic extract by UHPLC-ESI/MS, evaluated its acute toxicity, and investigated its protective effect against cyclophosphamide-induced nephrotoxicity in rats. UHPLC-ESI/MS tentatively identified phenolic and flavonoid constituents, including isorhamnetin derivatives, taxifolin, epicatechin, and hydro-p-coumaric acid hexoside. No mortality or treatment-related toxicity was

Microplastics and IgA nephropathy: a novel environmental hypothesis of immune dysregulation.

IgA nephropathy (IgAN) is the most common glomerulonephritis worldwide, characterized by a 4-hit pathogenesis primarily driven by mucosal immune dysregulation in the gut-associated lymphoid tissue (GALT). However, the specific environmental triggers initiating this cascade remain elusive. We propose a novel hypothesis that chronic exposure to ubiquitous micro- and nano-plastics (MNPs) serves as a primary environmental catalyst for IgAN. Through a proposed 5-step cascade, we outline how ingested MNPs directly degrade intestinal tight junctions, precipitating a 'leaky gut' that permits the syner

Asymptomatic Bacteriuria in Kidney Transplant: Approaches to Screening and Treatment-A National Survey of Canadian Nephrologists.

BACKGROUND: It is estimated that up to half of kidney-transplant (KT) recipients develop asymptomatic bacteriuria (ASB) within three years posttransplantation. Despite limited evidence supporting screening or treatment, most centers continue routine surveillance and antibiotic therapy, raising concerns about antimicrobial resistance, drug toxicity, and costs. We sought to describe current Canadian practices and perceptions regarding ASB screening and treatment among nephrologists caring for KT recipients. METHODS: Nationwide cross-sectional survey. All nephrologists affiliated with the Canadi

Triazole Scaffold-based DPP-IV Inhibitors for the Management of Type-II Diabetes Mellitus: Insight into Molecular Docking and SAR.

Diabetes mellitus, characterized as a chronic metabolic disorder or a polygenic syndrome; is increasing at a very fast pace among every group of the population worldwide. It arises due to the inability of the body to produce enough insulin (the hormone responsible for controlling blood sugar levels) or inability to utilize the insulin, leading to hyperglycaemic condition, which, if left uncontrolled gives rise to chronic microvascular and macrovascular complications like retinopathy, neuropathy, nephropathy, coronary artery disease, cognitive impairment, etc. Several therapeutic approaches are

Living kidney donation prior to euthanasia: medical, psychological and ethical considerations.

BACKGROUND: Light chain deposition disease (LCDD) is a monoclonal immunoglobulin deposition disease (MIDD) that is characterized by the deposition of monoclonal light chains in multiple organs, including the kidney. It is a rare disorder caused by an underlying monoclonal plasma cell dyscrasia. LCDD with renal involvement causes proteinuria, which sometimes can lead to nephrotic syndrome. The monoclonal light chains are mostly in the κ form. Treatment of LCDD is the same as that for multiple myeloma (MM); however, some conventional anticancer drugs show substantial toxicity and therefore canno

A nephrology perspective on the clinical impact and management of delayed high-dose methotrexate elimination.

INTRODUCTION: Delayed methotrexate elimination (DME) is observed in ~15% of treatment cycles in patients with cancer receiving high-dose methotrexate (HDMTX). Given the strong association between DME and nephrotoxicity, oncologists might benefit from the input of a nephrologist/medical oncologist with a focus on nephrology when assessing and planning treatment for patients requiring HDMTX. AREAS COVERED: Identifying patients who are at risk of DME and related acute kidney injury (AKI), and developing a bespoke assessment and monitoring plan (with a focus on kidney assessment and monitoring),

Ameliorating Paraquat-induced Nephrotoxicity in Rats: Protective Effects of Nanocurcumin on Renal Histology and Molecular Pathways.

BACKGROUND: Paraquat (PQ) is a common herbicide, and its mortality results from injury to several organs, including the kidneys. Nanocurcumin and curcumin have anti-inflammatory and anti-oxidative activities, but their involvement in PQ-induced kidney damage is unclear. Therefore, the goal of our study was to compare nanocurcumin and curcumin in male rats whose kidneys were damaged by PQ. METHODS: 42 eight-week male albino Wistar rats were put into six groups at random as control, control + curcumin, control + nanocurcumin, PQ, PQ + curcumin, PQ + nanocurcumin for 7 days. The kidney tissues a

Therapeutic Effects of Tea Polyphenols on Renal Damage Induced by High Uric Acid.

BACKGROUND: Hyperuricemia (HUA) is a condition characterized by excessive uric acid production and/or inadequate uric acid excretion due to abnormal purine metabolism in the human body. Uric acid deposits resulting from HUA can lead to complications such as renal damage. Currently, drugs used to treat HUA lack specificity and often come with specific toxic side effects. OBJECTIVE: This study aimed to investigate the renal protective effects of an optimized tea polyphenol formula and allopurinol in a rat model of hyperuricemia following renal resection. The goal was to explore the mechanisms u

Multifactorial chronic kidney disease.

The burden of disease due to chronic kidney disease (CKD) has grown disproportionately in the last decades. This growth has not been similar in all regions. In the last few years, the results of clinical trials, particularly those evaluating sodium-glucose-cotransporter-2 (SGLT2) inhibitors and their efficacy in decreasing cardiovascular mortality and CKD progression, revealed the relevance of a common protective pathway that remains non-specific as it has been detected in multiple clinical scenarios. However, the injury pathways in kidney disease are now well-defined in immunological, toxic,

Peptide receptor radionuclide therapy: a new era of radiation nephropathy.

Peptide receptor radionuclide therapy (PRRT) has transformed the management of neuroendocrine tumors and other malignancies by enabling targeted radiation delivery while minimizing systemic toxicity. However, incidental renal radiation exposure presents a significant risk of radiation nephropathy, and this risk varies based on the unique characteristics of the radionuclide and its decay phenomenon as well as the peptide being utilized. This review explores the historical development of PRRT, the physics of radionuclide therapy as necessary for a practicing nephrologist, and key mechanisms of r

Nephrotoxicity of conventional chemotherapeutics: part II-non-platinum agents and miscellaneous nephrotoxic drugs.

While modern anticancer therapies have significantly improved survival in patients with cancer, their success is often hindered by unintended side effects and toxicities, particularly to the kidneys. Many conventional chemotherapeutic agents, despite their efficacy, exert nephrotoxic effects that can result in short- and long-term adverse kidney outcomes. This review critically examines the renal implications of non-platinum alkylating agents and other less commonly grouped cytotoxic drugs based on available literature. Nephrotoxicity remains a frequent and often dose-limiting challenge in pat
