OBJECTIVE: To investigate the diagnostic value of peripheral blood lymphocyte testing in children with infectious mononucleosis (IM).
METHODS: A total of 135 children with IM as the IM group and 100 healthy volunteers as the healthy group were included in this retrospective study. Peripheral blood lymphocyte subsets marked as CD3+, CD4+, CD8+, CD16 + CD56+, and CD19 + in the peripheral blood were quantified using flow cytometry. Statistical analysis was performed using the chi-square test, Kruskal-Wallis test, AUROC curve, and Kappa consistency test to assess the diagnostic value of these mar

BACKGROUND: Bilateral optic neuritis associated with optic disc swelling is a common feature of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD). However, extensive deep retinal haemorrhages have not been described in the context of MOG-associated optic neuritis. Here, we report a case of infectious mononucleosis with marked binocular peripapillary and perivascular haemorrhages as well as extensive deep retinal haemorrhages in the presence of MOGAD.
CASE REPORT: A 39-year-old Chinese woman presenting with subacute binocular vision reduction with no light perception was

Epstein-Barr virus (EBV), also known as human herpesvirus 4, is a member of the herpes virus family. EBV is a widespread virus and causes infectious mononucleosis, which manifests with symptoms such as fever, fatigue, lymphadenopathy, splenomegaly, and hepatomegaly. Additionally, EBV is associated with different lymphocyte-associated non-malignant, premalignant, and malignant diseases. So far, no effective treatment or therapeutic drug is known for EBV-induced infections and diseases. This study investigated natural compounds that inhibit EBV glycoprotein L (gL) and block EBV fusion in host ce

Aims/Background Atraumatic splenic rupture is a very rare and potentially life-threatening event usually associated with underlying pathological conditions. Splenic rupture in infectious mononucleosis occurs only in 0.1%-0.5% of patients. The aim of the present study was to publish a case report of the atraumatic splenic rupture and to present a mini-review of the international literature. Case Presentation A 30-year-old man of mixed races (Caucasian and African) diagnosed with acute cholecystitis was referred by a rural doctor to a secondary district hospital. His past medical and family hist

One of the main clinical manifestations of infection by the Epstein-Barr virus (EBV) is infectious mononucleosis. In this clinical syndrome, mild hepatitis with a slight elevation of aminotransferases is common. However, cholestasis is rare and usually occurs alongside a more severe, cytolytic hepatitis. This case report describes a 26-year-old male admitted to the emergency service with recurrent fever, odynophagia, and painful cervical lymphadenopathy, along with a skin rash after starting treatment with amoxicillin/clavulanate, and jaundice. The analytical assessment was consistent with cho

The molecular diversity of Epstein-Barr virus (EBV) is defined by mutations in specific EBV genes and has been insufficiently studied in infectious mononucleosis (IM). The aim of this study was to determine all variations of the EBV latency genes EBNA-1, EBNA-2 and LMP-1 in pediatric patients with EBV-associated IM in Croatia, including previously defined SNPs and indels as well as previously undocumented polymorphisms. The vast majority of EBV isolates (71/72) were determined as EBV type 1 while EBNA-1 genes were classified exclusively as previously defined EBNA-1 prototypes, with 22/72 seque

Epstein-Barr virus strains present in South-East Asia are tightly associated with nasopharyngeal carcinomas. We report the case of a Caucasian female who presented with an infectious mononucleosis syndrome. Sequencing demonstrated that she had been infected with a virus of Chinese origin that is thus spreading into European countries.

BACKGROUND: Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis (EBV-HLH) is a severe hyperinflammatory disorder induced by overactivation of macrophages and T cells. This study aims to identify the risk factors for the progression from infectious mononucleosis (EBV-IM) to EBV-HLH, by analyzing the laboratory parameters of patients with EBV-IM and EBV-HLH and constructing a clinical prediction model. The outcome of this study carries important clinical value for early diagnosis and treatment of EBV-HLH.
METHODS: A retrospective analysis was conducted on 60 patients diagnosed with

The Epstein-Barr virus (EBV) is widespread and has been related to a variety of malignancies as well as infectious mononucleosis. Despite the lack of a vaccination, antiviral medications offer some therapy alternatives. The EBV BZLF1 gene significantly impacts viral replication and infection severity. The current study performed computer-assisted techniques to analyses the potential drug candidates against Epstein-Barr virus toxin Zta [Human gamma-herpesvirus 4 (Epstein-Barr virus)] from phytochemicals derived natural inhibitors. Various bioinformatics methods were employed to predict and anal

Hemophagocytic lymphohistiocytosis (HLH) is a rare but potentially fatal entity characterized by an unregulated activation of the immune system. In the adult population, it is most commonly secondary to infectious, autoimmune, or neoplastic diseases. We present a case of a 23-year-old female diagnosed with infectious mononucleosis and hospitalized due to a persistent three-week fever and malaise with a new onset of jaundice and findings compatible with acute hepatitis and hepatosplenomegaly. Due to a lack of response to standard supportive care, maintained fever, persistent analytical worsenin

Epstein-Barr virus (EBV) usually causes mild, self-limiting, or asymptomatic infection in children, typically infectious mononucleosis. The severe course is more common in immunocompromised patients. Neurological complications of primary infection, reactivation of the latent infection, or immune-mediated are well-documented. However, few published cases of fatal EBV encephalitis exist. Case presentation We report a case of a 5.5-year-old immunocompetent girl with fulminant EBV encephalitis fulfilling the criteria for the recently proposed subtype Acute Fulminant Cerebral Edema: (AFCE). The chi

Splenic infarction with infectious mononucleosis (IM) caused by Epstein-Barr Virus (EBV) has been reported as a rare complication of IM. The monospot test, often used to diagnose EBV-related IM, may produce false-negative results, especially in atypical presentations or early stages of infection. This report describes the case of a monospot-negative patient who developed splenic infarction as a complication of IM. The pathophysiology of splenic infarction in IM remains poorly understood, though it is thought to be related to splenic congestion, thrombophilia, or the direct impact of EBV on the

Atraumatic splenic rupture (ASR) is a rare and life-threatening condition that presents diagnostic difficulties due to its rarity and non-specific clinical symptoms. It often requires computed tomography (CT) imaging for accurate diagnosis and surgical planning. Splenectomy is the standard treatment for unstable patients, but autotransplantation of splenic tissue may reduce the lifelong risks of overwhelming post-splenectomy infections (OPSI) by preserving some immunological function. However, autotransplantation is rarely performed in ASR due to the delicate nature of the spleen and the prese

Human cytomegalovirus infection usually proceeds asymptomatically in immunocompetent patients. In symptomatic forms, mononucleosis syndrome is the most common manifestation. However, atypical cases of cytomegalovirus infections in immunocompetent subjects are reported in the literature. Here, we describe a case of cytomegalovirus-related mononucleosis syndrome that presented with an atypical erythema multiforme-like skin rash and high fever. Very few cases have been described in the literature previously. In our case, the diagnosis was supported by specific serology, and human cytomegalovirus

PURPOSE OF REVIEW: Infectious mononucleosis (IM) is an infectious disease that presents clinically in only a small percentage of individuals despite almost universal infection with the causative agent. Here, we review the latest concepts in the clinical presentation, epidemiology, and host response of this disease.
RECENT FINDINGS: Several recently published papers/reviews describe IM as a condition caused by one of several etiologic agents including, cytomegalovirus (HHV-5), Roseola virus (HHV-6) and Toxoplasmosis amongst others; this review focuses on IM as solely caused by the human herpes

Circovirus human is a new viral species that includes the human circovirus (HCirV), which has been linked to hepatitis in immunocompromised persons. We investigated prevalence of HCirV infection in 278 patients with hepatitis and 184 asymptomatic persons using real-time PCR and sequencing assays. HCirV viremia and sequences were found in 8 (2.9%) hepatitis patients and no asymptomatic patients. Alternate causes of hepatitis (hepatitis E and cholangitis) were clearly identifiable in 2 HCirV-infected patients. HCirV could not be ruled out as a contributor to hepatitis in the remaining 6 patients

A 28-year-old female patient with no particular medical history had a sore throat seven days before admission. Subsequently, she developed malaise, right abdominal pain, and a fever of 38°C and visited our hospital. A blood test revealed a mild inflammatory response and elevated liver enzymes, and she was admitted to the hospital for detailed examination and acute liver injury treatment. Various viral tests and autoantibody measurements revealed elevated Epstein-Barr virus (EBV) immunoglobulin M and negative EB nuclear antigen antibodies. Therefore, she was diagnosed with primary infectious mo

BACKGROUND: The genetic susceptibility association between viral infection and the risk of colorectal cancer (CRC) has not been established.
METHODS: We conducted two-sample Mendelian randomization (MR) analysis using genome-wide association study (GWAS) data. In addition to traditional MR methods, we employed several other approaches, including cML, ConMix, MR-RAPS, and dIVW, to comprehensively assess causal effects. Sensitivity analyses were also performed to ensure the robustness of the results.
RESULTS: After sensitivity analysis, presence of SNPs linked to increased susceptibility to co
