The Epstein-Barr virus (EBV) is a prevalent virus linked to various diseases, including infectious mononucleosis (IM), nasopharyngeal carcinoma, and Hodgkin's lymphoma. Over the past few decades, EBV diagnostic strategies have evolved significantly-progressing from traditional serological assays and histopathology to more sensitive and specific molecular techniques such as nucleic acid amplification and high-throughput sequencing (HTS). While conventional methods remain valuable for their accessibility and established clinical use, they are often limited by sensitivity, speed, and multiplexing

Epstein-Barr Virus (EBV) is a causative agent of infectious mononucleosis and is strongly associated with Burkitt lymphoma, Hodgkin lymphoma, and nasopharyngeal carcinoma. EBV encodes a deubiquitinating enzyme, BPLF1, which is important for infectious virus production, B-cell immortalization, and tumorigenesis. To elucidate BPLF1's role, an affinity-based mass spectrometry screen was performed, which suggested that BPLF1 and mTOR interact. mTOR, a critical mediator within cellular signaling cascades and oncogenesis, exists in two distinct complexes: mTOR Complex 1 (mTORC1) and mTOR Complex 2 (

Infectious mononucleosis (IM), caused by the Epstein-Barr virus (EBV), is primarily a self-limiting illness. However, it often mimics bacterial infections, such as bacterial tonsillitis, which can represent a diagnostic pitfall and, consequently, increase the risk of developing complications. The aim of this case report is to highlight the complications resulting from the incorrect differential diagnosis of EBV-induced IM and its clinical significance. We present the case of a 24-year-old male who was initially misdiagnosed with bacterial tonsillitis. Amoxicillin-clavulanic acid was initiated

Mycoplasma pneumoniae (MP) is a bacterium commonly known to cause mild respiratory infections, especially in young children. Epstein-Barr virus (EBV) is a herpesvirus that causes infectious mononucleosis, typically a mild illness in younger individuals. However, in its severe form, EBV can cause pneumonia. Severe pneumonia resulting from co-infection with MP and EBV is rare, with few cases reported in the literature, mostly in the pediatric population. We report a case of a 20-year-old man who presented with a one-week history of progressively worsening right-sided pleuritic chest pain, subjec

IMPORTANCE: Epstein-Barr virus (EBV) is not only the principal cause of infectious mononucleosis (IM) but is also a precursor to several cancers and autoimmune disorders. Access to a prophylactic EBV vaccine early in life could be key for the prevention of these conditions. However, the incidence of primary EBV infection (pEBV) in preadolescent children is not currently known. We hypothesized that pEBV is clinically significant, but often undiagnosed.
METHODS: In this prospective study we screened and followed preadolescent children ages 1.5-11.99 years at a single pediatric primary care clin

Diffuse large B-cell lymphoma (DLBCL) is clinically heterogeneous malignancy with complex etiological factors. The role of Epstein-Barr virus (EBV) infection, particularly infectious mononucleosis (IM), in DLBCL development remains controversial. This study aims to elucidate the potential mechanisms underlying the inverse association between IM and DLBCL by investigating the causal relationship between EBV and DLBCL using Mendelian randomization (MR) and comprehensive miRNA profiling. We employed a two-sample MR approach to assess the causal effect of EBV infection on DLBCL risk, using genetic

BACKGROUND: Hypoglossal nerve palsy (HNP), although rare, can result from various causes and more commonly in children is secondary to infectious etiologies. Its optimal management requires accurate diagnosis and exclusion of severe complications like Lemierre's syndrome, vascular events, tumors, or demyelination/inflammatory processes).
CASE PRESENTATION: A previously healthy 16-year-old boy was diagnosed with HNP, infectious mononucleosis and Fusobacterium necrophorum tonsillitis. The patient presented with fever, neck swelling, and swallowing difficulty, accompanied by left-side tongue dev

Interferon-gamma (IFN-γ) and C-X-C motif chemokine ligand 9 (CXCL9) have been implicated as potential biomarkers for haemophagocytic lymphohistiocytosis (HLH); however, which of the two cytokines is better for the diagnosis and monitoring of HLH has not been clarified. We systematically evaluated both markers in HLH and control cohorts. While both IFN-γ and CXCL9 showed marked elevation in HLH patients, a subset (17.0%, 9/53) exhibited normal IFN-γ levels, whereas CXCL9 demonstrated near-universal positivity (98.1%, 52/53) across HLH subtypes. In the differential diagnosis of HLH versus non-Ep

Viral strategies to antagonize the robust host innate immune response have a major function in the pathogenicity of viral infection and virus-associated cancers. Epstein-Barr virus (EBV) infection causes infectious mononucleosis (IM) and several human cancers. While latent EBV can reactivate in some nasopharyngeal carcinoma (NPC) cells, the impact of EBV reactivation on the anti-viral innate immune and immunotherapy response of NPC patients remains incompletely understood. Here, we reveal the function of the EBV-encoded BRRF2 protein as a pivotal regulator of the host immune system. We show th

Infectious mononucleosis (IM) is a clinical entity caused by Epstein-Barr infection. Serious complications during the acute phase of the infection are rare, and very few cases with bilateral peritonsillar abscesses (PTAs) have been reported to date. A 23-year-old man presented to our emergency department with fever, dysphagia, and trismus that were unresponsive to prescribed antibiotics. Physical examination revealed a bilateral PTA. The patient was admitted to the otolaryngology department, where surgical drainage of both abscesses was performed by incision. The diagnosis of bilateral PTA can

BACKGROUND: Epstein-Barr virus (EBV) is the causative agent for infectious mononucleosis (IM), a diagnosis classically made in adolescence and young adulthood. IM is uncommon later in life generally because of previous exposure. When the disease does affect advanced age groups, diagnostic challenges exist and delays to appropriate treatment may occur.
CASE PRESENTATION: An 86-year-old man presented to the emergency department for evaluation of severe generalized weakness for greater than one week. Significant findings in the ED consisted of thrombocytopenia, elevated liver function testing, a

BACKGROUND: Epstein-Barr virus (EBV) is linked to a wide range of diseases, including infectious mononucleosis, hemophagocytic lymphohistiocytosis, and several malignancies. Quantitative EBV-DNA testing via polymerase chain reaction (PCR) is essential for diagnosis, but conventional testing using whole blood faces logistical challenges in resource-limited regions. Dried blood spot (DBS) samples offer a low-cost, stable alternative for EBV-DNA detection.
METHODS: Paired whole blood and DBS samples were collected from seven patients at Shinshu University Hospital, Japan, who were undergoing EBV

BACKGROUND: Infectious mononucleosis (IM) is an acute self-limited disease caused mainly by Epstein-Barr virus (EBV) that is prone to be missed or misdiagnosed in febrile children due to a lack of obvious clinical symptoms. Establishing a model based on blood cell parameters for identifying patients with IM is important for the subsequent diagnosis and treatment of children with fever. METHODS: From January to December 2023, data of a total of 5511 febrile children (including 518 IM patients with primary EBV infection) were collected from Children's Hospital Zhejiang University School of Medic
