Assessment of the causal influence of herpesvirus infections on the risk of depression phenotypes: A Mendelian randomization study.

OBJECTIVE: Previous findings of associations between herpesvirus infections and depression were mixed and did not implicate causality. This study used two-sample Mendelian randomization (MR) to clarify the causal impact of four symptomatic herpesvirus infections (shingles, chicken pox, cold sores, and mononucleosis) on three depression phenotypes, including depressive symptoms (DS), number of episodes with depressed mood (NEDM), and major depressive disorder (MDD). METHODS: Eligible SNPs were selected as instrumental variables (IVs). MR estimates were primarily based on the inverse variance w

Acute Liver Failure: Is Acetaminophen the Only Culprit?

Epstein-Barr virus (EBV) is a common herpesvirus associated with infectious mononucleosis and rare complications such as hepatitis. EBV-associated hepatitis during acute infection may alter liver metabolism, compounding the risk of drug-induced toxicity. We report the case of a 16-year-old female with acute EBV infection who developed severe acetaminophen toxicity after reported use for migraine relief. Her condition was refractory to N-acetylcysteine but improved with the addition of fomepizole, which may have mitigated liver injury by reducing N-acetyl-p-benzoquinone imine (NAPQI) production

Acute Epstein-Barr Virus Hepatitis With Cholestatic Jaundice and Hyperferritinaemia: A Case Report.

Epstein-Barr virus (EBV) is a common human pathogen often associated with infectious mononucleosis (IM), a typically self-limiting illness. EBV can affect the liver, with manifestations ranging from asymptomatic hepatitis to acute liver failure, particularly in immunocompromised individuals. Cholestatic jaundice and marked hyperferritinaemia are rare in EBV hepatitis, with ferritin levels correlating with disease severity. We report a case of EBV hepatitis in a 50-year-old immunocompetent man presenting with cholestatic jaundice, pharyngitis, cervical lymphadenopathy, and hyperferritinaemia (f

A Rare Case of Epstein-Barr Virus Hepatitis and Rash in an Adolescent.

Infection from Epstein-Barr virus (EBV) manifests with a diverse spectrum of presentations. One common manifestation is hepatobiliary involvement with hepatitis, which in most cases is mild and self-limited. Severe or fulminant hepatitis can potentially occur, though this is rare, particularly in immunocompetent pediatric populations. We present a previously healthy female adolescent with severe abdominal pain and jaundice, but no lymph node or tonsillar swelling. The workup revealed a marked elevation in liver enzymes, bilirubin, and mesenteric adenitis. A positive polymerase chain reaction (

A Rare Case of Acalculous Cholecystitis Secondary to Infectious Mononucleosis.

Infectious mononucleosis (IM) is a syndrome characterized by malaise, headache, lymphadenopathy, pharyngitis, rash, enlarged spleen, and fever. Epstein-Barr virus (EBV) is the most common cause of IM, though other organisms, such as cytomegalovirus (CMV), can cause it as well. Complications include transient hepatitis, splenic rupture, peritonsillar abscess, and other organ damage. This case study illustrates a case of acalculous cholecystitis caused by IM - a rare complication - with 44 EBV-associated acute acalculous cholecystitis (AAC) cases reported, and far fewer cases of CMV-associated A

Primary Epstein-Barr Virus Infection Presenting as Cholestatic Hepatitis Without Infectious Mononucleosis in an Immunocompetent Middle-Aged Adult: A Case Report.

Epstein-Barr virus (EBV) infections commonly present with a subclinical hepatitis, often in the context of infectious mononucleosis (IM). A 56-year-old male presented with fever, alongside signs of jaundice and right upper quadrant tenderness. Initial investigations revealed marked cholestatic hepatic dysfunction, but imaging showed no evidence of biliary tree pathology. Serological and molecular testing confirmed primary EBV infection, whilst excluding other viral and autoimmune causes of hepatitis. The patient was managed conservatively, with rapid normalisation of liver function during outp

Epstein-Barr virus (EBV)-induced acute acalculous cholecystitis in children: a case report.

BACKGROUND: The clinical manifestations of Epstein-Barr virus (EBV) infection are usually infectious mononucleosis, chronic active EBV infection, and related hemophagocytic lymphohistiocytosis. The incidence of non-calculous cholecystitis in children is not high, and non-calculous cholecystitis in children caused by EBV infection remains a rarity, which is often overlooked in clinical practice. The purpose of this study was to describe the clinical characteristics, laboratory tests, imaging findings, and treatment outcomes of a 9-year-old boy. CASE DESCRIPTION: This case details a 9-year-old

Epstein-Barr Virus Encephalitis as a Cause of Status Epilepticus - A Rare Encounter.

Epstein-Barr virus (EBV) is associated with infectious mononucleosis and is known to cause various neurological complications, albeit rarely. EBV encephalitis is a rare manifestation, and even rarer is its presentation as status epilepticus. To date, fewer than ten cases of EBV encephalitis presenting as status epilepticus have been reported globally. We report a case of an 18-year-old girl who presented with fever followed by seizures. She went into status epilepticus the next day. Cerebrospinal fluid (CSF) analysis showed lymphocytic predominance. Brain imaging showed bilateral symmetric hyp

Longitudinally Extensive Transverse Myelitis (LETM) and Meningoencephalitis Following Acute Epstein-barr Virus Infection in an Immunocompetent Male: Case Report and Review Literature.

INTRODUCTION: Epstein-Barr Virus (EBV) causes heterophile-positive Infectious Mononucleosis (IM), which manifests fever, sore throat, lymphadenopathy, and atypical lymphocytosis. In the Central Nervous System (CNS), EBV can cause acute encephalitis, cerebellar ataxia, Acute Disseminated Encephalomyelitis (ADEM), myelitis, meningitis, and radiculopathy. Reports of acute transverse myelitis linked to EBV infection are limited; therefore, Longitudinally Extensive Transverse Myelitis (LETM) due to EBV infection is extremely uncommon. Concurrent pneumonia and meningoencephalitis in an immunocompetent host: an atypical manifestation of Epstein-Barr virus.

Epstein-Barr virus (EBV) is a ubiquitous pathogen, with an estimated over 90% of adults worldwide having been exposed to it. EBV primarily affects immunocompromised individuals, with the most common presentation as infectious mononucleosis characterised by fever, sore throat, lymphadenopathy and fatigue. While these manifestations are well recognised, EBV-associated pneumonia and meningitis are rare complications, often posing diagnostic challenges due to their atypical presentation. Here, we present a male patient in his sixth decade with a 30-day history of prevailing symptoms, presenting wi

Scrub Typhus Masquerading As Infectious Mononucleosis: A Case Report.

Scrub typhus is a tick-borne disease caused by the intracellular organism Orientia tsutsugamushi. It typically presents with the cardinal "triad" of fever, rash, and eschar, along with other nonspecific symptoms. We report a case of scrub typhus in a 74-year-old man who did not exhibit the typical rash. He presented to the emergency room with a one-week history of generalized symptoms, including fever, throat pain, and myalgia, and was admitted due to suspected cholangitis based on elevated liver enzymes. However, computed tomography (CT) and magnetic resonance cholangiopancreatography (MRCP)

Non-antibiotic-Associated Petechial Rash in an Infectious Mononucleosis-Positive Patient: A Case Report.

Epstein-Barr virus (EBV) is the primary cause of infectious mononucleosis (IM). A cutaneous manifestation is typically observed after the administration of penicillin. We present a case of an otherwise healthy 18-year-old male patient with IM who developed a peripheral cutaneous petechial rash despite no recent exposure to penicillin. He exhibited classic signs of IM, including fever, fatigue, and pharyngitis. Ultrasound imaging demonstrated splenomegaly. He was confirmed to have IM via EBV antigen testing. Additional evaluation returned negative results for Chlamydia trachomatis, Neisseria go

Cohabitating in the City: A Case of Hemolytic Anemia in a Patient Coinfected With Babesiosis, Lyme Disease, and Mononucleosis.

In the past decades, the number of tick-borne illnesses reported in the United States has increased exponentially, leading to significant human morbidity and mortality. Among tick-borne illnesses, the most frequent in the United States is Lyme, a disease caused by the spirochetal bacteria Borrelia burgdorferi. Coinfection with Lyme disease and Babesia spp. is common among ticks, rodent reservoirs, and human hosts. At an early stage, both Lyme disease and babesiosis present with flu-like symptoms, such as fever, myalgias, fatigue, headache, and malaise. Due to the nonspecific presentation of th

Association of Epstein-Barr virus genomic alterations with human pathologies.

Epstein-Barr virus (EBV) infects >90% of humans and is associated with both hematological and epithelial malignancies. Here, we analyzed 990 EBV genomes (319 newly sequenced and 671 from public databases) from patients with various diseases to comprehensively characterize genomic variations, including single nucleotide variations (SNVs) and structural variations (SVs). Although most SNVs were a result of conservative evolution and reflected the geographical origins of the viral genomes, we identified several convergent SNV hot spots within the central homology domain of EBNA3B, the transactiva

Epstein-Barr virus-associated infectious mononucleosis exhibits substantially higher non-treponemal test titers in biological false-positive reactions.

OBJECTIVES: Biological false-positive (BFP) reactions in rapid plasma reagin (RPR) tests can complicate syphilis screening interpretation. Although Epstein-Barr virus-associated infectious mononucleosis (EBV-IM) is a known cause of BFP reactions, the characteristics of RPR titers in such cases remain unclear. This study aimed to compare the RPR titers in BFP reactions between patients with EBV-IM and those with other underlying conditions. METHODS: We retrospectively analyzed the RPR and Treponema pallidum hemagglutination assay (TPLA) results of all tests performed at a community hospital in
