The case for an Epstein-Barr virus vaccine: Lessons from its link to systemic lupus erythematosus.

Growing evidence has strengthened the longstanding hypothesis that Epstein-Barr virus (EBV) contributes directly to the development of systemic lupus erythematosus (SLE). While earlier work established epidemiologic associations and immune dysregulation in EBV-exposed individuals, recent multi-omic and single-cell studies now demonstrate mechanistic pathways that link viral infection to autoreactive B-cell activation. Notably, EBV has been shown to preferentially infect CD27+ CD21- memory B cells enriched for nuclear antigen-specific receptors, reprogramming them into potent antigen-presenting

Catastrophic ENT Complications of Pediatric Infectious Mononucleosis: Recent Experience at a Tertiary Pediatric Hospital.

Background: Epstein-Barr virus-related acute pharyngotonsillitis is common in children and adolescents and is generally managed successfully with positive outcomes by both ENT specialists and pediatricians. However, a variety of acute, life-threatening complications can occur, including upper airway obstruction and infectious or immune-mediated sequelae. Methods: This paper describes our recent experience with four pediatric patients presenting with severe ENT manifestations of infectious mononucleosis (IM) that led to life-threatening complications, all of whom were hospitalized and managed a

Plasma extracellular vesicle proteomic analysis identifies WARS as a potential biomarker of infectious mononucleosis.

BACKGROUND: Infectious mononucleosis (IM), caused by Epstein-Barr virus (EBV) infection, is a prevalent disease among children and young adults and may lead to a range of severe complications. Proteomic analysis of extracellular vesicles (EVs) has shown potential for identifying novel biomarkers and therapeutic targets. Consequently, elucidating the proteome signature of plasma EVs in IM may provide insights into pathological mechanisms of this disease, enabling improved diagnostic accuracy and targeted therapeutic development. METHODS: In this study, we employed tandem mass tag (TMT)-labeled

Microbial-Engineered Fluorescent Nanocarrier for Ion Ratiometric Sensing and sTREM-1-Mediated EBV Inhibition.

Epstein-Barr virus (EBV), a widespread human γ-herpesvirus, is associated with infectious mononucleosis and several malignancies, posing a major public health challenge. Acyclovir (ACV), the first-line antiviral drug, is limited by poor solubility, short half-life, and low bioavailability. To overcome these drawbacks, we designed a hyaluronic acid-based nanocarrier modified with microbial extracts and a fluorescent small molecule (1-HA-2), and constructed an ACV-loaded nanosystem (1-HA-2@ACV) with improved drug loading, fluorescence responsiveness, and aqueous stability, enabling both therapeu

Incidence age is bimodal for myalgic encephalomyelitis/chronic fatigue syndrome, with higher severity burden for early onset disease.

Myalgic Encephalomyelitis, or Chronic Fatigue Syndrome (ME/CFS), is a disease of uncertain origin. Studies of Norwegian health records have suggested that ME/CFS incidence across age groups is bimodal-a characteristic that could provide insight into the aetiology of the disease. Here, we analysed survey data from over 9000 respondents with ME/CFS from 10 European countries, and observe an early onset peak with a mean of 16.0 years old (standard deviation [SD]: 4.3) and a late onset peak at 36.6 years old (SD: 10.5). Statistical support for multimodal onset age was evident in 7 of the 10 countr

Comparing ME/CFS following mononucleosis with Long COVID.

ObjectivesLong COVID following SARS-CoV-2 and Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) following infectious mononucleosis (IM) are examples of post-infectious chronic illnesses. Behavioral and pathophysiological underpinnings of both ME/CFS following IM and Long COVID are not well understood.MethodsWe studied ME/CFS development following IM in a diverse group of college students who were enrolled before the onset of IM. We categorized those meeting either moderate or severe ME/CFS criteria. We subsequently recruited a matched sample of those infected with SARS-CoV-2, some of

Beyond the Usual Suspects: Emerging Associations Between Epstein-Barr Virus Infection/Infectious Mononucleosis and Cancers.

Epstein-Barr virus (EBV) is a ubiquitous herpesvirus and a causal factor for Burkitt Lymphoma (BL), Hodgkin Lymphoma (HL), gastric carcinoma (GC) and nasopharyngeal carcinoma (NPC). Whether EBV contributes to a wider spectrum of cancers remains uncertain. We reviewed MEDLINE, Embase and Web of Science on 30th July 2024 to identify observational studies that examined the association between EBV infection or EBV-infectious mononucleosis (IM) and cancers beyond BL, HL, GC and NPC. Evidence was appraised using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) framework.

Atraumatic Splenic Rupture as the Sole Manifestation of Infectious Mononucleosis: A Case Report and Literature Review.

Atraumatic splenic rupture is an uncommon but life-threatening complication of Epstein-Barr virus (EBV) infection. While infectious mononucleosis classically presents with fever, pharyngitis, and lymphadenopathy, we recorded a spontaneous splenic rupture even in the absence of these symptoms. We report a case of a 26-year-old male presenting to the emergency department with syncope. A progressive clinical hemodynamic deterioration was detected, ending hemorrhagic shock. Imaging revealed an atraumatic grade V splenic injury and massive hemoperitoneum. Laboratory tests showed lymphocytosis, prom

Case Report: Recurrent pulmonary infections with neck masses: a diagnostic challenge and literature review.

This article presents a case characterized by recurrent pulmonary infections and a poor therapeutic response. Initially, cystic fibrosis (CF) or Primary ciliary dyskinesia was suspected; however, the patient subsequently sought medical attention for a neck mass. The mass was initially misdiagnosed as lymphoma, but a comprehensive examination and assessment ultimately confirmed the diagnosis of activated phosphoinositide 3-kinase delta syndrome type I(APDS1). Through the patient's tortuous diagnostic journey, this report aims to enhance clinicians' understanding of this rare primary immunodefic

Chronic Active Epstein-Barr Virus Disease Causes Tubulointerstitial Nephritis and Growth Failure.

BACKGROUND: Chronic active Epstein-Barr virus (EBV) disease is a rare, potentially life-threatening condition characterized by the clonal proliferation of EBV-infected T or natural killer cells. Although it often manifests with systemic symptoms as seen with infectious mononucleosis, it rarely involves the kidneys or growth failure. CASE: A 9-year-old female patient presented with fever, malaise, anemia, kidney dysfunction and growth retardation. The initial diagnosis was acute tubulointerstitial nephritis based on a kidney biopsy, but further investigation, which included a polymerase chain

Nontranslocation follicular lymphoma mimicking infectious mononucleosis: a diagnostic challenge in a 39-year-old male.

Follicular lymphoma is an indolent B-cell non-Hodgkin lymphoma typically characterized by the t(14;18)(q32;q21) translocation resulting in B-cell lymphoma 2 (BCL2)/immunoglobulin heavy chain (IGH) fusion. Nontranslocation variants are rare and may present with atypical clinical features that complicate diagnosis. The patient's presentation initially resembled infectious mononucleosis due to a false-positive monospot test; however, subsequent workup revealed follicular lymphoma lacking the canonical BCL2/IGH translocation. Extensive infectious, autoimmune, and nutritional evaluation was unrevea

Case Report: Hepatitis and autoimmune hemolytic anemia induced by EBV-associated infectious mononucleosis.

Epstein-Barr virus (EBV) typically causes infectious mononucleosis, but in rare cases, it may lead to complications such as autoimmune hemolytic anemia (AIHA) and hepatitis. We report the case of an 18-year-old previously healthy female who presented with jaundice, red urine, and arthralgia. Laboratory workup revealed direct hyperbilirubinemia, elevated transaminases, and a direct antiglobulin test positive for IgG (negative for C3d), consistent with warm autoimmune hemolytic anemia in the context of EBV infection. Imaging ruled out biliary obstruction. The patient was managed conservatively,

Two-Trace Two-Dimensional (2T2D) Correlation Spectroscopy of Lymphocytes in Infectious Mononucleosis.

Raman spectroscopy has been widely demonstrated as a powerful tool for analyzing macromolecules characteristic of living organisms. This research focuses on a detailed spectroscopic analysis of lymphocytes affected by infectious mononucleosis caused by Epstein-Barr virus (EBV, HHV-4). As shown, at least 98% of the population is infected with EBV, making it one of the most common viruses. The main aim of this study is to evaluate the capability of Raman spectroscopy to distinguish between healthy lymphocytes from those infected with EBV at an early stage of infection. Blood samples were collect

Atypical presentation of Epstein-Barr virus infectious mononucleosis with bilateral dacryoadenitis in an elderly patient: a case report.

BACKGROUND: Epstein-Barr virus (EBV) infection is a highly prevalent condition and the primary cause of infectious mononucleosis (IM), typically affecting adolescents and young adults. Although IM is generally self-limited, EBV may involve multiple organ systems, presenting with atypical or localized manifestations. Ocular involvement is uncommon and includes conditions, such as conjunctivitis, uveitis, and, more rarely, dacryoadenitis. Acute dacryoadenitis remains an infrequent manifestation of EBV infection and is most commonly reported in younger patients; its occurrence in elderly patients

Increased Expression of CXCL9, CXCL10, and CXCL11 in Epstein-Barr Virus-Associated Infectious Mononucleosis and the Role of CXCL5 as a Candidate Biomarker of Disease Severity.

BACKGROUND: Epstein-Barr virus (EBV)-associated infectious mononucleosis (IM) elicits a robust cellular immune response; however, systemic chemokine profiles in pediatric IM and their diagnostic relevance remain insufficiently characterized. This study evaluated proinflammatory chemokine expression in children with acute EBV-associated IM and its relationship with disease presence and severity. METHODS: In this retrospective study, 64 children with confirmed acute EBV-associated IM and 16 healthy controls were included. Clinical severity was classified using the Severity of Mononucleosis (SOM

Age-Dependent Clinical Patterns of Primary Epstein-Barr Virus Infection in Children: Insights for Diagnostic Accuracy.

Primary Epstein-Barr virus (EBV) infection in children exhibits substantial clinical heterogeneity, often complicating early diagnosis and leading to unnecessary antibiotic use. This retrospective study evaluated 695 children (0-18 years) diagnosed with primary EBV infection at a tertiary pediatric center between 2010 and 2015, defined by positive viral capsid antigen (VCA) IgM and negative Epstein-Barr nuclear antigen (EBNA) IgG. Clinical, laboratory, and ultrasonographic findings were compared according to age group (≤4 vs. >4 years) and clinical setting (inpatient vs. outpatient). The media

A novel FcγRIIa-TRIM54-STAT2 axis negatively regulates type I interferon signaling and promotes viral susceptibility in human monocytes.

UNLABELLED: While adaptive humoral immunity and type I interferon (IFN-I) responses are known to cross-regulate each other during viral infection, the mechanisms by which antibodies (Abs) or immune complexes (ICs) modulate IFN-I signaling in immune cells remain unclear. Here, using plate-coated human IgG (cIgG) as an antigen-independent IC mimic, we show that FcγRIIa cross-linking in human blood monocytes induces the expression of TRIM54, an E3 ubiquitin ligase previously thought to be constitutively expressed only in muscle tissue. Mechanistically, TRIM54 binds to and promotes the ubiquitinat
