Biology of drug-induced acute nephrotoxicity: focus on the tubulointerstitium.

Drug-induced nephrotoxicity is a common and important cause of acute and chronic kidney injury. The kidneys receive ∼25% of the cardiac output exposing them to high drug concentrations. In addition, the kidneys actively participate in drug metabolism and excretion, thereby further increasing their susceptibility to drug-related toxic effects. Any compartment of the renal parenchyma such as the glomerulus, vasculature, tubules, and interstitium may be affected; however, the renal tubulointerstitium is the most frequent target. Several factors including patient's comorbidities, genetic predispos

Anti-inflammatory effects of ceragenin CSA-13 in acute pyelonephritis -insights from an animal model.

A hyperactive innate inflammatory response and oxidative stress are the primary contributors to kidney damage in acute pyelonephritis (APN). Consequently, effective APN management requires strategies that target both the bacterial etiology and the host immune response. Here, we report that ceragenin CSA-13, a cholic acid derived antimicrobial compound, is highly effective in treating Escherichia coli-induced model of APN in athymic mice. Specifically, we found that type 17 immunity plays a central role in host defense against E. coli infection, and it can be significantly suppressed by CSA-13

Lumpy Skin Disease Virus Infected Calves Showing Multisystemic Vasculitis on Postmortem Examination: A Summary of Six Cases.

Lumpy skin disease (LSD) is a rapidly spreading transboundary viral disease of cattle and water buffalo that poses a significant threat to livestock health and economies of Bangladesh. Calf mortality is steadily increasing over time. This study documented fatal calf mortality with vasculitis-driven multisystemic pathology, which has been rarely reported in Bangladesh. To investigate the rising incidence of calf mortality in Bangladesh, this study conducted a pathological investigation of six deceased calves and molecular analyses of the viruses. Clinically affected calves in north-central Bang

A case of chronic Campylobacter fetus infection-related glomerulonephritis, mimicking systemic autoimmune diseases.

Herein, we present the first case of chronic Campylobacter fetus infection-related glomerulonephritis (IRGN) in a 69-year-old man with a permanent cardiac pacemaker. The patient had a prior episode of fever and glomerulonephritis of undetermined etiology, and at that time, low-dose steroid therapy resulted in improved urinary findings and kidney function. This time the patient again developed deterioration of kidney function with microhematuria, proteinuria, high serum C-reactive protein levels, and positive titers of anti-double-stranded DNA antibody and proteinase 3 anti-neutrophil cytoplasm

Salvianolic Acid A Protects Against Angiotensin II-Induced Hypertensive Kidney Disease by Targeting TLR4 and Inhibiting TLR4/Myd88/PI3K/AKT/NF-κB Pathway.

Hypertensive nephropathy (HN), a significant consequence of chronic hypertension, remains a leading contributor to end-stage renal disease. Excessive Angiotensin II (Ang II)-driven inflammation, apoptosis, and fibrosis are central to its pathogenesis. Salvianolic acid A (SAA), a polyphenolic compound from Salvia miltiorrhiza, has anti-inflammatory properties, but its therapeutic potential in HN is unclear. HN was induced in mice by continuous subcutaneous infusion of Ang II (1.44 mg/kg/day) for 28 days. SAA (10 or 20 mg/kg/day) was administered orally every day during the final 14 days. Networ

Sodium glucose transporter 2 inhibition maintains kidney antibacterial response by decreasing complement C1q.

INTRODUCTION: Glucose promotes bacterial growth. Sodium-glucose transporter 2 inhibition (SGLT2i), which prevents glucose recovery from the urine, is standard therapy in chronic kidney diseases. However, kidney bacterial infection rates did not increase. Here, we investigated possible underlying mechanisms contributing to this effect. METHODS: We studied patients and a mouse model of pyelonephritis with and without inhibition of urinary glucose reabsorption using primary murine and human cell culture. Analysis involved a combination of histology, flow cytometry and gene expression analysis.

Taking the "Нет данных" out of CKDu-optimizing approaches to uncover the cause(s) of epidemic-level kidney disease in low- and middle-income settings: a report from the ISN's International Consortium of CKDu Collaborators (ISN i3C).

Chronic kidney disease of undetermined etiology (CKDu) is a major cause of mortality that affects communities, health systems, and national economies. Despite 2 decades of descriptive and analytical research, the causes of CKDu epidemics remain unclear. In February 2025, at the World Congress of Nephrology, the International Society of Nephrology's International Consortium of CKDu Collaborators met to discuss the obstacles to determining the cause(s) of the epidemics. Several key themes were identified: First, differences in CKDu burden between regions are poorly described, and this is complic

JAG1 of all trades, master of CKD? The role of JAG1 in autosomal dominant tubulointerstitial kidney disease.

The current study by Menguy et al. describes JAG1 as a novel candidate gene for the spectrum of autosomal dominant tubulointerstitial kidney disease. Patients have been shown to develop fibrosis and chronic kidney disease, when distinct pathogenic germline variants occur in JAG1. Interestingly, genetic alterations in the Jagged1/Notch2 pathway can cause the variable appearance of Alagille syndrome, which may also occur in the very same families. Haploinsufficiency is the likely pathomechanism, but future work will need to define the underlying mechanisms.

A case of tubulointerstitial nephritis with infiltration of neutrophils and interleukin-17-positive cells associated with Behçet's disease.

Behçet's disease (BD) is a non-infectious inflammatory condition characterized by neutrophilic infiltration. In addition to primary symptoms, including oral and genital ulcers, ocular involvement, and skin lesions, BD can also affect various organs. However, renal involvement, particularly in tubulointerstitial nephritis, has rarely been described. Herein, a rare case of acute tubulointerstitial nephritis in a patient clinically diagnosed with BD is reported. The renal lesion presented with other symptoms of BD and fever, and was considered to be BD-related due to the presence of neutrophilic

Shear wave elastography as a reliable tool in the prediction of renal histopathological abnormalities.

BACKGROUND: Chronic kidney disease (CKD) is a global health burden with irreversible progression and cardiovascular risk. Renal biopsy, while the gold standard for assessing kidney pathology, is invasive. Supersonic shear imaging (SSI) elastography is a promising non-invasive tool for quantifying tissue stiffness, potentially reflecting underlying fibrosis. This study aimed to assess the diagnostic value of SSI elastography for predicting renal histopathological abnormalities based on tissue stiffness and to evaluate its correlation with biopsy findings. METHODS: This cross-sectional study inc

Diabetes mellitus with anti-glomerular basement membrane disease and anti-tubular basement membrane disease: a rare case report of reversible dialysis dependence.

We present an instructive case of an elderly woman with a history of diabetes and diabetic retinopathy who was admitted due to rapidly progressive renal failure requiring dialysis. Although diabetic nephropathy was initially suspected, the patient's rapid clinical decline, significant hematuria, and proteinuria inconsistent with typical diabetic nephropathy prompted further serological evaluation and renal biopsy. Pathological examination revealed the rare coexistence of anti-glomerular basement membrane disease and anti-tubular basement membrane-associated interstitial nephritis. Following ag

Vedolizumab induced acute interstitial nephritis.

Vedolizumab is a gut selective anti-integrin monoclonal antibody with a very good and proven safety profile in the treatment of ulcerative colitis (UC). This case report highlights a rather unusual complication that every gastroenterologist ought to be aware of. It relates to the association of acute interstitial nephritis with vedolizumab usage in the management of UC. A 53-year-old man with liver cirrhosis due to Primary Sclerosing Cholangitis and UC was treated with vedolizumab following failed therapy with thiopurines and mesalazine. This led to improvement in his bowel function but his re

The accuracy of ultrasound viscoelastic imaging in evaluating renal fibrosis and interstitial inflammation of chronic kidney disease.

BACKGROUND: Accurate evaluation of renal fibrosis and interstitial inflammation grades is necessary to improve the prognosis of patients with chronic kidney disease (CKD). However, non-invasive and accurate diagnostic methods are lacking. In this regard, shear wave elastography, which measures tissue viscosity and elasticity, may be a promising alternative. Therefore, this study aimed to evaluate the diagnostic performance of individual viscoelastic parameters, combined viscoelastic parameter models, and combined clinical parameter models in assessing the grades of renal fibrosis and interstit

Renoprotective approaches against anthracycline nephrotoxicity.

Nephrotoxicity is a significant adverse effect of many cancer chemotherapies, often limiting treatment success and complicating clinical management. The kidneys are essential for removing antineoplastic drugs and their metabolites through glomerular filtration and tubular secretion. Unfortunately, both traditional cytotoxic agents and newer targeted therapies can harm various parts of the nephron, including the renal vasculature. This damage can present clinically as proteinuria, hypertension, electrolyte imbalances, glomerulopathies, acute and chronic interstitial nephritis, acute kidney inju

Urinary CXCL-9 outperforms TNF-α for non-invasive diagnosis of immune checkpoint inhibitor-associated acute interstitial nephritis.

BACKGROUND: Although kidney biopsy remains the gold standard for diagnosing immune checkpoint inhibitors-associated acute interstitial nephritis (ICI-AIN), several promising urinary biomarkers have emerged as potential non-invasive diagnostics. Notably, recent studies have highlighted tumor necrosis factor alpha (TNF-α), CXC motif chemokine ligand 9 (CXCL-9) and others as potential biomarkers to help distinguish ICI-AIN from other causes of acute kidney injury (AKI). However, a direct systematic comparison between these two biomarkers in a cohort of patients undergoing immunotherapy has not be

High-dimensional spatial proteomics and novel machine learning pipeline identifies disease specific renal damage states.

Lupus nephritis (LuN) and renal allograft rejection (RAR) manifest inflammation and fibrosis that ultimately lead to kidney failure. To quantitatively assess spatial injury patterns, we collected high dimensional spatial proteomics data from 23 LuN, 33 RAR, and 8 kidney control (KC) biopsies. We developed a computational pipeline to segment and classify tubules, capillaries, and glomeruli in whole-slide images using three trained neural networks (Renal Damage diagnosis, RDDx). RDDx achieved high accuracy and generalizability, reliably identifying small capillaries and differentiating tubular a
