Oxalate Nephropathy in a Patient With Chronic Pancreatitis and Recent Surgery: A Clinical Conundrum.

Calcium oxalate nephropathy is a rare condition with both primary and secondary causes. Primary hyperoxaluria, an inherited disorder, leads to liver oxalate overproduction, whereas secondary hyperoxaluria, or enteric hyperoxaluria, may be multifactorial and typically occurs during adulthood, with etiologies including increased dietary intake of oxalate, destruction of the microbiota in the gastrointestinal tract that break down oxalate, and malabsorptive disorders that increase serum oxalate levels. Calcium oxalate crystal deposition can cause irreversible kidney injury necessitating kidney re

Oxalate nephropathy in kidney transplant.

Kidney transplantation (KT) is the preferred treatment for end-stage renal disease, yet post-transplant complications and graft dysfunction remain significant challenges. Oxalate nephropathy, resulting from calcium oxalate crystal deposition due to hyperoxaluria, represents an under-recognised contributor to graft injury. We describe a woman in her 60s with obesity, chronic pancreatitis and type 2 diabetes who underwent KT after dialysis. Post-transplant, she developed recurrent pyelonephritis and impaired graft function. The biopsy revealed oxalate crystal deposits, tubular injury and interst

Emphysematous Pyelonephritis After Embolization of a Nonfunctioning Kidney Graft.

Emphysematous pyelonephritis (EPN) is a rare but severe infection with a high mortality rate characterised by gas formation within the renal parenchyma. Although primarily affecting native kidneys, EPN may exceptionally develop in nonfunctioning renal allografts. We report two cases of EPN occurring after therapeutic embolization of nonfunctioning kidney grafts because of graft intolerance syndrome (GIS). The first case involved a 50-year-old KT woman with a history of post-transplant diabetes and recurrent urinary tract infections, who developed EPN 3 months after embolization. Despite initia

Causes of morbidity and mortality in free-ranging Eurasian lynx (Lynx lynx) in Switzerland, 2000-2022.

Health monitoring based on post-mortem examination is essential for the management of endangered animal species. This is especially true for reintroduced species living in small populations with low genetic diversity, such as the Eurasian lynx (Lynx lynx) in Switzerland. Thanks to systematic post-mortem examinations, the Institute for Fish and Wildlife Health (FIWI), University of Bern, has acquired a comprehensive view of the lynx health in Switzerland. This study provides an updated overview of the causes of morbidity and mortality in the Eurasian lynx in Switzerland from 2000 to 2022. A tot

Tubulointerstitial inflammation in glomerular diseases: mechanistic pathways, prognostic value, and translational therapeutic targets.

Tubulointerstitial inflammation (TII) serves as a critical nexus connecting glomerular injury, interstitial fibrosis, and kidney function decline, and is commonly observed across various glomerular diseases. Pathogenic drivers and crosstalk along the glomerulus-tubule-interstitium axis promote a persistent interstitial inflammatory microenvironment, wherein chemokines, cytokines, and adhesion molecules activate specific pathways to recruit immune cells. Despite increasing attention, the independent prognostic significance of TII remains inconsistent across different diseases and populations. M

F18-FDG PET-CT in immune checkpoint inhibitor-associated acute interstitial nephritis: what is the diagnostic value?

BACKGROUND: Immune checkpoint inhibitor-associated acute interstitial nephritis (ICI-AIN) requires a kidney biopsy for a definitive diagnosis. A recent study suggested that 2-deoxy-2-[18F] fluoro-D-glucose positron emission tomography-computed tomography (F18-FDG PET-CT) may offer a noninvasive alternative, but lack of kidney biopsies in all patients and a proper control group receiving ICI therapy remained important limitations. We conducted the first study addressing these limitations and examined whether PET-CT could differentiate biopsy-proven ICI-AIN from other causes of acute kidney inju

Renal tubular acidosis complication of non-steroidal anti-inflammatory drugs induced interstitial nephritis and its complete resolution with steroids: A case report.

BACKGROUND: Non-steroidal anti-inflammatory drugs (NSAIDs) are extensively utilized for their analgesic and anti-inflammatory properties; however, their potential to induce renal adverse events remains a significant clinical concern. NSAID-induced interstitial nephritis (IN) and renal tubular acidosis (RTA) are rare but important complications of prolonged NSAID consumption. NSAID-induced RTA has been documented in limited case reports and usually resolves after stopping NSAIDs. However, our case shows a persistent, severe hypokalemia and hypophosphatemia linked to RTA from NSAID-induced IN, w

Lipocalin-2 Is Induced by Uromodulin Aggregates without Affecting Kidney Disease Progression in Autosomal Dominant Tubulointerstitial Kidney Disease- UMOD.

KEY POINTS: Lipocalin-2 upregulation paralleled intracellular uromodulin aggregates in mouse and cellular models of autosomal dominant tubulointerstitial kidney disease because of pathogenic uromodulin. The induction of lipocalin-2 was triggered by mutant uromodulin accumulation and endoplasmic reticulum stress in tubular cells. Genetic loss of lipocalin-2 reduced iron deposits in the kidney but did not affect kidney damage, indicating that lipocalin-2 was not driving disease progression. BACKGROUND: Autosomal dominant tubulointerstitial kidney disease due to pathogenic UMOD variants (ADTKD-

Predictive accuracy of SIRS, MEWS, and NEWS for intensive care unit admission to patients with stone-induced obstructive pyelonephritis: a retrospective cohort study.

Stone-induced obstructive pyelonephritis (siOPN) is a critical urological emergency that carries a high risk of rapid clinical deterioration. This study aimed to evaluate the predictive performance of the Systemic Inflammatory Response Syndrome (SIRS) criteria, the Modified Early Warning Score (MEWS), and the National Early Warning Score (NEWS) in identifying patients with siOPN who are at risk for intensive care unit (ICU) admission. This retrospective cohort study included 175 patients with radiologically confirmed siOPN. SIRS, MEWS, and NEWS scores were calculated at admission. Predictive a

Single-nucleus transcriptomics illuminates sex differences during murine Escherichia coli pyelonephritis.

There are profound sex differences in the prevalence and outcomes of urinary tract infections (UTI). While females comprise the majority of infections, males exhibit higher morbidity and mortality with upper-tract UTI. Correspondingly, preclinical modeling has demonstrated that male and androgen-exposed female mice are highly susceptible to severe high-titer pyelonephritis, a phenotype observed in < 20% of females. Here we subject kidneys from female, male, and androgen-exposed female C3H/HeN mice with pyelonephritis and PBS-exposed control mice to single-nucleus RNA sequencing, creating (to o

Presence of neutrophil extracellular traps (NETs) in different types of human urinary tract infections (UTI). A pilot study.

INTRODUCTION: Activated polymorphonuclear neutrophils (PMN) release neutrophil extracellular traps (NETs) composed of a web-like DNA core, concomitant with nuclear histones, granular peptides and enzymes. NETs in human urine and their potential role in human urinary tract infections (UTI) pathogenesis is still understudied. This pilot study aimed to analyze presence of NETs in urine samples of patients with different types of UTI. METHODS: Urine and blood samples were collected from three cohorts: group (A) included females (n = 24) with cystitis (n = 10), pyelonephritis (n = 6), and asymptom

Pyelonephritis Caused by Pseudomonas aeruginosa: From Treatable Infection to Therapeutic Dead-End, Rescued by an Investigational Antibiotic.

Pseudomonas aeruginosa is a major Gram-negative pathogen causing diverse infections, including complicated urinary tract infections (cUTIs). Although such cUTIs are usually treatable with standard antipseudomonal β-lactams, the rise of carbapenem-resistant P. aeruginosa, particularly those producing New Delhi metallo-β-lactamase (NDM), has made therapeutic options extremely limited. We report a 58-year-old man with diabetes, hypertension, and ischemic heart disease who presented with pyelonephritis in the setting of a long-retained ureteric stent. Urine culture grew P. aeruginosa resistant to

Monocyte Chemokines Enhance Atherosclerotic Plaque Necrosis After Bacterial Kidney Infection.

Cardiovascular event rate increases after acute infections, including urinary tract infections (UTI). Vascular inflammation is a major contributor to atherogenesis and plaque instability. The mechanistic pathway by which UTI impacts atherosclerosis has not been determined. Cardiovascular events were analyzed in a patient cohort, and atherosclerosis development was assessed in Ldlr-/- mice after a pyelonephritis (PN) episode. Inflammatory gene expression profiles of murine and human atherosclerotic vessels were analyzed. Monocyte mobilization was studied by Ccr2 ablation in mice and in human pr

Renal Diffuse Large B-cell Lymphoma Mimicking Acute Pyelonephritis on 18 F-FDG PET/CT.

A 58-year-old man with a history of lithotripsy for urolithiasis presented with persistent fever and pollakisuria. As empirical anti-infective therapy was ineffective, 18 F-FDG PET/CT was performed to detect potential malignancy, revealing bilateral renal enlargement with multiple nodular cortex uptakes and an enlarged left axillary lymph node, suggestive of pyelonephritis. However, biopsies of the axillary lymph node and right kidney confirmed high-grade diffuse large B-cell lymphoma (DLBCL). Following R-DA-EPOCH chemotherapy, the patient achieved sustained remission. This case highlights tha

Social Factors Associated with Ultrasound Versus Computed Tomography Utilization Among Children Diagnosed with Pyelonephritis in the Emergency Department.

RATIONALE AND OBJECTIVES: Acute pyelonephritis can complicate lower urinary tract infections (UTI) in children. While often unnecessary, imaging may be helpful if clinical symptomatology is atypical or complications are suspected. As social determinants of health (SDoH) can impact care, this study examines the correlational relationship between SDoH and computed tomography (CT) versus ultrasound (US) when imaging is used in pediatric patients with pyelonephritis in the emergency department. MATERIALS AND METHODS: A multivariate logistic regression analysis was performed using data from the Na

Transdiaphragmatic nephropleural fistula: A rare cause of empyema thoracic managed with single-stage surgical approach.

Empyema thoracic arising from a retroperitoneal source via a transdiaphragmatic nephropleural fistula is among its exceptionally rare causes. This report describes successful management with single-stage combined thoracic and urological surgery. A young female with long-standing nephrolithiasis and recent percutaneous nephrolithotomy, presented with respiratory distress. Initial management included emergent tube thoracostomy for a gross right pyopneumothorax confirmed by computed tomographic (CT) scan of chest. Escherichia coli was cultured from pleural fluid and urine samples. Further contras

Cost-effectiveness analysis of rapid antimicrobial susceptibility testing as a point of care for patients with uncomplicated urinary tract infection in Japan.

BACKGROUND: Empirical antibiotic therapy for patients with urinary tract infections (UTIs) has contributed to the rise of overuse or use of inappropriate antibiotics, which is a significant public health concern. The introduction of point-of-care diagnostic tests (POCTs) for the rapid determination of antimicrobial susceptibility is predicted to help reduce unnecessary broad-spectrum antibiotic use and produce actionable information for an approach to antibiotic stewardship. AIMS: To evaluate the cost-effectiveness of UTI diagnostic and treatment strategies, including a POCT in Japan. METHOD

Uncoupling glucosuria from infection: SGLT2 inhibition modulates kidney immune response during pyelonephritis.

Sodium-glucose cotransporter 2 inhibitors increase urinary glucose excretion, theoretically predisposing to urinary tract infection, yet clinical trials have not shown higher rates of severe infection. In this issue of Kidney International, Sendtner et al. provide a plausible mechanistic framework for this paradox. Combining human cohort data with experimental pyelonephritis, they demonstrate that sodium-glucose cotransporter 2 inhibition with empagliflozin is associated with reduced C1q expression, preserved antibacterial defenses, and altered inflammatory signaling. These findings suggest th

Piperacillin/Tazobactam-Induced Granulomatous Interstitial Nephritis in a Patient with Squamous Cell Lung Cancer Successfully Managed with Chemoradiotherapy.

Drug-induced acute interstitial nephritis (DI-AIN) is a common cause of acute kidney injury (AKI), particularly in patients receiving multiple medications. We report a case of AKI in a 47-year-old man with lung squamous cell carcinoma who was treated with carboplatin, paclitaxel, and tazobactam/piperacillin (TAZ/PIPC). A renal biopsy revealed acute tubulointerstitial nephritis with eosinophilic infiltration and non-caseating epithelioid granulomas. The drug lymphocyte stimulation test was positive for TAZ/PIPC, thus supporting a diagnosis of drug-associated DI-AIN. The discontinuation of TAZ/P
