BACKGROUND: Lupus nephritis (LN) is a significant cause of kidney morbidity in patients with systemic lupus erythematosus. While glomerular lesions are the primary focus of current classifications, tubulointerstitial involvement may significantly influence renal outcomes. There is limited data on tubulointerstitial inflammation (TII) and tubulointerstitial damage (TID) in Middle Eastern populations.
AIM: To examine the clinical, pathological, and prognostic significance of coexisting TII and TID in patients with biopsy-proven LN from Saudi Arabia.
METHODS: We retrospectively analyzed 100 pat

Proliferative lupus nephritis (LN) is triggered by deposition of autoantibodies in glomeruli and paralleled by a T cell-rich kidney infiltrate. Although these T cells have been attributed to the propagation of tissue injury, it is unclear how they are activated and whether T cell autoreactivity drives local inflammation. Kidney-infiltrating T cells are also observed in urine, where they have high resemblance to interstitial T cells. Therefore, urinary T cells are a proxy for investigating tissue pathogenesis. Here, we analyzed urinary T cells to elucidate whether a kidney-specific T cell autoi

BACKGROUND: Soluble urokinase plasminogen activator receptor (suPAR) is linked to organ damage in systemic lupus erythematosus (SLE). We investigated whether serum suPAR associates with histological activity and chronicity in diagnostic and 12-month repeat kidney biopsies in lupus nephritis (LN).
METHODS: suPAR levels were determined using ELISA in serum samples from 23 LN patients (19 incident) before and 12 months after initiation of immunosuppressive therapy, and from 15 age- and sex-matched healthy controls. Correlations with NIH activity index (AI) and chronicity index (CI) scores were a

Although rifampicin is one of the main drugs in the treatment of tuberculosis, its potential for causing nephrotoxic reactions may lead to renal injury. In this paper, we present a 23-year-old man who was treated for pulmonary tuberculosis and who developed a nephrotoxic syndrome caused by rifampicin. This syndrome was defined by abdominal distension due to fluid retention, swelling in both legs, nephrotic range proteinuria and biopsy findings showing tubular atrophy and interstitial fibrosis. The nephrotoxicity developed after a prolonged, uninterrupted course of treatment with rifampicin. Th

BackgroundTubulointerstitial lesions predict kidney prognosis in lupus nephritis (LN), but the molecular features of interstitial fibrosis remain incompletely defined. We previously showed that palladin, an actin-associated protein, drives kidney fibrosis through actin dynamics. This study examined whether interstitial palladin expression is associated with histopathology and prognosis in LN.MethodsWe retrospectively analyzed Japanese patients with biopsy-proven LN at Kanazawa University Hospital between 1990 and 2018. Palladin-positive area was quantified by immunohistochemistry and log2-tran

OBJECTIVES: To evaluate the prevalence, associated factors, and prognostic performance of NIH activity index–defined interstitial inflammation (NIH-TII) and interstitial fibrosis and tubular atrophy (IFTA) in lupus nephritis (LN). METHODS: This retrospective cohort study analyzed 195 renal biopsy episodes from 135 LN patients. Tubulointerstitial lesions were graded according to histopathological severity. NIH-TII was assessed semiquantitatively using the interstitial inflammation component of the modified NIH activity index. RESULTS: Among 195 renal biopsies, class IV LN was most frequent (49.

INTRODUCTION: Despite advances in lupus nephritis (LN) management, its heterogenous nature poses challenges in predicting treatment response. Multiplexed spatial proteomics offers a potential solution to dissect the heterogeneity of treatment response in LN.
METHODS: Renal response was assessed in 16 LN patients undergoing physician-directed induction therapy. Baseline biopsies from these patients were subjected to 34-plex spatial proteomics. A total of 53 glomerular and 49 tubulointerstitial regions of interest from these LN patients and renal cell carcinoma (RCC) controls were analyzed for

OBJECTIVES: Tuberculosis is a highly contagious disease that has a significant impact on global health. Emerging evidence suggests that tuberculosis can lead to an altered immune response. We investigated the association between tuberculosis and the onset of inflammatory arthritides (IA).
METHODS: Patients with incident tuberculosis in the South Korean National Claims database from 2010 to 2021 were included, and those who had undergone appendectomy during 2010-2011 served as controls. The onset of IA (including seropositive rheumatoid arthritis [SPRA], ankylosing spondylitis [AS], and psoria

INTRODUCTION: Human immunodeficiency virus (HIV)-associated kidney disease encompasses a diverse spectrum of glomerular, tubulointerstitial, and vascular pathologies. While HIV-associated nephropathy and immune complex-mediated glomerular disease in the setting of HIV are well-recognized entities, their simultaneous presentation with thrombotic microangiopathy (TMA) and diffuse infiltrative lymphocytosis syndrome (DILS) has not been previously documented. We report a novel case of advanced HIV infection manifesting as crescentic lupus-like glomerulonephritis (GN), CD8-predominant tubulointerst

Tubulointerstitial nephritis (TIN) with IgM-positive plasma cells (IgMPC-TIN) is an inflammatory disease characterized by the infiltration of IgM and CD138 dual-positive plasma cells into the renal interstitium. Steroid treatment is effective for many cases of IgMPC-TIN. However, the optimal dose and duration of steroid therapy remain poorly understood. In the present case, IgMPC-TIN was diagnosed in a 36-year-old woman with femoral head osteonecrosis. After administration of prednisolone (PSL, 20 mg/day), several markers of disease activity gradually decreased. The PSL dose was tapered to 5 m

OBJECTIVES: Snakebite envenomation is a major cause of acute kidney injury (AKI) in tropical regions. While acute tubular necrosis (ATN) and acute cortical necrosis have traditionally been described as predominant renal lesions, multiple factors, including recent improvements in early access to antivenom, may have altered the histopathological spectrum. This study aimed to evaluate renal histopathology in snakebite envenomation-associated AKI (SBE-AKI) and its clinicopathological correlates.
METHODS: In this prospective observational study conducted at a tertiary care centre in South India fr

Serum anti-phospholipase A2 receptor (PLA2R) antibody is widely used to diagnose primary membranous nephropathy (MN), but PLA2R positivity also occurs in secondary MN, especially nonsteroidal anti-inflammatory drug (NSAID)-induced MN, leading to a common diagnostic pitfall. We report a 71-year-old female with long-term unsupervised NSAID administration for polymyalgia rheumatica, presenting with recurrent nephrotic syndrome and acute kidney injury. Initial laboratory tests revealed severe hypoalbuminemia (15.7 g/L), elevated serum creatinine (3.98 mg/dL), massive proteinuria (10.24 g/24 h), an

Three clinical observations in patients with various manifestations of pyelonephritis are presented. Using 18F-FDG PET/CT and transmission electron microscopy of the renal parenchyma at 10,000x magnification, the ultrastructural and molecular-cellular features of acute purulent pyelonephritis, chronic pyelonephritis with neurogenic hypertension, and chronic pyelonephritis resulting in secondary contracted kidney are identified. The clinical significance of the identified changes, the reliability of the obtained results, and the prospects for their further application are discussed.

INTRODUCTION AND AIM: Immune-mediated nephropathies are a mechanistically diverse group of kidney disorders in which dysregulated innate and adaptive immunity drive sustained inflammation, tissue injury, and progressive fibrosis. The kidney is not a passive target of these processes but an active immunological organ, where crosstalk between resident parenchymal cells and infiltrating leukocytes shapes both initiation and progression of disease. Advances in immunogenetics and molecular pathology have allowed several previously idiopathic entities-lupus nephritis, IgA nephropathy, ANCA-associate

Acute kidney injury (AKI) caused by granulomatous interstitial nephritis (GIN) accompanied by chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) is extremely rare. A 71-year-old man with severe AKI was diagnosed with CLL/SLL based on bone marrow and lymph node biopsy results. A renal biopsy revealed dense cortical infiltration of CLL/SLL cells and severe interstitial inflammation with granuloma formation, consistent with GIN. Acalabrutinib-Obinutuzumab combination therapy was effective for CLL/SLL, but prednisolone was required to achieve a renal function improvement. A renal bi

Oxidative phosphorylation (OXPHOS) is the main source of cellular adenosine triphosphate (ATP) production and depends on proteins encoded by both mitochondrial and nuclear DNA (nDNA). Pathogenic variants affecting this dual genetic control cause primary mitochondrial disorders (MIDs), which follow either maternal inheritance when they affect mitochondrial DNA (mtDNA) or autosomal inheritance when they affect nuclear-encoded mitochondrial proteins. Once considered predominantly pediatric conditions, these disorders are increasingly recognized in adults where their clinical presentation is heter

BACKGROUND: Urolithiasis is a common disease associated with severe infectious complications such as acute obstructive pyelonephritis (AOP). This emergency requires antibiotic therapy and urinary drainage, with bladder and renal pelvic urine cultures (BUCap and PUCap, respectively) that are often discordant. However, microbiological evolution between AOP and delayed ureteroscopy remains poorly studied. This study evaluated the concordance between BUCap, PUCap, and preoperative bladder urine cultures before delayed ureteroscopy (poBUC).
METHODS: This retrospective single-center study included
