Uropathogens and prognosis among patients with hospital-diagnosed acute pyelonephritis: insights from a 19-year population-based cohort study.

PURPOSE: The microbial aetiologies of acute pyelonephritis (APN) may change over time. We aimed to describe long-term trends in microbiological diagnostics and pathogen distribution in patients with hospital-diagnosed APN, and to characterise clinical outcomes by pathogens. METHODS: We conducted a population-based, serial cross-sectional and cohort study of patients with hospital-diagnosed APN in North Denmark across three periods covering 2000-2018. National health registries were linked with microbiological data to describe temporal trends in microbiological diagnostics and pathogen distrib

A case of tubulointerstitial nephritis with infiltration of neutrophils and interleukin-17-positive cells associated with Behçet's disease.

Behçet's disease (BD) is a non-infectious inflammatory condition characterized by neutrophilic infiltration. In addition to primary symptoms, including oral and genital ulcers, ocular involvement, and skin lesions, BD can also affect various organs. However, renal involvement, particularly in tubulointerstitial nephritis, has rarely been described. Herein, a rare case of acute tubulointerstitial nephritis in a patient clinically diagnosed with BD is reported. The renal lesion presented with other symptoms of BD and fever, and was considered to be BD-related due to the presence of neutrophilic

Shear wave elastography as a reliable tool in the prediction of renal histopathological abnormalities.

BACKGROUND: Chronic kidney disease (CKD) is a global health burden with irreversible progression and cardiovascular risk. Renal biopsy, while the gold standard for assessing kidney pathology, is invasive. Supersonic shear imaging (SSI) elastography is a promising non-invasive tool for quantifying tissue stiffness, potentially reflecting underlying fibrosis. This study aimed to assess the diagnostic value of SSI elastography for predicting renal histopathological abnormalities based on tissue stiffness and to evaluate its correlation with biopsy findings. METHODS: This cross-sectional study inc

Diabetes mellitus with anti-glomerular basement membrane disease and anti-tubular basement membrane disease: a rare case report of reversible dialysis dependence.

We present an instructive case of an elderly woman with a history of diabetes and diabetic retinopathy who was admitted due to rapidly progressive renal failure requiring dialysis. Although diabetic nephropathy was initially suspected, the patient's rapid clinical decline, significant hematuria, and proteinuria inconsistent with typical diabetic nephropathy prompted further serological evaluation and renal biopsy. Pathological examination revealed the rare coexistence of anti-glomerular basement membrane disease and anti-tubular basement membrane-associated interstitial nephritis. Following ag

Vedolizumab induced acute interstitial nephritis.

Vedolizumab is a gut selective anti-integrin monoclonal antibody with a very good and proven safety profile in the treatment of ulcerative colitis (UC). This case report highlights a rather unusual complication that every gastroenterologist ought to be aware of. It relates to the association of acute interstitial nephritis with vedolizumab usage in the management of UC. A 53-year-old man with liver cirrhosis due to Primary Sclerosing Cholangitis and UC was treated with vedolizumab following failed therapy with thiopurines and mesalazine. This led to improvement in his bowel function but his re

The accuracy of ultrasound viscoelastic imaging in evaluating renal fibrosis and interstitial inflammation of chronic kidney disease.

BACKGROUND: Accurate evaluation of renal fibrosis and interstitial inflammation grades is necessary to improve the prognosis of patients with chronic kidney disease (CKD). However, non-invasive and accurate diagnostic methods are lacking. In this regard, shear wave elastography, which measures tissue viscosity and elasticity, may be a promising alternative. Therefore, this study aimed to evaluate the diagnostic performance of individual viscoelastic parameters, combined viscoelastic parameter models, and combined clinical parameter models in assessing the grades of renal fibrosis and interstit

Renoprotective approaches against anthracycline nephrotoxicity.

Nephrotoxicity is a significant adverse effect of many cancer chemotherapies, often limiting treatment success and complicating clinical management. The kidneys are essential for removing antineoplastic drugs and their metabolites through glomerular filtration and tubular secretion. Unfortunately, both traditional cytotoxic agents and newer targeted therapies can harm various parts of the nephron, including the renal vasculature. This damage can present clinically as proteinuria, hypertension, electrolyte imbalances, glomerulopathies, acute and chronic interstitial nephritis, acute kidney inju

Always Consider a Repeat Kidney Biopsy: Acute Interstitial Nephritis Soon After Membranous Nephropathy.

INTRODUCTION: The heterogeneity of membranous nephropathy is well described in the literature, and its clinical course and response to treatment vary. Similarly, acute interstitial nephritis (AIN) can present in unexpected and unusual ways and should always be considered within the differential diagnosis of worsening renal function. This case study describes the cross-section of these two disease entities. CLINICAL CASE: A 68-year-old male with a history of hypertension, chronic obstructive pulmonary disease (COPD), hyperlipidaemia, gout and treated prostate cancer presented with bilateral lo

Urinary CXCL-9 outperforms TNF-α for non-invasive diagnosis of immune checkpoint inhibitor-associated acute interstitial nephritis.

BACKGROUND: Although kidney biopsy remains the gold standard for diagnosing immune checkpoint inhibitors-associated acute interstitial nephritis (ICI-AIN), several promising urinary biomarkers have emerged as potential non-invasive diagnostics. Notably, recent studies have highlighted tumor necrosis factor alpha (TNF-α), CXC motif chemokine ligand 9 (CXCL-9) and others as potential biomarkers to help distinguish ICI-AIN from other causes of acute kidney injury (AKI). However, a direct systematic comparison between these two biomarkers in a cohort of patients undergoing immunotherapy has not be

High-dimensional spatial proteomics and novel machine learning pipeline identifies disease specific renal damage states.

Lupus nephritis (LuN) and renal allograft rejection (RAR) manifest inflammation and fibrosis that ultimately lead to kidney failure. To quantitatively assess spatial injury patterns, we collected high dimensional spatial proteomics data from 23 LuN, 33 RAR, and 8 kidney control (KC) biopsies. We developed a computational pipeline to segment and classify tubules, capillaries, and glomeruli in whole-slide images using three trained neural networks (Renal Damage diagnosis, RDDx). RDDx achieved high accuracy and generalizability, reliably identifying small capillaries and differentiating tubular a

The triglyceride-glucose index as an independent associate of severe tubulointerstitial fibrosis and a predictor of renal survival in IgA nephropathy.

BACKGROUND: To investigate the association of the triglyceride-glucose (TyG) index with severe tubular atrophy/interstitial fibrosis (T2 lesions) and renal outcomes in patients with IgA nephropathy (IgAN). METHODS: We retrospectively analyzed 1,791 biopsy-proven IgAN patients between October 2014 and September 2024. Receiver operating characteristic (ROC) curve analysis assessed TyG index performance for predicting T2 lesions. Multivariable logistic regression and restricted cubic spline (RCS) models were applied to assess the relationship between the TyG index and T2 lesions. Renal survival

An Atypical Presentation of Tubulointerstitial Nephritis With Uveitis (TINU) in an Older Patient Requiring Emergency Dialysis: A Case Report.

Tubulointerstitial nephritis with uveitis (TINU) is an immune-mediated systemic disorder most commonly described in adolescents and young adults, and its recognition in older patients remains challenging. We report the case of a 58-year-old woman who presented with systemic symptoms and rapidly progressive acute kidney injury. Renal function deteriorated significantly, necessitating emergency hemodialysis. Renal biopsy demonstrated acute tubulointerstitial nephritis with diffuse interstitial inflammation and minimal fibrosis. During hospitalization, ocular symptoms were identified, and a compr

Urinary Chemokines in the Diagnosis and Monitoring of Immune Checkpoint Inhibitor-Associated Nephritis.

Immune checkpoint inhibitors are essential treatments for many oncologic diseases, but with well-known immune-related adverse events, such as acute interstitial nephritis (ICI-AIN). We investigated novel potential biomarkers that could assist in the diagnosis and follow-up of this condition and that are related to the active pathogenic pathways involved. We measured urinary soluble PD-1, PD-L1 and PD-L2, as well as chemokines CXCL5, CXCL9, CXCL10, CXCL11, CCL2, CCL3, CCL5 and cytokines IL-6 and IL-12p70 performing a Luminex assay in urine from patients with ICI-AIN (n = 35) and compared them w

Sequential Multiple-Organ Immune-Related Adverse Events during and after Pembrolizumab Therapy in Triple-Negative Breast Cancer: A Case Report.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) are an important component of triple-negative breast cancer (TNBC) treatment, and the KEYNOTE-522 regimen has established pembrolizumab combined with chemotherapy, as the standard of care. While ICIs improve outcomes, they can also induce immune-related adverse events (irAEs), affecting diverse organs. However, renal, pancreatic, and ocular toxicities are rare, and there have been few reports of sequential multi-organ irAEs in breast cancer. CASE PRESENTATION: A 63-year-old woman with stage IIA TNBC was treated with the KEYNOTE-522 regimen. Du
