Immune Profiling Identifies High-Risk Neutrophil-Rich Subtype in Checkpoint Inhibitor Nephritis.

INTRODUCTION: Immune checkpoint inhibitor (ICI)-induced acute interstitial nephritis (AIN) (ICI-AIN) is the leading cause of ICI-associated acute kidney injury (AKI). ICI-AIN is characterized by mononuclear immune infiltrates, although the mechanisms behind their toxicity remain unclear. We aimed to characterize these infiltrates in kidney biopsies and assess their correlation with clinical outcomes and therapeutic responses. METHODS: We retrospectively analyzed 49 biopsy-proven ICI-AIN cases using multiplex immunofluorescence to quantify immune cells (macrophages, neutrophils, B cells, T cel

Pyelonephritis decreases serum cholesterol and mitigates atherosclerosis severity despite systemic inflammation.

Hypercholesterolemia and inflammation are main causes of cardiovascular disease. Urinary tract infections are common and frequently recur. We here tested how pyelonephritis affects atherosclerotic plaque development and lipid levels. LDL receptor-deficient (Ldlr-/-) and wild-type mice were infected with uropathogenic Escherichia coli. Renal and systemic inflammation, lipid levels, and atherosclerotic plaque development were assessed. Gene regulation was studied in pyelonephritis and in human cells in vitro. In patients admitted with urinary tract infections, serum lipids and disease severity w

[Influencing factors and development of a predictive model for the progression of glomerular injury in children with Henoch-Schönlein purpura nephritis].

OBJECTIVES: To investigate the influencing factors for the progression of glomerular injury in children with Henoch-Schönlein purpura nephritis (HSPN) and to develop a predictive model. METHODS: A total of 259 children with HSPN admitted to the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2022 to December 2024 were retrospectively enrolled. Based on renal biopsy pathology reports, the children were classified into a low-grade group (grade Ⅰ-Ⅱ glomerular injury; n=61), and a high-grade group (grade Ⅲ-Ⅵ glomerular injury; n=198). Clinical indicators

Where is the data? Delayed and chronic irAE surveillance and management after cessation of ICIs: expert insights from SITC on survivorship care and the need for long-term data.

Immune checkpoint inhibitors (ICIs) have revolutionized cancer therapy, offering durable responses and prolonged survival. However, these therapies also present unique challenges, particularly with the onset of immune-related adverse events (irAEs), which can manifest during treatment either acutely and/or become chronic or emerge long after treatment cessation. Delayed, chronic, and re-emergent irAEs often require tailored survivorship care, including coordination across multiple disciplines focused on oncology, specialty care, and primary care. Despite the increased usage of ICIs, there is l

TNF-α blockade mitigates immune checkpoint-related nephritis in a humanized mouse model.

Immune checkpoint inhibitors (ICIs) can cause immune-related adverse events (irAEs), with acute interstitial nephritis (ICI-AIN) being the most common irAE. While the exact mechanism remains unclear, upregulation of IFN-γ and TNF-α pathways has been implicated. This study used a humanized chimeric PD-1/PD-L1 mouse model to assess renal effects of ICIs, alone or combined with proinflammatory cytokines, and to test if selective TNF-α blockade could prevent ICI-AIN. Mice were randomly divided into 4 experimental groups: Control, ICI-Only, ICI-Cytokines (ICI-Cyt), and ICI-Block (ICI-TNF-α blockade

Kidney pathology findings in pediatric patients with kidney injury and inflammatory bowel disease: a case series.

BACKGROUND: Patients with inflammatory bowel disease (IBD) are at risk of kidney pathologies such as IgA nephropathy (IgAN) and tubulointerstitial nephritis (TIN). Data on kidney abnormalities and treatments in pediatric IBD patients with kidney disease are limited. This study describes kidney pathology, treatments, and outcomes in pediatric IBD patients who underwent kidney biopsy at our center. METHODS: We conducted a single-center retrospective case series at a quaternary pediatric hospital in Toronto, Canada. Pediatric IBD patients who underwent kidney biopsy between June 2018 and Februar

Clinical significance of renal histological diagnosis in patients with type 2 diabetes and proteinuria atypical for diabetic nephropathy.

BACKGROUND/AIM: Most patients with proteinuria are considered to have typical diabetic nephropathy (DN). However, when proteinuria occurs without diabetic retinopathy, with hematuria, or persists despite strict glycemic and blood pressure control, it is considered atypical for DN and warrants further evaluation for non-DN via kidney biopsy. Nevertheless, comprehensive information on renal histopathology and prognosis following kidney biopsy remains limited. This study aimed to clarify these issues through a retrospective analysis. METHODS: Among 490 patients who underwent kidney biopsy at Shi

Evaluation of the relationship between the severity of interstitial fibrosis in diagnostic renal biopsies and disease progression in primary glomerular diseases.

This single-center retrospective cohort study divided 176 adults with biopsy-proven primary glomerulonephritis (GN) between 2014 and 2021 into 4 groups based on cortical interstitial fibrosis (IF) percentage. The primary outcome was disease progression, defined as a ≥40% decline in estimated glomerular filtration rate (eGFR) and/or the need for dialysis. The secondary outcome was complete remission, defined as proteinuria <300 mg/24 hours. Baseline proteinuria and serum creatinine increased with greater IF severity, while eGFR decreased. Kaplan-Meier analysis showed a higher risk of progressio

An update on the epidemiology of chronic kidney disease of uncertain etiology.

PURPOSE OF REVIEW: Chronic kidney disease of uncertain etiology (CKDu) has devastating impacts on individuals and communities in affected regions, but it remains an ill-defined diagnosis of exclusion despite decades of environmental and epidemiologic research. The purpose of this review is to synthesize recent literature describing the global burden of disease and potential causal factors. RECENT FINDINGS: A large, international prevalence study quantitatively identified Uddanam, India, Northwest Nicaragua, and Anuradhapura, Sri Lanka as prominent CKDu hotspots. Stronger evidence for the role

Oxalate Nephropathy in a Patient With Chronic Pancreatitis and Recent Surgery: A Clinical Conundrum.

Calcium oxalate nephropathy is a rare condition with both primary and secondary causes. Primary hyperoxaluria, an inherited disorder, leads to liver oxalate overproduction, whereas secondary hyperoxaluria, or enteric hyperoxaluria, may be multifactorial and typically occurs during adulthood, with etiologies including increased dietary intake of oxalate, destruction of the microbiota in the gastrointestinal tract that break down oxalate, and malabsorptive disorders that increase serum oxalate levels. Calcium oxalate crystal deposition can cause irreversible kidney injury necessitating kidney re

Oxalate nephropathy in kidney transplant.

Kidney transplantation (KT) is the preferred treatment for end-stage renal disease, yet post-transplant complications and graft dysfunction remain significant challenges. Oxalate nephropathy, resulting from calcium oxalate crystal deposition due to hyperoxaluria, represents an under-recognised contributor to graft injury. We describe a woman in her 60s with obesity, chronic pancreatitis and type 2 diabetes who underwent KT after dialysis. Post-transplant, she developed recurrent pyelonephritis and impaired graft function. The biopsy revealed oxalate crystal deposits, tubular injury and interst

Emphysematous Pyelonephritis After Embolization of a Nonfunctioning Kidney Graft.

Emphysematous pyelonephritis (EPN) is a rare but severe infection with a high mortality rate characterised by gas formation within the renal parenchyma. Although primarily affecting native kidneys, EPN may exceptionally develop in nonfunctioning renal allografts. We report two cases of EPN occurring after therapeutic embolization of nonfunctioning kidney grafts because of graft intolerance syndrome (GIS). The first case involved a 50-year-old KT woman with a history of post-transplant diabetes and recurrent urinary tract infections, who developed EPN 3 months after embolization. Despite initia

Causes of morbidity and mortality in free-ranging Eurasian lynx (Lynx lynx) in Switzerland, 2000-2022.

Health monitoring based on post-mortem examination is essential for the management of endangered animal species. This is especially true for reintroduced species living in small populations with low genetic diversity, such as the Eurasian lynx (Lynx lynx) in Switzerland. Thanks to systematic post-mortem examinations, the Institute for Fish and Wildlife Health (FIWI), University of Bern, has acquired a comprehensive view of the lynx health in Switzerland. This study provides an updated overview of the causes of morbidity and mortality in the Eurasian lynx in Switzerland from 2000 to 2022. A tot

Tubulointerstitial inflammation in glomerular diseases: mechanistic pathways, prognostic value, and translational therapeutic targets.

Tubulointerstitial inflammation (TII) serves as a critical nexus connecting glomerular injury, interstitial fibrosis, and kidney function decline, and is commonly observed across various glomerular diseases. Pathogenic drivers and crosstalk along the glomerulus-tubule-interstitium axis promote a persistent interstitial inflammatory microenvironment, wherein chemokines, cytokines, and adhesion molecules activate specific pathways to recruit immune cells. Despite increasing attention, the independent prognostic significance of TII remains inconsistent across different diseases and populations. M

F18-FDG PET-CT in immune checkpoint inhibitor-associated acute interstitial nephritis: what is the diagnostic value?

BACKGROUND: Immune checkpoint inhibitor-associated acute interstitial nephritis (ICI-AIN) requires a kidney biopsy for a definitive diagnosis. A recent study suggested that 2-deoxy-2-[18F] fluoro-D-glucose positron emission tomography-computed tomography (F18-FDG PET-CT) may offer a noninvasive alternative, but lack of kidney biopsies in all patients and a proper control group receiving ICI therapy remained important limitations. We conducted the first study addressing these limitations and examined whether PET-CT could differentiate biopsy-proven ICI-AIN from other causes of acute kidney inju

Renal tubular acidosis complication of non-steroidal anti-inflammatory drugs induced interstitial nephritis and its complete resolution with steroids: A case report.

BACKGROUND: Non-steroidal anti-inflammatory drugs (NSAIDs) are extensively utilized for their analgesic and anti-inflammatory properties; however, their potential to induce renal adverse events remains a significant clinical concern. NSAID-induced interstitial nephritis (IN) and renal tubular acidosis (RTA) are rare but important complications of prolonged NSAID consumption. NSAID-induced RTA has been documented in limited case reports and usually resolves after stopping NSAIDs. However, our case shows a persistent, severe hypokalemia and hypophosphatemia linked to RTA from NSAID-induced IN, w

Lipocalin-2 Is Induced by Uromodulin Aggregates without Affecting Kidney Disease Progression in Autosomal Dominant Tubulointerstitial Kidney Disease- UMOD.

KEY POINTS: Lipocalin-2 upregulation paralleled intracellular uromodulin aggregates in mouse and cellular models of autosomal dominant tubulointerstitial kidney disease because of pathogenic uromodulin. The induction of lipocalin-2 was triggered by mutant uromodulin accumulation and endoplasmic reticulum stress in tubular cells. Genetic loss of lipocalin-2 reduced iron deposits in the kidney but did not affect kidney damage, indicating that lipocalin-2 was not driving disease progression. BACKGROUND: Autosomal dominant tubulointerstitial kidney disease due to pathogenic UMOD variants (ADTKD-
