A novel and two recurrent UMOD mutations in autosomal dominant tubulointerstitial kidney disease (ADTKD): a case series and literature review.

Autosomal dominant tubulointerstitial kidney disease (ADTKD) is a rare hereditary disorder characterized by slowly progressive loss of kidney function with absent or mild proteinuria and tubulointerstitial fibrosis. Mutations in the UMOD gene represent one of the primary genetic etiologies of ADTKD. This study presented the clinical data and genetic variant interpretation of three ADTKD patients diagnosed via a next-generation sequencing (NGS)-based diagnostic workflow at the Department of Nephrology, Zhejiang Provincial People's Hospital, along with a review of relevant literature. Genetic an

Atatürk's kidney disease: pyelonephritis or trench nephritis? A medical and historical analysis.

BACKGROUND: The aim of this study is to examine Atatürk's recurrent renal disease throughout his life in light of historical and medical evidence and to evaluate, from a medical history perspective, whether this condition is more consistent with pyelonephritis or with "trench nephritis," as described among soldiers during World War I. METHODS: This study was designed as a qualitative narrative review using document and content analysis. Primary and secondary sources obtained from the Archives of the Presidency of the Republic of Türkiye, the National Library, newspaper archives, physicians' r

Immunofluorescence Patterns in IgAN are Associated With Active, Necrotizing Lesions.

INTRODUCTION: IgA nephropathy (IgAN) is characterized by profound heterogeneity, yet therapeutic decisions often rely on a "1-size-fits-all" strategy. As the therapeutic landscape expands to include B-cell and complement-targeting agents, a better understanding of underlying disease mechanisms is increasingly important. This study aimed to identify distinct immunological endotypes and determine their associations with severe, active histological lesions - particularly glomerular necrosis - to guide precision medicine. METHODS: We performed a comprehensive retrospective analysis of 284 patient

[Environmental factors and tubulointerstitial nephropathy : the paradigm of «chronic interstitial nephritis in agricultural communities» (CINAC)].

Chronic Interstitial Nephritis in Agricultural Communities (CINAC) represents a major public health challenge, primarily affecting young to middle-aged agricultural workers. Clinically, CINAC is characterized by low-grade proteinuria, a bland urinary sediment, and rapidly progressive renal failure. Kidney biopsies disclose large dysmorphic lysosomes containing electron-dense non-membrane aggregates in proximal tubular cells. Although the disease was long attributed to recurrent episodes of dehydration, its pathophysiology is now recognized as more complex. Indeed, the heat-stress hypothesis al

Case Report: Sintilimab-associated acute interstitial nephritis in a patient with severe thrombocytopenia: clinical diagnosis and treatment strategy when renal biopsy is contraindicated.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have become one of the core drugs for the comprehensive treatment of advanced gastric cancer. As a representative domestic PD-1 inhibitor, sintilimab has shown significant efficacy in clinical application, but the management of immune-related adverse events (irAEs) remains a clinical challenge. Among them, the incidence of immune-related acute kidney injury (ir-AKI) is only 16.5%-17%, but it is mainly manifested as acute interstitial nephritis (AIN), which can progress to irreversible renal failure in severe cases. In particular, when ICIs are co

Acute kidney injury following household pyrethroid aerosol insecticide exposure: a case report and literature review.

BACKGROUND: Pyrethroids (PYRs) are increasingly detected in both environmental matrices and human populations. Emerging evidence underscores their potential for systemic toxicity. Biopsy-confirmed nephrotoxicity in humans remains sparsely documented, particularly in non-occupational exposure. We present a case of acute kidney injury (AKI) associated with household pyrethroid-containing aerosol insecticide exposure. CASE PRESENTATION: A 55-year-old previously healthy male presented with nausea and abdominal pain and was found to have AKI on laboratory evaluation. He denied prior medication use

Management of Xanthogranulomatous Pyelonephritis: A Review of Current Evidence and Treatment Strategies and a Proposed Clinical Management Algorithm.

Xanthogranulomatous pyelonephritis is an uncommon destructive inflammatory process of the renal parenchyma associated with urinary tract obstruction and infection. Preoperative diagnosis remains challenging with correct identification in less than half of cases. Management paradigms include immediate surgical intervention or staged approaches that emphasize preoperative optimization and percutaneous drainage. Radical nephrectomy remains the de facto definitive treatment though critical evidence gaps underscore the need for more rigorous study to guide management.

Light chain-mediated acute tubulointerstitial nephritis as the sole manifestation of monoclonal gammopathy of renal significance in a young adult.

Light-chain-mediated acute tubulointerstitial nephritis (LC-ATIN) is an uncommon, under-recognised lesion within the spectrum of monoclonal gammopathy of renal significance (MGRS). It typically presents with acute kidney injury (AKI), minimal proteinuria and nonspecific symptoms, complicating diagnosis and risking progression to end-stage kidney disease. We report a man in his early 40s with unexplained AKI whose diagnosis was established after kidney biopsy demonstrating kappa-restricted tubular basement membrane deposits on immunofluorescence and electron microscopy. Bone marrow evaluation i

Clinical value of repeat kidney biopsy in pediatric IgA vasculitis nephritis.

BACKGROUND: For pediatric patients with IgA vasculitis nephritis (IgAVN), therapeutic decisions during relapse are often guided by empirical adjustments based on clinical manifestations, lacking timely pathological evidence. We aimed to investigate the dynamic evolution of kidney pathology in children with IgAVN and evaluate the clinical utility of repeat kidney biopsy in treatment decisions. METHODS: Retrospective analysis was conducted on 30 children with IgAVN who underwent repeat kidney biopsy. We summarized demographic characteristics, clinical relapse features, and indications for repea

Targeting the Disease Response With NlpD and LytM for Effective Nonantibiotic Treatment of Urinary Tract Infections.

BACKGROUND: Finding new ways of treating bacterial infections is essential. The NlpD protein, which inhibits RNA polymerase II (Pol II), has shown therapeutic efficacy against urinary tract infection. This study investigated the mechanism of Pol II inhibition and protection by NlpD and its LytM peptide. METHODS: Recombinant NlpD and LytM were screened for interactions with constituents of the Pol II complex, using AlphaFold predictions and protein interaction technology. Treatment effects were quantified in infected tissues and regulated host response pathways identified by genome-wide transc

Proximal Tubule‑Derived Semaphorin 3C Promotes Fibrosis in Adenine-Induced Tubulointerstitial Nephritis.

KEY POINTS: Proximal tubule‑derived semaphorin 3C functioned as a promoter of inflammation and fibrosis in adenine-induced tubulointerstitial nephritis. Semaphorin 3C acted on renal fibroblasts to enhance the expression of profibrotic markers and to promote proliferation. Thus, proximal tubule‑derived semaphorin 3C may represent a potential therapeutic target in tubulointerstitial nephritis. BACKGROUND: Semaphorin 3C (SEMA3C) is a secreted protein that is essential for cardiovascular and renal development; however, its role in the development and progression of CKD remains unclear. METHODS:

[Interrelationship between immunological and oxidant disorders in acute gestational pyelonephritis at different trimesters of pregnancy].

INTRODUCTION: Urinary tract infection in pregnant women is one of the most common and formidable causes of miscarriage. The topic of gestational pyelonephritis originates from the high prevalence (up to 10%) and the unresolved nature of a number of pathogenesis problems, namely early and timely diagnostics, accurate assessment of the severity of the inflammatory process, the effectiveness and efficiency of treatment, and the prevention of complications. Despite the great successes of practical medicine in the diagnosis and therapy of acute inflammatory kidney diseases in pregnant women, this a

Causal relationship between diabetes and urinary tract infections: A Mendelian randomization study.

Diabetes mellitus (DM) is associated with many infectious diseases, but the relationship between diabetes and urinary tract infections remains controversial. Therefore, we investigated, by Mendelian randomization (MR), the causal relationship between type 1 DM (T1DM), type 2 DM (T2DM), and 3 common urinary tract infections at different sites. We evaluated the causal relationship between T1DM and T2DM and pyelonephritis, cystitis, and urethritis in a two-sample MR study, using the inverse variance weighted (IVW) method for the primary analysis and MR-Egger and weighted median estimation methods

Tubulointerstitial diseases: an updated framework for diverse and emerging entities.

Tubulointerstitial diseases represent a heterogeneous group of kidney diseases with diverse causes and overlapping histopathologic and immunophenotypic patterns. Unlike glomerular disease, tubulointerstitial diseases have lacked specificity and standardized classification, but greater diagnostic precision is now possible and is necessary as nephrology enters an era of personalized medicine. This review provides an updated etiologic and attributive framework for tubulointerstitial nephritis/nephropathy (TIN) and tubular injury with 8 categories: drug effect, autoimmune/immune-mediated, infectio
