Over a 15-year period we observed seven children (four girls, three boys) who presented within the first months of life with severe renal failure and acidosis, associated with hypertension in five patients and polyuria in four. In addition, one patient had a severe cholestatic liver disease. In two families, a similarly affected sibling had died previously. Four patients were referred with the clinical diagnosis of polycystic kidney disease because of moderate enlargement of kidneys, but renal imaging (intravenous pyelography and ultrasonography) did not confirm this diagnosis. A renal biopsy,

We studied the possibility that tubule fluid iron could be involved in the pathogenesis of the tubulo-interstitial injury associated with primary glomerular disease. Tubule fluid iron is determined by the magnitude of the glomerular leak for transferrin and the iron saturation of transferrin. To minimize tubule fluid iron in an experimental model of glomerulonephritis, iron deficiency was induced in rats prior to the induction of nephrotoxic serum nephritis. Iron deficiency did not effect the development of glomerular disease as determined by proteinuria, but had a marked effect on preventing

Renal involvement during Still's disease in the adult is rarely mentioned in the literature. Proteinuria and hematuria are frequently reported during systemic involvement in the disease but, conversely, observations including an anatomical account of the kidney are rare: amyloidosis is mentioned the most often (5 compatible cases), while other cases are more disparate: non specific glomerulitis (4 cases), glomerulonephritis with mesangial deposition of IgA (2 cases), tubulo-interstitial nephritis; these different non specific aspects may correspond to an immune complex disease. A case of subacute necrotizing encephalomyelopathy (SNE; Leigh's disease) in an infant aged 19 months is reported. In addition to the typical central nervous lesions, hyperlactatemia and hyperpyruvatemia and morphological changes in the kidney were documented and have been identified as a tubulo-interstitial disease with a focal sclerosing glomerulopathy.

Although acute tubulo-interstitial nephritis is increasingly recognized as a cause of acute renal failure, little is known about renal tubular function in this disease. We report on two patients with acute tubulo-interstitial nephritis who demonstrated abnormalities in proximal and distal tubular function. The first patient developed hyperkalemia presumably from a potassium secretory defect in the distal nephron. The second patient developed an incomplete Fanconi's syndrome with glycosuria and aminoaciduria and two heretofore unreported complications of acute interstitial nephritis: hypokalemi

The clinical course of a woman with systemic lupus erythematosus demonstrated that renal involvement in this disease can change, both in the predominant site of renal injury and in the pattern of glomerular lesions. Over 50 months she had first tubulo-interstitial lupus nephritis with no glomerular change, then a membranous nephropathy with disappearance of tubulo-interstitial inflammation, and finally crescentic glomerulonephritis. This case emphasizes the need for close follow-up of all lupus patients regardless of their initial renal manifestations and pathology.

To test the hypothesis that immune complex deposition can mediate tubulo-interstitial damage in lupus nephritis, a correlative immunofluorescent (IF) and morphometric study of 43 renal biopsies from 39 patients with diffuse proliferative lupus glomerulonephritis was done. The point-counting method was used to determine the percentage of the cortical tubulo-interstitial system occupied by the mononuclear cell infiltrates (infiltrate index) and by the interstititum (interstitial index). The estimated amount of deposits containing immunoglobulin along tubular basement membranes and within the int

Anatomical and immunological studies were performed in two brothers with membranous glomerulonephritis. The older child presented with renal failure, Fanconi syndrome and anti-TBM antibody in his plasma. Renal biopsy revealed severe tubulo-interstitial disease with membranous glomerulonephritis. Because of rapidly progressive renal insufficiency the patient was started on hemodialysis. A cadaver renal allotransplantation was performed without success. Renal transplant biopsy showed severe lesions of vascular rejection without recurrence of the primary disease. The younger child was examined at

A 34 year old female developed impaired function of her renal allograft 21 months post-transplant. This was associated with lethargy, pyrexia, tenosynovitis, pancytopenia and a colonic ulcer. Severe tubulo-interstitial changes with intranuclear inclusion bodies and intracytoplasmic herpes type viral particles were seen on renal biopsy. There was no evidence of rejection. Cytomegalovirus was cultured from the urine and there was a rise in CMV antibody titer. These findings suggested the renal impairment was due to a direct cytopathic effect of the CMV. Despite treatment with transfer factor and

Experimental models of immunologically induced tubulo-interstitial nephritis (TIN) in rats are described. Antitubular basement membrane (anti-TBM) nephritis was demonstrated to be a TIN resulting from an antigen antibody reaction. Renal transplant rejection and the local graft-vs-host (GVH) reaction were cell-mediated immune reactions in which the antigen was possibly shared by kidney structures. A cellular immune reaction to locally applied antigens was shown to be the basis for another experimental model of TIN, possibly originating from activated mediator systems.

In 18 patients who received renal allografts, tubulo-interstitial lesions were studied, according to the types of rejection or the possible recurrence of primary disease. Immunofluorescent (IF) investigation revealed tubular basement membrane (TBM) deposits of IgG and/or C3 and sometimes other components of Complement. The pattern of these deposits was: "linear" in 3 cases, "granular" in 9 cases and "atypical" with linear and granular segments in 6 cases. The IF study of own kidneys of 3 patients, before the transplantation, revealed no TBM deposits. TBM deposits were accompanied by arterial w

To assess the role of circulating immune complexes in the pathogenesis of acute rejection, sera were measured for such complexes by the (125)I-C1(q) binding assay in 45 normal subjects, 24 allografted patients undergoing acute rejection, and in 11 allografted patients in a quiescent phase. Increased C1(q)-binding activity (C1(q)-BA) was detected in 14 patients with acute rejection, 9 of whom had renal biopsies showing fibrin deposition in the vasculature together with cellular infiltrates in the tubulo-interstitial structures; renal histology was not available in the other 5 patients.
