Cardiorenal Benefits of SGLT2 Inhibitors in Patients with Chronic Kidney Disease and Concomitant Hypertension.

BACKGROUND: Uncontrolled hypertension is both a driver of chronic kidney disease (CKD) and a complication of the disease, as well as a risk factor for cardiovascular disease (CVD). Therefore, renal protective agents with antihypertensive properties are desirable for management of cardiorenal syndrome in CKD. Sodium-glucose cotransporter-2 (SGLT2) inhibitors are emerging as a new class of renal protective agents, with robust efficacy in delaying progression of CKD and reducing cardiovascular events. Here, we present an overview of SGLT2 inhibitors and discuss the alternative mechanisms contribu

Special Situations in Argentina: Addressing the Challenges Faced by Individuals on Dialysis Without a Diagnosis.

The third major subgroup requiring admission to dialysis in Argentina are individuals with kidney failure without a known cause, also known as chronic kidney disease of unknown origin (CKDUO). In this retrospective cohort study, the characteristics of this group were described using data from the National Dialysis and Transplant Registry. In Argentina, 113,352 patients commenced maintenance dialysis from April 1, 2004, to December 31, 2021 (213 months); 42% of patients were older than 65 years of age and 2.3% were younger than 18 years of age. There was a predominance of male patients (66,650

Full recovery of vision following early and intensive hemodialysis in an 18-year-old woman with methylmalonic acidemia-related optic neuropathy.

BACKGROUND: Methylmalonic acidemia (MMA) caused by complete or partial deficiency of the mitochondrial enzyme methylmalonyl-CoA mutase (mut0 or mut- enzymatic subtype, respectively) leads to the accumulation of toxic organic acids, causing severe organ dysfunction and life-threatening complications. Despite appropriate treatment, optic neuropathy has been reported as a vision-threatening complication of MMA, often resulting in debilitating sequelae. CASE REPORT: We present the case of an 18-year-old woman with isolated mut° MMA and with chronic kidney disease stage 3 who presented with a rapi

Comparative Renal Safety of Besifovir Dipivoxil Maleate and Tenofovir Disoproxil Fumarate in Chronic Hepatitis B Patients: Insights From a Nationwide Cohort Study.

BACKGROUND: Besifovir dipivoxil maleate (BSV) is a novel antiviral agent approved in Korea for chronic hepatitis B (CHB) treatment. While previous comparative studies between BSV and tenofovir disoproxil fumarate (TDF) have suggested lower renal toxicity in BSV, these findings were limited by small sample sizes. This study aimed to comprehensively assess the incidence of chronic kidney disease (CKD) in CHB patients treated with BSV versus TDF using a nationwide cohort. METHODS: In this retrospective cohort study, we analyzed treatment-naïve CHB patients who initiated BSV or TDF therapy betwee

Pregnancy in chronic hemodialysis in a resource-limited country: case report and literature review.

BACKGROUND: Conception in hemodialysis is a rare event with an incidence of 0.5-1.4% of women of childbearing age, and associated with a high maternal-fetal risk. We report a case of full-term pregnancy with live child that occurred in our hemodialysis center at Kayes Hospital in Mali. CASE PRESENTATION: The patient was 38 years old and had been on chronic hemodialysis for 4 years for hypertensive nephropathy. She was dialyzed twice a week for 8 h. We discovered a pregnancy of 17 weeks' amenorrhea (WA) in a context of acute pelvic pain. Hemodialysis was immediately intensified to 3 times/week

Early-stage therapeutic efficacy of TNAP inhibition using a novel milder murine model of CKD-MBD.

Chronic kidney disease (CKD) is a complicated systemic disease displaying various pathophysiological symptoms including mineral bone disorder (CKD-MBD). Ideally, early intervention for CKD-MBD would be desirable, however, there is not enough evidence regarding treatment of CKD-MBD, especially in its early stages, due to its multifactorial pathophysiology and the difficulty in generating adequate animal models. In this study, we evaluated the efficacy of a tissue nonspecific alkaline phosphatase (TNAP) inhibitor, SBI-425 in a CKD-MBD animal model, produced by a combination of nephrectomy and hi

Misplacement of a femoral venous catheter into the ascending lumbar vein: A case report and first use of balloon catheter compression hemostasis.

RATIONALE: This case report aims to highlight a rare but life-threatening complication of femoral venous catheterization and to describe a novel endovascular technique for its management. Non-tunneled femoral catheters provide rapid vascular access for emergency dialysis (e.g., arteriovenous graft [AVG] occlusion, hyperkalemia) but carry risks of vascular injury, potentially causing fatal bleeding. This is the first report of hemorrhagic shock due to ascending lumbar vein rupture from femoral catheter misplacement, successfully managed by balloon compression hemostasis. PATIENT CONCERNS: A 48

Atypical morphological features in secondary and tertiary hyperparathyroidism mimic malignancy: a detailed clinicopathological study.

Patients with secondary or tertiary hyperparathyroidism due to chronic renal failure who fail medical treatment require sub-total or total parathyroidectomy. The histological characteristics of parathyroid glands resected in this setting are not well studied; however, there is awareness that the findings may mimic malignancy. We conducted a detailed clinicopathological study of parathyroid resection specimens from an unselected Royal North Shore Hospital (RNSH) cohort (n=110) and an external consultation cohort (n=44). The most common atypical features observed in the RNSH cohort were fibrous

Two Cases of Acquired Reactive Perforating Collagenosis in Hemodialysis Patients.

Acquired reactive proliferating collagen (ARPC), usually associated with systemic diseases, is a rare skin disease characterized by the elimination of altered dermal collagen through the epidermis. Here, we report two cases of ARPC in maintenance hemodialysis patients, one of which is combined with type 2 diabetes, chronic renal failure, and hypertension and the other is combined with chronic renal failure and hypothyroidism. Two patients were treated with oral thalidomide and ebastine, local application of fluticasone propionate cream, and enhanced dialysis. After treatment, the itching and t

A study of three cases of acquired reactive perforating collagenosis mimicking calciphylaxis.

BACKGROUND: Calciphylaxis, also termed calcific uremic arteriolopathy (CUA), is a rare and potentially fatal vasculopathic disorder predominantly affecting patients with end-stage kidney disease (ESKD), particularly those undergoing dialysis or who have recently received kidney transplantation. The hallmark histopathological features of CUA encompass medial calcification of systemic arterioles, intimal fibrosis, and thrombosis. In contrast, Acquired Reactive Perforating Collagenosis (ARPC), a rare perforating dermatosis frequently associated with systemic diseases, is characterized by the tran

A New Perspective on the Management of Giant Rectal Polyps Presenting With McKittrick-Wheelock Syndrome: A Case Report on Treatment With Transanal Excision and Delorme's Plication.

McKittrick-Wheelock syndrome is characterized by fluid deficit, electrolyte derangements, and acute kidney injury caused by a villous colorectal polyp. Treatment depends on both the location and size of the polyp. Most small polyps are treated by endoscopic resection and larger polyps by either surgical segmental resection or endoscopic excision. The most challenging polyps are those involving the lower rectum, often associated with rectal prolapse. Endoscopic excision is much more difficult, and surgical resection remains the predominant treatment option. We discuss an 83-year-old female with

Fisetin ameliorates vascular smooth muscle cell calcification via DUSP1-dependent p38 MAPK inhibition.

Medial vascular calcification is highly prevalent in advanced age and chronic kidney disease (CKD), where it is associated with increased risk for cardiovascular events and mortality. Vascular smooth muscle cells (VSMCs) actively regulate this process, which can be augmented by inflammation and cellular senescence. Thus, the present study investigated the impact of fisetin, a flavonol with anti-inflammatory and senolytic properties, on VSMC calcification. Fisetin treatment suppressed calcific marker expression and calcification of VSMCs as well as p38 MAPK phosphorylation induced by pro-calcif

The role of the intestinal microbiome in cognitive decline in patients with kidney disease.

Cognitive decline is frequently seen in patients with chronic kidney disease (CKD). The causes of cognitive decline in these patients are likely to be multifactorial, including vascular disease, uraemic toxins, blood-brain barrier leakage, and metabolic and endocrine changes. Gut dysbiosis is common in patients with CKD and contributes to the increase in uraemic toxins. However, the gut microbiome modulates local and systemic levels of several metabolites such as short-chain fatty acids or derivatives of tryptophan metabolism, neurotransmitters, endocannabinoid-like mediators, bile acids, horm

Dietary Phosphorus and Metabolic Health in CKD and ESKD.

The contribution of dietary phosphate (P) in the pathogenesis of CKD-associated mineral bone disease and the management of P intake in patients with CKD are essential to slow down disease progression and improve patient outcomes. In patients with CKD, and most likely in the general population, P retention and overload can affect four critical aspects of the cardiovascular system: increased arterial BP, vascular and valvular calcification, and left ventricular hypertrophy. All of these factors contribute to increased cardiovascular risk and mortality. Intestinal absorption of P from a mixed die

Research progress of the intestinal axis in autologous arteriovenous fistula stenosis in maintenance hemodialysis patients.

The number of the patients treated with maintenance hemodialysis (HD) is increasing due to the increasing incidence of end stage renal disease (ESRD). Autologous arteriovenous fistula (AVF) is the preferable modality for long-term vascular access during HD. AVF stenosis is the main cause of AVF dysfunction in HD patients, but its mechanism has not been fully elucidated. Patients with ESRD often have various related complications due to intestinal microbiota disorders and their metabolites, and the intestinal axis reveals various metabolic disorders in patients with chronic kidney disease. This

Magnesium Balance in Chronic Kidney Disease: Mineral Metabolism, Immunosuppressive Therapies and Sodium-Glucose Cotransporter 2 Inhibitors.

It is now widely recognized that maintaining magnesium (Mg) homeostasis is critical for health, especially in the context of chronic kidney disease (CKD). Patients with CKD commonly develop hyperphosphatemia and secondary hyperparathyroidism, which are controlled by therapies targeting intestinal phosphate absorption and circulating calcium levels or by modulating parathyroid calcium sensing. Notably, Mg supplementation may provide dual benefits by promoting bone formation and maintaining normal mineralization with slightly elevated serum levels. Importantly, low Mg levels are associated with
