Chronic Kidney Disease in Common Variable Immunodeficiency: a Multicenter Study.

PURPOSE: There are few reports of renal involvement in Common Variable Immunodeficiencies (CVID) and, when present, is due to infections, inflammation, or treatments. The aim of this study was evaluating the prevalence of chronic kidney disease (CKD) and to identify CVID-related clinical, laboratory and therapeutic features inducing it. METHODS: A multicenter observational retrospective study on 367 adult CVID patients from five Italian Referral Centers for Primary Immunodeficiency. RESULTS: CKD was identified in 23 (6.27%) patients that were older (p < 0.001), had arterial hypertension (p <

Gut dysbiosis and brain microhemorrhages in young vs. aged mice with chronic kidney disease.

Intestinal dysbiosis and gut-derived toxins in chronic kidney diseases (CKD) are associated with vascular injury. This study examined the relationship between gut dysbiosis and cerebral microhemorrhages (CMH) in young and aged CKD mice (3 vs. 16 months of age) in both sexes. CKD was induced in C57BL/6J mice using a nephrotoxic adenine diet. Serum creatinine, trimethylamine N-oxide (TMAO), indoxyl sulfate (IS) and p-cresyl sulfate (pCS) were measured. CMH was quantified via brain histology, and gut microbial sequencing was analyzed from fecal pellets. Creatinine and uremic toxins were elevated

Serum potassium is a critical risk factor for cardiac arrhythmias in acute heart failure patients treated with levosimendan: A retrospective study in a cohort of 250 patients.

BACKGROUNDPrimary laminopathies are a heterogeneous group of rare diseases caused by nuclear lamina dysfunction due to pathogenic LMNA variants. However, despite their ubiquitous expression, LMNA variants have rarely been linked to chronic kidney disease (CKD). Here, we systematically investigate clinical implications and functional underpinnings of a distinct LMNA missense variant [lamin A/C p.(Arg349Trp)] that has sporadically been found in patients with a complex phenotype including lipodystrophy, proteinuria, and focal segmental glomerulosclerosis (FSGS).METHODSIn clinical and functional t

Incidence of major urological cancers in patients on dialysis: a systematic review and meta-analysis.

BACKGROUND: Studies have demonstrated an elevated risk of urological malignancies in individuals undergoing dialysis, which consequently leads to unfavorable prognoses and diminished quality of life for patients with end-stage kidney disease. Nevertheless, the absence of standardized recommendations for cancer screening and limited utilization of conventional screening methods within the dialysis population remain prevalent issues. METHODS: A meta-analysis was conducted on cohort studies published prior to June 2024, aiming to quantify the cancer risk among individuals undergoing dialysis. Ra

Ankle-brachial index trajectory before death in patients receiving maintenance hemodialysis.

BACKGROUND: The trajectory of the ankle-brachial index (ABI) over time in relation to death has not been fully described in patients undergoing hemodialysis. We modeled ABI trajectory pattern before death in patients undergoing maintenance hemodialysis. METHODS: Patients undergoing hemodialysis in a dialysis facility were retrospectively enrolled and categorized into two groups, deceased and surviving groups, based on their status at the end of the observation period. Linear mixed-effect model and a backward timescale from the year of death or study end date were used to compare ABI trajector

Gut alterations in a chronic kidney disease rat model with diet-induced vascular calcification.

Intestinal disorders and vascular calcification (VC) are often associated with chronic kidney disease (CKD). While gut barrier alterations have been reported in CKD (such as abnormal intestinal permeability, bacterial overgrowth, and inflammation), it is not clear if vascular calcification influences these alterations. To investigate whether the bidirectional relationships between VC and gut dysfunction could be mediated by increased inflammation and uremic toxin generation, we used the SNx-VC model of uremic vascular calcification (rats undergoing subtotal 5/6th nephrectomy and fed a procalci

The Gut Microbiome in Hyperuricemia and Gout.

Humans develop hyperuricemia via decreased urate elimination and excess urate production, consequently promoting monosodium urate crystal deposition and incident gout. Normally, approximately two-thirds of urate elimination is renal. However, chronic kidney disease (CKD) and other causes of decreased renal urate elimination drive hyperuricemia in most with gout. This places more demand on elimination of urate via the gut, where diet, purine metabolism, and microbiota intersect. Heritable impairment of urate transport into the gut is common and promotes hyperuricemia, renal urate overload, and

Volume Assessment in Patients Undergoing Long-Term Dialysis.

Accurate assessment of fluid status is a priority for patients with kidney failure undergoing long-term dialysis. There is wide variation in current volume-related practices between dialysis units and an urgent need to develop better evidence to guide practice. Clinical decisions relating to volume management are implicitly based on assessment of volume status, and there are numerous different but imperfect methods of assessment. Isotope-based dilutions are impractical for clinical use and may not be a gold standard for patients with kidney failure. Individual trends in body weight and BP have

Inter-organ crosstalk: The kidney's role in systemic health and disease.

This review elucidates the critical role of inter-organ crosstalk in systemic health, focusing on the kidney's interactions with the heart, bone marrow, lung, liver, intestine, bone-vascular, and nervous system. These interactions are vital for maintaining physiological homeostasis and are mediated by hormones, cytokines, and metabolites. The kidney's role in these networks is pivotal, as dysfunction can exacerbate systemic diseases, highlighting the need for integrated therapeutic strategies. Chronic kidney disease and acute kidney injury serve as key examples of how kidney dysfunction impact

Biomimetic Polythiophene: g-C3N4 Nanotube Composites with Induced Creatinine and Uric Acid Specificity for Portable CKD and Gout Detection.

Despite significant advancements in disease diagnostics, the development of portable, highly selective, and low-cost electrochemical sensors for real-time, noninvasive detection of chronic kidney disease (CKD) and gout biomarkers remains a challenge. In this work, we demonstrate an inexpensive CKD and gout diagnostic platform with the incorporation of biomimetic polymer matrix nanocomposites based on creatinine (CRE) and uric acid (UA) imprinted conducting polythiophene (MIP) and graphitic carbon nitride nanotubes (NTs). These nanocomposites are termed CRE-MIPNT or UA-MIPNT, depending on the t

[Miliary palmoplantar gout tophi in a patient with hyperuricemia and chronic renal insufficiency].

We report on a woman with palmoplantar whitish-yellowish aggregated, partly ulcerated papules on a slightly erythematous base. Histological examination showed pale amorphous nodular deposits in the dermis. On radiological examination, calcifications could be ruled out. Laboratory tests showed marked hyperuricemia and chronic renal insufficiency. We diagnosed miliary gout (MG) and initiated uric acid-reducing therapy with allopurinol 100 mg/day. First described in 2007, MG presenting with multiple whitish-yellowish papules is a rare intradermal variant of classic gout that is characterized by s

Is the Mayo prediction tool for MGRS ready for prime time?

Kidney disorders caused by a monoclonal Ig (monoclonal gammopathy of renal significance) are now considered treatable kidney diseases. Because chronic kidney disease and monoclonal gammopathy of undetermined significance occur frequently at older ages and because monoclonal gammopathy of renal significance is rare, it is difficult to select the right patient for a kidney biopsy. The "Mayo MGRS Prediction Tool" was developed to assist in decision making. Although there is some value in using the calculator, there are many caveats. Clinicians should be aware of the biases and limitations of the

Identification of key genes in membranous nephropathy and non-alcoholic fatty liver disease by bioinformatics and machine learning.

BACKGROUND: Chronic kidney disease (CKD) and non-alcoholic fatty liver disease (NAFLD) are closely associated. However, membranous nephropathy (MN), one of the causes of CKD, may contribute to NAFLD through abnormalities in lipid metabolism. METHODS: 93 patients diagnosed with MN by renal biopsy and admitted to Henan Provincial People's Hospital between August 2021 and August 2022 were enrolled in this study. Patients were divided into two groups based on the presence or absence of NAFLD. Publicly available datasets related to NAFLD and MN were obtained from the Gene Expression Omnibus (GEO)

Early Conversion to Once-Daily MeltDose® Extended-Release Tacrolimus (LCPT) in Liver Transplant Patients.

Background: Switching stable liver transplant (LT) recipients from twice-daily immediate-release tacrolimus (IR-Tac) to once-daily MeltDose® extended-release tacrolimus (LCPT) has been proven safe and well tolerated. Moreover, the switch has been associated with enhanced treatment adherence, improvement of tremors, and preserved renal function. Here, we hypothesized that switching to LCPT early after LT may enhance long-term patient outcomes significantly. Methods: This single-center, observational study investigated the long-term safety of LCPT in a large cohort of LT recipients (n = 100). Al

Multifaceted skeletal effects of sevelamer carbonate in a secondary hyperparathyroidism model.

INTRODUCTION: Hyperphosphatemia leads to abnormal mineralization of bones and soft tissues in patients with chronic kidney disease-induced secondary hyperparathyroidism (CKD-SHPT). Sevelamer lowers phosphate levels by binding to dietary phosphate in the gastrointestinal system, forming new bone and reducing the risk of renal osteodystrophy and fracture. However, the influence of sevelamer carbonate (SevC) on bone microarchitecture, material qualities, and mechanical behavior is unknown in CKD-SHPT conditions. MATERIAL AND METHODS: We utilized a rat model of CKD-induced hyperphosphatemia by fe

Dyslipidaemia and inflammation are risks for subclinical atherosclerotic vascular disease among chronic kidney failure patients in South Africa.

BACKGROUND: Chronic kidney disease (CKD) is associated with generalized inflammation. The presence of CKD-related (non-traditional) cardiovascular disease (CVD) risk factors such as inflammation, oxidative stress and uraemic toxins worsen the CVD. A distinct form of lipoprotein alteration known as uraemic dyslipidaemia, characterized by normal low-density lipoprotein (LDL), reduced high density lipoprotein (HDL) and elevated triglyceride and lipoprotein (a) has been described in CKD. The combination of all these factors increase the cardiovascular risk in CKD patients. We evaluated the relatio

Successful twin pregnancy in a patient with lupus erythematosus on hemodialysis: a case report and literature review.

BACKGROUND: Pregnancy rates in women on hemodialysis are low but increasing due to improvements in nephrological and obstetrical care. All pregnancies in dialysis patients qualify as high-risk pregnancies, but courses and outcomes of pregnancy vary depending on the underlying cause of renal failure. While more information about best practice in treatment of pregnant dialysis patients has accumulated recently, there are few cases described with systemic lupus erythematosus (SLE) as underlying disease and there is almost no information about twin pregnancies in this cohort. CASE PRESENTATION: We

Bilateral Renal Fungal Bezoars and Perinephric Abscess in an Infant With Arthrogryposis-Renal Dysfunction-Cholestasis Syndrome: A Clinico-pathologic Case Report.

The patients with Arthrogryposis-Renal dysfunction-Cholestasis (ARC) syndrome have genetic susceptibility to the opportunistic infections due to the involvement of VPS33B (vacuolar protein sorting 33 homolog B) in phagolysosome fusion in macrophages. Detailed pathologic studies in ARC patients are missing in literature due to the lack of autopsy. We described the first autopsy case of ARC syndrome in a 2-month-old male infant. His death was due to recurrent sepsis and multiorgan failure despite the appropriate poly-antibiotic therapy and supportive care. The autopsy showed invasive renal candi

COQ8B gene deficiency as a potential cause of retinal abnormalities in Pediatric kidney transplant recipients.

Coenzyme Q10 synthesis disorder caused by COQ8B gene deficiency is among the most prevalent causes of end-stage renal disease (ESRD) in children, which usually presents as isolated kidney disease, with sporadic cases associated with extrarenal symptoms such as retinitis pigmentosa (RP). Through long-term follow-up of 26 renal transplant children with COQ8B variants at our center, it is observed that, despite favorable renal transplant outcomes, 23.1% of children experienced night blindness or other ocular symptoms. Nine children were recruited for systematic ophthalmic examination and found th
