Predictive value of ferritin heavy chains in the development of coronary artery calcification in patients on maintenance hemodialysis: a prospective cohort study.

BACKGROUND: Vascular calcification (VC) is a well-established risk factor for cardiovascular disease (CVD) and mortality in patients on maintenance hemodialysis (MHD). These patients frequently present with hyperphosphatemia as well as disorders of iron metabolism. This study aims to explore the role of ferritin heavy chain (FTH) in the development and progression of coronary artery calcification (CAC) in patients on MHD and assess its predictive value. METHODS: Using a bioinformatics approach, we analyzed datasets related to VC. In our prospective study, we evaluated the Coronary Artery Calc

Phosphate and Inflammation in Health and Kidney Disease.

Phosphate is emerging as an active mediator of oxidative stress and vascular injury in chronic kidney disease (CKD). This emerging pathophysiological framework, referred to as "Phosphatopathy", describes the systemic syndrome driven by chronic phosphate overload and characterized by oxidative stress, inflammation, endothelial dysfunction, vascular calcification, cellular senescence, and metabolic imbalance. Beyond being a biochemical marker, phosphate overload triggers NOX-derived reactive oxygen species (ROS), activates Wnt/β-catenin and TGF-β signaling, and disrupts the FGF23-Klotho axis, pr

Recurrent immunotactoid glomerulopathy in a kidney transplant recipient: Case report.

RATIONALE: Immunotactoid glomerulopathy (ITG) is a rare glomerular disease characterized by protein deposition in hollow microtubules on electron microscopy. Patients may present with proteinuria, hematuria, hypertension, and renal insufficiency, and some patients even progress to end-stage renal disease (ESRD). In patients with ESRD, ITG recurs in more than 50% of patients after kidney transplantation; however, there is no clear treatment plan for these patients owing to the limited number of reported cases. PATIENT CONCERNS: In this study, we report a case of a 58-year-old male who was admi

Chronic kidney disease in adults with sickle cell trait: a systematic review and meta-analysis.

Sickle cell trait (SCT) may increase the risk of chronic kidney disease (CKD). We aimed to determine the pooled statistics of the association between SCT and CKD. Studies published up to May 2024 that were available on PubMed, Embase, Global Health Library, and Web of Science were screened. We included studies that reported odds ratios or hazard ratios (HRs) of CKD and/or end-stage renal disease (ESRD) and that compared adults with SCT to those without SCT. The risk of bias was evaluated using the Risk Of Bias In Nonrandomized Studies-of Exposures tool. The pooled SCT prevalence was calculated

Impact of physician awareness and microbiological examination on incidence of COVID-19-associated pulmonary aspergillosis: a retrospective study.

BACKGROUND: The reported incidence of aspergillosis among COVID-19 patients has varied significantly, which can be partly attributed to differences in diagnostic approaches and levels of physicians' proficiency in diagnosing COVID-19-associated pulmonary aspergillosis (CAPA). Consequently, we conducted a retrospective study to investigate the potential reasons for these discrepancies and analyzed the risk factors for pulmonary aspergillosis in patients with COVID-19. METHOD: Data were retrospectively collected from December 1, 2022, to September 30, 2023, from patients who were admitted to th

Rapidly progressive glomerulonephritis as an unusual type of renal involvement in sarcoidosis: a case report.

INTRODUCTION: Sarcoidosis is a multisystem inflammatory disease of unknown etiology characterized by the formation of noncaseating epithelioid granulomas. Clinically significant renal involvement is rare in sarcoidosis. It most commonly manifests as chronic tubulointerstitial nephritis and nephrocalcinosis with nephrolithiasis. Further, glomerular involvement is observed sporadically, mainly membranous glomerulopathy or focal segmental glomerulosclerosis. CASE PRESENTATION: We describe the clinical case of a 49-year-old patient of Caucasian ethnicity with a history of sarcoidosis of the lungs

Chronic kidney disease related to Loa loa microfilaremia in a rural area of the Republic of Congo: a population-based cross-sectional study.

BACKGROUND: Loiasis affects millions in Central Africa and, though historically considered benign, emerging data suggest possible renal involvement. This study investigated the association between Loa microfilaremia and renal function. METHODS: We conducted a cross-sectional study in the Republic of Congo in May-June 2022. Renal function was assessed via estimated glomerular filtration rate (eGFR) using Chronic Kidney Disease-Epidemiology Collaboration (CKD-EPI) and European Kidney Function Consortium (EKFC) equations, and proteinuria and/or haematuria (renal abnormalities, RAb). Valve-sparing aortic root and partial aortic arch replacement with reimplantation of the brachiocephalic trunk for chronic type ІA aortic dissection.

A 47-year-old female with dyspnoea, poor blood pressure control and a family history of aortic disease presented with chronic type IA aortic dissection (with a maximal aortic diameter of 50 mm at the ascending aorta level). Anatomically, the patient presented with involvement of the brachiocephalic trunk and the left renal artery originated from the false lumen. The aortic valve was trileaflet but demonstrated severe eccentric insufficiency. Due to this constellation of findings, urgent surgical repair was indicated. Cardiopulmonary bypass was established centrally by direct true lumen cannula

New Sarcoidosis Diagnosis Following Unresponsive Event During Hemodialysis.

Sarcoidosis is a systemic disease where extrapulmonary presentation of the disease often goes unrecognized as it overlaps with many disease manifestations. End-stage renal disease (ESRD) can be complicated by hypercalcemia and encephalopathy, both potential signs of sarcoidosis. These numerous extrapulmonary presentations may delay timely diagnosis and treatment. In this case report, a 53-year-old male with ESRD was admitted following an unresponsive episode during routine hemodialysis, later found to have sarcoidosis. Initial presentation notable for hypothermia, tachycardia, leukocytosis, an

Audiological Findings in Hemodialysis Patients.

Hearing loss in end-stage chronic renal failure is more common than in the general population. The etiopathogenetic mechanisms reported included the use of ototoxic drugs, hypertension, fluid and electrolyte disorders associated with kidney failure, and factors related to hemodialysis itself, such as acute hypotension, reduced blood osmotic pressure, acute urea clearance, and the immunological reaction caused by dialyzer membranes. The aim of our study was to evaluate auditory function in patients with end-stage chronic renal failure, to study the prevalence of hearing loss and the factors tha

Successful Treatment with Secukinumab in a Psoriasis Patient on Hemodialysis.

Moderate to severe psoriasis has been reported as an independent risk factor for IgA nephropathy (IgAN). IgAN is characterized by episodic microscopic hematuria, which can progress to end-stage renal disease (ESRD). Managing therapeutic interventions for psoriasis patients requiring dialysis due to ESRD presents significant challenges. We present a case of severe plaque psoriasis in a patient concurrently diagnosed with IgAN who is dependent on hemodialysis. Over the past two months, his condition has worsened without any identifiable triggers. Physical examination revealed generalized scaly p
