Impact of renal function on the efficacy of low-voltage area ablation after pulmonary vein isolation: a sub-analysis of the SUPPRESS-AF trial.

AIMS: The SUPPRESS-AF trial showed that pulmonary vein isolation (PVI) plus low-voltage area (LVA) ablation may reduce atrial fibrillation (AF) recurrence in some subgroups. Renal dysfunction is a cause of LVAs due to atrial cardiomyopathy and is also a risk factor for AF recurrence after catheter ablation. The aim of this study was to investigate the efficacy of LVA ablation after PVI stratified by renal function. METHODS AND RESULTS: This study was a sub-analysis of the SUPPRESS-AF trial, a multicentre, prospective, randomized, open-label trial. A total of 341 consecutive patients who under

Evolving landscape of thrombotic microangiopathy in kidney transplant recipients in the post-C5 inhibitor era.

A comprehensive analysis was performed on all consecutive biopsy-proven thrombotic microangiopathy (TMA) complicating kidney transplantation in the post-C5 inhibitor era (from 2009) to identify pathological profiles, determine causes, and establish risk factors associated with death-censored graft survival, in 2 French centers. Pathological criteria were assessed according to the TMA Banff Working Group, followed by an unbiased analysis to identify distinct subgroups. One hundred nineteen cases were identified, 8 (6.7%) primary TMA, and 23 (19.3%) antibody-mediated rejection. In 98 cases (82.4

Impact of chronic kidney disease on postoperative outcomes following craniotomy for aneurysmal subarachnoid hemorrhage.

PURPOSE: Aneurysmal subarachnoid hemorrhage is an urgent manner in which blood accumulates in the subarachnoid region. Presence of chronic kidney disease (CKD) is often a predictor of adverse outcomes perioperatively. Patients with CKD may have different perioperative outcomes during surgery for aneurysmal subarachnoid hemorrhage compared to patients without CKD, and we sought to retrospectively examine the effect of CKD on perioperative outcomes in aSAH patients receiving surgery. METHODS: Patients undergoing craniotomy for aneurysmal subarachnoid hemorrhage were analyzed using the ACS-NSQIP

Pleural malpositioning of a haemodialysis catheter causing haemothorax despite the use of ultrasound guided placement and chest radiograph confirmation.

We report an unusual case of haemodialysis catheter malposition into the pleural space causing haemothorax, despite ultrasound-guided insertion and X-ray confirmation. A patient with chronic kidney disease presented in respiratory failure due to pulmonary oedema and was scheduled for urgent haemodialysis. A haemodialysis catheter was inserted under ultrasound guidance, with apparent blood backflow, and postinsertion chest radiograph suggested proper placement. However, dialysis was interrupted multiple times due to persistently inadequate extracorporeal blood flow and visibly diluted blood in

Duration of Pneumocystis jirovecii pneumonia prophylaxis in adult kidney transplant recipients and outcomes: A population-based study.

Kidney transplant recipients are prescribed medications to prevent Pneumocystis jirovecii pneumonia (PJP), but there is no consensus regarding the optimal duration of therapy. We used linked health care databases in Alberta, Canada, to describe patterns of PJP prophylactic prescriptions in incident adult kidney transplant recipients (2008-2019) to compare clinical outcomes in recipients taking short-term (≤9 months) versus long-term (>9 months) PJP prophylaxis. We identified 1265 kidney transplant recipients. The median age was 53 years (interquartile range, 41-62 years), 35% were female, and

Risk factors and outcomes of Pneumocystis jirovecii pneumonia in kidney transplant recipients: A case-control study of the United States Renal Data System.

Kidney transplant recipients are at increased risk for Pneumocystis jirovecii pneumonia (PJP). We aimed to determine risk factors and outcomes of the disease using a large national database. We first conducted a nested case-control study to identify risk factors associated with PJP in patients who received a kidney transplant from January 1, 1998, through December 31, 2019. We then performed a matched cohort study to evaluate clinical outcomes following the diagnosis of PJP. We matched 407 patients with PJP to 1625 uninfected controls. PJP was diagnosed at a median time of 55 months after tran

Albuminuria Predicts a Rapid Decline in Kidney Function in 2 International, Longitudinal Cohorts of Adults With Sickle Cell Anemia.

Chronic kidney disease (CKD) is common and a major contributor to increased morbidity and early mortality in people with sickle cell anemia (SCA). Urine albumin-to-creatinine ratio (uACR) is recommended to identify patients with SCA-related CKD but its utility in predicting long-term kidney dysfunction remains unclear in this patient population. In two independent, longitudinal cohorts of patients with hemoglobin SS or Sβ0-thalassemia (USA: n = 268, median follow-up 6 years; France: n = 310, median follow-up 8.2 years) we investigated the utility of uACR, as well as other clinical and modifiab

Benefits of aldosterone receptor antagonism in chronic kidney disease: the BARACK-D RCT.

BACKGROUND: Chronic kidney disease affects around 10% of the global population and is associated with significant risk of progression to end-stage renal disease and vascular events. Aldosterone receptor antagonists such as spironolactone have shown prognostic benefits in patients with heart failure, but effects on patients with chronic kidney disease are uncertain. OBJECTIVES: To determine the effect of low-dose spironolactone on mortality and cardiovascular outcomes in people with chronic kidney disease stage 3b. DESIGN: Prospective randomised open blinded end-point trial. SETTINGS: Three

Causal relationship between branched-chain amino acids and their metabolites and frailty: A two-sample Mendelian randomization study.

Increasing evidence suggests that branched-chain amino acids (BCAAs) are associated with frailty. However, whether there is a causal relationship between them remains to be seen. In addition, the metabolism of amino acids in the body is complex, and many previous studies have neglected the effects of their metabolites. The aim of this study was to investigate the relationship between BCAAs and their metabolites and indicators of frailty through Mendelian randomization (MR) methods, so as to determine the causal relationship between the 2, and to provide potential information for preventive and

Impact of Left Ventricular Geometry on All-Cause Mortality in Patients Undergoing Hemodialysis: A Longitudinal Observational Study.

KEY POINTS: We evaluated the association between the four categories of left ventricular geometry and all-cause mortality in patients undergoing hemodialysis. Concentric hypertrophy was the most prevalent, followed by concentric remodeling, and these were associated with a high risk of mortality. Echocardiography screening for evaluating the four categories of left ventricular geometry may be useful for risk prediction in patients undergoing hemodialysis. BACKGROUND: Left ventricular (LV) geometry, which is classified into four categories on the basis of left ventricular mass index and relati

When Everything Else Fails: TricValve® in a Dialysis-Dependent Patient with Advanced Right Heart Failure.

We report the case of a 72-year-old man with end-stage renal disease on maintenance dialysis and advanced right heart failure with severe tricuspid regurgitation, chronic atrial fibrillation, and cardiac cachexia. The patient presented with profound hypotension and cardiogenic shock, leading to recurrent failure of renal replacement therapy despite inotropic support. Given the prohibitive surgical risk, transcatheter edge-to-edge repair was deemed unsuitable due to extensive annular dilation, and the patient underwent urgent percutaneous caval valve implantation with the TricValve® system. Inflammatory mediation by neutrophil percentage to albumin ratio in the association between chronic kidney disease and presbycusis.

This study assessed the association between chronic kidney disease (CKD) and presbycusis and investigated whether inflammatory neutrophil percentage-to-albumin ratio (NPAR) indicators influence this relationship. This study used data from the National Health and Nutrition Examination Survey and employed methods such as restricted cubic spline analysis, multiple logistic regression analyses, and mediation analyses to explore these relationships. High-frequency hearing impairment (HI) was correlated with moderate CKD (OR = 3.25, 95% CI:1.25- 8.46, P = 0.02). HI grades were correlated with modera
