Oral Anticoagulants in Patients With Atrial Fibrillation and Advanced or End-Stage Renal Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: Patients with atrial fibrillation (AF) and advanced chronic kidney disease (CKD) face a markedly elevated risk of both thromboembolic and bleeding events. While oral anticoagulation is the cornerstone of stroke prevention in AF, the optimal use of this treatment in patients with stage 4-5 CKD and end-stage renal disease (ESRD) undergoing dialysis remains uncertain, as these populations were largely excluded from the landmark trials that established the efficacy of direct oral anticoagulants (DOACs) in the general AF population. This study aimed to systematically evaluate and compar

Unmasking Iododerma: A Distinctive Histopathologic Pattern of Neutrophil-Rich Dermatosis With Haloed Cryptococcoid Bodies in the Setting of Hydralazine Exposure and Autoimmune Serologic Abnormalities.

Cutaneous iododerma is a rare adverse reaction to iodine-containing compounds that may clinically and histopathologically mimic infection, vasculitis, malignancy, and other neutrophilic dermatoses. Although most commonly associated with iodinated contrast exposure, a variety of predisposing factors and potential associations have been reported, including impaired renal function, repeated iodine exposure, and more recently, hydralazine therapy in isolated cases. We report two women with chronic kidney disease and recent iodinated contrast exposure who developed rapidly progressive, painful, bul

Association of waist circumference trajectories with incident chronic kidney disease.

BACKGROUND: Chronic kidney disease (CKD) is a major public health burden. Waist circumference (WC) is associated with CKD risk, but the prognostic significance of longitudinal WC change remains uncertain. We examined the association between WC trajectories and incident CKD in Korean adults. METHODS: This retrospective cohort study included 5,429 adults aged 40-69 years from the Korean Genome and Epidemiology Study Ansan-Ansung cohort who were free of CKD at baseline and during the 6-year exposure period. WC trajectories were classified as stable, increasing, or decreasing using Gaussian finit

Association of chronic kidney disease with major cardiovascular outcomes following lower extremity endovascular revascularization.

OBJECTIVE: Chronic kidney disease (CKD) amplifies cardiovascular risk, yet its stage-specific influence on perioperative outcomes following endovascular lower extremity revascularization (LER) for peripheral artery disease (PAD) remains poorly characterized. METHODS: We conducted a retrospective cohort study using the ACS NSQIP registry (2013-2022). Adults undergoing endovascular LER for PAD were stratified by preoperative estimated glomerular filtration rate (eGFR) via the CKD-EPI 2021 equation: G1 (≥90), G2 (60-89), G3 (30-59), G4 (15-29), and G5 (<15 mL/min/1.73 m2 or dialysis dependent).

A Visual Protocol for Selecting, Initiating, and Monitoring Hemodialysis or Peritoneal Dialysis in Adults with Uremic-Stage Chronic Kidney Disease.

Uremic-stage chronic kidney disease requires timely kidney replacement therapy, but the transition from advanced chronic kidney disease to hemodialysis (HD) or peritoneal dialysis (PD) is often fragmented. Late referral, incomplete modality education, delayed access planning, and insufficient early follow-up contribute to avoidable urgent starts, catheter exposure, hospitalization, and poor patient experience. This article describes a reproducible, visually demonstrable transition-care protocol for a hospital-based adult nephrology service. The workflow moves from eligibility screening and cli

"I Think It's Institutional Racism:" Pediatric Social Workers' Explanations of Racial Disparities in Kidney Transplant Access for Black Children in the United States.

BACKGROUND: Black children in the US with ESKD experience higher mortality, lower transplant rates, and longer time on dialysis than other children. Prior research on health disparities in adult kidney patients often emphasizes patient-level psychosocial factors, yet little is known about how providers understand these inequities for children. This study explored pediatric dialysis and transplant social workers' explanations for racial disparities affecting US black children in kidney transplant access. METHODS: Semi-structured interviews were conducted with 16 pediatric dialysis and transpla

A territory-wide study on adults hospitalized for rhinovirus infection - mortality, severe adverse respiratory and renal outcomes and relationships with multiple comorbidities.

Introduction. While rhinovirus was thought to cause mild infections such as the common cold, severe infections such as community-acquired pneumonia and asthma exacerbations can also occur. Children with chronic medical conditions have been shown to have increased risks of severe infections.Hypothesis/Gap statement. There is a difference in the risks of serious in-hospital outcomes among adults hospitalized for rhinovirus or influenza virus infection. Multiple morbidities in adult patients are associated with severe infections among patients hospitalized for rhinovirus infections.Aim. To compar

Potato and warm moist compresses to accelerate healing of limb swelling and bruising after venous stent placement: A case report.

RATIONALE: Hemodialysis is an essential renal replacement therapy for patients with end-stage renal disease, and an autologous arteriovenous fistula (AVF) is preferred because of its durability and low complication rate. Maintaining AVF patency is critical for effective dialysis. AVF stenosis may reduce blood flow, impair dialysis adequacy, and compromise vascular access function. Digital subtraction angiography-guided angioplasty and venous stent placement are commonly used to restore patency. Postoperative swelling, bruising, pain, and restricted limb movement may occur and delay recovery. T

National trends in chronic kidney disease prevalence by sleep duration in South Korea, 2007 to 2022: A nationwide cross-sectional study.

Despite growing evidence linking sleep duration to chronic kidney disease (CKD), long-term trend analyses specific to the Korean population remain limited, and most previous studies have relied on broad intermediate sleep categories. This study analyzed trends in CKD prevalence by self-reported sleep duration among Korean adults. We included 87,246 Korean adults aged ≥19 years from the Korea National Health and Nutrition Examination Survey, 2007 to 2022. CKD was defined as an estimated glomerular filtration rate <60 mL/minutes/1.73 m2. Sleep duration was categorized into 5 groups (<6, 6-7, 7-8

Short-term clinical efficacy and recorded adverse events of semaglutide combined with dapagliflozin in patients with type 2 diabetes mellitus and chronic kidney disease.

Type 2 diabetes mellitus (T2DM) complicated by chronic kidney disease (CKD) is associated with persistent hyperglycemia, progressive renal impairment, systemic inflammation, and an increased risk of adverse clinical outcomes. Sodium-glucose cotransporter 2 inhibitors and glucagon-like peptide-1 receptor agonists have shown metabolic and cardiorenal benefits, but clinical evidence regarding their combined use in patients with T2DM and CKD remains limited. This retrospective observational study included 126 patients with T2DM complicated by CKD who were treated at our hospital between March 2023

Epigenetic reprogramming by BCG (Bacillus Calmette-Guérin) vaccination: A novel strategy to reverse immunodeficiency in chronic kidney disease.

Chronic kidney disease (CKD) is associated with reflecting innate immune impairment. Chronic inflammation, oxidative stress, metabolic dysregulation, and prolonged uraemia are the primary causes of this condition. Monocytes, macrophages, and their progenitors may undergo drastic epigenetic changes in response to these systemic stresses. This has lowered the activation of histone marks, such as H3K4me3, and destabilised repressive marks, like H3K9me3. Critical immune response genes are silenced by chromatin closure. This epigenetic change exhausts the immune system and impairs its ability to cl

Impact of Revised Cardiac Risk Index, Individual Risk Factors, and Their Duration on Major Adverse Cardiac Events After Primary Total Knee Arthroplasty: A Nationwide Propensity-Matched Cohort Study.

BACKGROUND: Although the Revised Cardiac Risk Index (RCRI) is widely used to assess major adverse cardiac events (MACE) preoperatively, the impact of risk-factor duration and age stratification on its predictive performance after total knee arthroplasty (TKA) remains unclear. This study evaluated post-TKA MACE incidence and risk according to RCRI score, individual risk factors, their preoperative duration, and age strata. METHODS: This retrospective cohort study used a nationwide claims database, including primary TKA patients (2017 to 2021). The RCRI was modified for claims data by defining

Sofosbuvir/Daclatasvir versus Grazoprevir/Elbasvir in Patients with Chronic Hepatitis C on Haemodialysis: A Multicentre, Single-Blind, Randomised Controlled Trial.

BACKGROUND: Patients with chronic kidney disease undergoing hemodialysis are at risk of hepatitis C virus exposure due to nosocomial transmission and the duration of hemodialysis. The treatment for hepatitis C in patients with chronic kidney disease undergoing hemodialysis is oral direct-acting antiviral (DAA) therapy, which works directly as an antiviral. Currently, the DAA considered safe for use in patients with chronic kidney disease is grazoprevir/elbasvir. However, this DAA is expensive and effective in patients with genotypes 1 and 4. Recent studies have shown that sofosbuvir-based regi
