The Chronic Renal Insufficiency Cohort (CRIC) study measured hepcidin, cardiovascular, kidney and metabolic outcomes in chronic kidney disease.

INTRODUCTION: Abnormal iron homeostasis plays a critical role in extra-hematopoietic disorders. The relationship of hepcidin, the master iron regulator, to clinical outcomes, has not been characterized in a large cohort of chronic kidney disease (CKD). METHODS: To determine if plasma hepcidin level predicts cardiovascular (CV), kidney, and metabolic outcomes in patients with CKD, we studied 3826 patients enrolled in the Chronic Renal Insufficiency Cohort (CRIC) study. Hazard ratios for hepcidin tertiles were estimated from Cox Proportional Hazards regression for primary outcomes of incident h

Predicting Major Amputation Rates in Diabetic Hindfoot Ulcers With Critical Infection and Ischemia: A Risk-Factor-Based Model.

BACKGROUND: Diabetic hindfoot ulcers pose a substantially greater risk of progressing to major amputation than forefoot or midfoot ulcers. Patients with these ulcers often experience severe psychological distress at the prospect of major limb loss. Consequently, patients, their families, and healthcare professionals prioritize understanding the actual risk of major amputation based on the patient's current clinical status. However, practical tools addressing this critical and common concern remain limited. In a previous preliminary study, we proposed a model for predicting major amputation rat

Gut-targeted interventions to complement PTH-lowering therapy.

Secondary hyperparathyroidism is a major contributor to skeletal fragility in chronic kidney disease, yet fracture risk remains high despite parathyroid hormone-lowering therapy. In a rat model of chronic kidney disease, Allen et al. showed that combining the prebiotic fiber inulin with a calcimimetic further suppresses elevated bone turnover while improving cortical bone architecture and mechanical properties. These findings highlight the potential of gut-targeted interventions to complement established parathyroid hormone-lowering strategies for improving skeletal health.

Estimating GFR: what it is, what it is not, and why prediction cannot settle the question.

Schultheiss et al. show that replacing Chronic Kidney Disease Epidemiology Collaboration with European Kidney Function Consortium cystatin C-based equations reclassifies women, but not men, into milder chronic kidney disease stages, while outcome prediction is unchanged. We argue that prediction of kidney failure cannot validate a glomerular filtration rate equation: dialysis initiation is a clinical decision, not a filtration measurement. The analysis is cross-sectional, leaving longitudinal performance untested. Equations are population tools; at the bedside, non-glomerular filtration rate d

Transarterial Computed Tomographic Angiography in Patients With Impaired Renal Function Undergoing Endovascular Aortic Repair.

PurposeComputed tomography angiography (CTA) is crucial for preoperative planning and postoperative surveillance of endovascular aneurysm repair (EVAR). Renal function impairment however limits pre- and postoperative assessment. The aim of our study was to evaluate the safety and diagnostic value of a novel technique in the setting of patients undergoing EVAR.Methods216 patients with chronic kidney disease undergoing transbrachial placement of an angiography catheter in the aorta for contrast medium administration were analyzed retrospectively. 45 patients with normal kidney function and aorti

Maternal kidney disease and composite kidney outcomes in the offspring.

Maternal chronic kidney disease (CKD) is associated with adverse pregnancy outcomes, but its relationship with long-term offspring kidney health remains uncertain. This retrospective maternal-child cohort study examined whether maternal CKD or reduced kidney function before or during pregnancy was associated with a composite kidney outcome in offspring. Live births in the Taiwan Mother and Child Health cohort from 2007 to 2017 were followed through 2018. In the primary cohort, maternal kidney disease was defined as claims-based CKD meeting chronicity criteria before or during pregnancy; the ma

Study of Depression, Anxiety, Somatic Symptoms, and Quality of Life in Patients with Chronic Kidney Disease on Maintenance Hemodialysis in a Tertiary Hospital in Chennai.

BACKGROUND: Chronic kidney disease (CKD) is associated with substantial psychological morbidity, particularly among patients receiving maintenance hemodialysis (MHD). Depression, anxiety, and somatic symptoms adversely affect health-related quality of life (HRQoL), yet these factors remain under-recognized in routine clinical practice. OBJECTIVES: To assess the prevalence and severity of depression, anxiety, and somatic symptoms and to evaluate their association with quality of life among patients with CKD undergoing maintenance hemodialysis. MATERIALS AND METHODS: This hospital-based cross-

Association between dietary inflammatory index and mortality in adults with chronic kidney disease: a retrospective study from the National Health and Nutrition Examination Survey.

BACKGROUND: The prevalence and mortality of chronic kidney disease (CKD) are high. Systemic inflammation often occurs in patients with CKD and limits overall survival. The dietary inflammatory index (DII), developed to assess the inflammatory potential of diet, may be an underrecognized risk factor for CKD mortality. METHODS: This study included 7,113 adult participants from the US National Health and Nutrition Examination Survey conducted between 2005 and 2018. Multivariable Cox regression analysis was used to assess the association between DII scores and mortality in patients with CKD, with

Complement and immunoglobulin profiles in ANCA-associated vasculitis with renal involvement: a serotype-stratified analysis of disease activity and renal outcomes.

BACKGROUND: ANCA-associated vasculitis (AAV) is characterized by pauci-immune small-vessel inflammation driven predominantly by neutrophil activation, in contrast to immune-complex-mediated glomerulopathies in which complement consumption tracks disease activity and renal injury severity. Serum C3 at diagnosis has repeatedly been associated with renal and patient survival in AAV, yet it remains unresolved whether complement fractions reflect the extent of renal injury in a dose-dependent manner, as they do in lupus nephritis, and how complement relates to immunoglobulin class profiles across A

TGF-β in chronic kidney disease: a controversy for decades.

Chronic kidney disease (CKD) is characterized by persistent inflammation, maladaptive repair and progressive renal fibrosis, yet therapeutic strategies capable of effectively targeting these interconnected processes remain limited. Transforming growth factor-β1 (TGF-β1) is a central mediator of renal fibrosis but also exerts essential immunoregulatory and tissue-protective functions, creating a fundamental therapeutic paradox. Emerging evidence indicates that these divergent effects are determined not simply by TGF-β1 abundance, but by its cellular source and target, spatial and temporal conte

Structural Racism and Chronic Kidney Disease Among African Americans: A Research Process Guided by Public Health Critical Race Praxis.

Structural racism has a profound impact on the ability of Black patients to access needed chronic kidney disease (CKD) treatment and care. Studies that have documented various forms of inequities in CKD diagnosis and treatment of CKD among Black patients are mostly focused on kidney disease outcomes with little attention to the processes of intervention implementation. Focusing on outcomes alone tends to assume the process of research, including study design and implementation, is neutral to the outcome. In actuality, the process, including methods employed by the researcher, all influence and

KDOQI Commentary on the 2025 AHA/ACC Guideline for the Prevention, Detection, Evaluation, and Management of High Blood Pressure in Adults.

Hypertension prevention, diagnosis, and management are central to modifying the progression of chronic kidney disease (CKD). Although the nephrology community has long recognized CKD as an independent risk factor for cardiovascular disease, CKD has not historically been incorporated in cardiovascular risk prediction models. The new 2025 American Heart Association and American College of Cardiology (AHA/ACC) Guideline for the Prevention, Detection, Evaluation, and Management of High Blood Pressure in Adults addresses this gap by formally adopting the PREVENT calculator, which combines cardiovas

Unraveling the dual role of fibroblast growth factor 5 (FGF5) in cardiorenal diseases through multi-omics causal inference.

BACKGROUND: As populations age worldwide, multimorbidity has become an increasingly prominent public health challenge. Among age-associated conditions, cardiovascular diseases (CVDs) are among the most prevalent and life-threatening, and they frequently coexist with other chronic disorders, particularly chronic kidney disease (CKD). Pleiotropic molecules may provide an important entry point for elucidating comorbidity mechanisms and identifying therapeutic opportunities. However, the clinical translational value of different pleiotropic candidates exhibits substantial heterogeneity, and their

ApoB/ApoA-I ratio and all-cause mortality in maintenance hemodialysis: a cohort study.

BACKGROUND: Traditional lipid markers have limited and inconsistent prognostic value in maintenance hemodialysis (MHD), a phenomenon often described as the lipid paradox. We evaluated whether lipid and apolipoprotein-related markers, particularly the ApoB/ApoA-I ratio, were associated with mortality in MHD patients. METHODS: This single-center retrospective cohort study included 409 MHD patients. Lipid and apolipoprotein-related markers were analyzed as standardized continuous variables. Cox regression was used for all-cause mortality, and cause-specific Cox and Fine-Gray models were used for

Acute kidney disease after acute kidney injury: incidence, risk factors, and association with de novo chronic kidney disease.

Acute Kidney Disease (AKD) is defined as acute kidney injury (AKI) persisting for 7 to 90 days and represents a window for interventions to improve renal outcomes. However, data on AKD remain underexplored. This retrospective cohort study at Tongji Hospital, China, included 23,868 adult patients who survived at least one AKI episode between January 2015 and April 2024. Using electronic medical record data, we aimed to quantify the incidence of AKD among AKI survivors, identify independent risk factors, and evaluate the association between AKD and de novo chronic kidney disease (CKD). Among the

Low Nephron Number and Kidney Disease.

Reduced nephron endowment and acquired nephron loss increase susceptibility to hypertension and chronic kidney disease (CKD). Low birth weight, prematurity, genetic factors, and adverse prenatal exposures can limit nephron development, whereas aging and kidney injury reduce the number of functioning nephrons after birth. Compensatory glomerular hypertrophy and increased single-nephron filtration can initially preserve overall glomerular filtration rate, but sustained glomerular stress may promote podocyte injury, proteinuria, and glomerulosclerosis. These changes can accelerate nephron loss an

Maternal Obesity Induces DNA Methylation Changes in the Kidneys and Blood of Rodent Offspring at Adulthood.

AIM: To identify differentially methylated genes in offspring of obese versus lean mothers using a mouse model of maternal obesity. BACKGROUND: Adverse conditions during fetal development predispose offspring to adult diseases, supporting the theory of fetal programming. DNA methylation is suggested as a mechanism by which maternal obesity induces fetal programming. As maternal obesity predisposes offspring to chronic kidney disease (CKD), we aimed to investigate the role of DNA methylation in maternal obesity related to CKD in the offspring. METHODS: Maternal obesity was modelled by feeding
