Normothermic regional perfusion (NRP) is increasingly used in controlled donation after circulatory death (cDCD) to mitigate ischemia-reperfusion injury. While NRP reduces delayed graft function (DGF), its impact on long-term outcomes, particularly in elderly recipients, remains uncertain. We retrospectively analyzed first kidney transplants from cDCD donors in a national registry (2015-2023). Outcomes after NRP were compared with rapid recovery (RR) using 1:1 propensity score matching based on donor/recipient age, comorbidities, and transplant year. A predefined subcohort analysis focused on

Kidney transplantation is prioritized for children in the United States, yet equity for highly sensitized pediatric candidates remains uncertain. Using Organ Procurement and Transplantation Network data, we examined kidney-only candidates aged 0 to 17 years listed between December 4, 2014, and December 31, 2024, who were ever activated, comparing the Kidney Allocation System (KAS; Era 1) and the 2021 circle-based revision (KAS250; Era 2). Candidates were stratified by calculated panel-reactive antibody (cPRA) at first activation (0%-20%, 21%-79%, 80%-99%, 100%). The primary outcome was the 3-y

Frailty has emerged as a powerful risk stratification tool in kidney transplantation. Frail kidney transplant candidates are significantly more likely to experience posttransplant complications, including delayed graft function, rehospitalizations, and death. Although the transplant community's understanding of the relationship between frailty and clinical variables has exploded over the past several decades, our understanding of patient perspectives regarding frailty lags behind. Lack of knowledge regarding patient perspectives regarding frailty can limit the successful integration of clinica

Although disparities in access to kidney transplantation are well-documented, the impact of decentralized, institution-specific allocation systems remains understudied. This study evaluates how structural fragmentation shapes access to deceased donor kidney transplantation among candidates who remained alive and active on the waitlist in Mexico. We conducted a period-prevalent time-to-event analysis using National Transplant Registry data (2014-2024) encompassing 24 698 deceased donor kidney transplant candidates. Access disparities were evaluated across health care sectors (social security, p

PURPOSE: Cardiorespiratory fitness (CRF) is a modifiable factor associated with the progression of chronic kidney disease. However, whether higher CRF levels can help prevent age-related kidney function decline in the early stages before the onset of chronic kidney disease remains unclear. This study aimed to investigate the effect of CRF on subsequent changes in kidney function in individuals with preserved kidney function under a prespecified causal framework.
METHODS: This prospective cohort study assessed CRF by measuring oxygen uptake at the ventilatory threshold using gas exchange analy

Chronic kidney disease progresses through convergent mechanisms involving hemodynamic stress, proteinuria, inflammation, fibrosis, and metabolic disturbance. Barry Brenner's hyperfiltration hypothesis established these processes as drivers of ongoing injury, and landmark trials established that renin-angiotensin-aldosterone system (RAAS) inhibition slows progression and improves clinical outcomes, marking a shift from supportive care to disease-modifying treatment. Treatment options have since expanded, with newer therapeutic classes providing additional kidney and cardiovascular benefit along

This study aimed to assess whether various pathomic features in automatically segmented glomeruli and tubules were associated with death-censored graft loss in kidney transplant recipients. We studied 906 patients who were transplanted between 2000 and 2013, returned to Mayo Clinic for a 5-year surveillance biopsy, and had available subsequent follow-up. Banff scores were obtained from the pathology reports and clinical characteristics from the patients' charts. A multicompartment segmentation model segmented nonsclerotic glomeruli and tubules, and within these compartments, morphometric and t

Kidney retransplantation-the reuse of a previously transplanted allograft-is rare, although limited reports suggest clinical feasibility and an underrecognized opportunity within the existing donor pool. We present a novel case involving a living individual donating a previously transplanted deceased donor kidney after early graft failure from recurrent focal segmental glomerulosclerosis. Five days later, that kidney was transplanted into a second recipient, where it has continued to function for over 8 years. This case report also exposed structural barriers that limit broader adoption. The c

OBJECTIVES: Oliguria is a key prognostic indicator of acute kidney injury (AKI), yet the consensus definition (< 0.5 mL/kg/hr) is increasingly questioned. We aimed to determine thresholds of lowest mean diuresis (LMD) associated with AKI, chronic kidney disease (CKD), and mortality.
DESIGN: Retrospective observational study.
SETTING: Uppsala University Hospital ICU, Sweden.
PATIENTS: A total of 4999 adults (≥ 18 yr) without preexisting CKD admitted to the ICU between 2017 and 2024.
INTERVENTIONS: None.
MEASUREMENTS AND MAIN RESULTS: LMD was calculated over 3, 6, 12, and 24 hours (LMD3h-LM

The transition from acute kidney injury (AKI) to chronic kidney disease (CKD) remains a major clinical problem with unclear underlying mechanisms. Emerging evidence suggests that alterations in tryptophan metabolism, particularly in the production of downstream metabolites such as quinolinic acid (QA), play a role in renal pathophysiology. QA, an intermediate in de novo nicotinamide adenine dinucleotide biosynthesis, is metabolized by quinolinate phosphoribosyltransferase (QPRT). We investigated whether QA contributes to kidney injury using models of folic acid-induced AKI-to-CKD transition an

BACKGROUND: Voriconazole is the recommended first-line treatment for invasive pulmonary Aspergillosis. However, its nonlinear pharmacokinetics and marked interindividual variability increase the risk of drug-induced liver injury, particularly in older patients with multiple comorbidities.
CASE PRESENTATION: A 76-year-old Asian man with chronic obstructive pulmonary disease, type 2 diabetes mellitus, hypertension, polycythemia, and glucocorticoid-induced adrenal insufficiency was admitted with an infective exacerbation of COPD. Sputum culture identified Aspergillus fumigatus, and oral voricona

OBJECTIVES: The hemoglobin, albumin, lymphocytes and platelets (HALP) score, a novel nutritional and inflammatory biomarker, has been used in various chronic disease studies. However, the relationship between the HALP score and chronic kidney disease (CKD) remains poorly elucidated. This study aimed to explore the association between the HALP score and CKD.
METHODS: Our analysis encompassed 25,181 adult participants drawn from NHANES cycles spanning 2009-2018. Weighted multivariable logistic regression and generalized additive models (GAMs) were employed to evaluate the independent associatio

BACKGROUND: Shiga toxin-producing Escherichia coli (STEC) infections can cause significant long-term renal sequelae. Intermediate-term outcomes for patients who do not develop hemolytic uremic syndrome (HUS) remain unclear. We evaluated clinical and laboratory outcomes in children with laboratory-confirmed STEC O157 infection in a large childcare outbreak.
METHODS: This prospective longitudinal cohort study enrolled children aged 9 months to <6 years attending childcare centers in Calgary, Alberta, Canada during an STEC outbreak in September 2023. Among cases, clinical presentation was catego

BACKGROUND: Although increases in plasma levels of non-esterified fatty acids (NEFA) during hemodialysis (HD) have been reported, the precise time course of this elevation, its underlying pathophysiology, and effective therapeutic strategies remain unclear.
METHODS: The time course of plasma NEFA and immunoreactive insulin (IRI) concentrations during HD sessions was examined in seven patients with end-stage kidney disease across three phases : no intervention (control phase), intradialytic parenteral nutrition with an amino acid solution (IVAA phase), and intradialytic parenteral nutrition wi

BACKGROUND: Guideline recommendations on the timing of pre-dialysis care coordination remain largely consensus-based, with limited outcome data supporting specific referral thresholds. This study evaluated the association between the timing of referral to a Renal Coordinator (RC) and the risk of unplanned initiation of dialysis in patients with advanced chronic kidney disease (CKD).
METHODS: We performed a retrospective cohort study of 282 adults with end-stage kidney disease (ESKD) who were referred to RC at a tertiary hospital in Singapore between January 2022 and December 2024. All patient

BACKGROUND: Chronic kidney disease (CKD) in children is characterized by persistent inflammation and oxidative stress, contributing to progressive loss of renal function and multisystem involvement. Maresin 1 (MaR1), a specialized pro-resolving lipid mediator derived from docosahexaenoic acid (DHA), plays a role in the resolution of inflammation and tissue repair. However, its circulating concentrations in pediatric CKD have not been examined. This study aimed to determine serum MaR1 levels across CKD stages in children and to assess its potential relevance as a biomarker reflecting disease se

OBJECTIVES: To determine whether Raman spectroscopy-based urine molecular fingerprinting (UMF) may predict delayed graft function (DGF) and assess renal allograft quality prior to transplant and monitor its state of health after transplantation.
BACKGROUND: Current methods for assessing the quality of the renal allograft at baseline and its state of health post-transplantation are inadequate.
METHODS: We conducted a pilot study in 26 patients receiving DCD allografts. UMF was performed on urine samples collected from study participants immediately before transplantation (baseline), on postop

The 2023 organ procurement and transplantation network (OPTN) policy "Modify waiting time for candidates affected by race-inclusive eGFR calculations" for kidney transplantation (KT) was analyzed at our kidney transplant center over a 15-month period following its implementation. A total of 372 adult Black patients active on our KT waiting list were identified, of whom 104 (28%) qualified for waiting-time modification (WTM). The WTM group gained a median total waiting time of 2.4 years, with 36 (35%) receiving KTs compared to 107 (42%) in the no wait-time modification (NWTM) group. Pre-listing
