Evaluating the kidney disease progression using a comprehensive patient profiling algorithm: A hybrid clustering approach.

BACKGROUND: Acute kidney injury (AKI) can lead to an approximate ninefold increased risk for developing chronic kidney disease (CKD). Despite this, many AKI survivors lack proper nephrology follow-up, highlighting the urgent need to identify patient profiles before onset CKD. Thus, we aimed to develop a patient profiling algorithm to identify clinical phenotypes from AKI to CKD progression. METHODS: This retrospective study utilized electronic health records data from 2010 to 2022. We classified AKI into three groups: Hospital Acquired AKI (HA-AKI), Community Acquired AKI (CA-AKI), and No-AKI

Impact of Telomere Shortening on Post-transplant Outcomes in Interstitial Lung Disease.

Shortened telomere length (STL) is associated with increased rates of interstitial lung diseases, malignancy, hematological disorders, and immunosuppressive treatment toxicities. In this single-center retrospective study, we aim to determine whether patients with interstitial lung diseases who have STL, as determined by quantitative PCR of buccal epithelial cells, exhibit worse post-transplant outcomes compared to recipients with normal telomere length. In our series of 26 patients, STL was associated with a higher incidence of chronic kidney disease following lung transplantation (100% vs 55%

Pancreas irradiation for treatment-resistant acute cellular rejection in a severely immunocompromised pancreas-after-kidney transplant recipient-case report.

Treating acute rejection of a pancreas transplant in a severely immunocompromised patient with viral opportunistic infection is challenging due to the balance of rescuing from rejection without worsening the morbidity of infection and prolonging the infection episode. We present a case involving a pancreas-after-kidney transplant in a patient with cytomegalovirus (CMV) high-risk discordance (donor positive/recipient negative) and chronic lymphopenia who developed difficult-to-treat CMV disease approximately 6 months after pancreas transplantation. Following the withdrawal of the antimetabolite

First Use of Maribavir in Poland to Treat Refractory CMV Disease in a Patient After Kidney Transplantation.

Cytomegalovirus (CMV) is one of the most common opportunistic infections affecting solid organ transplant recipients (SOTRs). In this article, we presented the case of a 39-year-old patient with end-stage renal disease after kidney transplantation with refractory CMV infection, who was successfully treated with maribavir for the first time in Poland. The use of maribavir resulted in a significant reduction of CMV viremia in ganciclovir/valganciclovir-resistant CMV infection and resolution of CMV disease symptoms in the absence of drug-related adverse events.

Phase 2, Randomized, Double-blind, Placebo-controlled Study of Fiztasovimab (NPC-21) for Kidney Transplant Recipients at High Risk of Cytomegalovirus Infection (LionHeart21).

BACKGROUND: Kidney transplantation (KT) has dramatically improved the quality of life of patients with end-stage kidney disease. However, the incidence of opportunistic infections has also increased because of immunosuppression. A common infection after KT is cytomegalovirus (CMV). In KT recipients, we assessed the efficacy and safety of fiztasovimab (NPC-21), an anti-CMV monoclonal antibody. METHODS: This was a multicenter, randomized, double-blind, placebo-controlled, phase 2 study of NPC-21 for KT recipients with CMV donor-positive (D + )/recipient-negative (R - ) serostatus. Patients were

Risk of cytomegalovirus infection in solid organ transplant recipients: A population-based cross-sectional study.

BACKGROUND: Relative to healthy individuals, in severely immunocompromised transplant recipients, the presentation of cytomegalovirus (CMV) is characterized by delayed clearance, more recurrent episodes, and faster changes in their condition over time. This study investigates CMV occurrence in solid organ transplantation (SOT) recipients, addressing a gap in epidemiological research. METHODS: Using National Health Insurance Research Database data, a cross-sectional study observed SOT recipients (2003-2019) with propensity score matching. The case group was 11,028 SOT recipients and 44,112 pat

Impact of kidney function on 200 days of antiviral prophylaxis for cytomegalovirus disease in cytomegalovirus-seronegative recipients of cytomegalovirus-seropositive donor kidneys: Post hoc analysis of a randomized, phase 3 trial of letermovir vs valganciclovir prophylaxis.

This post hoc analysis was conducted to understand the impact of kidney function on 200 days of cytomegalovirus (CMV) prophylaxis in CMV donor-positive/recipient-negative (D+R-) kidney transplant recipients (KTRs). Adult CMV D+R- KTRs were randomized (1:1) posttransplant to letermovir 480 mg (with acyclovir and valganciclovir placebo) or valganciclovir 900 mg (with acyclovir and letermovir placebos) daily for 28 weeks (NCT03443869). Everolimus and Low-Dose Tacrolimus After Heart Transplant in Children: A Randomized Clinical Trial.

IMPORTANCE: Studies suggest that everolimus may reduce the risk of rejection, cardiac allograft vasculopathy (CAV), chronic kidney disease (CKD), and cytomegalovirus (CMV) after heart transplant. Everolimus use is controversial because of data demonstrating higher infection deaths when everolimus is introduced de novo after transplant. It is unclear whether everolimus is safe and effective when initiated at 6 months posttransplant in children, a population in which median graft survival is limited to 15 years and randomized clinical trials are lacking. OBJECTIVE: To evaluate the safety and ef

Lack of evidence for viral etiology in symptomatic chronic kidney disease of unknown etiology (CKDu) in Sri Lanka.

BACKGROUND: Chronic kidney disease of unknown etiology (CKDu) is a major health problem in Sri Lanka. A transient symptomatic episode, characterized by fever, dysuria, and arthralgias, accompanied by kidney dysfunction, has been reported in endemic areas of CKDu. Sym-CKDu has shown with histological features consistent with acute interstitial nephritis and is considered a potential initiating event of CKDu. Among various potential etiologies, viral infections such as Hantavirus, Cytomegalovirus (CMV), and Epstein-Barr virus (EBV) have been implicated in the causation of CKDu. METHODS: Here, w

Determinants of virological outcomes in kidney transplant recipients treated with maribavir for refractory cytomegalovirus infection.

Maribavir has demonstrated promising efficacy in treating refractory cytomegalovirus (CMV) infections among solid organ transplant recipients; however, factors influencing favorable outcomes remain uncertain. We conducted a retrospective cohort study that involved 88 kidney transplant recipients (KTRs) from 22 transplantation centers, all of whom had refractory CMV infections treated with maribavir (63 with and 25 without ganciclovir resistance mutations). CMV clearance at week 8 was achieved in 45 KTRs (51.1%). Among the 49 KTRs (55.7%) who achieved CMV clearance within 16 weeks, 37 (42%) did

Updating the kidney donor risk index: Removing donor race and hepatitis C virus status.

This study reports the results of a recalculation of the kidney donor risk index (KDRI) formula requested by the Organ Procurement and Transplantation Network's Minority Affairs Committee to remove the donor race and hepatitis C virus (HCV) status variables. The updated KDRI model was fit on adult, deceased donor, solitary kidney, first-time transplants from 2018-2021. Deceased donors from 2018 through 2021 were included in a counterfactual analysis to evaluate how the kidney donor profile index (KDPI) would change if race and HCV seropositivity were excluded. When recalculating the original K

The landscape of hepatitis C virus infection in pediatric kidney transplantation.

Hepatitis C virus (HCV) infection is increasing in prevalence due to the growing opioid epidemic; however, its impact on pediatric kidney transplantation is unknown. This study compared kidney transplant outcomes between HCV-positive and propensity score-weighted HCV-negative pediatric recipients. It also examined HCV-positive kidney utilization for pediatric transplantation in the United States. We used the Scientific Registry of Transplant Recipients to identify pediatric kidney transplants (aged < 18 years) performed between April 1, 1994 and December 1, 2022. We used propensity score weigh

Practice and Outcome Trends of Peritoneal Dialysis in Chronic Kidney Disease Patients With Liver Cirrhosis and Chronic Heart Failure Complicated by Symptomatic Ascites-A Scoping Review.

BACKGROUND: The rising growth of patients with end-stage kidney disease (ESKD) associated with chronic liver disease (CLD) and refractory chronic heart failure (CHF) associated with advanced chronic kidney disease (CKD) complicated by ascites presents serious renal replacement therapy (RRT) challenges. Haemodialysis is often poorly tolerated owing to increased hemodynamic instability, bleeding, and encephalopathy risks. Peritoneal dialysis (PD) has emerged as a promising alternative, but its adoption and efficacy are not consistently supported by existing literature, and there lacks guideline

Chronic heart failure and hypoalbuminemia are risk factors of poorer mRNA SARS-CoV-2 vaccine response in maintenance Hemodialysis patients (the COVaccinDia study).

BACKGROUND: Chronic kidney disease represents an immunocompromising condition and a cause of a lower vaccine efficacy, even in patients undergoing maintenance dialysis. Recent SARS-CoV-2 outbreaks have prompted clinicians to better understand the underlying mechanisms and establish more suitable vaccination schedules. METHODS: In a single-center, retrospective, observational study of patients undergoing maintenance dialysis in France, we studied the factors associated with the intensity of the humoral response to a SARS-CoV-2 vaccine in this population, including specific dialysis-related var
