Impact of Revised Cardiac Risk Index, Individual Risk Factors, and Their Duration on Major Adverse Cardiac Events After Primary Total Knee Arthroplasty: A Nationwide Propensity-Matched Cohort Study.

BACKGROUND: Although the Revised Cardiac Risk Index (RCRI) is widely used to assess major adverse cardiac events (MACE) preoperatively, the impact of risk-factor duration and age stratification on its predictive performance after total knee arthroplasty (TKA) remains unclear. This study evaluated post-TKA MACE incidence and risk according to RCRI score, individual risk factors, their preoperative duration, and age strata. METHODS: This retrospective cohort study used a nationwide claims database, including primary TKA patients (2017 to 2021). The RCRI was modified for claims data by defining

Textbook outcomes as a composite quality metric following fenestrated-branched endovascular aortic repair.

OBJECTIVES: Fenestrated-branched endovascular aortic repair (FB-EVAR) has been increasingly utilized to treat complex abdominal aortic aneurysms (CAAAs) and thoracoabdominal aortic aneurysms (TAAAs). Textbook Outcome (TO) is a composite quality metric designed to reflect the ideal postoperative course. The aim of this study was to evaluate the incidence of TO following FB-EVAR and to identify predictors of TO failure. METHODS: Consecutive patients enrolled in a prospective, non-randomized sponsor-initiated investigational device exemption study to assess FB-EVAR for CAAAs and TAAAs between 20

Prognostic Significance of Noninvasive Simultaneous Renal and Cardiac Perfusion: Interrogating Mechanisms of Cardiovascular-Kidney Interactions.

BACKGROUND: Physiologic interaction between the cardiovascular and renal systems is pivotal in the understanding of disease and as a target for therapeutic interventions, as highlighted in the cardiovascular-kidney-metabolic syndrome. This study explores the association of renal blood flow, derived noninvasively from cardiac positron emission tomography-computed tomography, with cardiovascular and renal outcomes. METHODS: We evaluated the association between renal blood flow and outcomes in a retrospective cohort of 295 consecutive patients who underwent 13N-ammonia positron emission tomograp

Reproductive risk factors and prevalence of cardiovascular-kidney-metabolic syndrome among young women.

INTRODUCTION: Cardiovascular-Kidney-Metabolic (CKM) syndrome provides a framework for early cardiometabolic and kidney dysfunction. While reproductive risk factors are associated with cardiovascular disease, their relationship to CKM syndrome stages remains unclear. We estimated CKM prevalence among reproductive-aged US women and evaluated associations with reproductive risk factors. METHODS: Using the National Health and Nutrition Examination Survey (2013-2020), we included 3,917 non-pregnant women aged 20-49 years. Reproductive risk factors included early menarche (≤10 years), infertility,

Defining lactation outcomes, milk composition, and breastfeeding safety for women with chronic kidney disease: protocol for a prospective observational study.

BACKGROUND: Among persons of childbearing age, advanced chronic kidney disease (stage 3–5) reportedly affects approximately 1 in 150 individuals. Birth rates compared to the general population are lower in kidney transplant recipients and in individuals undergoing dialysis, estimated at approximately 1:10 and 1:100, respectively. Despite these challenges, the incidence of pregnancy among those with chronic kidney disease is increasing, and birthing parents with advanced chronic kidney disease, including those undergoing dialysis or with a kidney transplantation, express a strong desire to brea

Effects of multicomponent exercise on frailty in older adults with type 2 diabetes: Differential responses by metabolic frailty phenotype.

BACKGROUND: Frailty in older adults with type 2 diabetes mellitus (T2DM) is metabolically heterogeneous, but whether multicomponent exercise (MCE) produces phenotype-specific functional benefit remains unclear. We asked two linked clinical questions: does a supervised 24-week MCE program improve physical function and frailty versus usual care, and are those effects modified by metabolic frailty phenotype (anorexic-malnourished [AM] phenotype vs sarcopenic-obese [SO] phenotype)? METHODS: In this 24-week, phenotype-stratified, parallel-group randomized controlled trial, 146 community-dwelling a

Electronic alert-based computerized decision support for detection of chronic kidney disease in type 2 diabetes mellitus: CKD-DETECT.

BACKGROUND: Urine albumin-to-creatinine ratio (UACR) provides a reliable method for chronic kidney disease screening in patients with type 2 diabetes, yet its use remains underutilized. METHODS: The CKD-DETECT randomized clinical trial included outpatients ≥18 years with type 2 diabetes without UACR testing in the prior 12 months and no chronic kidney disease. Physicians were randomized to receive either an alert prompting UACR testing or no alert (control). The primary outcome was the proportion of UACR orders within 90 days. The secondary outcome was new chronic kidney disease stage 3-5 dia

Iron Deficiency Screening in Patients Admitted with Acute Heart Failure Decompensation.

Correction of iron deficiency (ID) with intravenous iron among patients with heart failure (HF) may improve outcomes but is rarely assessed in real-world studies. Hospital admission for decompensated HF presents an opportunity to identify patients for treatment of ID. Of 2,275 patient admissions for decompensated HF, iron studies were available for 526 (23.1%) and ID was identified in 332 (63.1%) patients. Iron replacement was oral in 241 (10.6%) or intravenous in 285 (12.5%) with 132 (46.3%) patients receiving treatment during the admission. Anemia, chronic kidney disease stage 3 (CKD3), and

Relationship between vitamin D status, glycaemia and nephropathy in adults with type 2 diabetes mellitus.

BACKGROUND: Diabetes mellitus (DM) remains a major cause of morbidity and mortality. Poor glycaemic control is a key determinant of diabetes-related complications, and vitamin D deficiency has emerged as a potential contributor to impaired glycaemic control. End-stage renal disease is one of the most severe complications of DM, imposing substantial individual and public health burdens. This study aimed to determine the prevalence and risk factors of vitamin D deficiency among patients with type 2 diabetes mellitus (T2DM) and to evaluate its association with glycaemic control and renal function

Zero-Contrast Percutaneous Renal Denervation Guided by Multimodal Image Fusion.

As a second-line treatment for resistant hypertension or uncontrolled hypertension, renal denervation (RDN) has been widely adopted globally. Conventional RDN procedures require the use of contrast agents, which pose risks of exacerbating renal impairment and increasing cardiac burden for patients with concurrent renal or cardiac insufficiency. A 43-year-old man, diagnosed with hypertension for >10 years, presents with combined chronic kidney disease stage 3 and heart failure. Despite standardized medication therapy, his blood pressure remains poorly controlled. Due to concerns from the patien

When Dysphagia Is Not Psychogenic: An Atypical Presentation of Schatzki Ring in a Patient With Schizophrenia.

Schatzki rings are benign distal esophageal rings that are often asymptomatic but may cause intermittent solid-food dysphagia or acute food bolus impaction. Symptoms are classically localized to the lower chest; however, atypical throat-localized symptoms may obscure the diagnosis, particularly in patients with psychiatric comorbidity. We report the case of a 74-year-old woman with schizophrenia, gastroesophageal reflux disease, chronic kidney disease stage 3, type 2 diabetes mellitus, hypertension, and hyperlipidemia who presented with three days of globus sensation and progressive inability
