A Rare Presentation of Pseudomonas Putida Bacteremia Secondary to Skin and Soft Tissue Infection.

Pseudomonas putida is a rod-shaped, flagellated, non-lactose fermenting, gram negative bacterium, usually found in water and soil habitats. P. putida generally causes nosocomial infections in immunocompromised patients. Increased infection rates are seen in those patients with medical devices inserted, due to this organism's innate ability to attach to moist and inanimate objects. In this case report we present a 63-year-old male patient with a past medical history of type II diabetes mellitus, chronic kidney disease stage 5, and hypertension who presented with P. putida cellulitis and bactere

Ambulatory Blood Pressure Monitoring During 52 Hours in Patients With Chronic Kidney Disease and Haemodialysis Treatment-An Exploratory Pilot Study.

BACKGROUND: Hypertension in patients with haemodialysis is mainly characterised by high pre-dialysis blood pressure (BP) due to body fluid retention before dialysis, and the BP tends to decrease after dialysis due to removal of water during dialysis. Intervals between haemodialysis treatments vary and a long inter-dialytic interval dialysis is associated with increased mortality. OBJECTIVE: To examine variations in BP; how ambulatory BP vary during a dialysis treatment performed after the long inter-dialytic interval, that is, the first treatment of the week, compared to dialysis after a shor

Biomarkers to predict kidney outcomes in children with IgA vasculitis.

Immunoglobulin A (IgA) vasculitis (IgAV, also known as Henoch-Schoenlein purpura, HSP) is a small vessel vasculitis, most commonly presenting in childhood. In most, it has a straightforward, self-limiting disease course, however some children may develop kidney involvement (IgAV-N) which occurs 4-12 weeks following disease onset and remains the biggest contributor to long-term morbidity. Therefore, children undergo a six-month period of kidney monitoring to identify nephritis via surrogate markers including urinalysis and blood pressure measurements. On-going efforts aim at earlier identificat

A Case Report of Successful Use of Phenytoin/Fosphenytoin in a Pediatric Kidney Transplant Recipient With Nirmatrelvir/Ritonavir-Induced Tacrolimus Allograft Injury.

BACKGROUND: Paxlovid, a fixed combination nirmatrelvir and ritonavir (NIM-RTV), is a potent inhibitor of cytochrome P450 3A4 (CYP3A4) isoenzyme. It is approved for the treatment of mild to moderate COVID-19 infections in patients at risk for serious infection. The metabolism of tacrolimus, a CYP3A4 substrate, is significantly reduced in those receiving NIM-RTV. Coadministration of NIM-RTV without tacrolimus dose reduction may result in toxicity. CYP3A4-inducing medications, including phenytoin, fosphenytoin or rifampin, may reverse toxicity while achieving rapid clearance. CASE PRESENTATION:

Twice against thrice-weekly hemodialysis (TATH): a multicenter nonrandomized trial.

BACKGROUND: The optimal frequency of maintenance hemodialysis remains a subject of debate. In many countries, twice-weekly hemodialysis is still commonly practiced. This trial aimed to compare the outcomes of patients undergoing twice-weekly versus thrice-weekly hemodialysis. METHODS: This prospective, multicenter, nonrandomized trial included incident adult patients, with chronic kidney disease stage 5, initiating hemodialysis between January 2018 and August 2021. Patients were allocated to either a twice-weekly or thrice-weekly regimen, and monitored at 1, 3, 6, 12 and 24 months. This trial

Changes in Protein Expression of Renal Drug Transporters and Drug-Metabolizing Enzymes in Autosomal Dominant Polycystic Kidney Disease Patients.

Autosomal dominant polycystic kidney disease is the most prevalent inherited kidney disease and leads to bilateral kidney enlargement and progressive loss of renal function, often over decades. Comorbidities include hypertension, flank pain, and bacterial infections. The condition often necessitates prolonged multidrug therapy. Given the kidneys' critical role in drug excretion, the progressive functional impairment in the disease can lead to complications such as drug overdosing and unexpected levels of drug-drug interactions. Studies of drug-metabolizing enzyme and transporter expression in

Retrospective Analysis of the Association of Preoperative Sex Hormone Therapy With the Development of Postoperative Acute Kidney Injury.

BACKGROUND: Postoperative acute kidney injury (AKI) worsens surgical outcomes. Previous studies have observed an age- and sex-dependent effect on postoperative AKI rates. The objective of our study was to determine whether preoperative exposure to male or female sex hormone therapies modified AKI risk after both noncardiac and cardiac surgery. We hypothesized that women older than 55 years on estrogen/progesterone replacement therapy and men on antiandrogen therapy would have lower odds of postoperative AKI compared to counterparts not receiving sex hormone therapies. METHODS: We conducted a

Application of a reverse stethoscope to overcome communication barriers: A case report of an elderly patient with illiteracy and profound hearing loss.

RATIONALE: As a common clinical diagnostic tool, stethoscopes have the ability to amplify sound, isolate ambient noise, and locate the source of sound. However, we reversed the stethoscope innovatively. This changed its standard use, delivering sound directly to the patient's ear. It successfully overcame communication barriers between clinicians and patients who have both illiteracy and severe hearing loss (HL). PATIENT CONCERNS: This paper reports a case of a 74-year-old elderly male patient with illiteracy combined with very severe HL requiring a hearing decibel of 82 dB, living alone, who

Neutrophil extracellular traps drive peritoneal inflammation and tissue remodeling in pediatric peritoneal dialysis.

BACKGROUND: Peritoneal dialysis (PD) sustains children with chronic kidney disease stage 5 (CKD5) but promotes peritoneal membrane remodeling. Neutrophil extracellular traps (NETs) orchestrate antimicrobial defense and sterile inflammation; their involvement in PD-induced transformation is unknown. METHODS: Forty-five children were enrolled in the International Pediatric Peritoneal Biobank. Peritoneal biopsies taken at PD initiation and after ≥ 12 months of low-glucose-degradation-product PD were compared with surgical biopsies from non-uremic peers. Histomorphometry quantified microvessel de

Mono-allelic pathogenic variants in JAG1 cause autosomal dominant tubulo-interstitial kidney disease (ADTKD-JAG1).

INTRODUCTION: Autosomal dominant tubulointerstitial kidney disease (ADTKD) is a common monogenic kidney disease leading to kidney failure usually during mid adulthood. It is due to pathogenic variants in at least five genes. However, despite thorough screening of UMOD, MUC1, REN, HNF1B and SEC61A1, 25 to 50% of families remain without molecular diagnosis. METHODS: Here, we investigated a cohort of 203 families with ADTK, as well as sporadic cases of kidney disease of unknown etiology and cases of chronic kidney disease stage 5 from the Genomics England 100,000 Genomes Project. Expression of J

Non-valvular atrial fibrillation in patients on peritoneal dialysis, prevalence, treatment and professionals involved.

Atrial fibrillation is the most frequent chronic arrhythmia in patients with chronic kidney disease. Oral anticoagulation with vitamin K antagonists and now direct oral anticoagulants have been and are the fundamental pillars for the prevention of thromboembolic events. However, there are no randomized clinical trials on the risk-benefit profile of oral anticoagulation in patients with chronic kidney disease stage 5 on peritoneal dialysis and there is little evidence in the literature in this population. The objective of our study was to know the prevalence, treatment and professionals involve
