KEY POINTS: During follow-up, 326 (1%) critical illness survivors with AKI developed ESKD; 2723 (9%) died. Black race and lack of health insurance were independently associated with higher risk of developing ESKD. Residence in a socioeconomically deprived neighborhood was independently associated with higher risk of ESKD.
BACKGROUND: Adverse social determinants of health (SDOHs) are associated with higher risk of AKI. This study evaluated the association of individual- and neighborhood-level SDOHs with incident ESKD among intensive care unit (ICU) survivors of AKI.
METHODS: Thirty thousand f

BACKGROUND: Chronic kidney disease (CKD) care is increasingly reliant on digital tools. However, diverse abilities among patients may create a digital divide, exacerbating disparities in health outcomes. Measuring digital health literacy is the first step towards gaining knowledge to develop targeted, patient-centred strategies. Due to a lack of available evidence, this study aimed to assess digital health literacy levels among patients with CKD.
METHODS: A multicentre cross-sectional study was conducted between July 2024 - January 2025, enrolling CKD patients from three Nephrology Units in c

BACKGROUND: Sarcopenia is an increasingly recognized complication of chronic kidney disease (CKD), contributing to impaired physical function and higher mortality. While most evidence derives from elderly populations, its frequency and inflammatory correlates in younger adults remain underexplored. This study aimed to investigate the prevalence of sarcopenia and its association with inflammatory markers in a non-geriatric CKD cohort, including both hemodialysis and non-dialysis patients.
METHODS: This cross-sectional observational study included 128 non-geriatric CKD patients (18-65 years), 6

While a few studies have investigated renal manifestations associated with mitochondrial DNA (mtDNA) variants, detailed renal histopathological changes and long-term outcomes remain poorly characterized. This study reported a family in which all 5 siblings presented with hyperuricemia and chronic kidney disease (CKD); the proband died of end-stage kidney disease at 16 years of age, while both parents were unaffected. Renal histology from 3 siblings revealed multiple foci of sclerotic and atubular glomeruli, as well as focal tubular atrophy, making early pathologic diagnosis challenging. Ten ye

PURPOSE OF REVIEW: Healthcare significantly impacts global warming, with nephrology and dialysis being major contributors. This has led to the emergence of the green dialysis movement. However, literature summarizing recent advances and providing clear implementation frameworks remains limited.
RECENT FINDINGS: In this narrative review, we explore interventions that reduce the global footprint of dialysis. Through the conceptual framework of the Six W's of Sustainable Dialysis, we aim to facilitate the implementation of green dialysis initiatives by improving knowledge and raising awareness.

The year 2025 was crucial for Nephrology: both the World Health Organization (WHO) and the United Nations (UN) formally recognized chronic kidney disease (CKD) as a global health priority among noncommunicable diseases in May and December 2025, respectively. This important milestone is expected to increase worldwide awareness of the urgent need to address the growing burden of CKD. Nephrology is currently undergoing a major paradigm shift: a "back to the future" transition from replacing kidney function through dialysis to preserving kidney function through nephroprotective strategies, which r

Peritoneal dialysis (PD)-associated peritonitis is a leading cause of technique failure. We aimed to develop and internally validate a Cox regression‑based nomogram for predicting peritonitis‑free survival in incident PD patients. This single-center retrospective cohort study included 372 patients who underwent peritoneal dialysis between June 2018 and December 2024, with follow-up through December 2025. LASSO‑Cox regression was used to select predictors from 23 candidate variables. A nomogram was constructed to predict 6‑, 12‑, and 24‑month peritonitis‑free survival. Model performance was ass

OBJECTIVE: Healthcare disparities between rural and urban regions highlight barriers to care, yet access to nephrology services in nonmetropolitan areas remains poorly characterized. This study evaluates the distribution of nephrologists and dialysis centers across Georgia and examines how these patterns relate to county-level chronic kidney disease (CKD) prevalence.
METHODS: The Georgia All-Payer Claims Database estimated CKD prevalence, the Centers for Medicare & Medicaid Services Dialysis Facility Care Compare database identified dialysis centers, and the Georgia Composite Medical Board Ph

The impact of ureteroscopy (URS) and retrograde intrarenal surgery (RIRS) on renal function in patients with chronic kidney disease (CKD) remains unclear, particularly given concerns regarding intrarenal pressure and repeated interventions. We performed a systematic review and meta-analysis to evaluate changes in estimated glomerular filtration rate (eGFR) following URS or RIRS in patients with pre-existing CKD. A systematic review and meta-analysis were conducted according to PRISMA guidelines and registered in PROSPERO (2025). Databases were searched in March 2026. Studies reporting pre- and

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disorder and a leading cause of kidney failure. Although typically considered an adult-onset condition, a subset of patients present in utero or within the first 18 months of life (very early-onset ADPKD; VEO-ADPKD). This subgroup may experience a more severe disease course.
METHODS: A systematic search of MEDLINE was conducted to identify studies reporting clinical, genetic, radiologic and kidney outcome data in children (< 18 years) with VEO-ADPKD. Case series including fewer than three VEO-

Cardiovascular disease is the leading cause of death in chronic kidney disease (CKD), largely driven by accelerated atherosclerosis and vascular calcification. These complications cannot be fully explained by traditional cardiovascular risk factors, indicating that CKD-specific mechanisms may contribute to persistent vascular inflammation. Trained immunity, a long-lasting functional reprogramming of innate immune cells mediated by metabolic and epigenetic remodeling, has emerged as a plausible mechanistic framework linking uremic toxin accumulation to cardiovascular injury. However, the level

BACKGROUND: Chronic kidney disease (CKD) associated osteoporosis leads to an increased risk of bone fracture in patients with CKD. Evidence on the efficacy and safety of anti-osteoporotic medications is limited for this group. This systematic review aimed to examine the efficacy and safety of anabolic parathyroid hormone (PTH) treatment in patients with CKD Stages 4-5D.
METHODS: MEDLINE, EMBASE and ClinicalTrials.gov were searched systematically for randomized controlled trials (RCTs) investigating teriparatide, natpara, or abaloparatide in adult patients with CKD Stages 4-5D, including kidne

BACKGROUND: Chronic kidney disease (CKD) is a major global health burden characterized by progressive loss of renal function and high cardiovascular morbidity and mortality. Sodium-glucose cotransporter 2 (SGLT2) inhibitors, initially developed as glucose-lowering agents, have demonstrated pleiotropic metabolic and cardiorenal benefits. However, previous evidence syntheses have not fully clarified the magnitude of metabolic, hemodynamic, renal surrogate, and safety effects specifically in patients with diabetic CKD.

Two patients undergoing hemodialysis with long-standing, refractory restless legs syndrome were treated with pharmacologic therapy and high-efficiency online hemodiafiltration, achieving an α1-microglobulin removal rate of ≥ 40%. However, sufficient symptom improvement was not achieved. Therefore, online hemodiafiltration using a vitamin E-bonded polysulfone membrane, which has antioxidant effects, was introduced. Although only limited improvement was observed during predilution online hemodiafiltration, marked clinical benefit was obtained after switching to postdilution online hemodiafiltrat

Roxadustat, a hypoxia-inducible factor prolyl hydroxylase (HIF-PH) inhibitor, is effective for treating renal anemia, with efficacy noninferior to erythropoiesis-stimulating agents. However, data on its adverse effects remain scarce. In medical practice, roxadustat administration reportedly leads to central hypothyroidism; however, the frequency of its occurrence remains unclear. We investigated thyroid function in patients undergoing dialysis who received roxadustat at Saiseikai Yokohamashi Tobu Hospital, with particular focus on the timing of the decrease in serum thyroid-stimulating hormone

BACKGROUND: Pre-emptive kidney transplantation is the preferred treatment in patients with end-stage kidney disease (ESKD); however, many patients must unavoidably initiate dialysis prior to transplantation. Prolonged dialysis exposure is associated with increased morbidity and mortality, mandating prompt transition to transplant when possible. Delays in transplant readiness are unfortunately prevalent and are often driven by fragmented communication, limited education, and system-level barriers.
OBJECTIVE: To reduce time from dialysis initiation to deceased donor kidney transplant (DDKT) act

Chronic kidney disease (CKD) and cardiovascular disease (CVD) are major causes of morbidity and mortality in people with type 2 diabetes (T2D). Contemporary CKD guidelines increasingly support incorporation of glucagon-like peptide-1 receptor agonists (GLP-1RAs) as an additional pillar of cardiorenal therapy alongside renin-angiotensin-aldosterone system (RAAS) blockade, sodium-glucose co-transporter 2 inhibitors (SGLT2i) and finerenone. Semaglutide is the first GLP-1RA with kidney-outcome data from a dedicated trial in T2D-CKD: the Evaluate Renal Function with Semaglutide Once Weekly (FLOW) s

Chronic kidney disease (CKD) represents a significant health burden worldwide, characterized by a gradual decline in kidney function and increased risk of cardiovascular events. Current therapeutic strategies like inhibition of the renin-angiotensin-aldosterone system or sodium-glucose cotransporter 2 inhibitors (SGLT2i), while beneficial, often fail to fully halt CKD progression, and a significant residual risk of disease progression persists. Glucagon-like peptide-1 receptor agonists (GLP-1 RAs), initially developed for the treatment of type 2 diabetes (T2D), have emerged as promising agents
