BACKGROUND: End-stage renal disease patients on maintenance hemodialysis are at high risk of severe infections. The co-occurrence of methicillin-sensitive Staphylococcus aureus septicemia and penetrating aortic ulcer in this population is rare, posing notable therapeutic challenges.
METHODS: A case of a 73-year-old male end-stage renal disease hemodialysis patient was analyzed. The patient presented with left flank pain, nausea, and anorexia. Diagnoses and treatment adjustments were based on laboratory tests, computed tomography angiography, and serial monitoring of biomarkers and imaging.

BACKGROUND: Patients with chronic kidney disease (CKD) are more vulnerable to tuberculosis (TB) because of weakened immune systems, which is a serious health risk, especially in low-to-middle-income nations.
EPIDEMIOLOGY: CKD patients have a 6.9 to 52.5 times higher risk of developing TB, with dialysis patients at even greater risk.
PATHOGENESIS: Impaired cell-mediated immunity, malnutrition, Vitamin-D deficiency, and Zinc deficiency contribute to TB susceptibility in CKD patients.
DIAGNOSTIC CHALLENGES: Atypical symptoms, nonspecific presentation, and limited diagnostic tools hinder TB dia

RATIONALE: Cystic fibrosis (CF) is a common genetic condition in which progressive lung disease leads to morbidity and mortality. Non-tuberculous mycobacteria (NTM) are mycobacteria, other than those in the Mycobacterium tuberculosis complex, and are commonly found in the environment. NTM pulmonary infections affect a significant proportion of people with CF worldwide, which may be associated with a more rapid decline in lung function and even death in certain circumstances. Although there are guidelines for the antimicrobial treatment of NTM lung disease, there is no specific evidence from st

The "10 Recommendations for Global Kidney Health" developed at the Global Policy 2017 constitute a commitment by and challenge to all stakeholders. Achieving these goals in disadvantaged population such as the First Nation Peoples (FNP) is even more challenging. Qom and Wichi ethnics are isolated Argentine FNP with poor access to health care. Research on chronic kidney disease (CKD) in Qom FNP in the Chaco Province, Argentina showed high rates of proteinuria without a decrease of the glomerular filtration rate in a group of 385 persons in 2003. In 2018, a subsequent cross-sectional evaluation

The demand for resources for mycobacterial culture makes diagnosing non-tuberculous mycobacteria (NTM) difficult. This study assessed probe amplifications in the Xpert MTB/RIF Ultra (Xpert Ultra) and Xpert MTB/RIF assays for different NTM species identified from diabetes mellitus (DM) and chronic kidney disease (CKD) patients. Since the sources of NTM isolates were DM and CKD patients, the study was limited to those patients. We ran the Xpert Ultra and Xpert MTB/RIF assays on 87 NTM culture isolates identified from DM (41 isolates) and CKD (46 isolates) patients, as well as seven distinct NTM

BACKGROUND: There is increasing evidence that patients with chronic kidney disease (CKD) are at risk of acquiring latent tuberculosis infection (LTBI) or developing active tuberculosis (TB). However, the diagnosis of LTBI is still a challenge due to the limitations of current tests, which rely on the host’s cellular immune response against Mycobacterium tuberculosis (Mtb) – that may be compromised in this population. Herein, we assessed the prevalence of LTBI and its predictors among patients with CKD, including those undergoing routine hemodialysis (HD). METHODS: A prospective cross-sectional

Comorbidities are a significant factor affecting the prognosis of patients with rifampicin-resistant tuberculosis (RR-TB). We evaluated the prevalence of comorbidities and their prognostic effects in a Chinese RR-TB cohort. In this study, we reviewed the clinical data of RR-TB patients who started anti-tuberculosis treatment in China between May 2018 and April 2020 by conducting a multicenter cohort study. A log-binomial regression model was used to analyze the relationship between comorbidities and the treatment outcomes of RR-TB. The burden of comorbidities among RR-TB patients in China was

BACKGROUND: Existing studies indicate that infection may serve as both initiators and accelerators in the pathogenesis of Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) through multiple immunopathological pathways. We report a rare case of AAV combined with two fatal infections of Aspergillus fumigatus and Mycobacterium avium complex (MAC), in which a successful clinical outcome was achieved. CASE PRESENTATION: A 67-year-old female with a history of bronchiectasis was admitted due to poor appetite and fatigue, along with newly identified renal insufficiency. Her serum

BACKGROUND: Adequate vitamin D and calcium nutrition play a critical role in the maintenance of musculoskeletal health and are considered the first step in osteoporosis treatment.
ROUNDTABLE DISCUSSION: In February 2008 Merck Sharp & Dohme sponsored a 2-day, evidence-based expert panel on the benefits of vitamin D for the patient with osteoporosis and the role of vitamin D in combination with antiresorptive therapy for the management of osteoporosis. One of the primary objectives of the meeting was to review new data on the optimal serum 25-hydroxy vitamin D [25(OH)D] levels. The symposium wa

INTRODUCTION: From a public health perspective, it is essential to understand the burden of kidney involvement in leptospirosis. We aimed to assess the frequency of acute kidney injury (AKI) and chronic kidney disease (CKD) in patients with leptospirosis.
METHODOLOGY: This systematic review and meta-analysis included all articles up to 14.08.2024 from three databases (PubMed, Scopus, Web of Science) using search terms related to leptospirosis and kidney involvement. After de-duplication, two independent reviewers independently checked the articles in two phases (title-abstract and full-text),

Levofloxacin, a fluoroquinolone antibiotic, is essential in the treatment protocols for multidrug-resistant tuberculosis (MDR-TB). Generally well tolerated, it infrequently induces central nervous system adverse effects, especially in patients with renal impairment. We present a woman in her early 50s with chronic renal disease and hypothyroidism who experienced focal seizures after 48 hours of commencing levofloxacin as part of her multidrug-resistant tuberculosis treatment regimen. The seizures exhibited unilateral aberrant movements with initially intact consciousness, thereafter followed b

Chronic kidney disease (CKD) is a progressive ailment identified by renal fibrosis, inflammation, and mitochondrial dysfunction, necessitating the development of novel therapeutic interventions. Bifendate (dimethyl diphenyl bicarboxylate [DDB]), a known hepatoprotective agent, has shown promising antifibrotic properties, but its potential in CKD remains unexplored. This study integrates computational pharmacology, molecular docking, and experimental validation to explicate the mechanisms of renoprotection by DDB. Curative targets for CKD and DDB were identified from DisGeNET, GeneCards, and Sw

BACKGROUND: Feline morbillivirus (FeMV) has been associated with renal pathology in cats; however, the specific pathological alterations caused by FeMV infection remain controversial. This study aimed to investigate histopathological changes, viral localization, and apoptotic activity in the kidneys of FeMV-infected cats.
METHODS: Kidney tissues from 150 deceased cats with suspected or confirmed chronic kidney disease (CKD) were screened for FeMV using conventional reverse-transcription PCR (cRT-PCR). Positive cases were genotyped and quantified for viral load using reverse-transcription digi

BACKGROUND: Hypertension is a major risk factor for the progression of chronic kidney disease (CKD). However, the impact of blood pressure (BP) load, especially when office BP is normal, remains unclear. Thus, we conducted a retrospective cohort study to elucidate the impact of elevated BP load in patients with biopsy-proven CKD, BP load elevation, and a normal office BP compared with normotensive patients with CKD but without BP load elevation and those with hypertension.
METHODS: We retrospectively analyzed patients with histologically confirmed atherosclerotic CKD who underwent kidney biop

Emerging treatments for antibody-mediated rejection (ABMR, NEJM391 (2):122-132) have increased the importance of ABMR detection when donor-specific antibody (DSA) is negative. We addressed this issue in the Trifecta-Kidney study (ClinicalTrials.gov #NCT04239703) using 3 centralized tests in 690 kidney transplant biopsies: DSA (One Lambda Inc), blood donor-derived cell-free DNA (dd-cfDNA, Prospera™ test, Natera, Inc), and molecular biopsy assessment (MMDx). We used an "AutoBanff 2022" algorithm to model the impact of alternative DSA interpretations on the histologic diagnosis of DSA-negative AB
