Phenotypic Heterogeneity of Diabetic Kidney Disease and Its Association With Cardiovascular and Renal Outcomes: Evidence From a Chinese Multicentre Cohort.

BACKGROUND: Diabetic kidney disease (DKD), a major microvascular complication of diabetes, is the leading cause of chronic kidney disease (CKD) and progression to end-stage kidney disease (ESKD). DKD exhibits substantial clinical heterogeneity, ranging from the classic proteinuric phenotype to the increasingly recognised nonproteinuric phenotype. However, cardiovascular, kidney and all-cause mortality risks across these phenotypes remain incompletely characterised. METHODS: We conducted a retrospective cohort study of 14 367 adults with DKD in the China Renal Data System (CRDS). Major adverse

Laterality of Hand-Assisted Laparoscopic Donor Nephrectomy in Adult Kidney Transplantation.

INTRODUCTION: Living-donor kidney transplantation is widely performed because of shortage of deceased donors. The left kidney is generally preferred for donor nephrectomy because its longer renal vein facilitates venous anastomosis during recipient surgery. However, comprehensive evaluation of donor and recipient outcomes according to donor nephrectomy laterality remains limited. Therefore, we compared the short- and long-term outcomes of right hand and left hand-assisted laparoscopic donor nephrectomy (HALDN) in living kidney donors (LKDs) and recipients. METHODS: This single-center retrospe

Nephrotic range proteinuria and rapid progression to end-stage kidney disease in seronegative pauci-immune necrotizing glomerulonephritis masked by cutaneous lesions: a case-based review.

Antineutrophil cytoplasmic antibodies (ANCA)-negative pauci-immune necrotizing glomerulonephritis (PING) is a rare entity, representing a distinct subset of small-vessel vasculitis. The absence of ANCA does not exclude the diagnosis, and clinical suspicion should remain high in the presence of compatible systemic and renal findings. We present the case of a 22-year-old woman with recurrent, non-specific cutaneous manifestations since early childhood, who was hospitalized for an exacerbation of skin lesions. Laboratory results revealed nephrotic-range proteinuria of 17 g/d [N < 150 mg/d] and a

Lactylation-dependent DDX18 nucleolar escape regulates CD44 mRNA stability to drive fibrotic progression in kidney injury.

Following acute kidney injury (AKI), a substantial subset of patients experience an irreversible progression to chronic kidney disease (CKD), yet the molecular determinants governing this maladaptive transition remain elusive, and effective clinical interventions are lacking. Here, we identify lactate as a key metabolic determinant orchestrating the transition from AKI to CKD. Analysis of the UK Biobank cohort revealed that elevated circulating lactate independently predicts CKD development in patients with AKI and correlates with fibrotic progression. Using murine ischemia-reperfusion injury

Elevated peak width of skeletonized mean diffusivity in patients with end-stage kidney disease: evidence of diffuse white matter microstructural injury.

BACKGROUND: Peak width of skeletonized mean diffusivity (PSMD) is a diffusion tensor imaging-based marker of diffuse white matter microstructural injury that has been associated with cerebral small vessel disease (CSVD). This study aimed to investigate whether patients with end-stage kidney disease (ESKD) exhibit altered PSMD compared with healthy controls. METHODS: We prospectively enrolled 38 ESKD patients and 38 age- and sex-matched healthy controls. Diffusion tensor imaging (DTI) was performed using a three-tesla MRI scanner in patients with ESKD and healthy controls. We obtained PSMD bas

Optimizing omega-3 supplementation in chronically ill dogs: The impact of body weight and dietary inclusion rates.

Nutritional supplement dosages for dogs are often expressed relative to overall body weight (BW). However, daily food intake scales with metabolic weight (BW0.75). This discrepancy implies that the ingested dose of supplements included in diets does not increase linearly with BW. This study evaluated the impact of this phenomenon on omega-3 supplementation, comparing the theoretical efficacy of extruded therapeutic diets using cardiac (dosage expressed by BW) and chronic kidney disease (dosage expressed by BW0.75) dosage models. The theoretical dietary intake of eicosapentaenoic acid (EPA) and

Impact of peritoneal dialysis fluid composition and infusion timing on 18F-fluoro-2-deoxy-2-d-glucose positron emission tomography-computed tomography interpretability in peritoneal dialysis patients.

BackgroundGlucose and insulin blood levels are well known altering factors for 18F-fluoro-2-deoxy-2-d-glucose positron emission tomography-computed tomography ([18F]-FDG PET-CT) interpretability in peritoneal dialysis (PD) patients. A potential influence of glucose-based solutions and Icodextrin in PD patients has not been studied. The aim of this study was to assess PET-CT interpretability in PD patients according to the osmotic agent used in the dialysis fluid (Icodextrin vs. glucose) and the timing of dialysate infusion before imaging.MethodsWe searched for all PET-CT performed on individua

Unlocking the benefits of allopurinol: the role of timely initiation and adherence in cardiovascular and renal outcomes in hyperuricemia.

BACKGROUND: Increasing evidence links elevated serum uric acid levels to adverse cardiorenal outcomes. To date, limited data exist on whether the benefits of urate-lowering therapies (ULTs) depend on timely treatment initiation and sustained adherence in patients at high cardiovascular risk. METHODS: An economic evaluation was conducted, based on a simulation model populated by Italian real-world patient-level data from the Uric Acid Right for Heart Health (URRAH) cohort to evaluate long-term cardiovascular, renal, and economic outcomes associated with timely allopurinol use. 10,000 patients

The Intersection of Heart Failure with Preserved Ejection Fraction and Chronic Kidney Disease.

Heart failure and chronic kidney disease (CKD) often co-exist. Heart failure with preserved ejection fraction (HFpEF) contributes to significant morbidity and mortality among patients with CKD. Among patients with CKD, along with traditional risk factors for heart failure, numerous CKD-specific pathophysiological pathways contribute to the heightened risk of HFpEF development. However, the heterogeneity of HFpEF presents numerous challenges for diagnosis, treatment, and prevention, which has led to various phenotyping approaches. Phenotyping HFpEF may allow for better HFpEF characterization fo

A Novel Spatial Correlation Analysis of Peripheral Hemodynamics in a Murine Model of Vascular Calcification Using NIRS Imaging.

Vascular calcification (VC) is a major contributor to cardiovascular diseases (CVD) and progresses rapidly in patients with chronic kidney disease (CKD). Recently, we developed a low-cost, near-infrared spectroscopy (NIRS)-based imaging device (near-infrared optical scanner-NIROS) and used it to detect VC in the aortae via peripheral vascular imaging of the tail in mouse models. Our initial studies indicated distinct hemodynamic changes in the murine tail with and without the presence of VC. This study aims to assess the overall changes in peripheral hemodynamic flow patterns in mice with and

Oleanolic Acid-Enriched Olive Oil Attenuates Kidney Function Decline in Patients With Type 2 Diabetes: Results From the OLTRAD Randomised Controlled Trial.

AIMS: Chronic kidney disease (CKD) is a major complication of type 2 diabetes mellitus (T2DM), driven in part by oxidative stress and the accumulation of advanced glycation end products (AGEs). Oleanolic acid (OA), a natural triterpene present in olive-derived matrices, may modulate AGE metabolism. In this prespecified, exploratory secondary analysis of the OLTRAD trial, we evaluated whether long-term intake of OA-enriched olive oil attenuates kidney function decline in patients with T2DM. METHODS: In the OLTRAD randomised controlled trial, 97 patients with T2DM were assigned to consume OA-en

Differences in dietary intake assessed by dietary recall and that estimated by protein nitrogen appearance rate.

Peritoneal dialysis (PD) patients are at risk of muscle wasting and clinical guidelines recommend assessing dietary intake, by calculating protein equivalent of nitrogen appearance (PNA) to assure adequate intake. As the PNA equations were developed some time ago based on continuous ambulatory PD (CAPD) treatments, we re-evaluated them by comparing dietary recall protein intake. Dietary histories were obtained from 42 PD patients, mean age 62.2 ± 15.8 years, 58.1% male, 41.9% white, 39.5% treated with CAPD, median dialysis vintage 13.2 (3.1-23.3) months when attending for routine outpatient pe

Cardiovascular Risk Assessment Tools in CKD.

Cardiovascular (CV) risk calculators estimate the likelihood of CV events by integrating factors such as age, sex, BP, lipids, smoking, and diabetes. Commonly used tools in the general population include the Framingham Risk Score, Systematic Coronary Risk Evaluation, atherosclerotic cardiovascular disease Pooled Cohort Equations, and QResearch Cardiovascular Risk Algorithm (QRISK), the latter being regularly updated using large-scale UK health records and including a wider range of variables such as CKD. CKD is a strong, independent risk factor for CV disease, but most standard models omit kid
