OBJECTIVES: To determine the prevalence of chronic kidney disease (CKD) in internal medicine outpatients in a Central European tertiary care setting, identify associated risk factors and assess use and potential impact of renoprotective therapies. We hypothesised that CKD is common, underdiagnosed and undertreated.
DESIGN: Cross-sectional observational study.
SETTING: Tertiary care outpatient clinics in Central Europe.
PARTICIPANTS: 1933 adult outpatients at risk for CKD were screened; an expanded cohort of 2903 patients with available laboratory data was included for secondary analyses. In

BACKGROUND: In the UK there is variation in which tests are used for monitoring Long Term Conditions (LTCs). Guidance that underpins testing practices for LTCs is often based on expert opinion rather than evidence. There is little insight into the views of primary care staff as to why such variation exists or on whether current LTC blood test monitoring is optimal.
AIMS: To understand the views of Healthcare Professionals on blood test monitoring for three LTCs (type 2 diabetes, hypertension and chronic kidney disease), identify factors that shape monitoring practices for LTCs and inform the

Resident renal fibroblasts are a predominant source for myofibroblasts that produce ECM to contribute to renal fibrosis in the course of chronic kidney disease. The mechanism whereby fibroblast-myofibroblast transition (FMyT) is driven remains incompletely understood. In the present study we investigated the involvement of ten-eleven translocation 3 (TET3) in regulating fibroblast-myofibroblast transit ten-eleven translocation ion and renal fibrosis focusing on the regulatory mechanism and translational potential. We report that TET3 deletion in primary renal fibroblasts dampened myofibroblast

BACKGROUND: Immunological checkpoint inhibitors (ICIs) have shown promise in treating various malignancies but are understudied in genitourinary cancers among patients with advanced chronic kidney disease (CKD), who are typically excluded from clinical trials. We evaluated the efficacy and safety of ICIs in this high-risk patient group.
METHODS: This retrospective cohort study included patients with CKD diagnosed with renal cell carcinoma and urothelial carcinoma, utilizing data from 63 healthcare organizations in the TriNetX US Collaborative Network database between January 2015 and December

BACKGROUND: Chronic obstructive pulmonary disease (COPD) and chronic kidney disease (CKD) frequently coexist, yet mortality trends in patients with both conditions remain poorly characterized.
METHODS: Using the CDC WONDER Multiple Cause of Death database, we analyzed death certificates from 1999 to 2020 for adults aged ≥ 25 years with COPD (ICD-10: J40-J44) as the underlying cause and CKD (N18) as a contributing cause. Age-adjusted mortality rates (AAMRs) per 100,000 population were calculated using 2000 US standard population. Trends were assessed via Joinpoint regression, with stratificati

BACKGROUND: Heat waves are increasingly recognized as an environmental determinant of chronic kidney disease (CKD). However, the CKD burden attributable to heat waves and associated healthcare inequalities, particularly under future climate change, remains insufficiently characterized.
METHODS: Based on two nationally representative cross-sectional surveys in China, we established climate region-specific exposure-response functions between heat waves and CKD. Then, we conducted 1-km grid-level health impact assessments integrating the most up-to-date nationally representative CKD prevalence d

BACKGROUND: Chronic kidney disease (CKD) is a major global public health challenge associated with high morbidity, premature mortality, and substantial healthcare costs. In Sri Lanka, CKD is a public health priority due to the high prevalence of diabetes, hypertension, and chronic kidney disease of unknown etiology (CKDu). Although public healthcare services are largely free at the point of delivery, CKD imposes considerable financial pressure on households through out-of-pocket expenditure (OOPE). This study assessed the household economic burden among CKD patients attending a tertiary care h

Cellular senescence is a pivotal driver of the transition from acute kidney injury (AKI) to chronic kidney disease (CKD). We previously identified Pannexin 1 (Panx1) as an endoplasmic reticulum (ER)-resident calcium (Ca2+) leak channel that promotes renal tubular senescence; however, the precise downstream effectors remain elusive. Here, we establish a novel mechanistic link between Panx1 and the cGAS-STING pathway. We show that Panx1 triggers cGAS-STING activation by facilitating the cytosolic release of mitochondrial DNA (mtDNA). Mechanistically, we establish a strict hierarchical relationsh

KEY POINTS: The Predicting Risk of Cardiovascular Disease EVENTs equations optimize risk prediction in the modern era of increasing cardiovascular kidney metabolic disease prevalence. Risk prediction is most helpful to intensify therapy in those with less advanced kidney disease. In those with advanced kidney disease, risk indices provide little utility beyond biomarkers alone such as eGFR and albuminuria.
BACKGROUND: Cardiovascular kidney metabolic syndrome is a growing public health problem and leading cause of cardiovascular mortality. We evaluated model performance of the American Heart A

Chronic kidney disease-associated pruritus (CKD-aP) remains a prevalent and burdensome symptom whose management is often empirical and only partially effective. Its pathophysiology appears to arise from interacting systemic, cutaneous, neural, and central mechanisms rather than from a single dominant pathway. In this review, we synthesize the evidence for a broader neuroimmune framework encompassing retained uremic solutes, microinflammation, xerosis and barrier dysfunction, immune dysregulation, neuropathic sensitization, opioid imbalance, and neuropeptide signaling. On this basis, we propose

KEY POINTS: In nationwide nephrology-referred CKD, sodium-glucose cotransporter 2 inhibitor use was associated with lower cardiovascular event and KRT risk. Associations with cardiovascular events and KRT were consistent in advanced CKD, a group underrepresented in trials. Findings supported the applicability of trial results to a broad nephrology-referred CKD population.
BACKGROUND: Sodium-glucose cotransporter 2 inhibitors (SGLT2 inhibitors) reduce cardiovascular (CV) and kidney events in randomized trials, yet evidence on their effectiveness in routine nephrology practice remains limited.

AIMS: Chronic kidney disease (CKD) remains a leading complication of diabetes, and understanding population-level trends is crucial for prevention and healthcare planning. We examined national trends in CKD prevalence, incidence, and mortality among people with diabetes.
METHODS: In this nationwide register-based cohort study, we identified people with diabetes in Denmark between 2016 and 2024 using linked national registers. CKD was defined by ≥ 2 measurements of estimated glomerular filtration rate < 60 mL/min/1.73 m2 and urinary albumin-creatinine ratio ≥ 30 mg/g, or a hospital diagnosis o

AIMS: Chronic kidney disease (CKD) is associated with uraemic cardiomyopathy characterised by early metabolic dysfunction. Elevated myocardial intracellular sodium (Naᵢ) has emerged as a driver of cardiometabolic remodelling, however, its role in CKD and therapeutic modulation remains unclear. We investigated whether dual sodium-glucose cotransporter (SGLT)1/2 inhibition with sotagliflozin (SOTA) targets Naᵢ and improves cardiac metabolism in CKD.
METHODS AND RESULTS: CKD was induced in male Wistar rats by 5/6 nephrectomy and assessed after 4 weeks. Cardiac phenotype was evaluated using in vi

BACKGROUND: Diabetic kidney disease (DKD), a major microvascular complication of diabetes, is the leading cause of chronic kidney disease (CKD) and progression to end-stage kidney disease (ESKD). DKD exhibits substantial clinical heterogeneity, ranging from the classic proteinuric phenotype to the increasingly recognised nonproteinuric phenotype. However, cardiovascular, kidney and all-cause mortality risks across these phenotypes remain incompletely characterised.
METHODS: We conducted a retrospective cohort study of 14 367 adults with DKD in the China Renal Data System (CRDS). Major adverse
