Assessment of a novel bendable-tip ureteral access sheath with integrated suction: an in vitro study.

Surgical management of urinary stones has significantly evolved with the advent of extracorporeal shockwave lithotripsy, flexible ureterorenoscopy (fURS), and percutaneous nephrolithotomy. For lower pole stones larger than 1 cm, the choice between fURS and miniaturized percutaneous nephrolithotomy remains debated. fURS with ureteric access sheaths (UAS) is a widely utilized technique, but fragment clearance is challenging in unfavorable anatomical conditions and/or lower pole stones. This study compares a novel flexible suction UAS (FANS) with a standard UAS in an in vitro model of lower pole

A Rare Case of a Giant Prostatic Urethral Stone and Its Management.

Prostate calculi, or prostatic stones, are typically small, greyish-brown, ultrasound-dense formations composed mainly of carbonated calcium phosphate apatite. We present a 65-year-old male with severe lower urinary tract symptoms (LUTS), including weak urinary stream, difficulty urinating, and acute retention, who was found to have a 4 cm prostate stone. Imaging confirmed obstruction of the prostatic urethra. Surgical intervention with transurethral resection of the prostate (TURP) and Holmium laser lithotripsy successfully removed the stone and relieved symptoms. This case emphasizes the nee

Associations between ureteral stent indwelling time, patient characteristics, and stent pain from an international prospective registry.

OBJECTIVES: Previous studies have shown conflicting results concerning the optimal duration of ureteral stenting after endourologic treatment of stone disease, its effect on patient comfort, and the necessity for emergent, unscheduled care. This study assessed the impact of stent duration, sex, and other patient-associated factors on reported pain scores using a large, international prospective registry. METHODS: A prospective observational patient registry on ureteral stents from 10 institutions in 4 countries (United States, Canada, France, and Japan) from 2020-2023 was assessed. The primar

Novel Insights Into the Causal Association Between Dietary Factors and Risk of Urinary Calculus: A Multivariate and Two-Step Mendelian Randomization Analysis.

Previous observational studies preliminarily unveiled dietary habits that are associated with urinary calculus (UC). However, the causal association remained unclear. This study employed two-sample Mendelian randomization (MR) to assess the causal effects of dietary factors on UC using genome-wide association data. Multivariable MR (MVMR) was applied to identify independent dietary influences, and two-step MR explored mediation by 30 common biomarkers. Genetically predicted intake of alcohol, coffee (including decaffeinated, instant, and ground coffee), psychoactive drinks, tea, fruits (includ

From Prostate Enlargement to Bladder Stone: Large Bladder Stone as a Complication of Long-Standing BPH: A Case Study.

Vesicular calculi, or bladder stones, are a common occurrence in males of older age. Although there is a variety of different etiologies reported, one of the less common ones includes a background of benign prostatic hyperplasia (BPH). This case report describes a rare presentation of a large vesicular calculus in an old male patient that was diagnosed along with BPH. The patient, with a history of alcohol abuse and hypertension, previously had a percutaneous nephrolithotomy. Diagnosis was confirmed through transrectal ultrasound, x-ray Kidney-Ureter-Bladder, and cystourethroscopy. He underwen

Outcomes of transurethral resection of prostate for benign prostatic obstruction stratified as per duration of prior medical therapy.

INTRODUCTION/AIMS OF STUDY: This original work done in patients with Benign Prostatic Obstruction aims to find out the impact of duration of medication therapy on the outcomes of TURP. Medical management of BPO is recommended as the first line of treatment and surgery is usually reserved for those who have failure of medical management and/or complications of BPO (refractory retention, vesical calculus, recurrent UTI or hematuria). However, there is some pointing evidence that delay in the surgery may affect the overall outcomes of transurethral resection of prostate (TURP) [1, 2] Presently, t

Surgical management of incomplete duplex kidney with calculus and homolateral ureterovesical junction obstruction: a case report and literature review.

BACKGROUND: Congenital anomalies of the kidneys and urinary tract (CAKUT) comprise a large spectrum of congenital malformations, and combined manifestations of CAKUT spectrum could coexist in the same person, which make the management more challenging. We present a rare clinical case of left incomplete duplex kidney with calculus and homolateral ureterovesical junction obstruction (UVJO) that occurred in a female adult, and report the combined surgical management of lithotripsy by flexible ureteroscopy with holmium laser, and refitting the ureter into the bladder after resection of the affecte

The Great Pretender: Xanthogranulomatous Prostatitis Mimicking Prostate Cancer.

Xanthogranulomatous prostatitis (XGP) is a rare chronic inflammatory condition of the prostate that can clinically and radiologically mimic prostate cancer (PCa). We present a case of XGP in an elderly male with markedly elevated prostate-specific antigen (PSA) levels and imaging findings suggestive of locally advanced PCa. A 66-year-old male with a two-month history of intermittent fever was incidentally found to have two vesical calculi (13 mm and 9 mm) and grade 3 prostatomegaly (57 cc) with a large median lobe. He reported no lower urinary tract symptoms (LUTS). The patient underwent elect

How I do it: percutaneous cystolitholapaxy for bladder stones with complex lower urinary tract anatomy.

While cystolitholapaxy for bladder stones is commonly performed using a transurethral approach, large or complex stone burdens in patients with complex lower urinary tract anatomy may make this inefficient or infeasible. Percutaneous cystolitholapaxy is a safe, effective, minimally invasive alternative for diverse indications, including patients with benign prostatic hyperplasia, urethral stricture disease, closed bladder neck, continent catheterized channel, or other urinary diversion. In this article, we review the indications for and advantages of percutaneous cystolitholapaxy and describe

Female prostatic occlusive syndrome: A case report of MRI analysis and literature review.

Female prostatic occlusive syndrome (FPOS) is a rare condition caused by obstruction of the female urethra due to hypertrophic or cystic Skene's glands-the embryologic homologues of the male prostate. Because its clinical manifestations are nonspecific, FPOS is frequently misdiagnosed as urethral diverticulum, cystitis, or other lower urinary tract disorders, often leading to delayed or inappropriate treatment. This study aimed to characterize the MRI features of FPOS and to discuss the diagnostic challenges associated with this entity. We report the case of an 87-year-old woman who presented

Hyponatremia and rhabdomyolysis from excessive water intake for ureteral stone expulsion in a patient with mental disorder: A case report.

RATIONALE: Primary polydipsia refers to excessive water intake due to psychogenic or non-psychogenic causes without being secondary to conditions such as hyperglycemia or renal dysfunction. Most cases of primary polydipsia are psychogenic in nature, with few cases of non-psychogenic primary polydipsia reported in the literature. In this case, the patient's excessive water intake appeared to be influenced by both psychogenic and non-psychogenic factors. PATIENT CONCERNS: A 43-year-old male patient with a history of psychiatric illness was diagnosed with ureteral stones and was engaged in exces

What should the optimal access site be for percutaneous treatment of anterior lower pole calyx stones?

OBJECTIVES: Compared to other stone groups, anterior calyx stones are more challenging for endourologists to treat. This study aims to evaluate the differences between our latest technique and conventional techniques for treating anterior calyx stones in the lower pole. MATERIALS AND METHODS: Patients with isolated anterior lower pole calyx or complex lower pole stones with anterior calyx branching were included in the study. The first group included lower pole access, while the second group included percutaneous treatment through the middle or upper pole. In the first group, the posterior ca

[The 1-cm lower calyx calculus: SWT, URS or Mini-PCNL? Guidelines vs. reality.].

Urinary stones of the upper urinary tract can be considered a widespread public health concern due to their high incidence and prevalence and their health policy-related and financial implications. A significant proportion of newly diagnosed kidney stones are lower-pole stones, i.e., stones affecting the lower calyx group of the renal pelvicalyceal system. These are often diagnosed by chance, i.e., as incidental findings during ultrasound or CT scans performed for other reasons, or as "secondary stones" detected during the diagnostic work-up of symptomatic urinary stones in other locations. Endoscopic combined intrarenal surgery versus percutaneuos nephrolithotomy for complex pediatric stone disease: A comparative analysis of efficacy and safety.

INTRODUCTION: Decision-making for complex pediatric urinary system stone disease is still a challenge for pediatric urologists. The interest in supine percutaneous nephrolithotomy (PCNL) is increasing among centers to achieve high surgical success rates with less morbidity. Despite advanced retrograde intrarenal surgery armamentarium, percutaneous approaches remain the first-line surgical treatment modality for >2 cm and complex renal stones. There are no comparative studies yet in the literature for pediatric endoscopic combined intrarenal surgery (ECIRS) OBJECTIVE: In this study, we aimed to
