BACKGROUND: Killed whole-cell oral cholera vaccines can be used to prevent cholera but require multiple doses and have limited efficacy in young children. PanChol is a single-dose, live-attenuated, oral cholera vaccine derived from a current seventh pandemic Vibrio cholerae O1 strain. It co-expresses Inaba and Ogawa antigens, over-expresses the non-toxic cholera toxin B subunit, and is designed to minimise reactogenicity and be incapable of toxigenic reversion. We aimed to assess safety and immunogenicity of PanChol in a first-in-human trial.
METHODS: This phase 1a trial was conducted at the

BACKGROUND: Studies have shown that food insecurity (FI) during infancy is associated with higher odds of obesity in early childhood, but the evidence regarding the impact of FI interventions on improving child body mass index (BMI) remains inconclusive. This study explores whether reducing food insecurity during infancy is effective in preventing early life obesity. METHODS: Using a randomized comparative effectiveness design, NOURISH delivers two interventions: an “unrestricted benefit” group that receives a cash benefit in the form of study-specific debit cards, and a “grocery benefit” grou

The genus Vibrio encompasses globally relevant pathogens, of which Vibrio cholerae is the best known due to its role in cholera. Closely related species within the Cholerae clade - Vibrio paracholerae, Vibrio metoecus and Vibrio tarriae - were long misclassified as non-O1/O139 Vibrio cholerae. The objective of this study was to analyse all 13,000+ available V. cholerae genomes in GenBank to determine the presence of species from the Cholerae clade. Genome-wide analyses using Mash, whole-genome-based Average Nucleotide Identity and digital DNA-DNA hybridization reclassified 190 unique genomes a

BACKGROUND: Zonulin, a biomarker linked to intestinal barrier dysfunction, may contribute to fetal growth disorders such as intrauterine growth restriction (IUGR) and small-for-gestational-age (SGA) pregnancies. This study evaluated maternal serum zonulin levels as a diagnostic tool for distinguishing IUGR from SGA and investigated their associations with perinatal outcomes.
METHODS: This cross-sectional study included singleton pregnancies between 32 and 37 weeks. Maternal blood samples collected at 16-18 weeks of gestation were analyzed for serum zonulin levels using ELISA, with measurement

UNLABELLED: High genome variability of the 7th cholera pandemic agent, V. cholerae El Tor, led to emergence of genovariants with a distinct set of altered genes. The aim of the work was to analyze the dynamics of changes in pathogenicity, epidemicity, as well as drug resistance and phylogeny in toxigenic strains of V. cholerae El Tor isolated in Russia and endemic regions during three waves of ongoing pandemic.
MATERIALS AND METHODS: We used whole-genome nucleotide sequences of 155 strains, obtained by us (42) and taken from the NCBI Genbank (113). DNA sequencing was performed on Ion PGM plat

Activation of virulence in Vibrio cholerae, the etiological agent of cholera disease, is mediated by two transmembrane one-component signal-transduction proteins, ToxR and TcpP, which are also transcription factors. Using cryo-electron microscopy, we have solved five structures of the ompU and toxT transcription activation complexes, including the RNA polymerase (RNAP) holoenzyme, promoter DNAs, transcribed RNA, and their corresponding transcription factors, ToxR or TcpP and ToxR-TcpP, respectively. Activation is achieved through the interaction of ToxR or TcpP with the α-C-terminal repeat dom

Epidemiology has long been central to public health, guiding our understanding of the distribution and determinants of disease. As the field has evolved-from John Snow's cholera investigations to large-scale cohort studies and causal inference frameworks-it now faces a transformative juncture with the advent of artificial intelligence/machine learning (AI/ML). These technologies offer unprecedented opportunities to improve data measurement, inference, and population health insights, yet also pose methodological and ethical challenges. Anchored by the core epidemiologic domains of study populat

BACKGROUND: Cholera remains a persistent public health threat in fragile and conflict affected settings where inadequate water, sanitation, and hygiene infrastructure, population displacement, and climate shocks contribute to recurrent outbreaks. Somalia has experienced cyclical cholera epidemics for decades, yet recent national level analyses of mortality predictors remain limited. METHODS: A retrospective analysis of national cholera surveillance data was conducted using line lists from cholera treatment centers across Somalia for the period January to December 2024. Suspected and confirmed

Vibrio cholerae, the aetiological agent of cholera, is predominantly an imported pathogen in high-income countries. We report a case of a 49-year-old Nigerian male returning from Africa with abdominal pain and watery diarrhea, who rapidly developed acute kidney injury. The initial diagnosis was Clostridioides difficile enteritis, based on GDH antigen and toxin detection in stool samples. Microbiological cultures subsequently revealed concomitant growth of Vibrio cholerae. The patient required intensive care unit management, including aggressive fluid resuscitation and antimicrobial treatment w

BACKGROUND: In Bangladesh, cholera treatment focuses on acute watery diarrhea in symptomatic cases at health facilities, though asymptomatic infections are common. Understanding the role of asymptomatic infections in transmission is crucial for designing appropriate control strategies in this setting.
METHODS: We utilized data from household studies conducted in Dhaka, Bangladesh during 2006-2018 where a symptomatic confirmed cholera case and their household contacts were followed for thirty days. Vibriocidal antibodies, bacteriological culture, and symptom histories were collected at multipl

This study examines the spatiotemporal dynamics and determinants of waterborne diseases (WBDs) in Algeria between 2000 and 2023, using annual epidemiological reports from the National Institute of Public Health. Five major diseases are analyzed: foodborne disease outbreaks (FBDOs), hepatitis A, dysentery, typhoid fever, and cholera. Descriptive statistics, temporal trend analysis, and age-stratified profiling are applied to identify epidemiological patterns and underlying determinants. Results indicate a significant epidemiological transition: Typhoid fever and dysentery declined by nearly 98%

Niger State in central Nigeria faces a range of natural, biological, and security hazards. To inform preparedness and health security planning, a multi-hazard risk assessment was conducted using WHO's Strategic Tool for Assessing Risks (STAR), this is one of the first applications of WHO STAR at a state level in Nigeria. A cross-sectional study was conducted using the WHO STAR. Stakeholders involved identified hazards across natural, biological, technological, and societal domains through review of surveillance, disaster, and meteorological data. Hazards were scored for likelihood, impact, vul

The unfolded protein response (UPR) is a cellular stress response mechanism that maintains endoplasmic reticulum (ER) homeostasis through three signaling pathways mediated by IRE1α, PERK, and ATF6 sensors. While UPR's role in viral infections has been well documented, recent studies indicate that intracellular bacterial pathogens have evolved specific mechanisms to hijack UPR signaling for survival and replication. This review examines UPR manipulation strategies employed by major bacterial pathogens, including Brucella, Mycobacterium tuberculosis, Legionella, and Salmonella. These pathogens u

The O1 serogroup of Vibrio cholerae has caused all cholera pandemics and for over a century V. cholerae O1 outbreak strains have been characterized by their serotype. The two V. cholerae serotypes differ by the presence (Ogawa) or absence (Inaba) of methylation of the terminal sugar on the lipopolysaccharide O1-antigen. Serotype switching often occurs during epidemics and has historically been attributed to the pathogen adapting to immune pressures. Here we address the impact of serotype on V. cholerae pathogenicity using otherwise isogenic Ogawa and Inaba versions of several clinical V. chole

Autoimmunity is one of the potential mechanisms that has been proposed in the etiopathogenesis of chronic spontaneous urticaria (CSU). High serum zonulin levels are considered a marker for increased intestinal permeability, which has recently become an important research topic in autoimmune diseases. We designed this study to investigate the relationship between CSU and intestinal permeability based on serum zonulin levels. 65 patients with CSU and 65 age- and sex-matched healthy volunteers were included in the study. Serum zonulin levels were measured simultaneously with enzyme-linked immunos

INTRODUCTION: The Great Lakes region in eastern Democratic Republic of Congo (DRC), currently affected by armed conflict, remains the ecological niche of Vibrio cholerae. Massive population displacement has increased population density in areas surrounding Lake Tanganyika, particularly in South Kivu and Tanganyika Provinces.
OBJECTIVES: To describe the factors driving the persistence of cholera in this conflict-affected region and to underline the urgent need for integrated public health and humanitarian action.
KEY FINDINGS: Displaced populations, often lacking shelter, sanitation, and acce

BACKGROUND: The 7-1-7 targets are gaining traction as measurable targets for assessing a country's readiness to detect and respond to outbreaks of infectious diseases. The targets are outbreak detection within 7 days of emergence, notification to health authorities within 1 day, and key early response actions commenced within another 7 days. For outbreaks of measles, cholera, yellow fever, and meningococcal meningitis, we estimated the impact of initiating outbreak response immunisation (ORI) within 15 days of outbreak emergence, relative to the mean ORI response time for each disease in low-

BACKGROUND: Classical Swine Fever (CSF), or swine cholera, is a highly acute, febrile, and contagious disease caused by the Classical Swine Fever virus (CSFV) in pigs. RESULTS: A total of 205,622 samples were collected from 689 pig farms defined as individual production units nationwide across 20 Chinese provinces/cities from January to December 2022. The results showed that at the sample level, the prevalence of CSFV was 0.25% (95% confidence interval (CI) 0.22% − 0.27%). At the pig farm level, the prevalence of CSFV was 5.08% (95% CI: 3.44% − 6.72%). The risk factors for CSFV infection in pi
