Empowering communities in cholera outbreak response: Local perspectives from Sidama, Ethiopia.

BACKGROUND: Cholera remains a persistent public health challenge in low-resource settings, where effective multi-sectoral coordination and meaningful community engagement are often limited. This cross-sectional study examined the socioeconomic and contextual factors influencing community participation during a recent cholera outbreak in Ethiopia's Sidama Region. The study assessed levels of involvement across preparedness, response, and recovery phases to inform context-specific strategies. METHODS: A community-based cross-sectional survey was conducted between June and August 2023 among 613

Amplifying community perspectives in Medicaid policymaking: findings from North Carolina.

INTRODUCTION: In the midst of sweeping federal and state policy changes that will significantly impact Medicaid, effectively integrating community feedback into Medicaid policymaking will require identifying what Medicaid beneficiaries want and need to be healthy. We aimed to identify North Carolina community leaders' and community members' wants, needs, and experiences with Medicaid. METHODS: This qualitative study collected data using semi-structured interviews with 4 leaders of community-based organizations (CBOs), and 6 focus groups with 37 community members held across North Carolina bet

Conformations and sequence determinants in the lipid binding of an adhesive peptide derived from Vibrio cholerae biofilms.

Surface adhesion is critical to the survival of pathogenic bacteria both in natural niches and during infections, often via forming matrix-embedded communities called biofilms. Vibrio cholerae, the causal agent of pandemic cholera, is capable of forming biofilms adhering to both biotic and abiotic surfaces and the biofilm lifestyle has been implicated in promoting the survival of V. cholerae both in the natural reservoir and during host colonization. Previously, a 57-amino acid loop in the biofilm-specific adhesin Bap1 (Bap1-57aa) has been identified as a key contributor to the adhesion of V.

A Specific Signature of Circulating Free Fatty Acid Discriminates Bullous Pemphigoid From Pemphigus Vulgaris and Healthy Controls.

Bullous pemphigoid (BP) is an autoimmune skin disorder marked by antibodies targeting basement membrane proteins BP180 and BP230. Recent evidence suggests a role for the gut-skin axis and microbial metabolites, especially short-chain fatty acids (SCFAs), in modulating skin homeostasis and immune responses. In this study, we investigated the gut permeability and evaluated the circulating free fatty acids (FFAs) in BP patients, along with the assessment of the ability of each FFA to discriminate BP patients from both pemphigus vulgaris (PV) patients and healthy controls (HC). Thirty-six BP patie

ARF GTPases Function at the Golgi.

ADP-ribosylation factors (ARFs) are small (~21 kDa) GTPases that play a central role in intracellular transport across eukaryotic cells. Structurally related to Ras GTPases, ARFs were initially discovered as allosteric activators of cholera toxin but later emerged as key regulators of membrane trafficking. ARFs function through a GDP-GTP cycle, where guanine nucleotide exchange factors (GEFs) promote the exchange of GDP for GTP, activating ARFs and facilitating their membrane association. Conversely, GTPase-activating proteins (GAPs) stimulate GTP hydrolysis, leading to ARF inactivation and me

The Structure of the Full Catalytic Cycle of Vibrio cholerae NFeoB.

The acquisition of iron is critical for the survival and the virulence of numerous infectious pathogens, and most bacteria acquire ferrous iron (Fe2+) by utilizing the ferrous iron transport (Feo) system. FeoB is the main component of this system and its function is regulated by its soluble cytosolic domain, termed NFeoB. We have recently begun to define the structure and the mechanism of the Feo system from the bacterium Vibrio cholerae, the causative agent of the disease cholera. However, major structural gaps in our understanding of the nucleotide-promiscuous V. cholerae NFeoB still exist.

Development and optimization of in-house made indirect ELISA kit for the detection of antibodies against Pasteurella multocida in chicken.

Fowl cholera is caused by P. multocida. The currently available commercial kit is relatively expensive for Ethiopian laboratories. To address this issue indirect ELISA kit to detect antibodies against P. multocida in chicken was developed. In-house made indirect ELISA kit was developed and optimized. Re-constituted local avian P. multocida biotype A was used for the preparation of coating antigen. The purity of the P. multocida biotype A inoculum was checked by gram staining and centrifugation at 4500 rpm for 30 min. The assay was developed and evaluated using 128 samples of chicken serum. Opt

In silico design of a multiepitope subunit vaccine targeting Salmonella enterica serovar Infantis: an immunoinformatics and reverse vaccinology approach.

Salmonella enterica serovar Infantis is an emerging zoonotic pathogen increasingly linked to poultry and multidrug-resistant human infections. Existing vaccines lack serovar-specific efficacy, underscoring the need for targeted immunization strategies. In this study, we employed an immunoinformatics and reverse vaccinology pipeline to design a multiepitope subunit vaccine (MEV) against S. Infantis. A subtractive proteomics analysis of 692 poultry-derived genomes identified CsgA as a highly conserved, immunogenic, and non-host homologous antigen. Selected cytotoxic T lymphocyte (CTL), helper T

Epidemiological analysis of a cholera outbreak in pastoral communities in the Somali region of Ethiopia.

BACKGROUND: Cholera is an acute diarrhea disease caused by infection of the intestine with the toxigenic bacterium Vibrio cholerae serogroup O1 or O139. It is a waterborne disease of global health importance with an estimated 3 to 5 million cases and over 100,000 deaths annually. METHODS: Our key objective was to analyze the epidemiology of a cholera outbreak in the Somali region of Ethiopia, which took place in 2017, using an electronic surveillance cholera database. We investigated 7 cholera-affected-districts. In total, 4115 cases of cholera were reported, of which 3846 (93.5%) records were

Gut microbiota dysbiosis in Indian women with PCOS may be linked to metabolic and hormonal dysregulation.

AIM: Polycystic ovary syndrome (PCOS) is a common gynecological and cardiometabolic disorder in reproductive-aged women. Recently gut microbiota alterations have been identified as key contributors to PCOS pathophysiology, but it remains understudied in Indian population. METHODS: 16S rRNA gene amplicon sequencing using Illumina-MiSeq platform was conducted in 57 PCOS and 30 control women. Diversity indices were assessed and significantly altered taxa were identified by differential abundance tests (Wilcoxon-rank-sum test, ANCOMBC2 and LEfSe) in total and hyperandrogenism-based PCOS subgroups

Targeting the MR1-MAIT Cell Axis for Vaccination Against Infectious Disease.

Mucosal-associated invariant T (MAIT) cells exist in high numbers in the body and have a unique and highly conserved T cell receptor (TCR). They can be activated in a TCR-dependent manner by ligands presented on the monomorphic protein MHC class I-related protein 1 (MR1) which is found on many cell types, including professional antigen presenting cells (APCs) and epithelial cells. This has sparked interest in the potential to exploit the MR1-MAIT cell axis for the development of vaccines against infectious disease. Within this context an MR1 ligand, typically 5-(2-oxopropylideneamino)-d-ribity

Statistical Modelling of Waning Immunity After Shanchol™ Vaccination: A Prospective Cohort Study.

Introduction: Cholera remains a major public health threat in endemic settings, and oral cholera vaccine (Shanchol™) campaigns are increasingly used amid constrained global supply. However, practical decisions on revaccination require clearer, setting-specific estimates of how rapidly vaccine-induced vibriocidal antibodies peak and wane. Methods: We conducted a prospective cohort kinetics analysis in Lukanga Swamps (Central Province, Zambia), enrolling adults (18-65 years) stratified by prior Shanchol™ exposure (0, 1, or 2 previous doses). All participants received two Shanchol™ doses 14 days

Strategies for achieving a healthy oral cholera vaccine market: Model-enabled scenario exploration of supply and demand dynamics.

Following decades of progress, recent years have seen a resurgence of cholera. This has led to unprecedented demand for vaccines from the global emergency stockpile of oral cholera vaccines (OCVs), for outbreak and humanitarian use. As a consequence of chronic supply shortages, preventive vaccination has been suspended since 2022. Although strategic demand scenarios have been published for OCV, models that integrate OCV supply and demand across long time horizons are lacking. Therefore, a quantitative system dynamics model is presented to simulate OCV market dynamics between 2013-2035. The mod

Attenuation mechanisms and vaccine potential of the serial passage-derived Pasteurella multocida strain PMZ8 in ducks.

Fowl cholera, caused by Pasteurella multocida, causes major losses in poultry worldwide, but its molecular pathogenesis remains poorly understood, limiting rational control. This study characterized an attenuated strain, PMZ8, obtained after 50 rounds of serial passaging from a highly virulent duck-derived isolate. Long-read whole-genome sequencing identified three fixed nonsynonymous mutations, in which Fis (R86S) and WaaF (G7R) contribute to virulence attenuation. PMZ8 exhibited an acapsular phenotype, a truncated LPS core, enhanced biofilm formation, defective envelope integrity, and increa

The Interplay Between Non-Instantaneous Dynamics of mRNA and Bounded Extrinsic Stochastic Perturbations for a Self-Enhancing Transcription Factor.

In this work, we consider a simple bistable motif constituted by a self-enhancing Transcription Factor (TF) and its mRNA with non-instantaneous dynamics. In particular, we mainly numerically investigated the impact of bounded stochastic perturbations of Sine-Wiener type affecting the degradation rate/binding rate constant of the TF on the phase-like transitions of the system. We show that the intrinsic exponential delay in the TF positive feedback, due to the presence of a mRNA with slow dynamics, deeply affects the above-mentioned transitions for long but finite times. We also show that, in t

Walking for Health: Franz Tappeiner (1816-1902), Meran, and the Origins of Public Health-Oriented Physical Activity.

BACKGROUND/OBJECTIVES: Franz Tappeiner (1816-1902) is often celebrated as a pioneer of alpine medicine and the founder of Tappeiner Promenade in Meran (South Tyrol, Italy). However, his legacy extends far beyond the scenic infrastructure, encompassing a comprehensive vision of physical activity as a public health intervention. His multidisciplinary practice anticipated the principles of contemporary rehabilitation, preventive medicine, and climate-sensitive public health. METHODS: This historical public health analysis, combining biographical, contextual, and material-spatial approaches, rein
