Saliva and Anterior Nasal Samples for Detecting Respiratory Viruses in Children.

For detecting respiratory viruses, a nasopharyngeal swab sample for polymerase chain reaction (PCR) assays has been the benchmark. Less discomfort related to sampling is requested especially for children, and thus other sample types have been suggested. We examined anterior nasal samples and saliva samples of 59 pediatric patients with respiratory symptoms and compared the results to nasopharyngeal swab samples. The collection of saliva and anterior nasal samples was tolerated better than nasopharyngeal samples in children. Anterior nasal samples were more accurate than saliva samples in detec

Molecular epidemiology and genetic diversity of human metapneumovirus among hospitalized children in Saudi Arabia during the COVID-19 era.

INTRODUCTION: Human metapneumovirus (HMPV) is a major viral cause of acute respiratory tract infections, especially in young children and older adults. However, data on its molecular epidemiology in the Middle East remain scarce. As the COVID-19 pandemic may have altered the circulation and molecular epidemiology of respiratory viruses, this study represents one of the first efforts to assess the genetic diversity and circulation patterns of HMPV in Saudi Arabia during the COVID-19 era. METHODS: A total of 216 nasopharyngeal aspirates were collected from hospitalized children with suspected r

Expression Patterns of Interferons and Proinflammatory Cytokines in the Upper Respiratory Tract of Patients Infected by Different Viral Pathogens: Correlation with Age and Viral Load.

Respiratory tract infections are a major cause of morbidity and mortality. After the SARS-CoV-2 pandemic, pathogenetic mechanisms leading to more severe outcomes were investigated, including uncontrolled viral replication in the upper airways. This was only partially investigated for other respiratory viruses. We measured mucosal expression of IFN-β1, IFN-λ1, IFN-λ2/3, IL-1β, and IL-6 in patients infected by human metapneumovirus, human rhinovirus, human respiratory syncytial virus or type A influenza virus. A total of 806 nasopharyngeal swabs were collected from patients presenting at emergen

Pathogenesis and Transmissibility of Middle East Respiratory Syndrome Coronaviruses of African Origin in Alpacas.

The Middle East Respiratory Syndrome coronavirus (MERS-CoV) remains a highly significant threat to global public health. Dromedary camels are the zoonotic source of human infection. All cases of zoonotic Middle East Respiratory Syndrome (MERS) have occurred in Middle Eastern countries despite MERS-CoV infection of camels being widespread in Africa. This disparity in the geographic burden of the disease may be due to genomic differences between MERS-CoV circulating in Middle Eastern countries (clades A and B) versus those infecting camels in Africa (clade C), although the precise genetic determ

Genomic and pathogenic insights into a new GVI-1 infectious bronchitis virus strain isolated in Northeast China.

Infectious bronchitis is a severe disease caused by the infectious bronchitis virus (IBV), resulting in significant economic losses to the poultry industry. We isolated an IBV strain from H120-vaccinated layers in Northeast China called CK/CH/LN/2301. Phylogenetic analysis of the S1 gene classified the isolate within the GVI-1 genotype. Further analysis revealed that CK/CH/LN/2301 is a recombinant strain derived from the GVI-1, GI-19, and 4/91 genotypes, with recombination events primarily located in the open reading frame (ORF) 1a region. Experimental infection of 1-day-old specific pathogen-

Pre-existing and early cellular immune factors correlate with functionally complete protection against primary controlled human SARS-CoV-2 infection.

Identifying host factors that mediate protection against newly-emergent viruses is needed for improved pandemic preparedness. Here, we analysed pre- and early post-exposure immune factors associated with resisting SARS-CoV-2 infection after human challenge in seronegative individuals, using multiplex protein, cytometric and RNA sequencing approaches in the nasopharynx and circulation. Pre-existing cross-reactive antibodies correlate poorly with clinical outcome. Instead, protection is associated with heightened nasopharyngeal CCL13 levels locally produced by conventional dendritic cells and mo

Clinical Usefulness of the Loop-Mediated Isothermal Amplification Assay for SARS-CoV-2 Detection.

BACKGROUND: Loop-mediated isothermal amplification (LAMP) is a molecular diagnostic method known for its rapid processing and operational simplicity due to its isothermal amplification process. While LAMP has demonstrated comparable diagnostic accuracy to PCR in certain applications, its performance may vary depending on assay design and implementation. This study aimed to evaluate the diagnostic performance of the MmaxSure™ assay (MmaxSure™; Mmonitor, Daegu, South Korea) in detecting SARS-CoV-2, comparing it with the STANDARD™ M nCoV Real-Time Detection Kit (STANDARD; SD BioSensor, Suwon, Sou

Influenza C Virus in Children With Acute Bronchiolitis and Febrile Seizures.

BACKGROUND: Influenza C virus (ICV), a lesser-known member of the Orthomyxoviridae family, usually causes mild respiratory illness in children. Due to its low prevalence and clinical similarity to other respiratory infections, it is not routinely screened. This study investigates the detection of ICV in children with acute bronchiolitis (AB) and febrile seizures (FS), comparing the findings with a healthy control group. METHODS: Between October 2009 and September 2011, 499 nasopharyngeal swabs were collected from children up to 6 years old at the University Medical Center Ljubljana. The study

Viral lineage and mode of exposure modulate within host spatial dynamics of influenza A viruses.

The upper and lower respiratory tracts (URT and LRT) present distinct environments for influenza A virus (IAV) replication. Their differential features have major implications for viral evolutionary dynamics, transmission potential, and pathogenesis. To investigate the implications of differential viral replication in the URT and LRT, we assessed dispersal of IAVs throughout the guinea pig respiratory system. Guinea pigs were inoculated intranasally with a 300 μL volume to deliver inoculum to both the URT and LRT. Two strains were used to represent the circulating seasonal IAV lineages: influe

Human Bocavirus Infection in Turkish Children.

The human bocavirus (HBoV) in children has been identified with an acute lower respiratory tract disease (LRTD). Its prevalence, clinical characteristics, and function as an RTD causal agent are unclear. This study sought to evaluate the laboratory and clinical data of HBoV patients, pinpoint the clinical characteristics linked to HBoV, and examine the distinctions between detecting HBoV alone and in combination with other agents. We used molecular testing to confirm the presence of HBoV DNA and 20 other respiratory pathogens in 1,739 nasopharyngeal samples from hospitalized children at the Pe

The impact of heat inactivation on RT-qPCR detection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2): an experience from the University Clinical Centre of Vojvodina, Serbia.

Handling clinical samples from patients suspected of SARS-CoV-2 infection puts healthcare workers at risk of exposure to infectious particles. To reduce this risk, samples are often heat-inactivated before nucleic acid isolation, but this procedure may affect the analytical sensitivity of the test. The aim of this study was therefore to evaluate the effects of heat inactivation (56 °C for 30 min) on RT-qPCR results of samples taken from nasopharyngeal and oropharyngeal (NP/OP) swabs collected from 200 symptomatic patients. Each sample was split into two aliquots - one subjected to heat inactiv

Host gene expression in the Nasopharynx can discriminate microbiologically confirmed viral and bacterial lower respiratory tract infection.

INTRODUCTION: Distinguishing viral versus bacterial lower respiratory tract infection (LRTI) is challenging. We previously developed a rapid, host response-based test (Biomeme HR-B/V assay) using peripheral blood samples to identify viral versus bacterial infection. We assessed the performance of this assay when using nasopharyngeal (NP) samples. METHODS: Patients with LRTI were enrolled, and a NP swab sample was run using the HR-B/V assay (assessing 24 gene targets) on the FranklinTM platform. The performance of the prior classifier at identifying viral versus bacterial infection was assesse

An integrative viro-bacterial signature based on viral load and high-resolution microbiome profiling predicts COVID-19 mortality.

BACKGROUND: The nasopharyngeal (NP) microbiota may play a critical role in modulating host immune responses during SARS-CoV-2 infection, yet its utility for predicting clinical outcomes is not fully defined. We aimed to determine if an integrative approach, combining NP microbial profiles with virological and clinical data, could improve mortality prediction in COVID-19 patients. METHODS: We analyzed nasopharyngeal swabs from 81 COVID-19 patients and 70 non-infected controls. Full-length 16S rRNA sequencing was used for microbiome profiling. Predictive models were developed using machine lear

Cats are more susceptible to the prevalent H3 subtype influenza viruses than dogs.

Recent reports have highlighted the increasing frequency of influenza A virus (IAV) spillover events from other species to dogs and cats. IAV, particularly the H3 subtype, exhibits a broad host range and a propensity for interspecies transmission, as exemplified by the sustained circulation of H3N2 and H3N8 canine influenza viruses in dog populations. This raises concerns about the potential role of companion animals as intermediate hosts in influenza virus transmission. To evaluate the susceptibility of dogs and cats to the prevalent H3 subtype influenza viruses, we experimentally inoculated

Sustained inflammation during human T-lymphotropic virus type 1 infection: a wildfire contributing to disease progression.

Human T-lymphotropic virus type 1 (HTLV-1) is a retrovirus affecting 10-20 million people worldwide. While many carriers remain asymptomatic, HTLV-1 infection can trigger intense inflammatory responses which are defined by the sustained release of pro-inflammatory cytokines and chemokines. Central to this process is the HTLV-1 encoded Tax oncoprotein, a viral regulator that drives uncontrolled inflammation by hijacking multiple cellular signaling pathways, such as the RelA/NF-κB signal transduction pathway. CD4 T-cells are the primary targets of Tax-mediated transformation, undergoing uncontro

Primary Aggressive Oral Lymphomas (PAOL): A Narrative Review of Diagnosis, Molecular Features, Therapeutic Approaches, and the Integrated Role of Dentists and Hematologists.

Primary aggressive oral lymphomas (PAOL) are a rare subset of extranodal non-Hodgkin lymphomas arising in the oral cavity without evidence of other systemic involvement at diagnosis. PAOL accounts for only about 2-3% of all lymphomas. They most commonly belong to aggressive B-cell subtypes such as Diffuse large B-cell lymphoma (DLBCL) and plasmablastic lymphoma (PBL), with occasional cases of Burkitt lymphoma and T-cell/NK-cell lymphomas. Clinically, these malignancies often present with non-specific symptoms (e.g., swelling, pain, ulceration, tooth mobility) that mimic benign dental condition

Disruption of influenza A virus transmission in gene-edited pigs.

Influenza A virus (IAV) infection in pigs causes significant economic losses in the swine industry. More importantly, pigs serve as an intermediate host for generating reassortant viruses with pandemic potential. Previously, gene-edited pigs lacking the TMPRSS2 gene were generated and tested for susceptibility to IAV infection. Experimental infection of TMPRSS2 knockout pigs (TMPRSS2 KO pigs) resulted in limited nasal shedding, posing the question about their ability to transmit IAV. Therefore, this study evaluated IAV transmission from principal-infected (donor) TMPRSS2 KO pigs to sentinel (r

The biology of congenital urinary bladder outflow obstruction.

Congenital urinary bladder outflow obstruction (BOO) is a major cause of kidney failure in children. Sometimes, it is caused by anatomical obstruction of the urethra. In other cases, functional failure of voiding occurs without anatomical obstruction. Bladder development is driven by genes expressed in smooth muscle, neural and urothelial cell lineages. Variants in the coding regions of such genes cause a subset of BOO. In other cases, such as posterior urethral valves (PUV), noncoding variants may influence susceptibility. By modelling BOO in mutant mice, combined with viral vector mediated g

Infectious complications in pediatric acute lymphoblastic leukemia treatment: A comparison of ALL-IC BFM 2009 vs. modified St. Jude total XV in a single-center retrospective cohort study.

Infectious complications remain the major cause of treatment-related morbidity and mortality in pediatric patients with acute lymphoblastic leukemia (ALL). This study retrospectively compared the patterns of infection in children treated with either the modified St. Jude Total XV protocol (n = 181) or the ALL-IC BFM 2009 protocol (n = 61) at a single center. Although the overall number of infection episodes was similar between the two groups, their distributions across treatment phases differed markedly. The ALL-IC BFM 2009 protocol was associated with a significantly greater incidence of infe
