Timing of Earliest Documented Positive Hepatitis C Laboratory Result Relative to Delivery Among Pregnant Persons With Active Infection in West Virginia, 2022-2023.

Direct-acting antiviral therapy can cure hepatitis C virus (HCV) infection; however, because treatment is not recommended during pregnancy, preconception treatment remains the primary strategy to prevent perinatal HCV exposure. West Virginia birth records from 2022 to 2023 were linked with statewide HCV laboratory surveillance data. This linked dataset was used to identify births to persons with active HCV infection and characterize the timing of the earliest documented positive HCV laboratory result relative to delivery. A total of 426 deliveries resulting in 433 HCV-exposed live-born infants

Clinical and metagenomic characterization of HHV-6 versus HHV-7 encephalitis in immunocompetent children.

BACKGROUND: Human herpesvirus 6 (HHV-6) and human herpesvirus 7 (HHV-7) are betaherpesviruses that can cause encephalitis/encephalopathy in immunocompetent children. However, early clinical manifestations, routine laboratory tests, and neuroimaging findings often lack specificity, making etiological characterization challenging. This study aimed to characterize the clinical features and metagenomic findings of HHV-6 and HHV-7 encephalitis in immunocompetent children. METHODS: This single-center retrospective cohort study included children (<18 years) with suspected central nervous system infe

Prolonged Primary Cytomegalovirus Infection in an Immunocompetent Adult Successfully Treated With Valganciclovir.

Cytomegalovirus (CMV) infection in immunocompetent adults is generally asymptomatic or causes a mild, self-limited illness, but it may occasionally present as CMV mononucleosis with prolonged fever. We report a case of primary CMV infection in an immunocompetent adult presenting with prolonged fever and subsequent clinical improvement following antiviral therapy. A 26-year-old woman presented with a three-week history of intermittent fever reaching 39°C. Laboratory findings revealed atypical lymphocytes and elevated liver enzymes. Serological findings were consistent with primary CMV infection

Engineered extracellular vesicles: pharmacological barriers, engineering strategies, and translational opportunities.

Extracellular vesicle (EV) therapeutics are progressing from broad proof-of-concept studies toward product-specific engineering and clinically defined applications. Native EVs offer biological membrane functions and the capacity to transport diverse molecular cargoes, but their therapeutic performance is limited by low cargo stoichiometry, rapid mononuclear phagocyte system clearance, clearance-dominated biodistribution, and inefficient functional cargo release. This Review examines how EV engineering strategies are used to address these barriers. We compare approaches for cargo loading, circu

Persistent HyperCKemia Beyond Post-Viral Myositis: A Case of Anti-HMGCR Immune-Mediated Necrotizing Myopathy.

Anti-3-hydroxy-3-methylglutaryl-coenzyme A reductase (anti-HMGCR) immune-mediated necrotizing myopathy (IMNM) is a rare autoimmune myopathy that should be suspected in patients with progressive proximal muscle weakness and marked creatine kinase (CK) elevation persisting despite statin discontinuation, although distinguishing it from more common conditions such as post-viral myositis and toxic statin-associated myopathy remains a major diagnostic challenge. We report the case of a 64-year-old man receiving long-term atorvastatin therapy who presented with progressive proximal muscle weakness,

Ocular Immune Toxicity in Cancer Immunotherapy, From Immune Privilege Breakdown to Treatment Rechallenge: A Review.

Immune checkpoint inhibitors (ICIs) have transformed cancer therapy but can disrupt ocular immune privilege and produce heterogeneous immune-related adverse events (Eye-irAEs). This structured narrative review with evidence mapping integrates clinical, epidemiologic, mechanistic, pharmacovigilance, and management data to define translational pathways from checkpoint blockade to ocular toxicity and treatment decisions. In a real-world cohort of 41,020 ICI-treated patients, ophthalmic irAEs occurred in 4.18% within 1 year, including dry eye disease in 2.00%, conjunctivitis in 0.87%, blepharitis

Retrospective Analysis of the Clinical Findings of 114 Horses with Eosinophilic Keratoconjunctivitis.

Eosinophilic keratoconjunctivitis (EKC) is an inflammatory eye disease of horses with an unknown cause and suspected immune-mediated pathogenesis. This single-center retrospective case series included 114 horses (163 affected eyes) examined at a German equine referral hospital from January 2012 to December 2020. Of these horses, 71 (62.3%) were Warmbloods, 77 (67.5%) were presented from July to September, and 49 (43.0%) had bilateral disease. The cornea was involved in 139 of 163 eyes (85.3%). During the study period, 114 EKC cases occurred among 5742 ophthalmology cases (2.0%), with an increa

Flipons, Noncoding Transcripts and Retroelements: A Universal Host Defense.

Encoding genetic information involves both structure and sequence. Non-protein-coding (NCR) RNAs modulate host immune responses by guiding and assembling stress-specific protein effectors, with flipons enabling dynamic transitions between states. Small NCRs transcribed by both RNA Polymerases 2 and 3 normally modulate these pathways. The miRNAs, tRNA fragments, mitrons, piRNAs, and endo-small-interfering RNA products target the guide effector complexes to their targets. These NCRs filter the subset of RNAs that are translated, a process modulated by other NCRs that fine-tune RNA stability, loc

Oncolytic virotherapy for cancer: biological mechanisms, clinical advances, and future perspectives.

Oncolytic virotherapy has emerged as a promising cancer treatment that combines selective tumor destruction with potent stimulation of antitumor immunity. Unlike conventional therapies, oncolytic viruses (OVs) preferentially infect and replicate within malignant cells while sparing normal tissues, leading to direct tumor lysis and the release of tumor-associated antigens, pathogen-associated molecular patterns, and damage-associated molecular patterns that activate both innate and adaptive immune responses. Advances in molecular virology and genetic engineering have transformed naturally occur

Possible Epstein-Barr virus-associated central nervous system involvement with reversible electroencephalographic abnormalities in a child: a case report.

BACKGROUND: Epstein-Barr virus (EBV)-associated central nervous system (CNS) involvement in children is uncommon and may present with nonspecific neurological symptoms. Brain magnetic resonance imaging (MRI) may remain normal, making diagnosis challenging. Cerebrospinal fluid (CSF) targeted next-generation sequencing (tNGS) has emerged as a useful diagnostic tool, although positive EBV results require careful clinical interpretation. CASE DESCRIPTION: A previously healthy 5-year-old boy presented with fever (38.5 ℃), headache, dizziness, and nausea for 3 days without seizures, altered conscio

Epigenetic Priming with Oral Decitabine/Cedazuridine Combined with Nivolumab in Unresectable or Metastatic Mucosal Melanoma: Clinical and Correlative Findings from a Phase Ib/II Trial.

Background/Objectives: Mucosal melanoma is a rare, aggressive melanoma subtype with lower response rates to immune checkpoint inhibitors than cutaneous melanoma. We previously identified reduced expression of innate immune pathogen-sensing pathway genes in mucosal melanoma, including components of the retinoic acid-inducible gene-I (RIG-I) pathway, and found that decitabine can induce re-expression of these immune genes and tumor-associated antigens. This study evaluated epigenetic immune priming with oral decitabine/cedazuridine (DEC-C) combined with nivolumab in patients with unresectable or

Chronic infectious keratitis: A review of etiopathogenesis, clinical presentation and management challenges.

Chronic infectious keratitis represents a distinct clinical entity characterized by indolent progression, diagnostic difficulty, and prolonged therapeutic courses. Unlike acute bacterial keratitis, chronic infections often present with subtle inflammatory signs, recurrent episodes, or poor response to empirical antimicrobial therapy. Among the most important causes are herpetic keratitis, acanthamoeba keratitis, Microsporidial stromal keratitis, and atypical (non-tuberculous) mycobacterial keratitis. Herpes simplex virus keratitis may manifest as recurrent epithelial disease or immune-mediated

Histopathologic spectrum of focal liver lesions diagnosed by ultrasound-guided percutaneous liver biopsy and predictors of hepatocellular carcinoma: A single-center experience from Sub-Saharan Africa.

BACKGROUND: Focal liver lesions (FLLs) encompass a wide spectrum of benign and malignant pathologies, and accurate diagnosis is essential for appropriate management. Although advances in imaging have improved lesion characterization, histopathologic assessment remains the diagnostic gold standard for indeterminate lesions. Data on the histopathologic spectrum and diagnostic utility of ultrasound-guided percutaneous liver biopsy (US-PLB) in sub-Saharan Africa (SSA) are limited. This study aimed to characterize the histopathologic findings of US-PLB performed for FLLs at a tertiary referral cent

Beyond HPV16/18: genotype distribution of high-risk HPV and cycle threshold values in cervical carcinogenesis spectrum.

BACKGROUND: Cervical cancer is a virus-dependent gynaecologic malignancy and according to GLOBOCAN 2022 cervical cancer is the fourth frequent women cancer in all over the world. Despite this frequency it is rarely seen in developed countries because of the enforcement of cervical screening programmes with human papillomavirus (HPV) test and Pap test called cotest. The burden of this women cancer is high despite the implementation of HPV-based vaccines and screening strategies, could nearly eliminate cervical cancer as a public health problem. Dengue virus infection.

Dengue is an acute febrile disease caused by the Aedes mosquito-borne dengue viruses (DENV-1 to DENV-4). Annually, approximately 100 million people fall ill with dengue, which can cause debilitating pain and discomfort, reducing both physical functioning and quality of life. Some patients progress to life-threatening inflammation-driven circulatory failure. Disease burden remains primarily in urban centres; however, factors including increased travel and climate change are rapidly expanding the geographical distribution of dengue to rural and peri-urban environments. Dengue is diagnosed based

RSV Molecular Epidemiology Pre- and Post-COVID-19 Pandemic Periods in Seattle, Washington, USA.

BACKGROUND: Globally, RSV declined to historically low levels following stay-at-home orders in 2020 and re-emerged as restrictions lifted, with large inter-seasonal outbreaks and other shifts in epidemiology. The effect of the pandemic restrictions on genomic diversity has not been well studied. METHODS: Individuals with respiratory symptoms were enrolled into community-based surveillance studies and provided nasal swabs over three respiratory virus seasons from 2019 to 2022 in the Seattle metropolitan area in Washington State, USA. Samples were tested for RSV by RT-qPCR, with whole genome se

Characterization of the Peromyscus leucopus immunoglobulin germline repertoire and anti-OspA antibody response reveals conserved germline IGHV features among cricetid rodents.

Peromyscus leucopus (P. leucopus) is an important reservoir host for several zoonoses, including Borrelia burgdorferi (B. burgdorferi), the causative agent of Lyme disease. However, the immunoglobulin germline repertoire underlying antibody responses in this reservoir host remains poorly defined. In this study, we characterized the immunoglobulin variable genes of P. leucopus and identified 12 IGHV, 12 IGKV, and 3 IGLV gene families comprising 129, 87, and 12 genes, respectively. These genes exhibited similar usage patterns to those of Mus musculus (M. musculus). We also identified a high-dive

First Phylodynamic Analysis of HRSV-A and HRSV-B in Rio de Janeiro, Brazil, Reveals Recurrent Dispersals From the Megalopolis Center to Suburban Cities.

Human respiratory syncytial virus (HRSV) is the leading cause of acute lower respiratory tract infections in infants and represents a major burden to public health systems worldwide. Despite its epidemiological relevance, genomic surveillance of HRSV in Brazil remains limited. Here, we investigated the genetic diversity and dispersal dynamics of HRSV-A and HRSV-B in the state of Rio de Janeiro, Brazil, during the period immediately preceding the implementation of maternal vaccination and prophylactic monoclonal antibodies. A total of 136 near-complete HRSV genomes were recovered from respirato
