Effects of microplastics and nanoplastics on the kidneys.

Plastics are hydrophobic carbon polymers with a half-life of ∼500 years. The widespread production and environmental accumulation of plastics pose significant toxicity concerns. Humans are routinely exposed to micro- and nanoplastics (MNPs), which can enter rate the bloodstream and reach various organs, including the kidneys. Here, we review research on nephrotoxic effects of plastics and the underlying mechanisms. The results of several studies of kidneys in mammals and kidney cells from humans suggest that MNPs induce renal toxicity. Although the underlying mechanisms remain to be characteriz

Insulin resistance and hyperinsulinaemia in kidney disease: mechanisms and metabolic effects.

The impact of insulin resistance and compensatory hyperinsulinaemia on kidney function, and their contribution to the development and progression of chronic kidney disease, have largely been overlooked in people with or without diabetes. Beyond its classical metabolic targets (muscle, liver and adipocytes), insulin has an essential role in kidney metabolism and physiology, regulating podocyte integrity, tubular sodium handling and gluconeogenesis. In states of insulin resistance, these homeostatic functions are disrupted, leading to glomerular hyperfiltration, proteinuria, sodium retention, pr

Cytochrome b5 reductase-3 overexpression reduces renal damage associated to intravascular hemolysis.

Intravascular hemolysis can cause both acute kidney injury (AKI) and chronic kidney disease (CKD). When erythrocytes break down in the bloodstream, they release hemoglobin and heme derivatives, which accumulate in renal cells and lead to increased oxidative stress, inflammation, cell death, and mitochondrial dysfunction. The enzyme NADH-cytochrome b5 reductase 3 (CYB5R3) plays a role in NAD+ metabolism and helps reduce oxidative stress and inflammation while enhancing mitochondrial function. In this study, we examined whether overexpressing CYB5R3 offers kidney protection against AKI caused by

A heterobifunctional IRAK4-targeting degrader impairs myddosome signaling in acute kidney injury and ameliorates kidney fibrosis.

Heterobifunctional degraders emerge as a novel pharmacologic strategy to treat inflammatory conditions. Previous data have indicated that inflammatory signaling via the myddosome complex is critical for kidney injury and fibrosis development. Within the myddosome, modulation of interleukin-1 receptor associated kinase 4 (IRAK4) by small molecule kinase inhibitors has been shown to reduce fibrosis, but has limited anti-inflammatory effect. Here we used a novel strategy to abrogate IRAK4 by means of targeted ubiquitination triggering proteasomal degradation. We tested the antifibrotic and anti-i

Unexpected aggressiveness in VHL-associated clear cell renal cell carcinoma: insights from two patients highlighting biological heterogeneity.

Von Hippel-Lindau (VHL) disease is a hereditary cancer syndrome predisposing to renal cell carcinoma (RCC), typically considered more indolent than sporadic RCC. The '3 cm rule' has guided nephron-sparing interventions, as tumours smaller than 3 cm rarely metastasise. However, accumulating evidence suggests this paradigm is not universal, with some VHL-associated RCCs demonstrating aggressive biological behaviour despite their small size. We report two cases of aggressive clear cell RCC (ccRCC) in patients with suspected VHL. The first case involved a middle-aged male presenting with bilateral

Burden and correlates of anemia in IgA nephropathy versus other primary glomerulonephritides.

BACKGROUND: Anemia is a frequent but often underrecognized complication of primary glomerulonephritis (PGN). This study aimed to determine the prevalence and determinants of anemia in a large multicenter PGN cohort, with particular focus on immunoglobulin A nephropathy (IgAN), and to evaluate its clinical, laboratory, and histopathological correlates. METHODS: Data were retrieved from the Turkish Society of Nephrology Glomerulonephritis Database (TSN-GOLD). A total of 5,500 patients with biop-valuesy-proven PGN were included, of whom 1,571 (28.6%) had IgAN. Demographic, clinical, laboratory,

Early-onset kidney failure in a girl with autosomal dominant tubulointerstitial kidney disease due to a de novo UMOD variant.

Autosomal dominant tubulointerstitial kidney disease (ADTKD) is characterized by renal tubular and interstitial abnormalities and slow progressive loss of kidney function. Patients with ADTKD rarely progress to kidney failure in early childhood. A 7-year-old Japanese girl was admitted to the hospital due to afebrile seizures and was later diagnosed with Panayiotopoulos syndrome. Blood examinations showed that she had a serum creatinine level of 1.57 mg/dL (Cr-eGFR 28 mL/min/1.73 m²), consistent with chronic kidney disease stage 4. Ultrasonography showed bilateral small to normal-sized kidneys,

Severe Chronic Kidney Disease Presenting as Asymptomatic Normocytic Anemia in a Child.

Anemia is a common finding in pediatric practice and is most often attributed to nutritional deficiency; however, it may also represent the earliest manifestation of underlying systemic disease. We describe the case of a six-year-old girl who presented for a routine well-child visit and was found to have severe normocytic, normochromic anemia despite appearing clinically asymptomatic. Subsequent laboratory evaluation revealed profound renal dysfunction with metabolic acidosis and electrolyte abnormalities. Renal imaging demonstrated bilaterally small kidneys without hydronephrosis, consistent

Multifaceted mechanisms of unconventional T cells in ischemia-reperfusion injury.

Ischemia-reperfusion injury (IRI) constitutes a common pathological basis for organ dysfunction across various critical clinical conditions, including ischemic stroke, myocardial infarction, and organ transplantation. This review focuses on the functions of unconventional T cells in IRI in the brain, liver, kidneys, heart, and mucosal barrier organs, highlighting their diverse regulatory mechanisms in various tissue injury patterns. It also examines the dual functions of these cells in IRI: instead of solely acting as consistent pro-inflammatory effectors, they demonstrate a context-dependent

Bone turnover markers in the management of CKD-associated osteoporosis-a European consensus.

Bone turnover abnormalities are common in chronic kidney disease (CKD) and contribute to bone fragility. Recently, the Kidney Disease: Improving Global Outcomes (KDIGO) introduced the term CKD-associated osteoporosis to describe bone fragility in CKD and highlighted the role of bone turnover markers (BTM) in clinical evaluation and management of bone fragility. However, current clinical practice guidelines lack specific treatment targets for bone turnover in the setting of CKD. The aim of this consensus was to present recommendations for the use of BTM in the management of CKD-associated osteo

Sulfate handling in proximal renal tubular defects: an exploratory study.

BACKGROUND: Renal tubular disease causes loss of electrolytes and other molecules. One of these electrolytes is inorganic sulfate, which is reabsorbed by dedicated transporters, including NaS1, encoded by the SLC13A1 gene. Still, inorganic sulfate is rarely measured in clinical practice, although it is essential for the development and functioning of several organ systems. The importance of adequate sulfate stores is emphasized by genetic disorders impairing sulfate metabolism in the brain, bone, and the endocrine system, causing severe neurodevelopmental disorders, skeletal dysplasia, and hor

MINOCA (Myocardial Infarction With Non-obstructive Coronary Arteries) Due to Diffuse Coronary Microvascular Dysfunction in a Patient With Systemic Autoimmune Disease and Chronic Kidney Disease.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) represents a heterogeneous entity requiring systematic exclusion of alternative diagnoses and identification of the underlying mechanism. Coronary microvascular dysfunction (CMD) is an important but often underdiagnosed cause. Systemic inflammatory flares may precipitate microvascular ischemia, yet direct multimodality confirmation remains limited. A 51-year-old woman with active perinuclear anti-neutrophil cytoplasmic antibody (P-ANCA) vasculitis, rheumatoid arthritis, and stage 3b chronic kidney disease presented with acut

Cytokine Release Syndrome and Cardiogenic Shock Following Anti-Thymocyte Globulin Induction in a Kidney Transplant Recipient: A Case Report.

Cytokine release syndrome (CRS) is a rare but serious complication of rabbit anti-thymocyte globulin, a common induction medication for solid organ transplant recipients. We present a case of grade 4 cytokine release syndrome in a renal transplant patient in whom tocilizumab successfully salvaged the graft function and stabilized the patient. A 22-year-old woman with end-stage renal disease on hemodialysis and a complex history including systemic lupus erythematosus nephritis underwent a deceased donor kidney transplant with thymoglobulin induction. Hours postoperatively, she developed hemodyn

Role of Lipid Signaling by Platelet-Activating Factor Receptor in Tubular Epithelial Cells in Acute Kidney Injury-to-CKD Transition.

KEY POINTS: Platelet-activating factor receptor contributed to renal tubular epithelial cells G2/M arrest through suppressing mouse double minute 2-mediated p53 ubiquitin degradation in AKI-to-CKD transition. Phosphatidylethanolamine (18:0/18:1) served as a novel endogenous ligand of platelet-activating factor receptor, inducing the downstream signaling. WAY-639497, a small-molecule platelet-activating factor receptor antagonist identified by virtual screening, mitigated renal tubular epithelial cells G2/M arrest and AKI-to-CKD transition. BACKGROUND: AKI represents a critical clinical compli

Fibroblast Growth Factor 23 as a Prognostic Biomarker in Post-Myocardial Infarction Outcomes: Influence of Renal Function and Its Modulation by Klotho.

BACKGROUND: Elevation of FGF23 (fibroblast growth factor 23) and decreased Klotho levels have been associated with various cardiovascular and renal diseases. However, the combined study of the FGF23-Klotho axis in ischemic heart disease remains elusive. METHODS: We analyzed the associations between circulating FGF23 and Klotho levels with cardiac and renal parameters, as well as mortality outcomes following myocardial infarction (MI). Cardiac tissue from patients with ischemic heart disease and a post-MI mouse model were analyzed to assess myocardial FGF23 expression. Proteomic analysis was p

Silent Myocardial Infarction Revisited: Immuno-metabolic Mechanisms, Multimodal Biomarkers, and Translational Diagnostics.

Silent Myocardial Infarction (SMI) is a clinically underrecognized phenotype along the myocardial infarction continuum that progresses without anginal symptoms. Its prevalence in diabetes, chronic kidney disease, and the elderly reflects contributions from neuropathy, autonomic dysfunction, and neurogenic silencing. Emerging evidence indicates that SMI reflects a biologically biased phenotype within the myocardial infarction continuum shaped by immune-metabolic and neurogenic modulation rather than representing a distinct entity. Biomarkers such as sCD36, galectin-3, sST2, and GDF-15 capture f

Practical Interpretation of Bone Scintigraphy: Metastases, Fractures, and Beyond.

Bone scintigraphy is one of the most heavily used imaging studies in nuclear radiology. The ability of technetium 99m-diphosphonate radiotracers to adhere to and form a complex with hydroxyapatite through the process of chemisorption highlights the sites and degree of osteoblastic activity. This mechanism results in a highly sensitive study for blastic processes of bone and serves as a valuable adjunct to cross-sectional imaging. Bone scintigraphy and its ability to capture the entire skeleton are especially useful to determine the extent of disease, most commonly for blastic metastases of bre
