Nephrogenic Diabetes Insipidus Affecting Three Males in Two Generations-Case Report and Review of the Literature.

Background: Nephrogenic diabetes insipidus (NDI) is defined as the inability of the kidney to concentrate urine owing to the insensitivity of the distal nephron to the antidiuretic hormone, arginine vasopressin. NDI is a heterogeneous rare autosomal dominant or X-linked disease. Objective: We present a family with nephrogenic diabetes affecting three males in two generations. Methods: We report two boys with NDI: a 4-month-old infant who was treated for fever, vomiting, and failure to thrive, and his 10-year-old uncle (the mother's brother), who was admitted concurrently for consuming 11 L of

Severe consequences of uncontrolled tertiary hyperparathyroidism in a limited resources setting: a case report.

Tertiary hyperparathyroidism (THPT) is a severe complication of uncontrolled secondary hyperparathyroidism, typically associated with advanced-stage chronic kidney disease (CKD). We present the case of a Honduran patient with a long-standing history of CKD secondary to severe preeclampsia, who developed THPT following the discontinuation of her treatment due to financial constraints and the COVID-19 pandemic. The patient exhibited severe maxillofacial deformities, functional limitations, and widespread skeletal abnormalities. Despite initial management with medications such as paricalcitol and

Bone marrow adipose tissue expansion and bone loss in experimental chronic kidney disease is independent of altered bone marrow stromal cell lineage determination.

INTRODUCTION: Chronic kidney disease-mineral bone disorder is the irreversible loss of kidney function leading to altered mineral homeostasis and bone loss, commonly referred to as renal osteodystrophy. Bone marrow adipose tissue (BMAT) accumulates in clinical CKD and animal models of this disease, but the mechanism(s) responsible are unclear. This study sought to determine the relationship between BMAT distribution and bone structure and to establish whether disease progression directly affected: 1) the commitment of bone marrow mesenchymal stromal cells (BMSCs) to osteoblastic (OPC) and adip

Functional characterization and cAMP-mediated rescue of a novel truncating AVPR2 mutation causing nephrogenic diabetes insipidus.

Vasopressin plays a central endocrine role in water homeostasis by activating the arginine vasopressin receptor 2 (AVPR2) receptor in renal collecting duct cells. Mutations in AVPR2 are a leading cause of X-linked nephrogenic diabetes insipidus (NDI), a disorder marked by renal insensitivity to vasopressin, leading to polyuria, polydipsia, and hypernatremia. We identified a novel truncating AVPR2 mutation (c.570dup; D191*) in a pediatric patient with NDI and investigated its molecular and functional consequences using a renal epithelial cell model. The D191* mutant exhibited marked reduction i

Renal Impact of Prolonged Sevoflurane Sedation in Intensive Care Unit Patients: An Observational Study.

BACKGROUND: Due to their rapid onset and minimal accumulation, volatile agents are used for prolonged sedation in intensive care unit (ICU) patients. However, potential renal effects, particularly polyuria and nephrogenic diabetes insipidus, remain a concern during sevoflurane use. OBJECTIVE: To evaluate the prevalence of and the factors associated with polyuria during prolonged sevoflurane sedation in ICU patients. METHODS: This observational study included consecutive ICU patients (July 2019 to July 2023) receiving sevoflurane, excluding those with prior renal replacement therapy. Sevoflur

Clinical outcomes among children with primary nephrogenic diabetes insipidus.

BACKGROUND: Primary nephrogenic diabetes insipidus (NDI) is a rare inherited disorder with limited data on long-term outcomes. This study assessed longitudinal outcomes with primary NDI. METHODS: This multicenter retrospective study included 63 patients with primary NDI. Growth rates and estimated glomerular filtration (eGFR) were analyzed via a piecewise linear mixed-effects model during the pediatric period. RESULTS: AVPR2 and AQP2 mutations were identified in 74.6% and 9.5% of patients, respectively. The median ages at diagnosis and last follow up were 0.41 and 12.37 years, respectively.

Failure to thrive in children with tubulopathies.

Failure to thrive (FTT) is a common clinical concern in pediatric patients with tubulopathies. Defective tubular handling of solutes, electrolytes, and acid-base balance contributes to impaired growth through both renal and systemic mechanisms. This educational review summarizes current evidence on the pathophysiological mechanisms, clinical presentation and management of FTT in pediatric tubulopathies.

The true cost of phosphate control in chronic kidney disease.

The loss of kidney function entails the development of a positive phosphate balance. The burden of addressing elevated phosphate levels is high. Both parathyroid hormone (PTH) and fibroblast growth factor 23 (FGF23) are increased to promote phosphaturia, thereby preventing the rise in serum phosphate. However, if the phosphate load is excessive, the corresponding phosphaturia is maximal, kidney function deteriorates and hyperphosphataemia becomes clinically evident in advanced stages of chronic kidney disease (CKD). In addition to its role in CKD progression, hyperphosphataemia has been linked

Severe lithium-induced nephrogenic diabetes insipidus: The diuresis paradox.

We report a case of profound nephrogenic diabetes insipidus (NDI) in which renal resistance to antidiuretic hormone results in dilute polyuria despite normal circulating concentrations. A 28-year-old man with bipolar mood disorder presented to his local clinic with symptoms suggestive of lithium toxicity. Plasma lithium concentrations and thyroid-stimulating hormone (TSH) were taken, but results were not acted upon. One week later, he presented obtunded, severely dehydrated and in renal failure. His plasma lithium concentration was 4.3 mmol/L (toxic threshold >1.5 mmol/L) and TSH >100 mIU/L. A

Development of animal models with chronic kidney disease-mineral and bone disorder based on clinical characteristics and pathogenesis.

Chronic kidney disease-mineral and bone disorder (CKD-MBD) is a systemic complication of chronic kidney disease (CKD), resulting in high morbidity and mortality. However, effective treatment strategies are lacking. The pathogenesis of CKD-MBD is unclear but involves feedback mechanisms between calcium, phosphorus, parathyroid hormone, vitamin D and other factors, in addition to FGF23, Klotho, Wnt inhibitors, and activin A. Construction of a perfect animal model of CKD-MBD with clinical characteristics is important for in-depth study of disease development, pathological changes, targeted drug s

Endocrine and Metabolic Challenges of Long-Term Lithium Therapy: Diagnostic and Management Considerations.

Lithium is a key treatment for bipolar disorder but is associated with significant endocrine and metabolic side effects, particularly lithium-induced hyperparathyroidism (LIH). Characterized by hypercalcemia and inappropriately normal or elevated parathyroid hormone (PTH) levels, LIH can lead to skeletal complications such as osteoporosis. Managing this condition is particularly challenging in psychiatric patients due to adherence issues with follow-up care. We describe the case of a 56-year-old Caucasian female patient with bipolar disorder on long-term lithium therapy (16 years), presenting

Comparison of parathormone lowering effects of paricalcitol and calcitriol in hemodialysis patients.

BACKGROUND: Parathyroid hormone (PTH) levels may fluctuate in patients undergoing hemodialysis because of changes in calcium, phosphorus, and vitamin D levels. For these patients, the "Kidney Disease: Improving Global Outcomes" clinical practice guidelines recommend PTH levels be maintained in the range of two to nine times of the upper normal limit. Maintaining this balance is critical to prevent renal osteodystrophy. When the severity of hyperparathyroidism exceeds the recommended limits, vitamin D receptor agonists may be used for lowering PTH levels. Paricalcitol, as a biologically active

Renal Osteodystrophy as a Risk Factor for Postoperative Complications after Knee Arthroplasty: A National In-Patient Sample Study.

ABSTRACT: Renal osteodystrophy (ROD), a skeletal complication of chronic kidney disease (CKD)-mineral and bone disorder, may influence perioperative outcomes after total knee arthroplasty (TKA), but its impact remains unclear. This study examined patient characteristics, hospital resource utilization, and postoperative complications in ROD patients undergoing primary TKA. We performed a retrospective cohort analysis of the National Inpatient Sample (2010-2019). Adults undergoing primary TKA were identified and stratified by ROD status. Propensity score matching (PSM; 1:20) was used to balance

Advances in noninvasive diagnosis of renal osteodystrophy.

Renal osteodystrophy (ROD) refers to the histological abnormalities in bone associated with chronic kidney disease (CKD), closely linked to the progression of CKD, increased risk of fractures, cardiovascular events, and mortality. Early diagnosis and comprehensive assessment of ROD are crucial for enhancing the quality of life of CKD patients and reducing mortality rates. However, the diagnosis and evaluation of ROD remain significant challenges. Although tetracycline double-labeling of iliac crest biopsies is considered the gold standard for diagnosis, its invasiveness and limitations in dyna

The impact of bone histomorphometry, bone-related biomarkers, and FRAX® on fractures patients with predialysis CKD.

INTRODUCTION: Chronic kidney disease (CKD) patients are at increased risk of fracture. Whether the type of renal osteodystrophy (ROD) contributes to fracture risk is not currently established since bone biopsies are not frequently performed in clinical practice. We aimed to evaluate the association of ROD subtypes, bone biomarkers, and fracture risk assessed with the FRAX® tool with the occurrence of fractures in predialysis CKD patients. METHODS: Retrospective study with patients followed between 2014-2023. Blood tests, including bone-related biomarkers (BRB), and bone biopsies were performe

Effects of minas frescal cheese enriched with Lactobacillus acidophilus La-05 on bone health in a preclinical model of chronic kidney disease.

OBJECTIVES: Milk products are good vehicles for probiotics due to their physical-chemical characteristics, improving probiotic survival in food and in the gastrointestinal tract. Lactobacillus acidophilus La-05 is known to modulate gut microbiota, and probiotics have been reported to influence mineral absorption by improving the gut microbiota profile, highlighting the gut-bone axis. This intervention could be relevant in nephrology due to the high prevalence of renal osteodystrophy in chronic kidney disease (CKD). Given the potential role of gut microbiota in bone metabolism, this is the firs

An Unusual Case of Lithium-Induced Nephrogenic Diabetes Insipidus.

We report a case of a 53-year-old woman with bipolar affective disorder on long-term lithium therapy who presented with confusion, acute right lower quadrant abdominal pain, fever, vomiting, and increased urinary frequency, with suspected urinary sepsis. Initial assessment revealed very high C-reactive protein (CRP), severe hyperglycemia, and acute kidney injury (AKI Stage 3). She was treated for urinary sepsis as computed tomography (CT) imaging demonstrated bilateral pyelonephritis, and Escherichia coli was isolated from blood cultures. Lithium was withheld initially due to renal impairment.
