A rare case report on diabetes insipidus following living donor liver transplantation in patient with alcoholic liver disease.

A rare occurrence of nephrogenic diabetes insipidus (DI) following liver transplantation despite elevated plasma antidiuretic hormone levels and normal brain imaging findings is of interest. On the sixth day post-transplantation, the patient developed polyuria, a hallmark symptom of DI, prompts a comprehensive differential diagnosis. Desmopressin therapy was initiated, leading to rapid symptom resolution and full recovery. This case underscores the diagnostic challenges of DI in liver transplant recipients, particularly in differentiating between central and nephrogenic forms. Conventional dia

"Metabolic Superscan" Pattern in a Case of Secondary Hyperparathyroidism With Parathyroid Hyperplasia Detected on 68 Ga-Trivehexin PET/CT.

Parathyroid hyperplasia frequently causes secondary hyperparathyroidism (SHPT) in chronic kidney disease (CKD), necessitating bilateral neck exploration. Although preoperative imaging is limited, it is essential for reducing recurrent or persistent disease. This case highlights the potential of 68 Ga-trivehexin PET/CT in SHPT, demonstrating better identification of hyperfunctioning parathyroid glands, potentially assisting surgical planning. In addition, diffuse skeletal tracer uptake revealed a "metabolic superscan" pattern consistent with renal osteodystrophy and rapid bone-turnover. 68 Ga-t

Aerobic exercise prevents renal osteodystrophy via irisin-activated osteoblasts.

Renal osteodystrophy is commonly seen in patients with chronic kidney disease (CKD) due to disrupted mineral homeostasis. Given the impaired renal function in these patients, common antiresorptive agents, including bisphosphonates, must be used with caution or even contraindicated. Therefore, an alternative therapy without renal burden to combat renal osteodystrophy is urgently needed. Here, we report that clinically relevant aerobic exercise significantly prevents high-turnover renal osteodystrophy in CKD mice and patients with CKD without compromising renal function. Mechanistically, 4-week

Measuring bone metabolic activity in dialysis patients with renal osteodystrophy using [18F]-Sodium Fluoride positron emission tomography- comparison between static and dynamic measurements.

BACKGROUND: Dynamic [18F]NaF PET has shown promising results in the measurement of bone metabolism in patients with renal osteodystrophy. Dynamic PET scans are challenging in clinical practice, and the objective of this study was to evaluate whether standardized uptake values measured by [18F]NaF PET could be a feasible method. METHODS: Twenty-eight patients on maintenance dialysis with confirmed renal osteodystrophy underwent a dynamic [18F]NaF PET scan. As a reference for bone metabolism, a bone biopsy was obtained from the anterior iliac crest (AIC). Tracer activity in bone was measured us

Copeptin in the diagnosis and management of renal tubular disorders.

Copeptin, a stable glycopeptide derived from the precursor of arginine vasopressin (AVP), has emerged as a valuable surrogate biomarker for AVP due to its stability and ease of measurement. This narrative review explores the physiological role of copeptin, its utility as a diagnostic and prognostic biomarker in different kidney diseases, and its clinical relevance in renal tubular disorders. The clinical application of copeptin as a diagnostic biomarker is best established in the differential diagnosis of polyuria-polydipsia syndrome (PPS), distinguishing nephrogenic diabetes insipidus (NDI) f

ENaC inhibitors for the management of lithium related polyuria: a systematic review.

BACKGROUND: By entering collecting duct principal cells via the epithelial sodium channel (ENaC), lithium is capable of inducing vasopressin insensitivity, resulting in excessive urine production, nephrogenic diabetes insipidus (NDI) and potential for other long-term forms of renal dysfunction. ENaC inhibitors (ENaC-I) such as amiloride have been shown in animal models to minimise this adverse effect, and while ENaC-I are often considered an effective strategy, the literature on ENaC-I for lithium-related polyuria has not yet been synthesised despite the importance of this topic. This review a

Transcriptional regulatory factors of the Aqp2 gene.

Aquaporin-2 (AQP2), the water channel protein exclusively expressed in the kidney collecting duct, plays an essential role in water reabsorption and fluid homeostasis. Dysregulation of Aqp2 transcription and changes in protein abundance lead to water balance disorders such as nephrogenic diabetes insipidus or water overload states. Recent studies have revealed that Aqp2 transcription is highly selective, with only 35 of 3,659 genes exhibiting vasopressin-dependent regulation. Two enhancer elements within a CCCTC-binding factor-insulated topologically associating domain containing the Aqp2 gene

Multifaceted skeletal effects of sevelamer carbonate in a secondary hyperparathyroidism model.

INTRODUCTION: Hyperphosphatemia leads to abnormal mineralization of bones and soft tissues in patients with chronic kidney disease-induced secondary hyperparathyroidism (CKD-SHPT). Sevelamer lowers phosphate levels by binding to dietary phosphate in the gastrointestinal system, forming new bone and reducing the risk of renal osteodystrophy and fracture. However, the influence of sevelamer carbonate (SevC) on bone microarchitecture, material qualities, and mechanical behavior is unknown in CKD-SHPT conditions. MATERIAL AND METHODS: We utilized a rat model of CKD-induced hyperphosphatemia by fe

Dysregulated neutrophils contribute to bone loss in renal osteodystrophy by enhancing osteoclastogenesis: Insights from integrated bioinformatics and experimental validation.

Renal osteodystrophy (ROD) is a major debilitating skeletal complication of chronic kidney disease characterized by dysregulated bone remodeling. Neutrophils are essential for bone homeostasis. However, their functionality in the ROD osteoimmune microenvironment remains unclear. To investigate the crosstalk between neutrophils and bone metabolic imbalance in ROD, we firstly performed transcriptomic profiling in patients with ROD, revealing a pronounced disruption of bone homeostasis. These findings were corroborated in a murine model of ROD, which demonstrated an osteoporotic phenotype and inc

Miliary tuberculosis with hypercalcaemia-induced nephrogenic diabetes insipidus in an infant.

Miliary tuberculosis (TB) is a rare but severe manifestation in infancy. Hypercalcaemia, well documented in granulomatous diseases like TB and sarcoidosis in adults, is infrequently reported in children. Nephrogenic diabetes insipidus induced by hypercalcaemia is a known complication. A female infant with right-sided neck swelling for 1 month (fine-needle aspiration showed tubercular lymphadenitis), on four-drug anti-tubercular therapy (ATT), admitted with complaints of polyuria, polydipsia and progressive weight loss over the last 2 weeks. Her father was on ATT for pulmonary TB. Investigation

Ifosfamide-induced nephrotoxicity in oncological patients.

INTRODUCTION: Ifosfamide is an alkylating chemotherapeutic agent used in the treatment of various neoplasms. Its main adverse effects include renal damage. AREAS COVERED: A comprehensive review was conducted, including 100 articles from the Scielo, Scopus, and EMBASE databases. Ifosfamide-induced nephrotoxicity is attributed to its toxic metabolites, such as acrolein and chloroacetaldehyde, which cause mitochondrial damage and oxidative stress in renal tubular cells. Literature review found a 29-year average age with no gender predominance and a mortality of 13%. Currently, no fully effective

Targeted long-read sequencing identified a causal structural variant in X-linked nephrogenic diabetes insipidus.

BACKGROUND: X-linked nephrogenic diabetes insipidus (NDI) is a rare genetic renal disease caused by pathogenic variants in the AVPR2 gene. Single nucleotide variants and small insertions/deletions in AVPR2 are reliably detected by routine clinical sequencing. Nevertheless, structural variants involving AVPR2 are challenging to identify accurately by conventional genetic testing. Here, we report a novel deletion of AVPR2 in a Czech family identified for the first time by targeted long-read sequencing (T-LRS). METHODS: A male proband with X-linked NDI underwent clinical sequencing of the AVPR2

Nephrogenic diabetes insipidus in a geriatric patient affected by SARS-CoV-2: complexity of a diagnosis, complexity of a virus.

BACKGROUND: Coronavirus disease 2019 (COVID-19) has an important impact on the kidney through direct and indirect damage mechanisms. Most previous studies have highlighted lesions caused by this virus in the early segments of the nephron. However, due to the antigenic characteristics of the virus, with almost ubiquitous receptors, and the molecular release it triggers, the distal segments of the nephron could also be affected. METHODS: A 71 year-old-man with respiratory failure while suffering from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pneumonia presented with typical s

Congenital nephrogenic diabetes insipidus treated with acetazolamide.

Congenital nephrogenic diabetes insipidus (CNDI) is a rare disorder. The condition is characterised by an inability of distal nephron segments to respond to normal or raised concentrations of serum antidiuretic hormone. In this report, we describe the case of a 13-year-old male known with CNDI who experienced a pedestrian vehicle accident leading to coma following a head injury. Intra-operatively, severe hypernatraemia and polyuria were observed. Following an inadequate response to conventional therapy, acetazolamide was prescribed resulting in an immediate response to therapy. To the best of
