Renal osteodystrophy in Egyptian CKD patients: observations from clinically indicated bone biopsies.

BACKGROUND: Renal osteodystrophy (ROD) varies among populations. Different genetic and environmental factors and prescription patterns may explain this variation. Egyptian ROD is not sufficiently studied. Although bone biopsy is the gold standard tool, no single study reported the actual ROD spectrum based on bone biopsy in Egypt or Africa. International guidelines must consider the different patterns in developing countries. METHODOLOGY: The ISN-sistership program enabled us to create an Egyptian bone biopsy consortium that provided a nationwide specialized CKD-MBD service. We included all CK

Recurrent Catatonia in the Setting of Urinary Tract Infection and Medical Comorbidity: A Case Report.

Catatonia is a neuropsychiatric syndrome characterized by disturbances in motor activity, affect, and autonomic function. Although often associated with primary mood or psychotic disorders, it can be precipitated or sustained by systemic infections such as urinary tract infection (UTI). We describe a 60-year-old man with recurrent catatonia and diagnoses of schizophrenia, bipolar disorder, and major neurocognitive disorder, whose latest episode of catatonia, described in this case report, was preceded by psychosocial stress and appeared to be exacerbated by polymicrobial UTI, bacteremia, and u

The Janus face of parathyroid hormone in renal osteodystrophy.

This editorial comment discusses novel insights into renal osteodystrophy from a parallel publication, highlighting that uremia per se independently impairs bone cell viability and induces skeletal parathyroid hormone resistance. Using multimodal bone analyses, Wagner et al. show that parathyroid hormone exerts compartment-specific effects: protective for trabecular osteocytes yet detrimental to cortical integrity via increased porosity and by bone fibrosis and increased resorption. These findings redefine optimal parathyroid hormone targets and emphasize addressing uremic toxicity to reduce f

Diagnostic agreement of bone histomorphometry for renal bone disease.

INTRODUCTION: An iliac bone biopsy with semi-quantitative histomorphometry is the gold standard for evaluating renal osteodystrophy (ROD). However, lack of standardization of the bone biopsy procedure, sample handling, reading and reporting represent barriers to research synthesis and clinical collaboration. This study assessed diagnostic agreement across European laboratories performing histomorphometry for evaluation of ROD. METHODS: Five-millimeter transiliac biopsy slides from 5 cases were converted to digital images for analysis by experts from 7 laboratories. Parameters included quantit

A need for normalization of bone histomorphometry parameters in chronic kidney disease.

PURPOSE OF REVIEW: Bone histomorphometry remains the only method capable of providing a direct, tissue-level assessment of bone remodeling, mineralization, and bone volume in patients with chronic kidney disease (CKD). The introduction of the TMV (turnover, mineralization, volume) classification represented a major conceptual advance by offering a unified framework to describe the heterogeneous skeletal abnormalities of renal osteodystrophy. However, nearly two decades after its introduction, the clinical and research application of the TMV system remains inconsistent. This limitation reflects

Renal osteodystrophy in pediatric patients. A systematic review and meta-analysis.

Chronic kidney disease (CKD) in children is associated with significant comorbidities, including mineral and bone disorder (CKD-MBD). A key manifestation of CKD-MBD is renal osteodystrophy (ROD), diagnosed by bone biopsy followed by histomorphometric analysis. In pediatric patients, ROD can lead to bone deformities and impaired growth. However, the limited use of bone biopsy in this population hampers the understanding of ROD patterns and severity. This systematic review and meta-analysis aimed to evaluate histomorphometric findings in children with CKD. We screened 5082 abstracts published up

Gut-derived uremic toxin indoxyl sulfate and aryl hydrocarbon receptor in renal osteodystrophy.

Chronic kidney disease (CKD) affects 14% individuals worldwide. Changes in bone and mineral metabolism in CKD are ubiquitous (CKD- Mineral and Bone Disorder) including renal osteodystrophy with a fracture incidence of up to 5 times greater than the general population. While PTH lowering is the mainstay of therapy for decades this has not reduced fractures in patients or improve bone mechanics in pre-clinical models. This suggests additional factors involved in the pathogenesis of renal osteodystrophy such as uremic toxins due to either increased production or decreased renal excretion. One gro

Multifocal Uremic Tumoral Calcinosis From Prolonged Hyperparathyroidism in Autosomal Dominant Polycystic Kidney Disease.

Secondary hyperparathyroidism (SHPT) is a common complication of chronic kidney disease. Prolonged stimulation of the parathyroid glands can lead to severe mineral bone disorders. Uremic tumoral calcinosis (UTC) is a rare but debilitating manifestation of prolonged hyperparathyroidism characterized by periarticular calcium-phosphate deposits. This case describes the progression of SHPT to multifocal UTC in a patient with autosomal dominant polycystic kidney disease (ADPKD). A 46-year-old female with ADPKD-related end-stage renal disease (ESRD) was evaluated for renal transplantation and found

Pre-Parathyroidectomy PTH as an Integrated Biomarker of Glandular Remodeling and Skeletal Turnover in Secondary Hyperparathyroidism.

Secondary hyperparathyroidism (SHPT) is a major component of chronic kidney disease-mineral and bone disorder (CKD-MBD), reflecting progressive disturbances in mineral metabolism, endocrine signaling, skeletal remodeling, and parathyroid-gland biology. Traditionally, preoperative parathyroid hormone (PTH) has been used primarily as a biochemical threshold for surgical referral. However, persistent PTH elevation in advanced CKD-related SHPT may reflect more than isolated endocrine activity; available evidence suggests it integrates parathyroid-gland remodeling, receptor resistance, skeletal tur

Combination of Biomarkers and Novel Diagnostic Tools in the Management of Osteoporosis in Chronic Kidney Disease: An Update.

Chronic kidney disease-mineral and bone disorder (CKD-MDB) is a systemic disorder that occurs as a complication of advanced chronic kidney disease. It includes biochemical alterations (calcium, phosphorus, parathyroid hormone, vitamin D), abnormalities in bone turnover and mineralization, and vascular and soft-tissue calcifications. The development of secondary hyperparathyroidism and profound alterations in bone remodeling culminate in renal osteodystrophy, which contributes to adverse cardiovascular outcomes and increased mortality. This narrative review article aims to summarize the role of

"The Renal Trail Reveals Masked Face" - A Unique Case Series.

Chronic renal disease (CRD) is a progressive, irreversible loss of renal mass and function, posing a significant health concern. Secondary hyperparathyroidism caused by hypokalcemia in CRD often leads to renal osteodystrophy (ROD), a bone pathology resulting from alterations in mineral metabolism. ROD typically occurs early in renal failure and progresses as kidney function deteriorates. Marked jaw enlargement remains a rare but severe complication of the ROD, causing significant facial disfigurement. This case series highlights the four cases of ROD with extensive orofacial involvement, empha

Mineral and Bone Disease in CKD and Kidney Transplantation: Controversies, Gaps, and a Path Forward.

The pathophysiology of chronic kidney disease (CKD)-related mineral and bone disorder (CKD-MBD) extends far beyond simple mineral imbalances. Evolving from the traditional understanding of secondary hyperparathyroidism, the conceptual framework has recently been updated to recognize 2 distinct but overlapping clinical syndromes: CKD-associated osteoporosis, which encompasses the significantly increased fracture risk and microarchitectural deterioration in this population; and CKD-associated cardiovascular disease, which accounts for vascular and structural cardiac abnormalities, including medi

Severe Tertiary Hyperparathyroidism with Brown Tumor and Multisystem Complications Mimicking Plasma Cell Dyscrasia in a Middle-Aged Man with Childhood-Onset FSGS.

Chronic kidney disease-mineral and bone disorder (CKD-MBD) is a well-recognized complication of end-stage kidney disease (ESKD), encompassing abnormalities in calcium, phosphorus, parathyroid hormone (PTH), vitamin D metabolism, and bone turnover. In advanced cases, secondary or tertiary hyperparathyroidism may lead to severe skeletal disease, including osteitis fibrosa cystica and brown tumors, which can mimic hematologic malignancies. We present the case of a 47-year-old man with childhood-onset focal segmental glomerulosclerosis (FSGS), two prior kidney transplants, and long-standing dialys

Accelerated osteocytic citrate production in chronic kidney disease is associated with protection of the kidney.

Metabolites mediate inter-organ communication and this metabolic crosstalk often goes awry in systemic diseases, including chronic kidney disease-mineral bone disorder (CKD-MBD), where CKD disrupts skeletal homeostasis, with significant bone loss and increased fracture risk observed in patients. While metabolites from the injured kidney have been identified to disrupt bone function, whether metabolites from the bone influence kidney function over the course of CKD progression is less-understood. Given that bone-derived factors such as FGF23 and sclerostin influence CKD-MBD disease progression,

Diffuse skeletal [18F]-FDG uptake from renal osteodystrophy with parathyroid hyperplasia in an ESRD patient: A case report.

Secondary and tertiary hyperparathyroidism (HPT) are common complications in patients with end-stage renal disease (ESRD) and may result in renal osteodystrophy. While [18F]-FDG PET/CT is primarily used to evaluate malignancy, it may occasionally reveal metabolic or endocrine abnormalities. A 45-year-old male with ESRD on hemodialysis presented with generalized bone pain and radiologic suspicion of malignancy. [18F]-FDG PET/CT demonstrated diffusely increased skeletal uptake and multifocal lytic bone lesions, consistent with metabolic bone disease secondary to renal osteodystrophy. Incidental

Efficacy and safety of doxercalciferol (1α hydroxyvitamin D2) on spine and femur bone mineral density in postmenopausal osteopenic women.

BACKGROUND: The use of small doses of vitamin D3 analogs or metabolites is common in the prevention of renal osteodystrophy. Their use in the treatment of osteoporosis has been limited by their potent adverse effects, causing hypercalcemia and hypercalciuria. We evaluated a vitamin D2 analog, i.e., 1α-hydroxyvitamin D2 (doxercalciferol), that has a lower incidence of adverse effects. METHODS: In a 1-year pilot randomized double-blind, placebo-controlled study, 55 postmenopausal women (aged 60-71 years) with osteopenia were treated for a year with either doxercalciferol or placebo, and 41 wome

Phlorizin attenuates chronic kidney disease induced bone loss by mitigating sclerostin mediated inhibition of Wnt/β-catenin signaling and ameliorating metabolic alterations.

CKD-MBD is a metabolic disorder characterized by disrupted mineral metabolism, leading to vascular calcification, renal osteodystrophy, and high fracture risk. Sclerostin, an osteocyte-derived Wnt inhibitor, is a key regulator of both bone loss and vascular calcification. This study explored the role of phlorizin (SGLT1 and SGLT2 inhibitor) on bone loss and metabolic profile in CKD-MBD. Calvarial osteoblasts were treated with phlorizin and evaluated for ALP activity, viability, mineralization, and gene/protein expression. Molecular docking and simulation studies were performed to check phloriz
