Prognostic values of proteinuria in patients with acute heart failure.

BACKGROUND: Renal dysfunction is significantly associated with poor prognosis in patients with heart failure. However, the prognostic significance of proteinuria as a potential marker of an impaired glomerular filtration barrier in acute heart failure (AHF) remains unclear. We aimed to investigate the prognostic value of urinary protein/creatinine ratio (PCR) in patients with AHF. METHODS: Urinary protein levels measured at admission were adjusted for urinary creatinine concentrations in 346 patients (75 ± 13 years; 61 % men) with AHF. Patients were categorized based on urinary PCR, adhering

Genetic, structural, and functional foundations of alkaline phosphatase measurement in clinical diagnostics.

Alkaline phosphatase (ALP) is a membrane-bound ectoenzyme with critical diagnostic relevance in clinical medicine, particularly for bone and liver diseases. This review examines the genetic, structural, and functional foundations of ALP, highlighting its distribution across tissues such as bone, liver, intestine, placenta, and neutrophils. The article details the evolution of the ALP gene family, including tissue-non-specific and tissue-specific isoenzymes, and discusses how genetic variation and post-translational modifications influence ALP activity, serum levels, and clinical interpretation

Investigation of the effects of silymarin and vitamin C on kidney damage and aquaporin-2 downregulation in lithium-induced nephrogenic diabetes insipidus in rats.

Although lithium (LIT) therapy is key in managing bipolar disorder long-term, prolonged use significantly contributes to acquired Nephrogenic Diabetes Insipidus (NDI). This study examined whether combining Silymarin (SIL) with Vitamin C (Vit C) enhances protection against lithium-induced nephrotoxicity in rats, comparing their individual antioxidant effects as well. Rats subjected to Li exposure were provided with a standard commercial diet supplemented with 80 mmol LiCl per kilogram for 28 days. Concurrently, SIL and Vit C were administered orally at dosages of 200 and 100 mg/kg body weight,

Renal Ischemia Induces Endoplasmic Reticulum Stress and Impairs the Reparative Potency of Scattered Tubular-Like Cells.

INTRODUCTION: CD24+/CD133+ scattered tubular-like cells (STCs) are surviving renal cells that acquire progenitor-like characteristics to repair other damaged kidney cells. Renal artery stenosis (RAS) impairs the reparative capacity of STCs, but the underlying mechanisms remain unknown. STCs contain abundant endoplasmic reticulum (ER), but its capacity to fold proteins could become saturated (ER stress), leading to STC dysfunction. We hypothesized that RAS alters the expression of genes implicated in ER stress in swine STCs. METHODS: STCs were harvested from pig kidneys after 10 weeks of RAS o

Central and nephrogenic diabetes insipidus: updates on diagnosis and management.

Diabetes insipidus (DI) is a rare endocrine disease involving antidiuretic hormone (ADH), encompassing both central and nephrogenic causes. Inability to respond to or produce ADH leads to inability of the kidneys to reabsorb water, resulting in hypotonic polyuria and, if lack of hydration, hypernatremia. DI cannot be cured and is an unfamiliar disease process to many clinicians. This diagnosis must be distinguished from primary polydipsia and other causes of hypotonic polyuria. The main branchpoints in pathophysiology depend on the level of ADH pathology: the brain or the kidneys. Prompt diagn

Dietary Polyunsaturated Fatty Acid Deficiency Impairs Renal Lipid Metabolism and Adaptive Response to Proteinuria in Murine Renal Tubules.

Background/Objectives: Kidneys are fatty acid (FA)-consuming organs that use adenosine triphosphate (ATP) for tubular functions, including endocytosis for protein reabsorption to prevent urinary protein loss. Peroxisome proliferator-activated receptor α (PPARα) is a master regulator of FA metabolism and energy production, with high renal expression. Although polyunsaturated fatty acids (PUFAs) are essential nutrients that are natural PPARα ligands, their role in tubular protein reabsorption remains unclear. As clinical PUFA deficiency occurs in humans under various conditions, we used a mouse

Lithium-induced nephrogenic diabetes insipidus following improved medication compliance: a case report.

BACKGROUND: Nephrogenic diabetes insipidus is a rare, often underrecognized complication of long-term lithium therapy. Lithium-induced nephrogenic diabetes insipidus results from chronic renal exposure, leading to significant polyuria, dehydration, and hypernatremia. CASE PRESENTATION: We describe a case of a 55-year-old White caucasian male with a schizoaffective disorder managed with lithium who presented with altered mental status and electrolyte abnormalities following a recent stroke. Evaluation revealed nephrogenic diabetes insipidus as the primary etiology, likely exacerbated by recent

Clinical and Molecular Genetic Analyses of a Girl With Isolated Nephrogenic Diabetes Insipidus due to Contiguous Gene Deletion Involving AVPR2 and L1CAM.

Loss-of-function mutations of AVPR2 and L1CAM result in nephrogenic diabetes insipidus (NDI) and L1 syndrome. These diseases are inherited in an X-linked recessive manner. Females with heterozygous variants can be affected owing to skewed X-chromosome inactivation (XCI). A 3-year-old girl with normal development was presented with polydipsia and polyuria, and diagnosed of NDI through an improper response to water restriction and desmopressin administration. A targeted genome capture sequencing for X chromosome confirmed Xq28 microdeletion involving AVPR2 and L1CAM, derived from mother with mos

Nitric oxide-primed engineered extracellular vesicles restore bioenergetics in acute kidney injury via mitochondrial transfer.

Background: The disruption of mitochondrial homeostasis in acute kidney injury (AKI) is an important factor that drives persistent renal dysfunction. Mesenchymal stem cell-derived extracellular vesicles (MSC-EVs) have shown great therapeutic potential in AKI, but insufficient specificity of targeting the impaired mitochondrial function. Herein, we developed an engineered nitric oxide (NO)-primed MSC-EVs (pEVs) to restore mitochondrial homeostasis for AKI therapy. Methods: A cisplatin-induced AKI model was established to investigate the therapeutic effects of MSC-EVs. Proteomic and Western blot

The acetylation GLI1 affects arsenical-induced renal fibrosis by mediating the Hedgehog signalling pathway.

Arsenic exposure by drinking water is a key challenge in the field of public health worldwide. Research has found a connection between arsenic exposure and renal fibrosis, yet the mechanisms behind this are still unclear. In this study, rats were exposed to 50 mg/L NaAsO2 or 200 mg/L dimethylarsinic acid (DMA) via drinking water for 12 weeks, and HK-2 cells were treated with 1 μM NaAsO2 for 4 weeks. The results showed that the renal tubular structure was destroyed, renal function was impaired, and fibrosis occurred in renal tissue in arsenic-exposed F344 rats. Arsenic-treated HK-2 cells presen

Integrated multi-omics profiling identifies Cmpk1 as a monocyte-specific therapeutic target for renal ischemia-reperfusion injury.

BACKGROUND: Acute kidney injury (AKI) driven by ischemia-reperfusion injury (IRI) involves poorly defined immune-metabolic mechanisms. Although nucleotide metabolism dysregulation is implicated in tubular injury, its cell type-specific roles remain unexplored. We aimed to dissect cell -specific metabolic reprogramming in renal IRI and identify therapeutic targets. METHODS: We employed an integrative multi-omics strategy including bulk transcriptomics, metabolomics, single-cell RNA sequencing, spatial transcriptomics, and functional validation through in vitro hypoxia-reoxygenation models and

Deoxynivalenol Exposure Leads to Abnormal Renal Tubular Autophagy Flow.

Deoxynivalenol (DON) is one of the common mycotoxins contaminating feed. Renal tubular epithelial cells may experience oxidative stress as a result of consuming DON over time. However, little is known about the biological mechanisms through which DON results in kidney damage. This study employed proteomics, atomic force microscope (AFM), transmission electron microscope (TEM), and fluorescent probes, among other techniques, to reveal that DON exposure caused disturbances in renal autophagy flux both in vivo and in vitro. Mechanistically, this study found that DON exposure led to impaired forma

Renal tubular (pro)renin receptor deletion exacerbates kidney injury in db/db mice.

The (pro)renin receptor (PRR) is a multifunctional protein implicated in blood pressure regulation and kidney fibrosis. Previous studies report enhanced PRR expression in nondiabetic and diabetic kidney disease. In this study, we investigated whether deletion of renal tubular PRR attenuates kidney injury in type 2 diabetes. Floxed PRR mice were bred with mice expressing Pax8 rtTA and LC1 transgenes and db/db mice (B6.BKS) to obtain renal tubular PRR knockout (KO)-db/db mice. Male, age-matched nondiabetic floxed controls, db/db mice, and PRR KO-db/db mice were studied at 16, 20, 26, and 30 wk o

Alport Syndrome is a Partial Tubulointerstitial Disease of the Kidney.

INTRODUCTION: Recent genetic studies have shown that Alport syndrome (AS) is much more prevalent than clinically recognized, suggesting that atypical cases may phenocopy other kidney diseases. To date, pathomechanistic studies of AS have focused exclusively on the glomerular membrane, yet equally strong expression of collagen α(IV) chains is found along the distal renal tubule. We hypothesized that genetically determined abnormality of the tubular collagen IV (α345) molecule contributes to kidney failure and may drive atypical phenotypes. METHODS: Histology and primary tubular cells (PTCs) of

Crosstalk between kidney and bones: New perspective for modulating osteoporosis.

Growing evidence indicates an interesting interplay between kidney and bone. The pathophysiological condition of the skeletal system is intricately associated with the normal functioning of the kidneys. This relationship is modulated by various factors, including calcium and phosphate, 1-α-hydroxylase, erythropoietin (EPO), klotho, fibroblast growth factor 23 (FGF23), bone morphogenetic protein-7 (BMP-7), and extracellular vesicles (EVs). These interactions are notably evident in conditions such as chronic kidney disease with bone mineral density (CKD-BMD), renal osteodystrophy (ROD), and oste

Using solid-state MRI and a double-tuned RF coil to quantify bone matrix and mineral densities in rat bones.

Quantitative information on the composition of bone, specifically the content of calcium phosphate mineral and organic matrix, is essential for accurate diagnosis of metabolic bone diseases such as osteoporosis, osteomalacia, and renal osteodystrophy, as well as for differentiating among these conditions. Conventional MRI fails to provide this information because these substances are solid and, therefore, yield no signal in conventional MRI scans, which typically employ spin or gradient echoes. In this report, we show how phosphorus and proton solid-state MRI yield the desired compositional in

Diagnosis, evaluation and management of osteoporosis in chronic kidney disease: navigating treatment approaches - Indian consensus statement.

BACKGROUND: Managing osteoporosis (OP) in chronic kidney disease (CKD) presents significant challenges due to altered bone metabolism. Given the lack of robust clinical trial data and a notable knowledge gap exists among nephrologists regarding an optimal management in this population, an expert consensus is crucial for developing tailored management strategies. This study aimed to gather an expert opinion to bridge this gap and establish consensus recommendations on the diagnosis and management of osteoporosis in CKD patients. METHODS: A panel of 13 Indian and 1 international experts, includ

Fracture Risk in Chronic Kidney Disease: Addressing an Overlooked Complication.

Fracture risk is a serious yet underrecognized complication among patients with chronic kidney disease (CKD), especially in those with stages G3-G5D. The overlap between CKD-Mineral and Bone Disorder (CKD-MBD) and osteoporosis leads to complex bone changes that increase the likelihood of fragility fractures. Studies show that 18% to 32% of CKD patients also have osteoporosis, and these individuals are more than 2.5 times as likely to suffer from fractures compared to those without CKD. In the advanced stages of the disease, fracture risk is up to four times higher than in the general populatio
